The Effect of Recombinant LH Addition to Recombinant FSH on Assisted Reproductive Technologies Outcomes in Overweight and Obese Patients without Polycystic Ovary Syndrome

Background: Obesity and overweight negatively affect fertility. Protocols need?to be developed in ART treatments for obese and overweight women. Objectives: To investigate the effect of recombinant luteinizing hormone (rLH) addition to recombinant follicle stimulating hormone (rFSH) on treatment outcomes?in assisted reproductive technologies (ART) cycles in obese and overweight women without polycystic ovary syndrome. Methods: This retrospective cohort study was carried out in Kocaeli University Faculty of Medicine, Assisted Reproductive Techniques Clinic between January 2016 and March 2019. To analyze the impact of rLH addition to rFSH in GnRH antagonist cycles in overweight and obese, the patients were divided into four groups according to body mass index (BMI) and gonadotropin type;Group 1: patients with BMI ≥ 25 stimulated with rFSH alone (n: 37), Group 2: patients with BMI ≥ 25 stimulated with rFSH + rLH (n: 37), Group 3: patients with BMI between 18.5 - 24.99 stimulated with rFSH alone (n: 33), Group 4: patients with BMI between 18.5 and 24.99 stimulated with rFSH + rLH (n: 30). Patients with polycystic ovary syndrome were excluded. Results: Basal LH levels were found to be significantly lower in obese and overweight patients compared to normoweight patients (p = 0.01). Grade 1 embryo ratio in obese and overweight patients was higher in rLH added obese group than in group LH not included (64.9%, p = 0.005). Ongoing pregnancy rates (OPR) in obese and overweight patients were significantly higher in rLH added group compared to rFSH only group (43.2% vs 18.9% respectively, p = 0.044). However, OPR did not differ significantly in rLH added and rFSH only groups in normoweight patients (p = 0.588). Conclusion: This study has shown that obese and overweight non-PCOS patients have lower endogenous LH levels. Takotsubo syndrome(TTS)is a rare clinical entity commonly seen in post-menopausal women after an emotional or sometimes physical stress.[1,2]This syndrome is thought to be caused by increased circulation catecholamine levels secondary to an adrenergic stimulus resulting in transient coronary spasm and microvascular dysfunction.[3]TTS is characterized by acute and reversible left ventricular dysfunction with the typical ECG and clinical findings of an acute coronary syndrome but no significant coronary stenosis.[4]Typically,in TTS,antero-apical ballooning is observed in ventriculography,and segmentary wall motion abnormalities are observed in magnetic resonance imaging and *** heart failure,left ventricular outflow tract obstruction,mitral regurgitation and cardiogenic shock may complicate this syndrome.[5]Herein,we present a case who is the oldest one in the literature with TTS and cardiogenic shock.

Derivation of energy-based base shear force coefficient considering hysteretic behavior and P-delta effects

A modified energy-balance equation accounting for P-delta effects and hysteretic behavior of reinforced concrete members is derived. Reduced hysteretic properties of structural components due to combined stiffness and strength degradation and pinching effects, and hysteretic damping are taken into account in a simple manner by utilizing plastic energy and seismic input energy modification factors. Having a pre-selected yield mechanism, energy balance of structure in inelastic range is considered. P-delta effects are included in derived equation by adding the external work of gravity loads to the work of equivalent inertia forces and equating the total external work to the modified plastic energy. Earthquake energy input to multi degree of freedom（MDOF） system is approximated by using the modal energy-decomposition. Energybased base shear coefficients are verified by means of both pushover analysis and nonlinear time history（NLTH） analysis of several RC frames having different number of stories. NLTH analyses of frames are performed by using the time histories of ten scaled ground motions compatible with elastic design acceleration spectrum and fulfilling duration/amplitude related requirements of Turkish Seismic Design Code. The observed correlation between energy-based base shear force coefficients and the average base shear force coefficients of NLTH analyses provides a reasonable confidence in estimation of nonlinear base shear force capacity of frames by using the derived equation.

An approach for optimization of the wall thickness (weight) of a thick-walled cylinder under axially non-uniform internal service pressure distribution

Today, improving the weight/load carrying capacity ratio of a part is the matter of studies in most of the scientific and industrial *** dimensions, the amount of material removed from outer and inner radius while manufacturing and the service pressure applied affect the residual stress distribution throughout the wall thickness and hence the load-bearing capacity of a thick-walled cylinder. Calculation of residual stresses after autofrettage process and optimization of autofrettage outline dimensions by using the amount of service pressures applied are common issues in *** this study, mandrel-cylinder tube interference dimensions were renovated by using traditional methods for swage autofrettage process of a gun barrel. Also, the residual stresses in the cylinder after autofrettage process, inside and outside material removal process and the variable service pressure throughout the cylinder applied were taken into consideration and incorporated into the design. By using the constrained optimization method, wall thickness(thus the weight) was optimized(minimized)to achieve the specified safety factor along the length of the cylinder. For the same cylinder, the results of the suggested analytical/with residual stress calculation approach were compared to analytical/without residual stress calculation results and numerical topology optimization method calculation results. Since the experimental measurement results are not yet available, it was not possible to compare them with the calculation *** suggested approach enabled 22.9% extra weight reduction in proportion to numerical topology optimization and enabled 4.2% extra weight reduction in proportion to analytical/without residual stress *** this approach, the gain from residual stresses after autofrettage operation, the loss of residual stresses after material removal, and the effects of service pressures can be taken into account for each stage of design.

