BACKGROUND Administrative data show that acute heart failure(HF)patients are older than those enrolled in clinical registries and frequently admitted to non-cardiological settings of *** purpose of this study was to describe clinical characteristics of old patients hospitalised for acute HF in Cardiology,Internal Medicine or Geriatrics *** Data came from ATHENA(AcuTe Heart failurE in advaNced Age)registry which included elderly patients(≥65 years)admitted to the above mentioned settings of care from december 1,2014 to december 1,*** We enrolled 396 patients,15.4%assigned to Cardiology,69.7%to Internal Medicine,and 14.9%to a Geriatrics *** age was 83.5±7.6 years(51.8%of patients≥85 years)and was higher in patients admitted to Geriatrics(Pdelines recommendation adherence for decongestion evaluation with comparison of weight and N-terminal pro-B-type natriuretic peptide levels on admission and at discharge(both P=0.035 and Pderly patients hospitalised for acute HF,clinical characteristics and management differ significantly according to the setting of admission.

Case Report: Complete Clinical Response of a Young Patient with a BRCA 2 Positive Metastatic Breast Cancer

Background: Metastatic breast cancer in BRCA 2 patients is lacking of target therapy studies. Case: We present a case of a 34-year-old woman affected by a recurrent metastatic breast cancer after 5 years treated with weekly nab-paclitaxel and three weekly Cisplatin obtaining a radiological complete response with 6 cycles. Conclusion: It is important to emphasize that patients with a molecular disorder who selected for “target” chemotherapy can obtain optimal response rate. Exploiting the mechanism of action of DNA damage in our favor, using cisplatin for example and basing on literature show the efficacy of using taxane in BRCA 2 patients, obtaining a very high response rate.

Psychiatric Morbidity of Chikungunya among Children in Bangladesh

Chikungunya occurred as a major epidemic form in Bangladesh in 2017 after a gap of 6 years. The first outbreak of Chikungunya was identified in Bangladesh in 2008. Another outbreak occurred at Dhaka in 2011. But the current outbreak is huge and effecting almost two third of the population. Methods: This was a retrospective case series study, consisted of 60 children, among which 35 were male and 25 female, ages ranging from 5 to 15 years. Those children were recruited from child health care clinic with a history of two to five days’ high grade fever. All patients recruited were IgM antibody for Chikungunya positive (after 5th day of onset of fever) and on the other hand antibody for dengue was negative. Two patients were excluded from the study due to Cerebral Palsy and Claustrophobia. Finally 58 patients were assessed. Informed written consent was obtained from all the legal guardians before data collection. Results: All patients presented with a history of two to five days’ high grade fever. The fever was preceded by a maculopapular rash over the trunk and extremities, headache, myalgia and arthralgia. delirium (non-specified) was most prevalent psychiatric symptoms 45 (77%). Other psychiatric symptoms were Panic Disorder (56%), Phobic Disorder (36%), Neurasthenia or Fatigue syndrome (27%), Hypersomnia (13%) and Claustrophobia (5%). Conclusion: This study suggested that psychiatric morbidity among pediatric Chikungunya patients is very common. Knowledge of the pathogenesis is required to reduce the psychiatric complications and to prevent the morbidity. There is an emergence to invent the prophylactic vaccination, proper and early diagnosis of Chikungunya.

钢筋纤维复合材料(SFCB)近些年可应用于混凝土构件。SFCB屈服前由内部钢筋提供高弹性刚度,屈服后由外部FRP提供屈服后刚度,且SFCB有着优异的抗腐蚀性能。可以通过更改钢筋与FRP的比例来调整SFCB的屈服后刚度。为了在新结构中充分发挥SFCB的作用,因此需对SFCB构件进行深入研究。本文对宽150mm、高250mm矩形截面的玻璃钢纤维复合材料筋(GSFCB)混凝土梁进行试验研究目的在于考察其受剪承载力。在两点加载装置下对51个梁进行试验,其中27根GSFCB混凝土梁、12根钢筋混凝土梁和12根GFRP混凝土梁。研究的参数包括变量剪切跨度与高度之比(a/d)、箍筋间距s和纵筋配筋率ρ。结果表明,GSFCB混凝土梁的抗剪承载力低于普通钢筋混凝土梁,但略高于GFRP混凝土梁。GSFCB混凝土梁与钢筋混凝土梁相比,裂缝更宽、更深,但与具有相同参数的GFRP混凝土梁相比,其裂缝更小。此外,钢筋混凝土梁的低抗剪承载力可以通过减小箍筋的间距来提高抗剪强度。通过钢筋混凝土梁与FRP混凝土梁的实验数据分别拟合出公式,然后再与规范公式对比,最后发现拟合出的公式与规范公式有一定的比例关系,故而以相同的比例进行计算并提出了SFCB混凝土梁的抗剪建议公式,希望对工程应用与理论分析提供一定的参考。

