AIM To determine the usefulness of assigning narrow-band imaging(NBI) scores for predicting tumor grade and invasion depth in colorectal *** A total of 161 colorectal lesions were analyzed from138 patients who underwent endoscopic or surgical resection after conventional colonoscopy and magnifying endoscopy with NBI. The relationships between the surface and vascular patterns of the lesions, as visualized with NBI, and the tumor grade and depth of submucosa(SM) invasion were determined *** were assigned to distinct features of the surface microstructures of tubular and papillary-type *** a multivariate analysis, a model was developed for predicting the tumor grade and depth of invasion based on NBI-finding *** NBI findings that correlated with a high tumor grade were associated with the "regular/irregular"(P < 0.0001)surface patterns and the "avascular area" pattern(P =0.0600). The vascular patterns of "disrupted vessels"(P = 0.0714) and "thick vessels"(P = 0.0133) but none of the surface patterns were associated with a depth of invasion of ≥ 1000 μm. In our model, a total NBIfinding score ≥ 1 was indicative of a high tumor grade(sensitivity: 0.97; specificity: 0.24), and a total NBIfinding score ≥ 9(sensitivity: 0.56; specificity: 1.0) was predictive of a SM invasion depth ≥ 1000 μm. Scores less than these cutoff values signified adenomas and a SM invasion depth < 1000 μm, respectively. 目的评估尼可地尔是否能减少危险手术进行过程中或术后心脏事件的发生。方案多中心研究。试验点日本的13家医院。患者通过心绞痛、心肌梗死病史、心脏衰竭、糖尿病和心电图异常等风险因素来评定的中危患者。给药方案在手术过程中给予这些患者尼可地尔。试验方法和主要结果术中和术后连续5天监控心脏事件,比较尼可地尔治疗组患者与非治疗组患者心脏事件的发生率。其中,尼可地尔治疗组患者84例,常规治疗组患者237例。与常规治疗相比,术中和术后给予尼可地尔治疗的患者心脏事件发生率均显著降低（术中给予尼可地尔：比值比0.15;95%置信区间,0.03～0.76;P=0.02;术后给予尼可地尔：比值比0.24;95%置信区间,0.06～0.90;P=0.04）。结论在非心脏手术术中或术后给予尼可地尔,均能降低心脏事件的发生率。

Emergence of Optochin Resistance among <i>Streptococcus pneumoniae</i>in Japan

The optochin susceptibility test is a key method for differentiating Streptococcus pneumoniae from other α-hemolytic streptococci;however, optochin-resistant (Optr) S. pneumoniae have been reported in the last two decades. In this study, we investigated the isolation frequency of Optr S. pneumoniae in the North Kyushu area of Japan, and biochemically and genetically characterized Optr S. pneumoniae clinical isolates. Seven (0.68%) out of 1032 S. pneumoniae isolates collected by the North Fukuoka Infectious Diseases Working Group were found to be Optr S. pneumoniae. Resistant strains had MICs of optochin 2- to 64- fold higher than susceptible strains, possessed different antimicrobial resistance profiles, and belonged to different serotypes. All the seven Optr isolates had mutations in the nucleotide sequence code for subunit c of F0F1 ATPase. Three isolates had mutations in codon 48 (deduced amino acid substitution of valine with phenylalanine) and two isolates had mutations in codon 49 (substitution of alanine with threonine or serine). Of the remaining two isolates, one had mutation in codon 50 (substitution of phenylalanine with leucine) and the other had mutation in codon 44 (substitution of methionine with isoleucine, which was a novel mutation in this position). From these results, we identified the mutation in the H+-ATPase subunit c gene (atpC) of S. pneumoniae, which was not recognized earlier, and determined that Opt resistance among Japanese pneumococcal isolates is not related to a specific pneumococcal serotype or antimicrobial resistance profile. Furthermore, the results indicate that when α-hemolytic streptococci resistant to optochin are isolated from patients with invasive infectious diseases, such as meningitis and pneumonia, we should perform additional examinations such as bile solubility tests or PCR assays before confirming isolates as viridans streptococci. This is the first report of the characterization of Optr S. pneumoniae in Japan.

