Mixed epithelial and stromal tumor of the kidney (MESTK) is a recently described rare neoplasm. Malignant transformation, recurrence and metastasis are rare, therefore histopathological distinction from other renal neoplasms, especially from renal cell carcinoma is important. Histologically the tumor is composed of biphasic components including cysts and tubules embedded in the spindle cell stroma. We report a case of a MESTK in 60-year-old postmenopousal woman who presented with an incidental solid renal mass but no urinary complaint.

Determination of Indicator Plants for Boron in the Kirka(Eskisehir/Turkey)Boron Deposit Area

The amount of elements in plants is important for biogeochemical explorations. Some plants which were accumulated extremely elements, are called indicator plants. In this study, the 11 plant species and soil samples were collected boron deposits area in Kirka (Eskisehir). Boron is determinated in plants (leaves and twigs) and soil samples. Additionally, statistical relations were established between the B values of plant and soil samples and three plants were only found. The boron in twigs of Puccinellia intermedia (n = 17, r = 0.7274), twigs of Genista aucheri (n = 26, r = 0.8581 and leaves of Pinus nigra (n = 16, r = 0.6805) species are found 119, 81 and 115 mg/kg respectively. This species was reflected boron in soil and their used indicator plant for boron. The indicator plants could be successfully used for biogeochemical prospecting and environmental monitoring.

Hysteroscopic septum resection in patients with recurrent abortions and infertility

Objective: The aim of this study was to assess the achievement of pregnancy as well as the reproductive outcome after hysteroscopic septum resection in women with recurrent abortions or infertility. Design: retrospective, clinical trial. Setting: University hospitals. Patient(s): Fourty women with a diagnosis of uterine septum who had a history of pregnancy loss or infertility. They were randomized into two groups. Group I includes 18 infertile women and group II includes 22 recurrent miscarriage women. Intervention(s): Hysteroscopic metroplasty was performed on all patients in both groups. Result(s): 40 patients who were diagnosed with a partial septate uteri had hysteroscopic septum resection and were divided into two groups: Group I (18 patients) presented with infertility and Group II (22 patients) with recurrent abortions. The mean post-operative follow up period was 24 months. Group I, 12 patients (66.6%) and Group II, 17 patients (77.2%) achieved pregnancy, with respective abortion rates of 2 (11.1%) and 4 (18.1%). Conclusion(s): Using hysteroscopy to perform metroplasty on septate uteri makes the procedure safer, easier, and less complicated. This procedure is recommended for all cases diagnosed with an uterine septum.

Association of the plasminogen activator inhibitor-1(PAI-1) gene 4G/5G promoter polymorphism in Buerger's disease (Tromboangiitis obliterans)

Thromboangiitis obliterans (TAO) is an unusual tobacco-associated vasculopathy that is a non- atherosclerotic inflammatory disorder of unkn- own etiology that affects small and medium- sized vessels of the extremities. The single guanosine nucleotide deletion/insertion polym- orphism (4G/5G) at -675 bp in promoter region of the PAI-1 gene is the major genetic determi- nant of PAI-1 expression. Plasma PAI-1 level is higher in people with the homozygous 4G genotype than in those with the 5G/5G genotype and renders higher transcription activity. The aim of this study was to determine the status and the role of PAI-1 gene 4G/5G promoter polymorphism in patients with Buerger's disease (Thromboangiitis obliterans—TAO). The current case-control study included 30 consecutive pat- ients with Buerger's disease (mean age 42.9 ± 14.3 years, 28 men and 2 women), and 30 healthy volunteers (mean age 40.9 ± 4.79 years, 27 men and 3 women) between January 2006 and September 2009. Patients and control cases were genotyped for the 4G/5G polymorphism using the multiplex PCR based stripassay reverse hybridisation technique. It was found that heterozygote PAI-1 gene polymorphisms (p < 0.05) was significantly more frequent in patients with TAO in the current results. There was a significant difference in genotype distribution between the two groups (P < 0.001). The 4G allele occurred more frequently in the patient group of heavy smokers (P = 0.05). The current study shows the higher prevalence of of 4G allele in TAO patients in Sivas population means gene may predispose to TAO.

Knowledge and Attitude of Family Physicians Regarding Adult Vaccination

Objective: The aim of this study was to establish the knowledge and attitude of family physcians offering primary health care, towards adult vaccination and their own vaccination coverage. Methods: This descriptive study was performed in May 2009 in Kayseri, Turkey. 282 family physician accepted to fill a questionnaire about adult vaccination. Results: Most of the family physcians (83.0%) thought adult vaccination as a modality of protection and 66.7% wanted a vaccination scheme just like the one in childhood. Their own vaccination rate for Hepatitis B was 67.0% and 58.5% for influenza at any time. Only half of the physcians were vaccinated within the season for influenza. Only 9.2% of the doctors have stated that they ask questions to their patients about their vaccination status. When physcians were questioned about the vaccines they advise to their patients aged 65 and more;70.6% stated influenza and 39.7% stated pneumococcus. Conclusion: In spite of being in the risk group, family physicians have low vaccination coverage rates. Suggestion of the necessary vaccines to the risk groups or to their patients at risk is low as well.

