Prevalence of Work-Aggravated Asthma among Bakery Workers in Abidjan (Cote d’Ivoire)

Introduction: Work-aggravated asthma is pre-existing or concomitant asthma whose symptoms are aggravated by the work environment. The aim of this study was to determine the prevalence of this pathology and its associated factors among bakery workers in Abidjan. Materials and Methods: This descriptive and analytical cross-sectional study was conducted over a period of four (4) months from 18 December 2019 to 18 April 2020. Two questionnaires were used, one on employees and the other on the indoor environment of bakeries. In addition, a spirometry test was performed on all bakery workers. Statistical analysis was performed using stata 15.1 software. Results: A total of 599 bakery employees, including bakers (59.73%), sales assistants/ cashiers (23.52%), cleaners (6.34%) and administrative staff (10.18%), were investigated. The mean age was 30.8 ± 8 years, with a sex ratio (M/F) = 2.2. Asthma symptoms were found in 95 (15.86%) employees, of whom 74 (77.9%) had work-related asthma and 11 (14.9%) had asthma aggravated by work. The factors associated with work-aggravated asthma were personal or family history of allergy or atopy [ORa = 3.75;CI95%: 1.56 - 8.93;p = 0.003], exposure to dust [ORa = 5.01;CI95%: 1.43 - 7.50;p = 0.011] and humidity level (60% - 70%) [ORa = 1.80;CI 95%: 0.99 - 3.28;p = 0.05]. Conclusion: Work-aggravated asthma is a reality in bakeries in Abidjan, with an estimated prevalence of 14.9%. Two of the three factors associated with this condition suggest a link with indoor air pollution. Combating air pollution in these establishments must therefore be a priority for the relevant authorities, in order to provide employees with a working environment that protects their health.

Introduction: Studies of abnormal ventilatory function in Côte d’Ivoire have been carried out in the workplace and in schools. The objective of this study was to determine the prevalence of respiratory symptoms and ventilatory function abnormalities in the population of the lagoon district of Attécoubé in Abidjan. Material and Methods: A cross-sectional study was carried out on 170 people in the municipality of Attécoubé Lagune. A questionnaire was used to collect information on sociodemographic, clinical, and environmental characteristics. A basic spirometry and a beta mimetic test were carried out on all the subjects surveyed. Data analysis was done with the stata 15.1 software. Results: The study population was composed of 103 women and 67 men with a sex ratio (M/F) of 0.65. The average age was 35.92 ± 15.28 years. The most frequent respiratory symptoms were chest tightness (29.41%), dyspnea (28.82%), sneezing (22.94%) and cough (22.35%). The prevalence of ventilatory function abnormalities was 43.24% among residents of Attécoubé Lagune and the most frequent abnormality was ventilatory restriction (35.15%) followed by obstruction (4.85%). The risk factor for ventilatory function abnormalities was heavy pollution [OR = 2.66;IC: 1.053 - 6.743;P = 0.038]. Conclusion: Residents of the Attécoubé Lagune district had many respiratory symptoms and a high prevalence of ventilatory function abnormalities. Improving air quality is urgently needed in this municipality.

Anionic Surfactants Levels in M’Koa Lake Water (Jacqueville, Cote d’Ivoire)

In this study, anionic surfactants concentrations and other physico-chemical parameters of surface water quality were assessed in the water of M’Koa Lake in the city of Jacqueville (C?te d’Ivoire). Three sampling campaigns were conducted at different seasons at six (6) sampling stations, identified in M’koa Lake by taking into account potential sources of pollution. The analyses of physico-chemical parameters were carried out using both the French AFNOR standard and the methods described by Rodier. The anionic surfactants concentrations were performed by using the methylene blue colorimetric method. The average concentrations of anionic surfactants in the water of M’koa Lake ranged from 0.23 ± 0.04 mg·L-1 to 1.43 ± 0.68 mg·L-1. They are high compared to both population density and socio-economic activities of Jacqueville’s town. These values can be explained by the characteristics of the studied Lake that do not allow enough dilution of the discharged effluent. The results indicate that M’koa Lake is subjected to harmful pollution by anionic surfactants. And among the physicochemical parameters studied, only turbidity and transparency values show water quality deterioration, favoured by wastewater discharge and direct human activities around the Lake.

