Background: A pseudoaneurysm of the internal carotid artery (ICA) at its cervicl part, is a rare and often lethal com-plication after cervical trauma. The treatment of choice is anticoagulation. Patients with a contraindication for anticoagulation or those who do not improve with standard treatment, warrant either a surgical repair or stent *** placement posses both short and long term risks such as immediate ischemic events, in-stent stenosis and stent breakage, thus the choice of stent type is critical. Advances in stent technology have made cervical stent placement a viable option. We report our experience with the LEO + (Balt Extrusion, Montmorency, France) nitinol flexible self expanding stent for post-traumatic ICA aneurysms. Methods: 5 patients suffering a post-traumatic cervical ICA pseudo-aneurysms refractory to standard treatment were treated with nitinol braided flexible self expanding stent. This stent has a shape memory and superelasticity virtues making it suitable. Diagnosis was made with CT angiography and confirmed by catheter angiography. All procedures were preformed under local anesthesia. Time between trauma and treatment ranged six days to over five years. Follow-up was performed by CT angiography or conventional angiography. Results: Five patients, having eight aneurysms were treated using 12 stents. In all cases, appropriate stent placement was achieved. No immediate or late complications, as well as no neurological sequele reported at 6 months. Radiological follow-up exams demonstrated complete pseudoaneurysm closure. Conclusion: Stent placement can be a safe and definitive treatment option for patients to conventional medical treatment.

When forced to drop from the plant,flightless arboreal insects can avoid reaching the risky ground by maneuvering their body through the *** wingless pea aphids(Acyrthosiphon pisum)are threatened by natural enemies,they often drop off their host plant while assuming a stereotypic posture that rotates them in mid-air,aligning them with their feet pointing *** position may increase their chances of re-clinging onto lower plant parts and avoid facing the dangers on the ground,although its effectiveness in realistic field conditions has not been *** performed both laboratory and outdoor experiments,in which we dropped aphids upon host plants to quantify clinging success in plants with different characteristics such as height and leaf *** aphids had twofold higher clinging rates than dead ones,indicating that clinging success is indeed affected by the active aerial-righting of dropping *** ability to cling was positively dependent on the plants’foliage cover as viewed in vertical direction from ***,we released aphids in commercial alfalfa(Medicago sativa)fields with varying plant heights and foliage cover and induced them to ***(up to 75%)of the aphids avoided reaching the ground in taller plants(65 cm),and 17%in shorter plants(21 cm),demonstrating the efficiency of the aphids’response in averting risks:both those of an approaching enemy on the plant and the plethora of new risks on the ***,even in complex field environment,the aerial-righting mechanism can substantially reduce the possible risks following escape from a predator.

Objective To compare evaluations of depressive episodes and suggested treatment protocols generated by Chat Generative Pretrained Transformer(ChatGPT)-3 and ChatGPT-4 with the recommendations of primary care *** Vignettes were input to the ChatGPT *** vignettes focused primarily on hypothetical patients with symptoms of depression during initial *** creators of these vignettes meticulously designed eight distinct versions in which they systematically varied patient attributes(sex,socioeconomic status(blue collar worker or white collar worker)and depression severity(mild or severe)).Each variant was subsequently introduced into ChatGPT-3.5 and *** vignette was repeated 10 times to ensure consistency and reliability of the ChatGPT *** For mild depression,ChatGPT-3.5 and ChatGPT-4 recommended psychotherapy in 95.0%and 97.5%of cases,*** care physicians,however,recommended psychotherapy in only 4.3%of *** severe cases,ChatGPT favoured an approach that combined psychotherapy,while primary care physicians recommended a combined *** pharmacological recommendations of ChatGPT-3.5 and ChatGPT-4 showed a preference for exclusive use of antidepressants(74%and 68%,respectively),in contrast with primary care physicians,who typically recommended a mix of antidepressants and anxiolytics/hypnotics(67.4%).Unlike primary care physicians,ChatGPT showed no gender or socioeconomic biases in its *** ChatGPT-3.5 and ChatGPT-4 aligned well with accepted guidelines for managing mild and severe depression,without showing the gender or socioeconomic biases observed among primary care *** the suggested potential benefit of using atificial intelligence(AI)chatbots like ChatGPT to enhance clinical decision making,further research is needed to refine AI recommendations for severe cases and to consider potential risks and ethical issues.

In recent years we have witnessed a large interest in surface deformation techniques. This has been a reaction that can be attributed to the ability to develop techniques which are detail-preserving. Space deformation techniques, on the other hand, received less attention, but nevertheless they have many advantages over surface-based techniques. This paper explores the potential of these two approaches to deformation and discusses the opportunities that the fusion of the two may lead to.

A ciliated hepatic foregut cyst (CHFC) is a rare foregut developmental malformation usually diagnosed in adulthood. Five percent of reported cases of CHFC transform into squamous cell carcinoma. We report the presentation, evaluation, and surgical management of a symptomatic 45-year-old male found to have a 6.2 cm CHFC. Contrast tomography-guided fine-needle aspiration demonstrated columnar, ciliated epithelium consistent with the histologic diagnosis of CHFC. The intracystic levels of carbohydrate antigen (CA) 19-9 and carcinoembryonic antigen (CEA) were extremely high (978118 U/mL and 973 μg/L, respectively). Histologically, the wall of the cyst showed characteristic pseudopapillae lined with a ciliated stratified columnar epithelium, underlying smooth muscle, an outer fibrous layer and no atypia. Immunohistochemistry for CA19-9 and CEA was positive. This is the first case report of a CHFC in which levels of CA 19-9 and CEA were measured. Our findings suggest that a large sized multilocular cyst and elevated cyst CA19-9 and CEA levels do not exclude a CHFC from consideration in the diagnosis. CHFCs should be included in the differential diagnosis of hepatic lesions. Accurate diagnosis of a CHFC is necessary given its potential for malignant transformation, and surgical excision is recommended.
