Hematopoietic stem cell transplantation for non-malignant gastrointestinal diseases

Both, autologous and allogeneic hematopoietic stem cell transplantation (HSCT) can be used to cure or ameliorate a variety of malignant and non-malignant diseases. The rationale behind this strategy is based on the concept of immunoablation using high-dose chemotherapy, with subsequent regeneration of naive T-lymphocytes derived from reinfused hematopoietic progenitor cells. In addition, the use of HSCT allows for the administration of high-dose chemotherapy (whether or not combined with immunomodulating agents such as antithymocyte globulin) resulting in a prompt remission in therapy-refractory patients. This review gives an update of the major areas of successful uses of HSCT in non-malignant gastrointestinal disorders. A Medline search has been conducted and all relevant published data were analyzed. HSCT has been proved successful in treating refractory Crohn’s disease (CD). Patients with refractory celiac disease type II and a high risk of developing enteropathy associated T-cell lymphoma have shown promising improvement. Data concerning HSCT and mesenchymal SCT in end-stage chronic liver diseases are encouraging. In refractory autoimmune gastrointestinal diseases high-dose chemotherapy followed by HSCT seems feasible and safe and might result in long-term improvement of disease activity. Mesenchymal SCT for a selected group of CD is promising and may represent a significant therapeutic alternative in treating fistulas in CD.

AIM: To report the outcome of surgery in patients with(pre)malignant conditions of celiac disease(CD) and the impact on ***: A total of 40 patients with(pre)malignant conditions of CD,ulcerative jejunitis(n = 5) and enteropathy associated T-cell lymphoma(EATL)(n = 35),who underwent surgery between 2002 and 2013 were retrospectively evaluated. Data on indications,operative procedure,post-operative morbidity and mortali ty,adjuvant therapy and overal l survival(OS) were collected. Eleven patients with EATL who underwent chemotherapy without resection were included as a control group for survival analysis. Patients were followed-up every three months during the first year and at 6-mo intervals ***: Mean age at resection was 62 years. The majority of patients(63%) underwent elective laparotomy. Functional stenosis(n = 1 3) and perforation(n = 12) were the major indications for surgery. In 70% of patients radical resection wasperformed. Early postoperative complications,mainly due to leakage or sepsis,occurred in 14/40(35%) of patients. Eight patients required reoperation. More patients who underwent resection in the acute setting(n = 3,20%) died compared to patients treated in the elective setting. With a median follow-up of 20 mo,seven patients(18%) required reoperation due to long-term complications. Significantly more patients who underwent acute surgery could not be treated with adjuvant chemotherapy. Patients who first underwent surgical resection showed significantly better OS than patients who received chemotherapy without ***: Although the complication rate is high,the preferred first step of treatment in(pre)malignant CD consists of local resection as early as possible to improve survival.

Management of recurrent rectal cancer:A population based study in greater Amsterdam

AIM： To analyze, retrospectively in a populationbased study, the management and survival of patients with recurrent rectal cancer initially treated with a macroscopically radical resection obtained with total mesorectal excision （TME）. METHODS： All rectal carcinomas diagnosed during 1998 to 2000 and initially treated with a macroscopically radical resection （632 patients） were selected from the Amsterdam Cancer Registry. For patients with recurrent disease, information on treatment of the recurrence was collected from the medical records. RESULTS： Local recurrence with or without clinically apparent distant dissemination occurred in 62 patients （10%）. Thirty-two patients had an isolated local recurrence. Ten of these 32 patients （31%） underwent radical re-resection and experienced the highest survival （three quarters survived for at least 3 years）. Eight patients （25%） underwent non-radical surgery （median survival 24 rno）, seven patients （22%） were treated with radio- and/or chemotherapy without surgery （median survival 15 mo） and seven patients （22%） only received best supportive care （median survival 5 too）. Distant dissemination occurred in 124 patients （20%） of whom 30 patients also had a local recurrence. The majority （54%） of these patients were treated with radio- and/or chemotherapy without surgery （median survival 15 mo）. Twenty-seven percent of these patients only received best supportive care （median survival 6 mo）, while 16% underwent surgery for their recurrence. Survival was best in the latter group （median survival 32 mo）. CONCLUSION： Although treatment options and survival are limited in case of recurrent rectal cancer after radical local resection obtained with TME, patients can benefit from additional treatment, especially if a radical resection is feasible.

