Histological evaluation for chemotherapeutic responses of metastatic lymph nodes in gastric cancer

AIM: To investigate the effect of preoperative chemotherapy(pre-CTx) for metastatic lymph nodes(MLNs) of gastric cancer(GC). METHODS: A retrospective cohort of patients with advanced GC, who underwent pre-CTx followed by gastrectomy, was reviewed. The histological tumor regression grade(TRG), which considered the percentage of residual cancer in the visible tumor bed, was applied to primary tumors and individual MLNs: G1a(complete response), G1b( 50%). The clinical response to pre-CTx was retrospectively evaluated using only MLNs information, and we compared the histological and clinical evaluations of ***: Twenty-eight patients were enrolled. A total of 438 MLNs were retrieved, and 22(5%), 48(11%), 63(14%) and 305(70%) LNs were assigned as G1 a, G1 b, G2 and G3, respectively. Stratification of the residual MLNs based on the TRGs was as follows: 28 G1 b MLNs(9%), 48 G2 MLNs(15%), and 253 G3 MLNs(76%) in the D1 region; 20(23%), 15(17%), and 52(60%) in the D2 region, respectively. However, no significant correlation was found between TRGs in MLNs and clinical response in the subgroup for which evaluation of clinical response was ***: Pre-CTx does not provide any outstanding histological benefit for MLNs, and an appropriate D2 lymphadenectomy should routinely be performed to offer the chance of curative resection.

Laparoscopic transhiatal approach for resection of an adenocarcinoma in long-segment Barrett's esophagus

Barrett's esophagus(BE) is a precursor of esophageal adenocarcinoma and is associated with gastroesophageal reflux disease, which is often preceded by a hiatal hernia. We describe a case of esophageal adenocarcinoma arising in long-segment BE(LSBE) associated with a hiatal hernia that was successfully treated with a laparoscopic transhiatal approach(LTHA) without thoracotomy. The patient was a 42-year-old male who had previously undergone laryngectomy and tracheal separation to avoid repeated aspiration pneumonitis. An ulcerative lesion was found in a hiatal hernia by endoscopy and superficial esophageal cancer was also detected in the lower thoracic esophagus. The histopathological diagnosis of biopsy samples from both lesions was adenocarcinoma. There were difficulties with the thoracic approach because the patient had severe kyphosis and muscular contractures from cerebral palsy. Therefore, we performed subtotal esophagectomy by LTHA without thoracotomy. Using hand-assisted laparoscopic surgery, the esophageal hiatus was divided and carbon dioxide was introduced into the mediastinum. A hernial sac was identified on the cranial side of the right crus of the diaphragm and carefully separated from the surrounding tissues. Abruption of the thoracic esophagus was performed up to the level of thearch of the azygos vein via LTHA. A cervical incision was made in the left side of the permanent tracheal stoma, the cervical esophagus was divided, and gastric tube reconstruction was performed via a posterior mediastinal route. The operative time was 175 min, and there was 61 m L of intra-operative bleeding. A histopathological examination revealed superficial adenocarcinoma in LSBE. Our surgical procedure provided a good surgical view and can be safely applied to patients with a hiatal hernia and kyphosis.

Epigenetic Programming： The Challenge to Species Hybridization

In many organisms, the genomes of individual species are isolated by a range of reproductive barriers that act before or after fertilization. Successful mating between species results in the presence of different genomes within a cell （hybridization）, which can lead to incompatibility in cellular events due to adverse genetic interactions. In addition to such genetic interactions, recent studies have shown that the epigenetic control of the genome, silencing of transposons, control of non-additive gene expression and genomic imprinting might also contribute to reproductive barriers in plant and animal species. These genetic and epigenetic mechanisms play a significant role in the prevention of gene flow between species. In this review, we focus on aspects of epigenetic control related to hybrid incompatibility during species hybrid- ization, and also consider key mechanism（s） in the interaction between different genomes.

