Modified malleable prosthesis with a fixed mesh:case series for the Ghattas technique

Penile prosthetic implantation represents a cornerstone for patients with organic erectile dysfunction(ED)that is refractory,unsatisfactory,or contra-indicated for other approved medical or mechanical *** this study,we introduce the“Ghattas technique,”wherein we constructed a polypropylene mesh sheath that surrounds and is fixed to a 13-mm malleable prosthesis cylinder,which can increase the cylinder diameter for cases that need a larger *** patients underwent preoperative evaluation and completed the five-item International Index of Erectile Function questionnaire(IIEF-5).Postoperative outcomes were evaluated by IIEF-5 and Erectile Dysfunction Inventory of Treatment Satisfaction(EDITS)questionnaires at final *** mean age of the 23 included patients was 57.9(standard deviation[s.d.]:11.4)years and the mean duration of ED was 8.5(s.d.:7.9)*** improvement was determined by comparing mean preoperative and postoperative IIEF-5 scores(8.3[s.d.:3.9]vs 24.6[s.d.:0.6],P<0.001).High treatment satisfaction was determined according to a mean EDITS score of 94.9(s.d.:9.9).The proposed Ghattas technique was safe and effective in our patients,and provides opportunity for cases that need a diameter larger than 13 *** studies are needed to confirm these results.

Background:Adult patients with congenital heart disease(ACHD)might be at high risk of Coronavirus disease-2019(COVID-19).This study aimed to report on a one-year tertiary center experience regards COVID-19 infection in ACHD ***:This is a one-year(March-2020 to March-2021)tertiary-center retrospective study that enrolled all ACHD patients;COVID-19 positive patients’medical records,and management were ***:We recorded 542 patients,205(37.8%)COVID-19-positive,and 337(62.2%)COVID-19-negative *** single ventricle and Eisenmenger syndrome patients were more vulnerable to COVID-19 infection(P<0.05*).Cardiovascular COVID-19 complications were arrhythmias in 47(22.9%)patients,heart failure in 39(19.0%)patients,cyanosis in 12(5.9%)patients,stroke/TIA in 5(2.4%)patients,hypertension and infective endocarditis in 2(1.0%)patients for each,pulmonary hypertension and pulmonary embolism in 1(0.5%)patient for each.11(5.4%)patients were managed with home isolation,147(71.7%)patients required antibiotics,32(15.6%)patients required intensive care unit(ICU),8(3.9%)patients required inotropes,7(3.4%)patients required mechanical ventilation,and 2(1.0%)patients required extracorporeal membrane oxygenation(ECMO).Thromboprophylaxis was given to all 46(22.4%)hospitalized *** College of Cardiology/American Heart Association classification revealed that complex lesions,and FC-C/D categories were more likely to develop severe/critical symptoms,that required mechanical ventilation and ECMO(P<0.05*).Mortality was reported in 3(0.6%)patients with no difference between groups(P=0.872).193(35.6%)patients were ***:COVID-19 infection in ACHD patients require individualized risk stratification and *** syndrome,single ventricle palliation,complex lesions,and FC-C/D patients were more vulnerable to severe/critical symptoms that required ICU admission,mechanical ventilation,and *** vaccine was mostly tolerable.

In the present study, 7 day postnatal C57/BL6 wild-type mice （hyperoxia group） and 7 day postnatal N-methyI-D-aspartate receptor subtype 3A knockout mice （NR3A KO group） were exposed to 75% oxygen and 15% nitrogen in a closed container for 5 days. Wild-type mice raised in normoxia served as controls. TdT-mediated dUTP nick end labeling （TUNEL）/neuron-specific nuclear protein （NeuN） and 5-bromo-2＇-deoxyuridine （BrdU）/NeuN immunofluorescence staining showed that the number of apoptotic cells and the number of proliferative cells in the dentate subgranular zone significantly increased in the hyperoxia group compared with the control group. However, in the same hyperoxia environment, the number of apoptotic cells and the number of proliferative cells significantly decreased in the NR3A KO group compared with hyperoxia group. TUNEL＋/NeuN＋ and BrdU＋/NeuN~ cells were observed in the NR3A KO and the hyperoxia groups. These results demonstrated that the NR3A gene can promote cell apoptosis and mediate the potential damage in the developing brain induced by exposure to non-physiologically high concentrations of oxygen.

The significance of clinical practice guidelines on adult varicocele detection and management

Varicoceles are the most common correctable etiology of male factor infertility. However, the detection and management of varicoceles have not been standardized. This has led to decades of debate regarding the effect of varicocele on male infertility and subsequently whether repair leads to an improved fertility status. The current body of evidence investigating the role of varicocele and varicocelectomy is weak and conflicting. The stance taken by the AUA and ASRM suggests that there is insufficient outcomes data to support evidenced-based guidelines, citing evidence used to provide current recommendations are generally of a low quality level. On the other hand, the EAU Guidelines give a level la of evidence for management of varicoceles that are clinically palpable, associated with subnormal semen analyses and having otherwise unexplained fertility. Besides aiding with clinical varicocele detection and management, clinical practice opinion statements and guidelines aim to direct and strengthen the infrastructure of future studies. We review the current status of opinion statements and guidelines in varicocele and management detection with focus on their application in practice.

