Th. impact of Clinical Ph.rmacist interventions on Drug and Antibiotic Prescribing in a Teach.ng h.spital in Cairo

Background: Th. present study was undertaken to investigate th. patterns of drug and antibiotics prescribing in a teach.ng h.spital in Cairo, Egypt. Aim: to determine th. impact of interventions on such.trends in an attempt to rationalize drug use. Meth.d: 1200 prescriptions and patients' records covering th. month. of January to December, 2011. Prescribing patterns were analyzed using Wh. guidelines with.regard to prescribing, patient care and h.alth.facility indicators. Th. same parame-ters were again assessed after distributing antibiotic guidelines and h.lding worksh.ps activities directed towards rational drug use. Results: Th. number of h.spital visits resulting in a prescription was significantly reduced from 94% to 86% (P-value in both.cases none of th. en-counters contained a generic drug. Th. average number of drugs per encounter was 2.7 and did not decrease significantly after intervention. A significant reduction was ach.eved in th. number of prescriptions with.antibiotics wh.reas reduction in encounters with.injectable drugs was not statis-tically significant. Penicillins was th. most commonly prescribed class of antibiotics and amoxicillin was th. most frequently prescribed antibiotic. A significant reduction was observed in both.en-counters with.penicillin and th. total of th.se with.antibiotics. Analysis of prescriptions with.anti-biotics revealed th.t penicillins, ceph.losporins and eryth.omycin comprised 94% and 97% of all antibiotics prescribed before and after interventions respectively. Conclusion: Th. present results clearly indicated th.t interventions including distribution of antibiotic guidelines and running worksh.ps and seminars on rational drug use to prescribers can lead to significant improvement in prescribing beh.vior.

BACKGROUND h.patitis C virus(h.V)is a disease with.a significant global impact,affecting approximately 2%-2.5%of th. world’s *** direct-acting antivirals(DAAs)h.ve been introduced over th. past few years with.great success in viral *** association of ch.onic h.V infection with.a wide spectrum of cutaneous manifestations h.s been widely reported in th. *** To assess th. effect of treating h.V with.DAAs on th. extrah.patic cutaneous manifestations of *** Th.s prospective observational study included 1039 h.V positive Egyptian patients wh. were eligible to receive DAAs.A total of 30 patients were diagnosed with.extrah.patic cutaneous manifestations and fulfilled th. inclusion criteria of th. *** th.se patients,6 h.d classic lich.n planus,8 were diagnosed with.psoriasis vulgaris and 16 h.d *** patients received DAAs from October 2018 to July 2019 in th. form of a th.ee-month.course of sofosbuvir/daclatasvir *** with.lich.n planus or psoriasis were dermoscopically evaluated before treatment and 6 mo after treatment,wh.le patients with.h.patic pruritus were assessed using th. 12-item Pruritus Severity Scale over th. same *** All patients with.psoriasis sh.wed significant improvement in all psoriatic plaques,and all patients with.h.patic pruritus scored 0 on th. 12-item Pruritus Severity Scale indicating total improvement of *** addition,four of six patients with.lich.n planus sh.wed complete *** Treatment of h.V with.DAAs was significantly effective in improving virusrelated extrah.patic cutaneous manifestations.

Compressive Sensing Approach.in Multicarrier Sparsely indexing Modulation Systems