Evaluation of Cladribine treatment in refractory celiac disease type Ⅱ

AIM: To evaluate cladribine [2-chlorodeoxyadenosine (2-CdA)] therapy in refractory celiac disease (RCD) Ⅱ. METHODS: An open-label cohort-study of RCD Ⅱ patients treated with 2-CdA was performed between 2000 and 2010. Survival rate, enteropathy associated T-cell lymphoma (EATL) occurrence, clinical course, and histological and immunological response rates were evaluated. RESULTS: Overall, 32 patients were included with a median follow-up of 31 mo. Eighteen patients responded well to 2-CdA. Patients responsive to 2-CdA had a statistically significant increased survival compared to those who were unresponsive. The overall 3- and 5-year survival was 83% in the responder and 63% and 22% in the non-responder group, respectively. The overall 2-year clinical, histological and immunological response rates were 81%, 47% and 41%, respectively. Progression into EATL was reported in 16%, all of these patients died. CONCLUSION: Treatment of RCD Ⅱ with 2-CdA holds promise, showing excellent clinical and histological response rates, and probably less frequent transition into EATL.

Dracunculiasis(Guinea worm disease) is a preventable waterborne parasitic disease that affects the poorest people living in remote rural areas in sub-Saharan African countries,who do not have access to safe drinking *** Guinea Worm Eradication Program,a 25-year old campaign to rid the world of Guinea Worm disease has now reached its final stage accelerating to zero cases in all endemic *** the 19th and 20th centuries,dracunculiasis was common in much of Southern Asia and the African *** overall number of cases has been reduced tremendously by≥99%,from the 3.32 million cases estimated to have occurred in 1986 in Africa to only I 797 cases reported in 2010 reported in only five countries(Sudan,Mali,Ethiopia,Chad and Ghana) and Asia free of the *** achievement is unique in its kind - the only previously eradicated disease is smallpox,a viral infection for which vaccination was possible - and it has been achieved through primary community-based prevention and health education *** efforts need to be taken in two countries,South Sudan(comprising 94%or I 698 out of 1 797 of the cases reported world-wide in 2010) and Mali because of frequent movements of nomads in a vast area inside and outside Mali’s *** factors favourable to dracunculiasis eradication are available including adequate financial resources,community and political support and high levels of *** there is no reason that this disabling parasitic disease cannot be eradicated soon before surprises arise such as new civil conflicts in currently endemic countries.

Multimodal Fused Deep Learning Networks for Domain Specific Image Similarity Search

The exponential increase in data over the past fewyears,particularly in images,has led to more complex content since visual representation became the new norm.E-commerce and similar platforms maintain large image catalogues of their *** image databases,searching and retrieving similar images is still a challenge,even though several image retrieval techniques have been proposed over the *** of these techniques work well when querying general image ***,they often fail in domain-specific image databases,especially for datasets with low intraclass *** paper proposes a domain-specific image similarity search engine based on a fused deep learning *** network is comprised of an improved object localization module,a classification module to narrow down search options and finally a feature extraction and similarity calculation *** network features both an offline stage for indexing the dataset and an online stage for *** dataset used to evaluate the performance of the proposed network is a custom domain-specific dataset related to cosmetics packaging gathered from various online *** proposed method addresses the intraclass variance problem with more precise object localization and the introduction of top result reranking based on object ***,quantitative and qualitative experiment results are presented,showing improved image similarity search performance.

