Two-stage treatment with sliding fibular flap technique for chronic infected nonunion of the tibia

Purpose:To assess the effectiveness of two-stage treatment with the fibular sliding technique in chronic infected nonunion of the tibia.Methods:The study included patients who were diagnosed with long-term chronic infected tibial nonunion following trauma and treated with the two-stage technique between January 2010 and November 2017.Patients with(1)intra-articular fractures of the distal third of the tibia and fibula,(2)pathological fracture resulting in bone loss or(3)neurological and vascular pathologies of the limbs were excluded.The operation consisted of two stages and the main goal in the first stage was to control the infection and in the second stage to control the healing of the bone.Functional&radiographic results and complications were evaluated according to Paley's criteria.Results:The patients comprised 14 males and 5 females with a mean age of 37.4 years(range,21-52 years).Patients were followed up for an average of 27 months(range,15-38 months).The microorganisms produced from these patients were Staphylococcus aureus in 13 patients,Pseudomonas aeruginosa in 4 patients and no bacteria in 2 patients.After the first stage operation,superficial skin necrosis developed in 1 patient.In another patient,there was a persistent infection,although union was achieved.For the entire patient group,union was observed at the end of 7.44 months(range,7-11 months).Based on Paley's criteria,there were 16(84.2%)patients with excellent scores,2(10.5%)good scores and 1(5.3%)fair scores radiologically;while regarding the tibial function,15(78.9%)patients had excellent scores,3(15.8%)good scores,and 1(5.3%)fair scores.No patients had poor radiological or functional score.Conclusion:Two-stage treatment can be considered as an alternative for fractures in regions that are susceptible to many and persistent complications,such as the tibia.This technique has the advantages of short operation time,minimal blood loss,no excessive tissue damage and not very technique-demanding(a short learning curve with no requirement for an

A Comparative Study of Metaheuristic Optimization Algorithms for Solving Real-World Engineering Design Problems

Real-world engineering design problems with complex objective functions under some constraints are relatively difficult problems to solve.Such design problems are widely experienced in many engineering fields,such as industry,automotive,construction,machinery,and interdisciplinary research.However,there are established optimization techniques that have shown effectiveness in addressing these types of issues.This research paper gives a comparative study of the implementation of seventeen new metaheuristic methods in order to optimize twelve distinct engineering design issues.The algorithms used in the study are listed as:transient search optimization(TSO),equilibrium optimizer(EO),grey wolf optimizer(GWO),moth-flame optimization(MFO),whale optimization algorithm(WOA),slimemould algorithm(SMA),harris hawks optimization(HHO),chimp optimization algorithm(COA),coot optimization algorithm(COOT),multi-verse optimization(MVO),arithmetic optimization algorithm(AOA),aquila optimizer(AO),sine cosine algorithm(SCA),smell agent optimization(SAO),and seagull optimization algorithm(SOA),pelican optimization algorithm(POA),and coati optimization algorithm(CA).As far as we know,there is no comparative analysis of recent and popular methods against the concrete conditions of real-world engineering problems.Hence,a remarkable research guideline is presented in the study for researchersworking in the fields of engineering and artificial intelligence,especiallywhen applying the optimization methods that have emerged recently.Future research can rely on this work for a literature search on comparisons of metaheuristic optimization methods in real-world problems under similar conditions.

Background:The field of personalized medicine has gained increasing attention in cancer care,with the aim of tailoring treatment strategies to individual patients for improved outcomes.Herbal medicine,with its long-standing historical use and extensive bioactive compounds,offers a rich source of potential treatments for various diseases,including cancer.Objective:To provide an overview of the current knowledge and evidence associated with incorporating herbal compounds into precision medicine strategies for cancer diseases.Additionally,to explore the general characteristics of the studies included in the analysis,focusing on their key features and trends.Search strategy:A comprehensive literature search was conducted from multiple online databases,including Pub Med,Scopus,Web of Science,and CINAHL-EBSCO.The search strategy was designed to identify studies related to personalized cancer medicine and herbal interventions.Inclusion criteria:Publications pertaining to cancer research conducted through in vitro,in vivo,and clinical studies,employing natural products were included in this review.Data extraction and analysis:Two review authors independently applied inclusion and inclusion criteria,data extraction,and assessments of methodological quality.The quality assessment and biases of the studies were evaluated based on modified Jadad scales.A detailed quantitative summary of the included studies is presented,providing a comprehensive description of their key features and findings.Results:A total of 121 studies were included in this review for analysis.Some of them were considered as comprehensive experimental investigations both in vitro and in vivo.The majority(n=85)of the studies included in this review were conducted in vitro,with 44 of them specifically investigating the effects of herbal medicine on animal models.Additionally,7 articles with a combined sample size of 31,271 patients,examined the impact of herbal medicine in clinical settings.Conclusion:Personalized medication can optimize the use of herbal m