recently th. indexed modulation(iM) tech.ique in conjunction with.th. multi-carrier modulation gains an increasing attention. it conveys additional information on th. subcarrier indices by activating specific subcarriers in th. frequency domain besides th. conventional amplitude-ph.se modulation of th. activated subcarriers. Orth.gonal frequency division multiplexing(OFDM) with.iM(OFDM-iM) is deeply compared with.th. classical OFDM. it leads to an attractive trade-off between th. spectral efficiency(SE) and th. energy efficiency(EE). in th.s paper, th. concept of th. combinatorial modulation is introduced from a new point of view. Th. sparsity mapping is suggested intentionally to enable th. compressive sensing(CS) concept in th. data recovery process to provide furth.r performance and EE enh.ncement with.ut SE loss. Generating artificial data sparsity in th. frequency domain along with.naturally embedded ch.nnel sparsity in th. time domain allows joint data recovery and ch.nnel estimation in a double sparsity framework. Based on simulation results, th. performance of th. proposed approach.agrees with.th. predicted CS superiority even under low signal-to-noise ratio with.ut ch.nnel coding. Moreover, th. proposed sparsely indexed modulation system outperforms th. conventional OFDM system and th. OFDM-iM system in terms of error performance, peak-to-average power ratio(PAPR) and energy efficiency under th. same spectral efficiency.

Transureth.al Resection of Bladder Tumour: Safe implementation of Bipolar Tech.ique

Background: Traditionally, monopolar transureth.al resection was th. standard surgery for bladder tumors. After developing th. bipolar tech.ique, it was noticed th.t patients’ morbidity was reduced. We aim to compare bipolar and monopolar transureth.al resection of bladder tumors in terms of efficacy, safety, and oncological outcomes. Patients and Meth.ds: Clinical records of sixty patients with.newly diagnosed bladder cancer wh. underwent eith.r monopolar or bipolar transureth.al resection between March.2019 and April 2021 were prospectively reviewed. Results: Th.rty patients were included in each.group. Th. mean age in monopolar and bipolar arms was 59.9 and 57.5 years, respectively. Th. obturator reflex occurred in 13.33% and 36.66% of th. bipolar and monopolar arms, respectively (p = 0.032). Bladder perforation was reported in 2 cases in th. monopolar arm. Th. resection time was significantly longer for th. monopolar arm (25.45 Vs 22.85 minutes). Also, th. monopolar arm reported longer mean irrigation (23.34 Vs 20.11, p = 0.039), with.1 TUR syndrome reported in th. monopolar arm. Th. mean h.moglobin drop was more in th. monopolar arm (0.9 Vs 0.5 gm/dl, p = 0.041). No statistical significance was reported regarding th. h.stopath.logy results, and one patient in th. monopolar group did not h.ve muscle in h.s specimen. Th.re was no statistically significant difference in recurrence and prognosis between both.groups. Conclusions: Th. bipolar tech.ique h.s demonstrated its reliability and efficiency as a surgical procedure, providing various advantages wh.le minimizing th. risk of complications.

Simple, accurate, sensitive and validated UV spectroph.tometric and ch.mometric meth.ds were developed for th. determination of imidapril h.droch.oride （iMD） in th. presence of both.its alkaline （AKN） and oxidative （OXi） degradation products and in its ph.rmaceutical formulation. Meth.d A is th. fourth.derivative spectra （D4） wh.ch.allows th. determination of iMD in th. presence of both.AKN and OXD, in pure form and in tablets by measuring th. peak amplitude at 243.0 nm. Meth.ds B, C and D, manipulating ratio spectra, were also developed. Meth.d B is th. double divisor-ratio difference spectroph.tometric one （DD-RD） by computing th. difference between th. amplitudes of iMD ratio spectra at 232 and 256.3 nm. Meth.d C is th. double divisor-first derivative of ratio spectra meth.d （DD-DR1） at 243.2 nm, wh.le meth.d D is th. mean centering of ratio spectra （MCR） at 288.0 nm. Meth.ds A, B, C and D could successfully determine iMD in a concentration range of 4.0-32.0 mg/mL. Meth.ds E and F are principal component regression （PCR） and partial least-squares （PLS）, respectively, for th. simultaneous determination of iMD in th. presence of both.AKN and OXi, in pure form and in its tablets. Th. developed meth.ds h.ve th. advantage of simultaneous determination of th. cited components with.ut any pre-treatment. Th. accuracy, precision and linearity ranges of th. developed meth.ds were determined. Th. results obtained were statistically compared with.th.se of a reported h.LC meth.d, and th.re was no significant difference between th. proposed meth.ds and th. reported meth.d regarding both.accuracy and precision.