Wound Infection Caused by Chromobacterium violaceum: A Case Report from a Tertiary Care Hospital in Bangladesh

Chromobacterium violaceum is a Gram negative, facultative anaerobe, generally present in water, soil in tropical and subtropical regions. This bacterium is an emerging environmental pathogen that causes life threatening infection in humans and animals. It can cause wound infection, visceral abscess, septicaemia, meningitis, diarrhoea, UTI. It is associated with significant mortality due to severe systemic infection. As the bacteria have high spreading tendency leading to sepsis, early identification and prompt treatment is necessary. Here we report a case of Chromobacterium violaceum wound infection in a 9 years old male from Dhaka, who was successfully treated with combination of cefixime and flucloxacillin antibiotics as per culture sensitivity report.

Prevalence of Antibiotic Resistant Bacteria on Tomato Surfaces and Effectiveness of Disinfectants in Reducing the Microbial Load

This study was conducted to isolate and identify antibiotic resistant bacteria in fresh tomato sold in three different local markets of Dhaka city. On an average 〉 5.0 log CFU/g of natural microbial populations and coliform bacteria was present in the samples. Prevalence of Salmonella spp., E. coli, Listeria spp., and Yersinia spp., were recorded through cultivation dependent assay of tomato samples of different markets. Almost all of the isolates were uniformly resistant to rifampicin, erythromycin, clindamycin and oxacillin （100%）, vancomycin （93%）, amoxicillin （87%）, whereas 60% of the isolates were found to be resistant to ampicillin and cephalexin. Antibiotic sensitivity test of the presumptive E. coli and Salmonella spp. and corresponding plasmid profiling indicated a correlation of plasmid mediated multidrug resistance （MDR） of these environmental bacteria. Washing with sanitizing agents such as acidified sodium chlorite （ASC） and 200 ppm chlorinated water was able to reduce _〈 2.0 log CFU/g of natural microflora and coliform bacteria. In contrast, ASC was able to reduce 〈 2.0 log CFU/g of other food borne pathogens and, chlorinated water was able to reduce up to undetectable level. Therefore, washing of tomato with 200 ppm chlorinated water could be useful in reducing the pathogen populations on tomatoes.

MAR Technique to Reverse the declining Trend of Groundwater Level in barind Area, NW, Bangladesh

The study was carried out in the drought prone water stressed barind area to know the state of rainfall, trend of groundwater level and groundwater management options including Managed Aquifer Recharge (MAR) techniques. Rainfall and groundwater level data have been used and shown a decreasing trend in Godagari and Mohonpur Upazila. But in Niamatpur Upazila rainfall is in reverse condition of rising trend. decreasing trend of groundwater level (GWL) has been found in the entire study area. The decreasing trend of GWL demands judicious management of groundwater for the area. For recovery of GWL as well as groundwater management, a recharge well as MAR structure has been installed and observed that the situation is better than the before and increasing GWL. More research and study are necessary to know about the trend of groundwater level for the whole barind area and to introduce proper designed and modified MAR techniques to reverse the declining situation of GWL.

Effect of aspirin on glucose-D transport in intestine of rat

Objective: The present study was designed to evaluate the effect a commonly prescribed Non Steroidal Anti Inflammatory Drug(NSAID) *** on brush border membrane in terms of changes in the intestinal transport level of glucose which is monosaccharide with absolute requirement in the body and hence its absorption is directly proportional on the morphology of the intestinal ***: Albino rats(Rattus Norvegicus) were divided into two different groups,Group I(Control),Group II(aspirin-treated,50 mg aspirin/kg of body weight).The treatment was continued for 28 *** the 29th day after overnight fasting,intestine was removed from animals of both *** in transport of glucose-D in intestine were ***: The results indicated a significant decrease in the transport of glucose-D in aspirin treated group as compared to the control ***: Cautious use of NSAID is recommended in commonly observed symptom such as headache and to those patients who are given as a prophylaxis for thrombosis.