Coupling coordinated development among digital economy,regional innovation,and talent employment:A case study in the Hangzhou Metropolitan Circle,China

Coordinative development across various systems,particularly the economic,social,cultural,and human resources subsystems,is a key aspect of urban sustainability and has a direct impact on the quality of urbanization.The Hangzhou Metropolitan Circle,comprising Hangzhou City,Huzhou City,Jiaxing City,and Shaoxing City,was the first metropolitan circle approved by the National Development and Reform Commission(NDRC)as a demonstration of economic transformation in China.To evaluate the coupling coordination degree of the four cities and analyze the coordinative development in three systems(including digital economy,regional innovation,and talent employment),we collected panel data during 2015–2022 from these four cities.The development level of the three systems was evaluated by the standard deviation method and comprehensive development index.The results are as follows:(1)the level of coupling coordinated development of the three systems in the Hangzhou Metropolitan Circle was relatively low;(2)the coupling coordination degree of the four cities in the Hangzhou Metropolitan Circle showed significant regional differences,among which Hangzhou City was in the leading position,and Huzhou,Jiaxing,and Shaoxing cities made steady but slow progress in the coupling development of the three systems;and(3)the development of digital economy and talent employment needs to be strengthened.This study contributes to the coordinative development of Hangzhou Metropolitan Circle by innovatively focusing on the coupling coordination relationship among digital economy,regional innovation,and talent employment,which also meets the industrial layout of Hangzhou Metropolitan Circle.In this way,the optimal allocation and sustainable development of digital economy,regional innovation,and talent employment in the Hangzhou Metropolitan Circle can be achieved.

Single nucleotide polymorphism within chromosome 8q24 is associated with prostate cancer development in Saudi Arabia

Objective: Genome-wide association studies have demonstrated that single nucleotide polymorphisms (SNPs) are important risk factors for the development of prostate cancer (PCa). Preliminary studies have suggested that the incidence of PCa in Saudi males is low but is probably familial or genetically related.Methods: To identify any possible association of SNP with PCa development in Saudi patients, we investigated a group of SNPs in Saudi PCa patients (n=85) and compared the outcomes to healthy normal controls (n=115) and nodular hyperplasia patients (n=120). DNA was extracted from paraffin-embedded formalin fixed tissue or whole blood from both patients’ groups and healthy control group. A total of thirteen SNPs were genotyped using TaqMan® minor groove binder polymerase chain reaction assay.Results: The rs16901979A, s629242T and rs1447295A alleles were found at significantly higher frequency in PCa patients than controls (p< 0.05). The rs16901979 CA genotype was found at significantly greater frequency in PCa patients than in healthy controls (43% vs. 14%, odds ratio=4.6, p=0.0001) and benign hyperplasia group (43% vs. 25%, odds ratio=2.2, p=0.009).Conclusion: Our study has highlighted the association of rs16901979 SNP with PCa in Saudi males. Such findings have important implications in the PCa diagnosis and in screening unaffected family members of Saudi patients.