AIM： To investigate the effcacy of effuent biomarkers for peritoneal deterioration with functional decline in peritoneal dialysis （PD）.METHODS： From January 2005 to March 2013, the subjects included 218 PD patients with end-stage renal disease at 18 centers. Matrix metalloproteinase-2 （MMP-2）, interleukin-6 （IL-6）, hyaluronan, and cancer antigen 125 （CA125） in peritoneal effluent were quantified with enzyme-linked immunosorbent assay. Peritoneal solute transport rate was assessed by peritoneal equilibration test （PET） to estimate peritoneal ***： The ratio of the effuent level of creatinine （Cr） obtained 4 h after injection （D） to that of plasma was correlated with the effluent levels of MMP-2 （ρ = 0.74, P 〈 0.001）, IL-6 （ρ = 0.46, P 〈 0.001）, and hyaluronan （ρ = 0.27, P 〈 0.001）, but not CA125 （ρ = 0.13, P = 0.051）. The area under receiver operating characteristic curve for the effluent levels of MMP-2, IL-6, and hyaluronan against high PET category were 0.90, 0.78, 0.62, and 0.51, respectively. No patient developed new-onset encapsulating peritoneal sclerosis for at least 1.5 years after peritoneal effuent ***： The effuent MMP-2 level most closely reflected peritoneal solute transport rate. MMP-2 can be a reliable indicator of peritoneal deterioration with functional decline.

AIM: To evaluate the efficacy of endoscopic submucosal dissection for superficial esophageal squamous cell neoplasms. METHODS: Between July 2007 and March 2009, 27 consecutive superficial esophageal squamous cell neoplasms in 25 enrolled patients were treated by endoscopic submucosal dissection. The therapeutic efficacy, complications, and follow-up results were assessed. RESULTS: The mean size of the lesions was 21 ± 13 mm (range 2-55 mm); the mean size of the resection specimens was 32 ± 12 mm (range 10-70 mm). The enblock resection rate was 100% (27/27), and en block resection with tumor-free lateral/basal margins was 88.9% (24/27). Perforation occurred in 1 patient who was managed by conservative medical treatments. None of the patients developed local recurrence or distant metastasis in the follow-up period. CONCLUSION: Endoscopic submucosal dissection is applicable to superficial esophageal squamous cell neoplasms with promising results.

Recently, we reported a case of gastric mucosaassociated lymphoid tissue (MALT) lymphoma presenting with unique vascular features. In the report, we defined the tree-like appearance (TLA) on the images of abnormal blood vessels which resembled branches from the trunk of a tree in the shiny mucosa, in which the glandular structure was lost. The 67-year-old female was diagnosed with gastric MALT lymphoma. The patient received eradication therapy for H. pylori. Conventional endoscopy revealed multiple ill-delineated brownish depressions in the stomach and cobblestonelike mucosa was observed at the greater curvature to the posterior wall of the upper gastric body 7 mo after successful eradication. Unsuccessful treatment of gastric MALT lymphoma was suspected on conventional endoscopy. Conventional endoscopic observations found focal depressions and cobblestone-like appearance, and these lesions were subsequently observed using magnified endoscopy combined with narrow band imaging to identify abnormal vessels presenting with a TLA within the lesions. Ten biopsies were taken from the area where abnormal vessels were present within these lesions. Ten biopsies were also taken from the lesions without abnormal vessels as a control. A total of 20 biopsy samples were evaluated to determine whether the diagnosis of MALT lymphoma could be obtained histologically from each sample. A positive diagnosis was obtained in 8/10 TLA (+) sites and in 2/10 TLA(-) sites. Target biopsies of the site with abnormal blood vessels can potentially improve diagnostic accuracy of gastric MALT lymphoma.