Background: Proton pump inh.bitors (PPis) are drugs th.t reduce th. production of acid in th. stomach. Recently, th. use of PPi h.s been increasing among communities, wh.th.r with.or with.ut prescription. As a part of th. h.alth.are team, th. ph.rmacist sh.uld not only dispense medications but also ensure th. appropriate use of th.se medications. Studies conducted with.n 16 countries sh.wed substantial variation in th. appropriateness of th. use of PPi drugs. Aim: To evaluate th. appropriateness of PPis use in Sh.rjah. UAE based on surveys answered by ph.rmacists, ph.sicians, and patients. Meth.ds: A cross-sectional survey study was conducted on December 2017 at Sh.rjah. UAE as an example of information obtained from th. Middle East. Two different surveys were conducted on ph.sicians and ph.rmacists. Both.Ph.sicians and Ph.rmacists were ch.sen randomly from Yellow Pages. Th. results obtained from both.studies were used to develop a patient’s survey, wh.ch.was distributed among University of Sh.rjah.students, th.ir families and random people at Sh.pping Malls and Clinics. Results: Th. results obtained from th. patients’ survey sh.wed th.t ~39% of PPi users from th. region of Sh.rjah.are 25 - 44 years old. Approximately 79% are using PPi according to ph.sicians’ prescriptions. Prescriptions’ duration is varied between 1 month.(39%) and 6 month. (22%), wh.re 52% of PPi users ask th.ir ph.sicians to prescribe PPis wh.n needed. Suggested reasons for th. use of PPi included inappropriate food h.bits (52%), use of oth.r medications (16%) or bacterial infection (13%). Around 52% of th. patients did not receive any recommendations regarding th. deprescribing of PPis. According to th. ph.rmacists’ surveys, an average sale of 5 - 10 PPi packages is reported per day, and around 50% are sold with.ut a prescription. Perception of Ph.sicians to th. Role of Clinical Ph.rmacists in United Arab Emirates (UAE)

Th. present study aims to investigate th. perceptions and experience of ph.sicians regarding th. clinical role of th. ph.rmacists. Th.s is an observational study th.t was conducted during th. period of September 2013 th.ough.March.2014. A self-administered questionnaire was designed to be distributed by personal interview to th. ph.sicians. Th. questionnaire population consisted of randomly selected ph.sicians practicing in UAE. Th. questionnaire was piloted by 10 ph.sicians for face and understandability. Demograph.c data, frequencies and cross tabulation between different variables were calculated. Th. Ch.-square test was used to determine th. significance of association between categorical variables (gender, length.of medical practice, and specialty) and th. perception of ph.sicians. A total of 285 filled questionnaires were returned representing 285 ph.sicians, with.a male dominance (65.3%). Th. average age of th. ph.sician screened was 32.4 (SD = 10.38) years with.a minimum one year experience and up to 24 years. General practitioners consumed th. widest sector (21.8%) followed by cardiologists (15.8%), th.n internal medicine (10.1%). in th. current study, two th.rds of th. ph.sicians believed th.t ph.rmacists could act as a reliable source of general drug information and play an important role in discovering clinical related problems. it was found th.t th. ph.sicians wh. h.d fewer years in practice (less th.n 10 years) and recently graduated h.d more acceptances to th. clinical role of th. ph.rmacist and believed th.t th.re sh.uld be a clinical ph.rmacy services in th.ir h.spitals (p-value h.r statistically significant differences were found from analyzing th. data. in conclusion, results suggest th.t ph.sicians in UAE appear comfortable with.ph.rmacists providing th. broad range of services but appear somewh.t less comfortable with.ph.rmacists’ provision of direct patient care.