Impacts of Environmental Management on the Quality of Traditional Well Water in the Soubre Region(South-West of Cote d’Ivoire)

Water supply is a global problem. A study was conducted on twelve (12) traditional wells water in Daba Dagnogo and Gnipi 2 to determine their physico-chemical quality. Two sampling campaigns (dry season and rainy season), were performed on 16 physical and chemical parameters. The results show that the water is acidic with an average pH of 6.41 ± 0.4;moderately mineralized with an average conductivity of 731.14 ± 611.98 μS·cm-1;moderately saline with an average rate of 1.56 ± 1.22. The average temperature of 28.07 ± 0.86°C is above the norm for all wells. All major ions are consistent with WHO standards except calcium and potassium. 41.67% of wells exceed the standard ammonium ion whose average was 1.43 ± 2.79 mg·L-1 nitrate and 100% with an average of 492.42 ± 434.02 mg·L-1. The ion concentrations are highest in the urban area and are higher in the dry season. The quality of the water sampled wells deteriorated. Several factors affect the degradation, namely the shallow wells, their proximity to toilet, the lack of sanitation and poor management of household waste. It will carry out a campaign to educate people to protect the immediate environment of the water points, avoiding the deposit of garbage and to provide the appropriate wells lids.

From (2,3-dihydro-1H-perimidin-2-yl)-phenyl, the substitution of OH group in ortho or para position on the phenyl ring, allows us to synthesize the studied compounds. These three compounds have been characterized by conventional spectroscopic methods (NMR and MS). The interest of this work is to review the antioxidant activity of our compounds. The antioxidant activity screening carried out according to FRAP and DPPH methods revealed significant anti-free radical properties for compounds 1 and 2 even at low concentrations. In contrast to the compound 2, compound 3 for which the OH group is substituted in para position has the lowest activity in both cases. Therefore the para position seems to be the least sensitive position to increase the antioxidant activity of this pharmacophore.

With aim of place at the disposal of the laboratories nonconventional food with food values and which do not have impact on the system haematopoietic of the rats of laboratory in particular the rat of the species Rattus norvegicus, the study of the haematologic parameters of rats nourished with nonconventional food makes containing powder of Moringa oleifera was undertaken. Indeed, analysis of the blood taking away carried out following the administration of five diets container various rates of powder of Moringa oleifera (0% (Control food), 25%, 50%, 75% and 100%) respectively codified L3P, L3P25, L3P50, L3P75 and L3P100. These different food formulations were administered to rats (Ratus norvegicus) during three months. The weight of these animals was daily taken, followed by blood samples every 30 days for the determination of the blood count. The results of the study were shown differences between the various food formulations for each period of blood samples in biological parameters. These differences were more significant for platelets. During the three months of study, the hematocrit, platelets, MCV and outside the control group, were reported a slight progressive increase. The leukocyte parameters of all the rats in the investigation, indicated an increase in the second month. In conclusion, the study shows that during the three months of investigation, the tested foods do not disturb the haematological parameters except for thrombocytes which reveal a modification.

Physico-Chemical Characterization of Industrial Liquid Discharges of Soap Factories in Abidjan, Côte D’Ivoire

This work deals with the efficient management of industrial liquid discharges from soap factories (ILDS) in the region of Abidjan. The aim of this work is to evaluate the physico-chemical quality of these discharges as well as the different types of pollution generated. Seventeen (17) industrial soap sources were studied through fourteen (14) global pollution parameters (T, pH, electrical conductivity EC, redox potential EH, suspended matter SM, COD, BOD5, biodegradability factor BF, , , , , total iron and ). A liquid waste collection campaign was done during six weeks from February to March 2016, corresponding to a period of intensive activity of these industries. The samples were analyzed according to the norms of the French Standardization Agency (AFNOR). Results were compared with the Ivorian guide values recommended by the Classified Installations Inspection Service (CIIS). Principal Component Analysis (PCA) is allowed to evaluate the pollution induced by these factories’ rejection. In addition, Ascending Hierarchical Classification (AHC) method leads to classify soap factories into three groups according to the physico-chemical quality of their releases. Moreover, the estimation of the biodegradability factor is permitted to know the state of the biodegradability of these effluents.