Potential of South African road technology for application in China

One of the main problems with roads and highways in China is the reflection cracking caused by the cement stabilized subbase layers passing through the overlying asphaltic layers. The cracks permit the ingress of moisture which softens the layers below the subbase resulting in loss of support and accelerated breakdown of the subbase layer and reduction in the tiding quality. The aim of this paper is to present the use of South African pavement design approach of deep structure and thin surfacing to overcome the existing problems. The deep pavement structure provides good long-term support and avoids the influence of moisture ingress, which means that only surfacing damage needs to be repaired. An unbound crushed stone base layer which is an integral component of the pavement structure limits reflection cracking. The paper first deals with the South African pavement design procedure and contrast this with the Chinese pavement design method. The inherent weaknesses of these methods are discussed and flowing from this discussion proposals for adapting the South African approach to China is presented. The resultant proposals have a high likelihood of success and will counteract the influences of extreme climate and rampant overloading that occurs on the Chinese roads.

Abstract: Cell death is a common event in all types of plant organisms. Understanding the phenomenon of programmed cell death (PCD) is an important area of research for plant scientists because of its role in senescence and the post-harvest quality of ornamentals, fruits, and vegetables. In the present paper, PCD in relation to petal senescence in ornamental plants is reviewed. Morphological, anatomical, physiological, and biochemical changes that are related to PCD in petals, such as water content, sink-source relationships, hormones, genes, and signal transduction pathways, are discussed. Several approaches to improving the quality of post-harvest ornamentals are reviewed and some prospects for future research are given.

A novel forecasting approach to schedule aggregated electric vehicle charging

To be able to schedule the charging demand of an electric vehicle fleet using smart charging,insight is required into different charging session characteristics of the considered fleet,including the number of charging sessions,their charging demand and arrival and departure *** use of forecasting techniques can reduce the uncertainty about these charging session characteristics,but since these characteristics are interrelated,this is not ***,forecasting frameworks that cover all required characteristics to schedule the charging of an electric vehicle fleet are absent in scientific *** cover this gap,this study proposes a novel approach for forecasting the charging requirements of an electric vehicle fleet,which can be used as input to schedule their aggregated charging *** the first step of this approach,the charging session characteristics of an electric vehicle fleet are translated to three parameter values that describe a virtual ***,optimal predictor variable and hyperparameter sets are *** serve as input for the last step,in which the virtual battery parameter values are *** approach has been tested on a real-world case study of public charging stations,considering a high number of predictor variables and different forecasting models(Multivariate Linear Regression,Random Forest,Artificial Neural Network and k-Nearest Neighbors).The results show that the different virtual battery parameters can be forecasted with high accuracy,reaching R^(2) scores up to 0.98 when considering 400 charging *** addition,the results indicate that the forecasting performance of all considered models is somehow similar and that only a low number of predictor variables are required to adequately forecast aggregated electric vehicle charging characteristics.

Flow hydrodynamics in embankment breach

Breaching flow occurs during the breach development of the embankment, dike, earthen dam, landslide barrier, etc. and plays an import role in the breaching erosion as the driving force. According to the previous research, the breaching process can be classified into initiation phase, breach widening phase and breach deepening phase. Based on the breaching development classifications, the breaching flow can be seen as a special compound weir flow when the breach channel is in the relatively equilibrium condition. There were five physical flow models were designed in the hypothesis of rectangular shape and trapezoidal shape for the breach channel cross sections to study the breaching flow characteristics. The distributions of water level and velocity were measured and analysed in the breaching flows in overtopping condition and emerged condition. There were two helicoidal flows above the breach channel slopes and triangular hydraulic jump in the downstream of the breach channel in the overtopping condition and emerged condition. The hydraulic energy loss was calculated according to the breaching velocity and water level distribution in the upstream and downstream of the model. It is found that the test results of the breach flow physical model can be valuable to bring insight of the breaching process of embankment and make contributions to the validations and verifications of breach numerical models.