In-silico clearing approach for deep refractive index tomography by partial reconstruction and wave-backpropagation

Refractive index(RI)is considered to be a fundamental physical and biophysical parameter in biological imaging,as it governs light-matter interactions and light propagation while reflecting cellular *** tomography enables volumetric visualization of RI distribution,allowing biologically relevant analysis of a ***,multiple scattering(MS)and sample-induced aberration(SIA)caused by the inhomogeneity in RI distribution of a thick sample make its visualization *** paper proposes a deep RI tomographic approach to overcome MS and SIA and allow the enhanced reconstruction of thick samples compared to that enabled by conventional linear-model-based RI *** proposed approach consists of partial RI reconstruction using multiple holograms acquired with angular diversity and their backpropagation using the reconstructed partial RI map,which unambiguously reconstructs the next partial *** this operation efficiently reconstructs the entire RI tomogram while suppressing MS and *** visualized a multicellular spheroid of diameter 140μm within minutes of reconstruction,thereby demonstrating the enhanced deep visualization capability and computational efficiency of the proposed method compared to those of conventional RI ***,we quantified the high-RI structures and morphological changes inside multicellular spheroids,indicating that the proposed method can retrieve biologically relevant information from the RI *** from the excellent biological interpretability of RI distributions,the label-free deep visualization capability of the proposed method facilitates a noninvasive understanding of the architecture and time-course morphological changes of thick multicellular specimens.

Effects of estradiol-17β and bisphenol A administered chronically to mice throughout pregnancy and lactation on the male pups＇ reproductive system

Aim： To assess the effect of estradiol-17β （E2） and bisphenol A （BPA） administered chronically by implanting a silicone tube throughout pregnancy and lactation on male pups＇ reproductive system in ICR mice. Methods： Female mice were implanted with a tube filled with 10 ng, 500 ng, 1 μg, or 10 μg of E2, or 100 μg or 5 mg of BPA, before mating. The tube was kept in the mice throughout pregnancy and lactation, until the pups had weaned at 4 weeks of age. During the period, E2 was released from the tube at 120 pg or 6, 12 or 120 ng/day, and BPA at 1.2 or 60 μg/day. Results： Most of the mice given 1 μg and 10 lag of E2 did not maintain their pregnancy. However, the other groups showed high rates of birth, more than 70%. At age of 4 weeks, the male pups were killed. Body weight and reproductive organ weights （testes, epididymides and accessory reproductive glands） in the treated groups did not differ from the control values, whereas the percentage of seminiferous tubules in the testis with mature spermatids was significantly lower in the groups given 10 ng and 500 ng of E2 and 5 mg of BPA than that in the control. Conclusion： Chronic exposure to E2 and BPA might disrupt spermatogenesis in male pups. （Asian JAndrol 2008 Mar; 10： 271-276）

Expulsion of geopressured hydrothermal system along active faults and its relation to the occurrence of earthquakes in the Shinanogawa seismic belt, Japan

The 1995 Northern Niigata Earthquake (M 6.0) occurred at a shallow depth in the Niigata seismic gap. The anomaly areas in temperature, electrical conductivity and Cl- concentration of groundwater trend northeast as linear distribution in the epicentral area and are approximately coincident with the area of the seismic intensity 6 (JMA scale). The distributions of seismic intensity 6 and groundwater anomalies convincingly imaged the presence of a buried active fault beneath the epicentral area. The occurrence of this earthquake and the anomalies of groundwater were related to the expulsion of geopressured hydrothermal system (GHS). All epicenters of the destructive earthquakes along the Shinanogawa seismic belt are actually located in the buried active fault zones characterized by the areas of temperature and geochemical anomalies of groundwater. These earthquakes might have been triggered by the activity of GHS. The expulsion of GHS along an active fault in combination with the thermal softening of fault zone attributed to high rock temperature may reduce fracture strength of the rock, and trigger earthquake occurrence. The dimension of the anomaly area of groundwater temperature at the epicentral area reflected the scale of the earthquake fault. The linear anomaly areas of groundwater resulting from the expulsion of GHS and having no historical destructive earthquake are of the predicated areas of future destructive earthquakes. Monitoring of groundwater conditions in these areas may provide useful information regarding the future occurrence of earthquakes.
