Novel technologies have been necessary for improving fruit quality and productivity of citrus,labor-saving and orchard conservation on steep slope lands since aging of growers and decrease in the number of successors is remarkable in mountain areas of southwestern *** purpose of this paper is to introduce new technologies for improving citrus production that have been developed in recent years.A new fruit quality control system using drip irrigation and liquid fertilization technique combined with year-round plastic mulching was developed,and it enables high quality and stable citrus fruit *** and/or nutrient solution is automatically supplied through drip tubes that are laid under the mulching sheets to give adequate water stress,so as to improve sugar and acid content of fruit.A new transportation system for steep sloping citrus orchards,which is a combination of the monorail system and contour narrow paths,was suggested.A small walking cultivator was developed to explain the procedure of narrow path *** introducing the narrow path,working hours for fertilizer and chemical herbicide application were *** prevention mapping of citrus orchards on slope lands was developed based on computer-aided seepage estimation and topographic *** mapping can show zones of both ascending flow and descending flow of underground water during heavy rains in citrus *** mapping is considered to be effective for the management of orchards and prevention of erosion on slope lands.

Effect of Foot Self-Care Behavior According to Severity of Diabetic Neuropathy in Japanese Patients with Diabetes

PURPOSE: Diabetic patients with severe diabetic neuropathy often have hypoesthesia and numbness. This study evaluated foot self-care behavior according to severity of diabetic neuropathy. METHODS: We used a hand-held nerve conduction test device DPN check (HDN-1000, Omron) to evaluate severity of diabetic neuropathy. Foot self-care was evaluated using the Japanese SDSCA (the Summary of Diabetes Self-Care Activities measure). Foot self-care comprised visual inspection, washing, wiping interdigital areas, and checking shoes, and was scored according to the number of days self-care which was performed in the previous week. RESULTS: The study evaluated 103 diabetic patients (age: 65.7 years, diabetes duration: 13.9 years, HbA1c: 7.3%). Total scores (out of 28 points) for self-care behavior according to neuropathy severity were 11.8 (Normal: n = 54), 10.6 (Mild: n = 27), 13.3 (Moderate: n = 17), and 10.8 (Severe: n = 5). Foot self-care scores were low in all groups, with particularly low scores in those with severe neuropathy. However, no statistically significant differences were observed. CONCLUSIONS: Foot self-care is essential in diabetes because lesions are more likely to occur in severe neuropathy. Living alone and the presence of recurrent foot lesions are associated with poor survival prognosis. Accordingly, foot-care intervention must take neuropathy severity and lifestyle factors into account. The severity of diabetic neuropathy must be determined and foot-care intervention should take lifestyle factors into account.

The term 'Satoyama' refers to traditional and unique secondary forests in Japan that occupy intermediate zones between villages ('sato') and hills or mountains ('yama'). Satoyama landscapes help sustain ecosystem services and the diversity of secondary natural environments. As Japan relies more heavily on foreign timber imports, the traditional role of Satoyama in providing forest products has diminished, and this has led to their abandonment and poor management. The chemical behavior of cations, anions, and dissolved organic matter in throughfall and stemflow from one such threatened Satoyama system in central Japan was investigated. From autumn to winter, the atmospheric deposition of sulfates and nitrates was 2.5–6.0 times higher compared to the amounts in summer due to the intrusion of air masses from the Asian continent. The dissolved organic matter in the throughfall and stemflow was composed mainly of humic substances and protein derivatives. The deposition fluxes of dissolved organic carbon from throughfall (7.31–10.1 g m^(−2) a^(−1)) and stemflow (1.79–3.84 g m^(−2) a^(−1)) in this study were within ranges seen in temperate forests in previous studies. The deposition flux of sulfates was low compared to that in other forest types because canopy interaction was lower, suggesting higher canopy openness than in primary forests. If a shift from a mixed species Satoyama forest to a conifer-dominated forest occurs after the mass mortality of oak, the deposition flux of dissolved organic carbon and K^(+) might decrease by 33% and 62%, respectively, while NO_(3)^(−) might increase by 20%. In the near future, the degradation of Satoyama landscapes might change the levels of dissolved organic carbon and nitrogen loads, resulting in imbalances in river-ocean linkages affecting forested catchments and aquatic ecosystems in Japan.