AIM: To study the effects of ferulic acid on gastrointe-stinal motility both in vitro and in ***: Ferulic acid induced concentration-dependent stimulation of the basal tone of isolated guinea pig ileum (2-20 μmol/L) and isolated rat fundus (0.05-0.4 mmol/L). RESULTS: Ferulic acid significantly accelerated the gastrointestinal transit and gastric emptying in rats in a dose-dependent manner (50-200 mg/kg, po). Cisplatin (2.5-20 mg/kg, ip) induced a dose-dependent delay in gastric emptying in rats. Pretreatment with ferulic acid dose-dependently, significantly reversed the cisplatin-induced delay in gastric emptying. CONCLUSION: The endogenous prostaglandins (PGs) are involved in mediating the stimulant effects of ferulic acid. This effect of dietary ferulic acid may help improve other accompanying gastrointestinal symptoms such as abdominal discomfort and also may protect against emesis induced by cytotoxic drugs.

Internet traffic encryption is a very common traffic protection *** internet traffic is protected by the encryption protocol called transport layersecurity (TLS). Although traffic encryption can ensure the security of communication, it also enables malware to hide its information and avoid being *** present, most of the malicious traffic detection methods are aimed at the unencrypted ones. There are some problems in the detection of encrypted traffic, suchas high false positive rate, difficulty in feature extraction, and insufficient practicability. The accuracy and effectiveness of existing methods need to be *** this paper, we present TLSmell, a framework that conducts maliciousencrypted HTTPs traffic detection with simple connection-specific indicators byusing different classifiers based online training. We perform deep packet analysisof encrypted traffic through data pre-processing to extract effective features, andthen the online training algorithm is used for training and prediction. Withoutdecrypting the original traffic, high-precision malicious traffic detection and analysis are realized, which can guarantee user privacy and communication *** the same time, since there is no need to decrypt the traffic in advance, the effi-ciency of detecting malicious HTTPs traffic will be greatly improved. Combinedwith the traditional detection and analysis methods, malicious HTTPs traffic isscreened, and suspicious traffic is further analyzed by the expert through the context of suspicious behaviors, thereby improving the overall performance of malicious encrypted traffic detection.

Nowadays, trends in wastewater treatment by zero-valent iron（ZVI） were turned to use bimetallic NZVI particles by planting another metal onto the ZVI surface to increase its reactivity. Nano size zero-valent iron/copper（NZVI/Cu0） bimetallic particles were synthesized in order to examine its toxicity effects on the wastewater microbial life, kinetics of phosphorus, ammonia stripping and the reduction of chemical oxygen demand（COD）.Various concentrations of NZVI/Cu0 and operation conditions both aerobic and anaerobic were investigated and compared with pure NZVI experiment. The results showed that addition10 mg/L of NZVI/Cu0 significantly increased the numbers of bacteria colonies under anaerobic condition, conversely it inhibited bacteria activity with the presence of oxygen. Furthermore,the impact of nanoparticles on ammonia stripping and phosphorus removal was also linked to the emitted iron ions electrons. It was found that dosing high concentration of bimetallic NZVI/Cu0 has a negative effect on ammonia stripping regardless of the aeration condition. In comparison to control, dosing only 10 mg/L NZVI/Cu0, the phosphorus removal increased sharply both under aerobic and anaerobic conditions, these outcomes were obtained as a result of complete dissolution of bimetallic nanoparticles which formed copper-iron oxides components that are attributed to increasing the phosphorus adsorption rate.

LogUAD: Log Unsupervised Anomaly Detection Based on Word2Vec

System logs record detailed information about system operation and areimportant for analyzing the system's operational status and performance. Rapidand accurate detection of system anomalies is of great significance to ensure system stability. However, large-scale distributed systems are becoming more andmore complex, and the number of system logs gradually increases, which bringschallenges to analyze system logs. Some recent studies show that logs can beunstable due to the evolution of log statements and noise introduced by log collection and parsing. Moreover, deep learning-based detection methods take a longtime to train models. Therefore, to reduce the computational cost and avoid loginstability we propose a new Word2Vec-based log unsupervised anomaly detection method (LogUAD). LogUAD does not require a log parsing step and takesoriginal log messages as input to avoid the noise. LogUAD uses Word2Vec togenerate word vectors and generates weighted log sequence feature vectors withTF-IDF to handle the evolution of log statements. At last, a computationally effi-cient unsupervised clustering is exploited to detect the anomaly. We conductedextensive experiments on the public dataset from Blue Gene/L (BGL). Experimental results show that the F1-score of LogUAD can be improved by 67.25%compared to LogCluster.

Image captioning aims to generate a corresponding description of an *** recent years,neural encoder-decodermodels have been the dominant approaches,in which the Convolutional Neural Network(CNN)and Long Short TermMemory(LSTM)are used to translate an image into a natural language *** these approaches,the visual attention mechanisms are widely used to enable deeper image understanding through fine-grained analysis and even multiple steps of ***,most conventional visual attention mechanisms are based on high-level image features,ignoring the effects of other image features,and giving insufficient consideration to the relative positions between image *** this work,we propose a Position-Aware Transformer model with image-feature attention and position-aware attention mechanisms for the above *** image-feature attention firstly extracts multi-level features by using Feature Pyramid Network(FPN),then utilizes the scaled-dot-product to fuse these features,which enables our model to detect objects of different scales in the image more effectivelywithout increasing *** the position-aware attentionmechanism,the relative positions between image features are obtained at first,afterwards the relative positions are incorporated into the original image features to generate captions more *** are carried out on the MSCOCO dataset and our approach achieves competitive BLEU-4,METEOR,ROUGE-L,CIDEr scores compared with some state-of-the-art approaches,demonstrating the effectiveness of our approach.
