Viral hepatitis prevalence in patients with active and latent tuberculosis

AIM: To assess the prevalence of hepatitis B virus(HBV) and hepatitis C virus(HCV) infection and association with drug induced liver injury(DILI) in patients undergoing anti-tuberculosis(TB) ***: Four hundred and twenty nine patients with newly diagnosed TB- either active disease or latent infection- who were due to commence antiTB therapy between September 2008 and May 2011 were included. These patients were prospectively tested for serological markers of HBV, HCV and human immunodeficiency virus(HIV) infections- hepatitis B core antigen(HBc Ag), hepatitis B surface antigen(HBs Ag), hepatitis B e antigen, Ig G and Ig M antibody to HBc Ag(anti-HBc), HCV Ig G antibody and HIV antibody using a combination of enzyme-linked immunosorbent assay, Western blot assay and polymerase chain reaction techniques. Patients were reviewed at least monthly during the TB treatment initiation phase. Liver function tests were measured prior to commencement of antiTB therapy and 2-4 wk later. Liver function tests were also performed at any time the patient had significant nausea, vomiting, rash, or felt non-specifically unwell. Fisher's exact test was used to measure significance in comparisons of proportions between groups. A P value of less than 0.05 was considered statistically ***: Of the 429 patients, 270(62.9%) had active TB disease and 159(37.1%) had latent TB infection. 61(14.2%) patients had isolated anti-HBc positivity, 11(2.6%) were also HBs Ag positive and 7(1.6%) were HCV-antibody positive. 16/270 patients with active TB disease compared to 2/159 patients with latent TB infection had markers of chronic viral hepatitis(HBs Ag or HCV antibody positive; P = 0.023). Similarly the proportion of HBs Ag positive patients were significantly greater in the active vs latent TB infection group(10/43 vs 1/29, P = 0.04). Left-sided periappendiceal abscesses occur in association with two types of congenital anomaly: intestinal malrotation and situs inversus. It is diffi cult to obtain an accurate preoperative diagnosis of these abscesses due to the abnormal position of the appendix. We present an unusual case of a left-sided periappendiceal abscess in an adult with intestinal malrotation, the diagnosis of which was a challenge.

Advanced simulation of die wear caused by wire vibrations during wire-drawing process

An advanced simulation that considers the effect of wire vibrations was proposed for predicting accurately wear profiles of a die used in a wire-drawing *** effect of wire vibrations,the changes in the wear profiles,and the generation of ringing during die approach were investigated by this *** vibrations occurring between the die and the drum are governed by a partial differential equation called the wave equation,which is a function of the wire length,tension,density,and initial wire *** wire-drawing process was simulated by the commercial code Abaqus FEA,and the die wear profiles were predicted by Archard’s wear *** predicted profiles were compared with measured profiles of a worn drawing die after producing 5 t of AISI 1010 wire;the die was made of tungsten carbide with a Brinell hardness of HB *** profiles predicted by considering the effect of wire vibrations are in good agreement with the experimental data,indicating that the advanced simulation can be used to accurately predict the die wear profiles when ringing is observed during die approach.

From pollutant to solution of wastewater pollution: Synthesis of activated carbon from textile sludge for dye adsorption

Adsorption is an important process in wastewater treatment,and conversion of waste materials to adsorbent offers a solution to high material cost related to the use of commercial activated *** study investigated the adsorption behaviour of Reactive Black 5（RB5）and methylene blue（MB）onto activated carbon produced from textile sludge（TSAC）.The activated carbon was synthesized through chemical activation of precursor followed with carbonization at 650℃ under nitrogen *** of time（0–200 min）,pH（2–10）,temperature（25–60℃）,initial dye concentration（0–200 mg·L^-1）,and adsorbent dosage（0.01–0.15 g）on dye removal efficiency were *** screening revealed that TSAC synthesized via H2SO4activation showed higher adsorption behaviour than TSAC activated by KCl and *** adsorption capacity of TSAC was found to be 11.98 mg·g^-1（RB5）and 13.27 mg·g^-1（MB）,and is dependent on adsorption time and initial dye *** adsorption data for both dyes were well fitted to Freundlich isotherm model which explains the heterogeneous nature of TSAC *** dye adsorption obeyed pseudo-second order kinetic model,thus chemisorption was the controlling *** study reveals potential of textile sludge in removal of dyes from aqueous solution,and further studies are required to establish the applicability of the synthesized adsorbent for the treatment of waste water containing toxic dyes from textile industry.

Endoscopic bilateral stent-in-stent placement for malignant hilar obstruction using a large cell type stent

BACKGROUND： Bilateral stent-in-stent （SIS） self-expandable metal stent placement is technically chghallenging for palliation of unresectable malignant hilar obstruction. In the SIS technique, the uniform large cell type biliary stent facilitates contralateral stent deployment through the mesh of the first metallic stent. This study aimed to assess the technical success and clinical effectiveness of this technique with a uniform large cell type biliary stent. METHODS： Thirty-one patients who underwentlbilateral SIS placement using a large cell type stent were reviewed retrospectively. All patients showed malignant hilarl obstruction （Bismuth types Ⅱ, Ⅲ, Ⅳ） with different etiologies. RESULTS： Sixteen （51.6%） patients were male. The mean age of the patients was 67.0±14.0 years. Most patients were diagnosed as having hilar cholangiocarcinoma （58.1%） and gallbladder cancer （29.0%）. Technical success rate was 83.9%. Success was achieved more frequently in patients without masses obstructing the biliary confluence （MOC） than those with MOC （95.2% vs 60.0%, P=0.03）. Functional success rate was 77.4%. Complications occurred in 29.0% of the patients. These tended to occur more frequently in patients with MOC （50.0% vs 19.0%, P=0.11）. Median time to recurrent biliary obstruction was 188 days and median survival was 175 days. CONCLUSIONS： The large cell type stent can be used efficiently for bilateral SIS placement in malignant hilar obstruction. However, the risk of technical failure increases in patients with MOC, and caution is needed to prevent complications for these patients.