Comparison of the Five Different Methods in Arterial Diameter Measurement

Background/Aims: In this study, the different types of arterial diameter measurement methods were com-pared with each other on rat model. Methods: The study was planned in three phases. In phase 1, all subjects (n = 30) underwent high resolution B-mode ultrasound examination and external diameter of the right com-mon carotid artery (RCCA) was measured. In phase 2, RCCA was explored. In phase 3, rats were put into three groups. Group 1 was kept at -20oC, group 2 was embedded in 4% formaldehyde solution. In group 3, circulatory system was filled with %20 latex solution. In all groups, digital image of the RCCA was taken after the processes mentioned above. Images were assessed by Image – Pro Plus software. Results: The greatest average diameter was observed in the high resolution B-mode ultrasound (phase 1). In group 1 and 2, direct measurement of the RCCA on living animal (phase 2) gave significantly wider diameter than those obtained at phase 3 in the same groups. Direct (phase 2) and latex (phase 3) measurements were equal in group 3. Conclusion: High resolution B-mode ultrasound gives larger, probably actual diameter of the artery. Latex injection results might correspond those encountered during operation.

Association of Tetanus-Diphtheria Vaccination during Pregnancy with Preeclampsia

Introduction: The tetanus-diphtheria vaccine administered during pregnancy is used as a toxoid vaccine with an intact antigen structure. In this study, we investigated the association of tetanus-diphtheria vaccination (TDV) during pregnancy with adverse maternal and neonatal outcomes and particularly preeclampsia that may occur during pregnancy. Materials and Methods: From the patients who presented to our clinic between 01.01.2016 and 31.12.2016 and underwent a cesarean section, a total of 435 patients, 327 who received TDV and 108 who did not receive TDV, were included in the study. The groups were compared with respect to their demographic characteristics. The patient data on the gestational week, birth weight, normal newborn, preeclampsia, small for gestational age (SGA), early membrane rupture, preterm labor, diabetic mother’s baby, prematurity, temporary neonatal tachypnea, newborn’s need for intensive care, and neonatal mortality were recorded. Results: While 14.7% of the women who had been administered TDV had preeclampsia diagnosis during their pregnancy, 12% of those who had not been vaccinated were diagnosed with preeclampsia. This showed no significant correlation between having been vaccinated during pregnancy and preeclampsia (p = 0.474). The other outcomes we had specified had also no statistically significant correlations with vaccination during pregnancy. Having been vaccinated during pregnancy was found correlated only with the place of residence and nulliparity (p = 0.004 and p = 0.002, respectively). Conclusion: No correlation was found in the study between TDV during pregnancy and preeclampsia. According to the data obtained from this study, vaccination was found to have no negative effect on preeclampsia or the other outcomes.

Prognostic Factors and Treatment Outcome in 178 Locally Advanced Cervical Cancer Patients

Background: To evaluate local control, survival, radiation side effects and treatment outcome in locally advanced cervical cancer patients. Materials and Methods: Among 2006-2011, 178 patients with locally advanced cervical cancer were treated with chemoradiotherapy +/-?radiotherapy and high dose rate (HDR) brachytherapy. Follow-up was complete for all patients. Concomitant chemotherapy was not administered in 44 patients due to renal impairments and ECOG of 2 - 3. Results: The median follow-up period was 34.5 months (range, 5 to 93) and 42 months (range, 14 to 93 months) for alive patients. Five years local-regional control, progression-free survival and overall survival rates were 87.8%, 58.9% and 67.3% in all patients, respectively. In this retrospective study young age, tumor diameter, stage, presence of residual tumor and administration of chemotherapy were effected in survival analysis. The parameters which affected the complete response of patients were defined as presence of concomitant chemotherapy and number of courses <5. Central region recurrence rate was defined higher in the group with treatment duration of 9 weeks and higher (p = 0.044). Conclusion: Primary chemoradiotherapy +/-?radiotherapy achieved a satisfactory rate of local control and survival rates with acceptable complications in locally advanced cervical cancer. Concomitant chemotherapy and treatment duration were the important prognostic factors for completed response locally advanced cervical cancers.

The Frequency of K-ras Mutation in Colorectal Adenocarcinomas with Absence of Distant Metastasis at Diagnosis

Purpose: K-ras mutations were reported to be in 40% - 60% of patients with sporadic colorectal cancers (CRCs). HER-2/neu oncogene that is important in breast carcinoma was reported in CRCs in several studies. The aims of our study are to determine;the frequency of K-ras mutation in CRC and positivity of HER-2/neu, the relation between K-ras mutation and HER-2/neu positivity, and also the relation between clinicopathological findings with K-ras mutation and HER-2/neu expression. Methods: Total of 35 colon resection specimen from patients without distant metastasis who were operated due to colorectal adenocarcinoma were included in the study. The sections were examined with light microscopy. Vascular and perineural invasions and pericolonic tumor deposits (PCTD) were investigated. HER-2/neu was applied immunohistochemically. DNA sample obtained from tumor paraffin block was screened for presence of 20 mutations in K-ras gene-codon 12, 13, 61 by AutoGenomics and Infinity Analyzer. Results: K-ras gene mutation was detected in 14 of 35 patients (40%). HER-2/neu positivity was detected in 7 cases (28.57%). There was not any significant correlation between HER-2/neu positivity, K-ras gene mutation and clinicopathological findings. There was direct correlation between PCTD and vascular invasion. Conclusions: There was not correlation between K-ras mutation and clinicopathological findings similar to several other studies. The relation between HER-2/neu expression and clinicopathological findings was not found.
