BACKGROUND In 2016,the Food and Drug Administration approved the first hybrid closed-loop(HCL)insulin delivery system for adults with type 1 diabetes(T1D).There is limited information on the impact of using HCL systems on patient-reported outcomes(PROs)in patients with T1D in real-world clinical *** this independent study,we evaluated glycemic parameters and PROs over one year of continuous use of Medtronic’s 670G HCL in real-world clinical *** To assess the effects of hybrid closed loop system on glycemic control and quality of life in adults with *** We evaluated 71 patients with T1D(mean age:45.5±12.1 years;59%females;body weight:83.8±18.7 kg,body mass index:28.7±5.6 kg/m2,A1C:7.6%±0.8%)who were treated with HCL at Joslin Clinic from 2017 to *** measured A1C and percent of glucose time-in-range(%TIR)at baseline and 12 *** measured percent time in auto mode(%TiAM)for the last two weeks preceding the final visit and assessed PROs through several validated quality-of-life surveys related to general health and diabetes *** At 12 mo,A1C decreased by 0.3%±0.1%(P=0.001)and%TIR increased by 8.1%±2.5%(P=0.002).The average%TiAM was only 64.3%±32.8%and was not associated with A1C,%TIR or ***,provided at baseline and at the end of the study,showed that the physical functioning submodule of 36Item Short-Form Health Survey increased significantly by 22.9%(P<0.001).Hypoglycemia fear survey/worry scale decreased significantly by 24.9%(P<0.000);Problem Areas In Diabetes reduced significantly by-17.2%(P=0.002).The emotional burden submodules of dietary diversity score reduced significantly by-44.7%(P=0.001).Furthermore,analysis of Clarke questionnaire showed no increase in awareness of hypoglycemic ***-5 showed no improvements in subject’s wellbeing among participants after starting the 670G HCL ***,analysis of Pittsburgh Sleep Quality Index showed no difference in sleep quality,sleep latency,or duration o

Experimental and Numerical Study of Dilute Gas-Solid Flow inside a 90°Horizontal Square Pipe Bend

A pneumatic test rig is built to test a curved 90° square bend in an open-circuit horizontal-to-horizontal suction wind tunnel system. Sand particles are used to represent the solid phase with a wide range of particle diameters. Velocity profiles are constructed by measuring the gas velocity using a 3-hole probe. Flow patterns inside the bend duct are introduced using sparks caused by burning sticks of incense with the air flow inside the piping system for flow visualization purpose. Numerical calculations are performed by Lagrangian-particle tracking model for predicting particle trajectories of dispersed phase, and standard k-ε model for predicting the turbulent gas-solid flows in bends. Comparisons made between the theoretical results and experimental data for the velocity vectors and particle trajectories show good agreement.

Behavior and Flexural Prediction of Special Cementitious Bonding Material for Fiber Reinforced Strengthening Systems

Strengthening of RC structures with externally bonded FRP （fiber reinforced polymers） has become an important challenge in civil engineering. Epoxy is the main bonding agent used so far, but in the case of a fire, it is subjected to complete loss of his bonding capabilities. Mineral based composites strengthening systems consist of FRPs and a cementitious bonding agent which form a repair or strengthening system that is more compatible with the concrete substrata, and roved its efficiency. The current research introduces the use of a special cementitious material ＂Grancrete＂ as a bonding agent. Test results of 32 T-section RC beams strengthened with various FRG （fiber reinforced Grancrete） strengthening systems are presented. The results demonstrated that most of the specimens were likely to fail by debonding of the FRP from the concrete either at the ends or at intermediate flexural cracks. This paper presents an in-depth study aimed at the development of a better understanding of debonding failures in RC beams strengthened with externally bonded FRP systems. Different analytical models, published in the literature for plate end debonding, are reviewed and compared to test results. The results also demonstrated that when using U-wraps, the specimens were likely to fail by FRP sheet rupture.