Early and Late Postoperative Pain and Side Effects after Mastectomy: A Comparison of Ketamine and Thiamylal Administered for Anesthetic Induction

Objective: To compare acute and long-term postoperative pain and side effects in patients undergoing mastectomy for breast cancer under general anesthesia induced with ketamine or thiamylal. Methods: Twenty four ASA physical status I-III patients undergoing mastectomy were randomly assigned to one of two groups. Ketamine group received intravenous ketamine, 1 mg/kg, and thiamylal group received intravenous thiamylal, 4 mg/kg, at the induction of general anesthesia. Anesthesia was maintained with sevoflurane, N2O and fentanyl. The intensity of pain was assessed by using visual analog scale (VAS) 3 and 16 hr and 2, 3 and 4 weeks after surgery. Postoperative side effects, including nausea, vomiting and hallucination were also recorded. Results: At 16 hr after surgery, VAS in ketamine group was significantly lower than that in thiamylal group. However, there were no statistically significant differences between the two groups in the VAS at 3 hr and 2, 3 and 4 weeks after surgery. There were no differences in the incidence of side effects such as nausea, vomiting and hallucination between the two groups. Conclusion: Intravenous ketamine at the induction of anesthesia could reduce acute postoperative pain but not long-term pain after mastectomy.

Although some studies had suggested a bidirectional relationship between diabetes and periodontal disease, there were no appropriate tools for nurses to evaluate oral status and oral health behaviors in patients with diabetes. Therefore, the Diabetes Oral Health Assessment Tool (DiO-HAT ?) was developed with items contributed by health care professionals (diabetologists, periodontal specialists, a preventive dentist, a Certified Nurse in Diabetes Nursing, a national registered dietitian, registered nurses, a dental hygienist, and nursing researchers) who were involved in the medical care of patients with diabetes. Subsequently, a survey of 700 Diabetes Nurse Specialists (DNS) was conducted to determine their score of recognition and implementation of the DiOHAT ?, however, 304 participants (43.4%) responded. Constructive concept validation and the Cronbach’s alpha coefficient for all assessment items were 0.931, indicating high reliability: Factor 1, Patient’s oral health status (α = 0.831);Factor 2, Implementation of oral health behaviors (α = 0.890);Factor 3, Information transmission regarding dental visits (α = 0.862);and Factor 4, Perceptions and knowledge of oral health behaviors (α = 0.793). Although the mean score of recognition of DiOHAT ? was 3.5 ± 0.4 points, the mean value of the implementation score was 1.5 ± 0.5 points (obtained using a 4-grade scale). The implementation scores were significantly lower than the recognition scores for all items (p < 0.001). The findings suggested that the DNS were not inclined to implement all items of DiOHAT ?, despite recognizing their importance.

Background: Diabetic foot ulcers with exposed bones commonly result in amputation. Objectives: To determine whether exposure of bone marrow cells and subsequent grafting of epi dermal sheets accelerates healing and reduces the need for amputation. Methods: Thirty-eight patients with chronic wounds caused by diabetes mellitus were enrolled in this study. Epidermal sheets obtained from suction blisters of each patient were grafted on to diabetic foot ulcers without exposed bones (n=10) and were compared with the standard treatment of local wound care, debridement with a scalpel when indicated, bed rest and parenteral antibiotics (n=8). In another group of patients, diabetic wounds with exposed bones were treated either with the standard procedure (n=9) or with a newly developed experimental procedure(n=11). In that newprocedure, the affected bone was initially exposed by debridementwith a scalpel, followed by partial excision with a bone scraper until fresh bleeding was observed from the exposed bone. The lesions were then immediately covered with an occlusive dressing, and finally the wound was covered with an epidermal graft of skin harvested from suction blisters. Patients in each group were matchedwith their counterparts by age, sex, wound size, wound infection and wound duration, to compare the time needed for total skin repair and rates of amputation. Results: Epidermal grafting significantly accelerated the healing of diabetic foot ulcers (P=0.042) without exposed bones, with site-specific differentiation. The newly developed combination therapy resulted in the healing of all diabetic ulcers with exposed bones without the occurrence of osteomyelitis or the necessity for amputation (P < 0.0001). Conclusions: Our study indicates that early aggressive debridement of diabetic foot ulcers with exposed bones down to a bleeding vascularized base and then grafting epidermal sheets significantly improves healing and reduces the rate of amputation.