A Comprehensive Analysis of the Thermo-Chemical Properties of Sudanese Biomass for Sustainable Applications

The chemical composition and thermal properties of natural fibers are the most critical variables that determine the overall properties of the fibers and influence their processing and use in different sustainable applications,such as their conversion into bioenergy and biocomposites.Their thermal and mechanical properties can be estimated by evaluating the content of cellulose,lignin,and other extractives in the fibers.In this research work,the chemical composition and thermal properties of three fibers,namely bagasse,kenaf bast fibers,and cotton stalks,were evaluated to assess their potential utilization in producing biocomposites and bioenergy materials.The chemical composition analysis followed the Technical Association of the Pulp and Paper Industry Standards(TAPPI)methods.The total phenol content was quantified using the Folin-Ciocalteu method,while Fourier Transform Infrared Spectroscopy(FTIR)was employed to assess the light absorption by the bonds.To evaluate thermal stability and higher heating values,Thermogravimetric Analysis(TGA),Differential Scanning Calorimetry(DSC),and bomb calorimetry were performed.The chemical analysis revealed that bagasse contained 50.6%cellulose and 21.6%lignin,kenaf bast fibers had 58.5%cellulose and 10%lignin,and cotton stalks exhibited 40.3%cellulose and 21.3%lignin.The FTIR curves demonstrated a notable similarity among the fibers.The TGA analysis showed degradation temperatures of 321°C for bagasse,354°C for kenaf bast fibers,and 289°C for cotton stalks.The DSC analysis revealed glass transition temperatures of 81°C for bagasse,66.3°C for cotton stalks,and 64.5°C for kenaf bast fibers.The higher heating values were measured as 17.3,16.6 and 17.1 MJ/kg for bagasse,kenaf bast fibers,and cotton stalks,respectively.The three fibers have a high potential for biocomposites and bioenergy material manufacturing.

In this editorial review,we comment on the article published in the recent issue of the World Journal of Gastrointestinal Surgery.Carcinoembryonic antigen(CEA)is a fetal glycoprotein and can be secreted in very small amounts from healthy adults after birth.CEA is widely used not only for diagnostic tumor markers but also importantly for the management of some gastrointestinal tumors.The most common clinical use is surveillance for the monitoring of colorectal carcinoma.However,CEA can become elevated in several malign or benign characterized pathologies.Serum CEA level may vary depending on the location of the lesion,whether it metastasizes or not,and its histopathological characteristics.It has been determined that cases with high preoperative CEA have a more aggressive course and the risk of metastasis to the lymph tissue and liver increases.In this editorial review,we focused on evaluating the role of CEA in clinical practice with a holistic approach,including the diagnostic and prognostic significance of CEA in patients with focal liver lesions,the role of CEA in follow-up after definitive surgery,and also hepatic resection for metastasis,and the management of all patients with raised CEA.

Evidence supporting the relationship between maternal asthma and risk for autism spectrum disorders

During pregnancy,maternal immune activation(MIA),due to infection,chronic inflammatory disorders,or toxic exposures,can result in lasting health impacts on the developing fetus.MIA has been associated with an increased risk of neurodevelopmental disorders,such as autism spectrum disorder(ASD)in the offspring.ASD is characterized by increased repetitive and stereotyped behaviors and decreased sociability.As of 2020,1 in 36 children are diagnosed with ASD by the age of 8 years,with ASD rates continuing to increase in prevalence in USA(Tamayo et al.,2023).Post-mortem brain studies,biomarker and transcriptomic studies,and epidemiology studies have provided compelling evidence of immune dysregulation in the circulation and brain of individuals diagnosed with ASD.Currently,the etiology of ASD is largely unknown,however,genetic components and environmental factors can contribute to increased susceptibility.Maternal allergic asthma(MAA),a form of MIA,has been identified as a potential risk factor for developing neurodevelopmental disorders(Patel et al.,2020).Asthma is a chronic inflammatory condition driven by a T-helper type(TH)2 immune response.

Effect of dates on blood glucose and lipid profile among patients with type 2 diabetes

Poor fruit and vegetable consumption is one of the 10 major risk factors for mortality.There is a misconception regarding the consumption of dates among patients with diabetes.This manuscript assessed the effects of date consumption on fasting and postprandial blood glucose,glycated hemoglobin,total cholesterol,triglycerides,low-density lipoproteins,high-density lipoproteins,and microbial markers.Four literature databases were searched for relevant articles.Of the 595 studies retrieved,24 assessed the effects of dates on glycemic control and lipids.Overall,the evidence suggests that dates have a lowering effect on blood glucose.Dates reduce total cholesterol and triglyceride levels and increase high-density lipoprotein levels.Dates also promote the abundance of beneficial gut microbiota.Therefore,patients with diabetes and dyslipidemia can consume dates to reduce their blood glucose,cholesterol,and triglycerides.