本文是一项正在研究项目的部分内容,该项研究主要目的是:探究中等高度的配筋混凝土砌块砌体结构作为主承重墙时,建筑整体结构的力学响应。本文以多层的单片墙构件为模拟对象,建立数值模型、并对其进行校准和修正,以模拟分析该类构件在侧向荷载作用下的力学行为。因为一个建筑模型中所包含墙片数目必然是复数,故本研究项目组认为:墙片的力学模型公式应当尽量简化,并且在应用该公式进行分析时,要在能够保证精度前提下尽可能地提高计算效率。配筋砌体墙片由多种不同材料组合而成,致使该类构件在承受往复荷载作用时呈现的力学响应,具有明显的非线性特征。本文使用Open Sees和Response-2000软件建立墙片模型,以模拟分析其在往复荷载作用下的挠曲变形、剪切变形,以及刚度变化。使用Open Sees提供的"二维基于强度框架单元"(英文原文:2D Force-Based frame element)来模拟墙片;在此基础上,对共计十片配筋砌体结构墙片试件进行拟静力往复荷载试验,用试验结果对数值模型进行校准和修正。本文主要叙述了数值模型的建立和验证工作,包托两个主要阶段:第一阶段,对墙体模型施加单一均速递增的侧向荷载,直至其进入弹塑性阶段,以获取滞回骨架曲线,通过该方法能够获取具有较高精度的曲线;第二阶段,再对第一阶段中的模型进行修正,以进一步完善滞回曲线的绘制。最终可掌握该类构件内部变化的规律,得到力学计算模型中有关参数的合理取值范围。结合Open Sees软件模型,可以对配筋砌体墙片在弯剪组合作用下的力学行为进行模拟。

Tissue Doppler,speckling tracking and four-dimensional echocardiographic assessment of right ventricular function in children with dilated cardiomyopathy

BACKGROUND Right ventricular(RV)function is frequently overlooked during dilated cardiomyopathy(DCM)*** To evaluate RV function in children with idiopathic DCM using relatively recent echocardiographic *** We prospectively studied the cardiac function in 50 children with idiopathic DCM and 50 healthy children as a control group,using four-dimensional echocardiography(4-DE),Tissue Doppler Imaging(TDI),and two-dimensional-speckles tracking echocardiography(2-D-STE).RV EF was measured by *** The auto left(LV)ejection fractions(EF)measured by 2-D-STE were significantly lower in the patients'group than in the *** sphericity index was also significantly lower in children with DCM than in the *** EF measured by 4-DE was significantly lower in the patient's group than the *** S wave,e´/a'ratio,myocardial performance index(MPI),and tricuspid annular plane systolic excursion(TAPSE)were significantly impaired in children with DCM than in *** LV and RV global longitudinal strains(GLS)were significantly reduced in children with DCM than in *** was significantly associated with the duration since diagnosis,tricuspid annulus S wave,RV MPI,and TAPSE,but not with the age of the patients,RV EF,or e´/a'*** There was impairment of the RV LGS and other systolic and diastolic parameters in children with *** and TDI can help to detect the early decline of RV function.

Latissimus Dorsi Mini-Flap as a Volume Replacement Technique after Partial Mastectomy for Breast Cancer in the Upper and Central Breast Quadrants: A Single Center Experience

Background: The latissimus dorsi (LD) muscle flap plays an essential role in breast reconstruction after partial mastectomy for cancer because of its stability and versatility. We evaluated both oncologic and aesthetic outcomes in addition to the related complications of this flap as an adjunct to breast conserving surgery in the management of breast cancer patients. Methods: All patients underwent a one-stage procedure with immediate reconstruction through two-steps operation;wider local excision utilizing oncoplastic principles and mini flap harvest & volume replacement. Results: The study included 34 cases with early breast cancer;30 patients had partial breast resection and defect refilling by LD mini-flap, three patients underwent mastectomy and one patient underwent extended LDF. The mean defect volume was (212.63 cm3 ± 59.57) cm3, while the mean flap volume was (218.27 cm3 ± 53.64 cm3). Patient self-evaluation of the cosmetic outcome was excellent in 20%, good in 60% and satisfactory in 20% of patients. Panel evaluation according to Harvard scale showed excellent in 36.7%, good in 36.7%, fair in 26.7% of patients. The median hospital stay was 4 days. The postoperative complications included wound gap in 4 patients (13.3%), postoperative donor site seroma in 16 patients (53.3%). No flap loss or necrosis, no affection on arm or shoulder mobility occurred. Lastly, no tumor recurrence till now. Conclusion: Latissimus dorsi mini-flap can achieve adequate cosmetic and oncologic outcomes with a low incidence of complications in patients with early stage (I/II) breast cancer and small to medium sized breasts.

