Minimizing treatment complexity of combat-related soft tissue injuries using a dedicated tension relief system and negative pressure therapy augmented by high-dose in situ antibiotic therapy and oxygen delivery: a retrospective study

Background:Following combat-related,extensive soft tissue injury from gunshot wounds or blasts,prolonged duration from injury to full wound closure is associated with infection,increased morbidity and mortality,failure to mobilize,poor functional outcome and increased *** purpose of this study was to evaluate a novel treatment enabling early primary closure of combat ***:This was a retrospective study of 10 soldiers and civilians with extensive combat-related soft tissue limb injuries(5 gunshot wounds,5 blasts)treated using the TopClosure^(■)Tension Relief System(TRS)with simultaneous administration of regulated oxygen-enriched and irrigation negative pressure-assisted wound therapy(ROINPT)via the Vcareα^(■)***:Nine patients were treated during the acute phase of injury and one was treated following removal of a flap due to deep infection 20 years after injury and flap *** patients had upper limb injury and the rest lower limb *** the aid of the TRS and/or ROINPT,immediate primary closure during reconstruction was achieved in 6 patients and delayed primary closure in *** one patient required a skin graft to close a small area of the wound after most of the wound had been closed by delayed primary *** closure was achieved within 0–37 days(median:12.5 days,interquartile range:2.75–19.75)from ***:The TRS is a novel device for effective,early skin stretching and secure wound closure through the application of stress relaxation and mechanical creep,achieving primary closure of large defects using a simplified surgical technique and reducing the need for closure using skin grafts and flaps and the use of tissue *** supplemental oxygen to the wound by ROINPT reverses the reduced oxygen levels inherent in conventional negative pressure-assisted wound therapy,mitigating anaerobic contamination and reducing *** may accelerate the evacuation of infectious materia

Limnological instrumentation in the middle of the 19^(th) century:the first temperature and density profiles measured in the Dead Sea

The first modem survey of the Dead Sea was performed by the Geological Survey of Israel in 1959-1960, and the report published remains the baseline study for our understanding of the physical and chemical properties of the lake. At the time the Dead Sea was a meromictic lake with a strong salinity gradient separating the deep waters （〉40 m depth） from the less saline surface waters. A few results of analyses of deeper water samples collected in the 1930s were reported, but overall we have very little information about the structure of the lake＇s water column before the 1959-1960 survey. However, it is little known that data on the physical and the chemical structure of the Dead Sea water column were obtained already in the middle of the 19~ century, and the information collected then is highly relevant for the reconstruction of the limnological properties of the lake in earlier times. The expedition of Lieutenant William Lynch （U.S. Navy） in 1848 reported the presence of a temperature minimum at a depth of-18 m, and also retrieved a water sample collected close to the bottom for chemical analysis. In 1864, the French Dead Sea exploration by the Duc de Luynes and his crew yielded detailed density and salinity profiles for a number of sampling stations. The results of these pioneering studies are discussed here, as well as the sampling equipment and measuring instruments used by the 1848 and the 1864 expeditions.

microRNAs and Alu elements in the p53-Mdm2-Mdm4 regulatory network

p53 is a transcription factor that governs numerous stress response pathways within the ceil. Maintaining the right levels of p53 is crucial for ceil survival and proper cellular homeostasis, The tight regulation of p53 involves many cellular components, most notably its major negative regulators Mdm2 and Mdm4, which maintain p53 protein amount and activity in tight check, microRNAs （miRNAs） are small non-coding RNAs that target specific mRNAs to translational arrest and degradation, miRNAs are also key compo- nents of the normal p53 pathway, joining forces with Mdm2 and Mdm4 to maintain proper p53 activity. Here we review the current knowledge of miRNAs targeting Mdm2 and Mdm4, and their importance in different tissues and in pathological states such as cancer. In addition, we address the role of Alu sequences-highly abundant retroelements spread throughout the human genome, and their impact on gene regulation via the miRNA machinery. Aius occupy a significant portion of genes＇ 31UTR, and as such they have the potential to impact mRNA regulation. Since Alus are primate-specific, they introduce a new regulatory layer into primate genomes. Atus can influence and alter gene regulation, creating primate-specific cancer-preventive regulatory mechanisms to sustain the transition to longer life span in primates. We review the possible influence of Alu sequences on miRNA functionality in general and specifically within the p53 network.

Earthquake engineering research and development have received much attention since the first half of the twentieth century. This valuable research presented a huge step forward in understanding earthquake hazard mitigation,which resulted in appreciable reduction of the effects of past earthquakes. Nevertheless,the 2011 Tohoku earthquake and the subsequent tsunami resulted in major damage. This paper presents the timeline of earthquake mitigation and recovery,as seen by the authors. Possible research directions where the authors think that many open questions still remain are identified. These are primarily based on the important lessons learned from the 2011 Tohoku earthquake.