Safety of endoscopic ultrasound-guided ethanol ablation for pancreatic cystic lesions: A single-center experience of 214 patients

Background:Endoscopic ultrasound-guided ethanol ablation(EUS-EA)for pancreatic cystic lesions(PCLs)has been used in recent years as a feasible treatment modality for low malignant probability PCLs or patients considered high-risk for *** present study aimed to confirm the safety of EUS-EA and to find predictive factors for adverse event(AE).Methods:A retrospective review was performed from the prospectively maintained database of patients who underwent EUS-EA for PCLs from June 2006 to April 2018 at Seoul National University *** primary outcomes of the study were the rates of AEs and severe AEs by *** secondary outcome was the predictive factors of AEs including acute pancreatitis and abdominal ***:A total of 214 patients were evaluated and the diagnoses of PCLs according to cystic fluid analysis and clinical features were as follows:serous cystic neoplasm(32.2%),mucinous cystic neoplasm(26.6%),branch duct type intraductal papillary mucinous neoplasm(BD-IPMN)(29.4%),and pseudocyst(11.7%).Three patients(1.4%)experienced severe ***,AEs occurred in 71(33.2%)***-IPMN(OR:2.87;95%CI:1.05–7.84;P=0.040),multilocular cysts(OR:3.59;95%CI:1.09–11.85;P=0.036),suspected ethanol leakage during procedure(OR:10.68;95%CI:1.98–57.53;P=0.006),and sticky cystic fluid(OR:3.83;95%CI:1.20–12.24;P=0.024)were predictive factors for post-procedural acute *** of uncinate process(OR:2.99;95%CI:1.22–7.35;P=0.017)and PCLs with exophytic portion(OR:3.70;95%CI:1.96–7.01;Pminal ***:EUS-EA is a safe procedure with a very low rate of severe *** seems possible to predict the AEs according to the features of the procedure and PCLs.

Survival benefit of adjuvant treatment for ampullary cancer with lymph nodal involvement:A systematic review and meta-analysis

Experimental Investigation of Fuel Quality and Contaminant Materials from Liquefied Petroleum Fuel

In this study, the quality characteristics and residue analysis in circulated LPG fuel were investigated experimentally in Korea. Quality characteristics in circulated LPG fuel were examined with samples of LPG in the supply chain (refinery, petrochemical, and imported LPG), transport, gas stations, and vehicles. The experimental results showed that quality of all circulated LPG was well within the quality standard guideline of LPG in Korea. Especially, it has shown average 13 wt ppm in sulfur content over the full circulated LPG. The residue samples in LPG fuel were extracted on 2 L scale with acetonitrile and analyzed by gas chromatography-mass spectrometry (GC-MS). The components of residues in LPG were composed of 62 organic chemicals with C3 ~ C28 and the main ingredients of residue were plasticizers ((di-octyl phalate (DOP), di-octyl adiphate (DOA) etc.), lubricant oil and amine compounds. It was also showed that mass of residue in vehicles was increasing compared with supply (refinery, petrochemical, and imported LPG). It was presumed that this residue had been originated from automotive LPG fuel, vehicle components, and lubricant oil in infrastructure.

Risk factors of organ failure in cholangitis with bacteriobilia

AIM: To identify the risk factors for organ failure(OF) in cholangitis with ***: This study included 182 patients with acute cholangitis who underwent percutaneous transhepatic biliary drainage between January 2005 and April 2013. We conducted a retrospective analysis of comprehensive clinical and laboratory ***: There were 24 cases(13.2%) of OF and five deaths(2.7%). Bile culture was positive for microbial growth in 130 out of 138(94.2%) patients. In multivariate analysis of 130 patients with positivebile cultures, significant predictive factors for OF were the presence of extended-spectrum beta-lactamase(ESBL) organisms in blood cultures, pre-existing renal dysfunction, and choledocholithiasis as an etiology, with odds ratios of 15.376, 6.319, and 3.573, respectively. We developed a scoring system with a regression coefficient of each significant variable. The OF score was calculated using the following equation:(2.7 × ESBL organisms in blood cultures) +(1.8 × pre-existing renal dysfunction) +(1.3 × choledocholithiasis). This scoring system for predicting OF was highly specific(99.1%) and had a positive predictive value of 86.2%.CONCLUSION: ESBL organisms in blood cultures, preexisting renal dysfunction, and choledocholithiasis are risk factors for OF in cholangitis with bacteriobilia. The OF scoring system may aid clinicians to identify a poor prognosis group.