Practical Artificial Ventilation in an African Tropical Environment: Experience of the Intensive Care Unit of the University Hospital of Treichville

Objective: To describe the practice of artificial ventilation (VA) in a resuscitation unit of a developing country with a view to its improvement. Patients and Methods: Prospective study for descriptive and analytical purposes, carried out in the intensive care unit of the University and Hospital Center of Treichville (Ivory Coast) from April 2009 to June 2010. All the patients having benefited from a artificial ventilation for a duration greater than 6 hours were included in this study. The studied parameters were: Socio-demographic (age, sex), diagnostic, therapeutic (indications, duration and complications of artificial ventilation), evolutionary. Results: Out of a total of 204 admissions during the study period, 81 patients received artificial ventilation, an incidence of artificial ventilation in the order of 39.7%. There were 49 men and 32 women. The ventilated patients had an average age of 43.9 years (range: 4 years and 85 years). Pathologies requiring artificial ventilation were neurological (46%) and traumatic (28%). Stroke was the leading medical condition (65%) while polytrauma was the major traumatic condition (65%). The most commonly used ventilatory modes were controlled volume ventilation (52.4%) and assisted ventilation (34.9%). The mean duration of artificial ventilation was 5.98 ± 3.73 days (range: 1 day and 21 days). The nosocomial pneumonia acquired under mechanical ventilation (PAVM) constituted 27% of the complications observed under artificial ventilation. The average length of ICU stay for all ventilated patients was 9.85 +/- 7.51 days (range: 1 day and 31 days). The lethality in our series was 80%. Patient age was the only prognostic factor associated with death (P = 0.003). Conclusion: The practice of artificial ventilation is still difficult in Ivory Coast and is at the origin of many complications such as nosocomial pneumonia acquired under mechanical ventilation which complicate the life threatening of the patients.

Anesthesiological Management of Eclampsia in Tropical Africa: Experience of the University Hospital Center of Bouake

Introduction: Eclampsia is a common cause of maternal mortality in developing countries. The objective of this study is to evaluate the practice of anesthesia for eclampsia in tropical Africa. Patients and Methods: Retrospective descriptive study on the parturients who benefited from eclampsia anesthesia in the operating theaters of the gynecology and obstetrics department, the University Hospital Center (CHU) of Bouake over a two-year period (January 2015 to December 2016). The parameters studied were: anesthetic risk assessment, anesthetic management, immediate anesthetic and post-anesthetic accidents and incidents. Results: Out of a total of 3831 emergency cesarean sections, 65 were for eclampsia, or 1.69% of obstetrical surgical activity. The mean age of the patients was 20.30 ± 3.34 years (range: 14 years and 39 years). Prenatal consultation was not performed in 70% of cases and 85% of patients were primiparous. Patients classified as ASA IIIU accounted for 75% of the population and in 62% of cases, the Glasgow score was between 9 and 12. Induction was achieved with thiopental in 98% of patients and vecuronium was the only muscle relaxant used. The most common fetal complications were hypotrophy (50%), prematurity (30%) and respiratory distress (40%). Postoperative anesthetic complications were agitation (70%) and wake delay (15%). Maternal mortality was 5% and the neonatal death rate was around 7.69%. The elements of poor maternal prognosis were the ASA IVU class (P = 0.015) and the Aldrete awakening score between 3 and 5 awakening (P = 0.001). Conclusion: Anesthesia for eclampsia at CHU of Bouake is difficult. It needs to be improved thanks to better equipment of the health structures.