Small for size syndrome difficult dilemma: Lessons from 10 years single centre experience in living donor liver transplantation

AIM To analyze the incidence, risk factors, prevention, treatment and outcome of small for size syndrome(SFSS) after living donor liver transplantation(LDLT). METHODS Through-out more than 10 years: During the period from April 2003 to the end of 2013, 174 adult-to-adults LDLT(A-ALDLT) had been performed at National Liver Institute, Menoufiya University, Shibin Elkoom, Egypt. We collected the data of those patients to do this cohort study that is a single-institution retrospective analysis of a prospectively collected database analyzing the incidence, risk factors, prevention, treatment and outcome of SFSS in a period started from the end of 2013 to the end of 2015. The median period of follow-up reached 40.50 m, range(0-144 m). RESULTS SFSS was diagnosed in 20(11.5%) of our recipients. While extra-small graft [small for size graft(SFSG)], portal hypertension, steatosis and left lobe graft were significant predictors of SFSS in univariate analysis(P = 0.00, 0.04, 0.03, and 0.00 respectively); graft size was the only independent predictor of SFSS on multivariate analysis(P = 0.03). On the other hand, there was lower incidence of SFSS in patients with SFSG who underwent splenectomy [4/10(40%) SFSS vs 3/7(42.9%) no SFSS] but without statistical significance, However, there was none significant lower incidence of the syndrome in patients with right lobe(RL) graft when drainage of the right anterior and/or posterior liver sectors by middle hepatic vein, V5, V8, and/or right inferior vein was done [4/10(28.6%) SFSS vs 52/152(34.2%) no SFSS]. The 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients with SFSS were 30%, 30%, 25%, 25%, 25% and 25% respectively, while, the 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients without SFSS were 70.1%, 65.6%, 61.7%, 61%, 59.7%, and 59.7% respectively, with statistical significant difference(P = 0.00). Outcome of Laparoscopic Resection for Left Sided Colon and Rectal Cancer

Background: For many years, traditional surgery for left colon and rectal cancers had developed with variable degrees of morbidity. With the evolution of laparoscopy and by the aid of better visualization and magnification, laparoscopic colorectal surgery had appeared, but technically challenging as it involves almost all advanced laparoscopic techniques, with the benefits of minimal morbidity, less pain, earlier recovery, shorter hospital stay, without compromising oncological results. Aim: The aim of this work was to evaluate laparoscopic resection for left sided colon and rectal cancer as regard feasibility, safety and outcomes. Patients and Methods: This prospective study was conducted on 40 patients having left sided colon and rectal cancer, including 29 patients with rectal cancer and 11 patients with left sided colon cancer within the inclusion criteria are evaluated by clinical examination, radiological and colonoscopic study and biopsy and treated by laparoscopic resection and followed ranged from 6 months to 2 years with mean of 20 months. Results: Twenty seven patients (67.5%) underwent laparoscopic anterior resection, 11 patients (27.5%) underwent laparoscopic left hemicolectomy and only 2 patients (5%) underwent laparoscopic abdominoperineal resection, minimal morbidity, no cancer related mortality and no recurrence during the period of follow up either local or systemic. Conclusion: Laparoscopic resection for left sided colon and rectal cancer is technically feasible, oncologically safe and has more benefits on postoperative recovery.

This present study includes twelve species that represent the Ficus genus,namely;aspera,carica,tinctoria ***,hirta,hispida,neriifolia,palmata,pumila,racemosa,septica,sur,and sycomorus,belonging to the Moraceae *** species samples were collected from various locations in *** study focused on the anatomical and molecular characteristics of mature foliage *** the identification and classification of taxa are highly dependent on the anatomical features of leaves,the anatomical characteristics were recorded in the form of a comparison between the examined plants in the data *** study aims to contribute to the identification of the studied species based on the anatomical details of the matured *** characterization includes the variations in upper and lower epidermal layers that are covered by a thin or thick cuticle;the number of palisade and spongy layers;crystals;secretory elements;lithocysts;the midrib zone has parenchyma associated with mechanical tissue,vascular system,and investigation of trichomes;on the other hand,in the current study,the phylogenetic analysis was conducted by using the ITS and 5.8 S *** the analysis of all the available data,it could be stated that there is an overall agreement with the anatomical character dendrogram.