Determination of discontinuities in marble blocks via a nondestructive ultrasonic technique

Miners working in the marble industry have always been interested in identifying structural weaknesses in marble blocks before they are transported to marble processing plants. To achieve this difficult task, several simple methods have been developed among miners but observation-based methods do not consistently provide satisfactory results. A nondestructive method developed for testing concrete could be used for this purpose. In this study, this simple method based on differences in ultrasonic wave propagation in different materials was presented, and the test results performed both in the laboratory and a marble quarry were discussed.

Stem taper equations for diameter and volume predictions of Abies cilicica *** the Taurus Mountains,Turkey

Taper equations are an important tool in estimating stem volumes at a multi-product level for sustainable forest *** taper equations are tested from three categories to estimate diameter at specified point on the stem,height at specified diameter,volumes of any desired portion of the stem,and whole tree volume of Taurus fir in Taurus Mountains of *** account for autocorrelation and multicollinearity present among multiple stem data observations collected from the same *** statistical approaches were used in model *** are made to determine which equation provides the best overall fit to all data based on four goodness-of-fit statistics,Coefficient of determination(R2);Root mean square error(RMSE),Akaike’s Information Criterion(AIC),and Bayesian information criterion(BIC).Results indicated that all taper equations tested could be used to accurately estimate section diameter at given height and stem *** et al.’s taper equation provided better results than the others for Taurus fir when an additional stem diameter observation at 5.30 m was *** and variable-form taper models consistently provided better results than the simple taper models except for Max and Burkhart’s *** et al.’s and Kozak’s taper models showed equally good performance to describe stem taper and to predict tree stem ***,these taper equations are able to be used to estimate diameter and volume for Taurus fir trees,if an upper stem diameter measurement was not available.

Colorectal stenting for palliation and as a bridge to surgery:A 5-year follow-up study

AIM: To evaluate the long-term effectiveness of colonic stents in colorectal tumors causing large bowel ***: We retrospectively analyzed data from 49 patients with colorectal cancer who had undergone colorectal stent placement between January 2008 and January 2013. Patients' symptoms,characteristics and clinicopathological data were obtained by reviewing medical records. The obstruction was diagnosed clinically and radiologically. Histopathological diagnosis was achieved endoscopically. Technical success rate(TSR)was defined as the ratio of patients with correctly placed SEMS upon stent deployment across the entire stricture length to total number of patients. Clinical success rate(CSR) was defined as the ratio of patients with technical success and successful maintenance of stent function before elective surgery(regardless of number of SEMS deployed) to total number of patients. The surgical success rate(SSR) of colorectal stent as a bridge to surgery was defined as the ratio of patients with successful surgical procedures. Unsuccessful surgical outcomes were defined as being due to insufficient colonic decompression. The technical,clinical,surgical success rates and complications after stenting were ***: The median age of patients was 64(36 to 89). 44.9% of patients were male and 55.1% were female. Eighteen patients had the obstruction located in the rectum,15 patients in the rectosigmoid region,10 patients in the sigmoid region,and 6 patients had a tumor causing obstruction in the proximal colon. Each patient was categorized pathologically as stage 2(32.7%,16 patients) or stage 3(42.9%,21 patients) and 12 patients(24.4%) had metastatic disease. None of the patients received chemotherapy before stenting. Stenting was undertaken in 37 patients as a bridge to surgery,and in 12 patients stents were used for palliation. Surface-to-bed heat transfer for high-density particles in conical spouted and spout–fluid beds

Bed-to-surface heat transfer experiments from a vertically submerged cylindrical surface were conducted in laboratory-scale (Dc = 25 cm) conical spouted and spout–fluid beds at two different conical angles (31° and 66°) in the high particle density range (2500 kg/m3 ≤ρp ≤ 6000 kg/m3). The effects of the bed design parameters (conical angle and inlet diameter of spouting gas entrance) and operating conditions (static bed height, particle size, density, and spouting and fluidization gas flow rates) on the heat transfer characteristics were investigated in detail. The heat transfer coefficients were shown to be dependent on the density and size of the particles. The minimum stable spouting velocities of the denser and larger particles were higher, which led to higher operational spouting velocities and thereby resulted in higher heat transfer coefficients. The positive effect of increasing the particle diameter on heat transfer was more pronounced in the spout and at the spout–annulus interface, whereas this effect was diminished in the annulus region. The heat transfer coefficient increased with increasing spouting gas velocity up to 1.0Ums–1.1Ums, beyond which no significant change was observed regardless of the particle type. The heat transfer coefficient in the annulus decreased with increasing conical angle because of reduced particle circulation. The spout–fluid operation increased the heat transfer coefficient by a maximum of 10% at the expense of a significant increase of the total gas flow rate. This result was attributed to the inability of the fluidizing gas to penetrate the annulus. An empirical correlation for the average heat transfer coefficient in the annulus was also proposed based on the data obtained in this work.
