Concise review of radiosurgery for contemporary management of pilocytic astrocytomas in children and adults

Pilocytic astrocytoma(PA)may be seen in both adults and children as a distinct histologic and biologic subset of low-grade *** is the principal treatment for the management of PAs;however,selected patients may benefit from irradiation particularly in the setting of inoperability,incomplete resection,or recurrent *** conventionally fractionated radiation therapy has been traditionally utilized for radiotherapeutic management,stereotactic irradiation strategies have been introduced more recently to improve the toxicity profile of radiation delivery without compromising tumor *** may be suitable for radiosurgical management due to their typical appearance as well circumscribed *** and precise targeting of these well-defined lesions under stereotactic immobilization and image guidance may offer great potential for achieving an improved therapeutic ratio by virtue of radiosurgical *** the high conformality along with steep dose gradients around the target volume allowing for reduced normal tissue exposure,radiosurgery may be considered a viable modality of radiotherapeutic *** advantage of radiosurgery may be the completion of therapy in a usually shorter overall treatment time,which may be particularly well suited for children with requirement of anesthesia during *** studies have addressed the utility of radiosurgery particularly as an adjuvant or salvage treatment modality for ***,despite the growing body of evidence supporting the use of radiosurgery,there is need for a high level of evidence to dictate treatment decisions and establish its optimal role in the management of ***,we provide a concise review of radiosurgery for PA in light of the literature.

Detecting early changes in choroidal vascularity and thickness using optical coherence tomography in patients with corneal crosslinking for keratoconus

AIM:To investigate changes in choroidal thickness and vascularity in keratoconus patients treated with corneal ***:This study evaluated 28 eyes of 22 patients with keratoconus who underwent corneal *** choroidal thicknesses were evaluated on enhanced depth imaging optical coherence tomography at the preoperative and postoperative 3d,1,and *** thickness in the four cardinal quadrants and the fovea were *** choroidal vascularity index was also ***:There was no significant difference in central choroidal thickness between the preoperative and postoperative 3d,1mo(P>0.05).There was a significant increase in the 3mo(P=0.034)and a significant decrease in the horizontal choroidal vascularity index on the postoperative 3d(P=0.014),there was no statistically significant change in vertical axes and other visits in horizontal sections(P>0.05).CONCLUSION:This study sheds light on choroidal changes in postoperative corneal crosslinking for *** it suggests the procedure’s relative safety for submacular choroid,more extensive research is necessary to confirm these findings and their clinical significance.

Mountainous regions have disadvantages in economic development because of harsh physical and climatic ***,winter tourism activities are one of the key components for supporting economic development in the *** a ski resort area supports direct and indirect employment in a region,and it stops immigration from mountainous regions to other *** research aimed to assess the potential ski areas using a multi criteria evaluation technique in the Van region which is located in the eastern part of Tü*** this context,snow cover duration,sun effect,slope,slope length,elevation,population density,distance from main roads and lake visibility were used as input factors in the decision making *** factor was standardized using a fuzzy technique based on existing well-known ski centers in Tü*** weight of inputs was defined by applying a survey to the professional *** most important factors were detected as transportation opportunities and snow covers whereas,the least important factors were elevation and population ***,lake visibility was very important to make a difference from other existing facilities in the ***,it was included as constraints and lake visible areas were extracted at the final stage of the *** ski areas were mapped in three levels as professional,intermediate and beginner *** of the suitable areas was selected as a sample projection and for the 3D simulation of the ski investment *** costs and benefits were *** was found that a ski tourism area investment can be amortized in 3 years in the region.

In today’s rapidly evolving landscape of communication technologies,ensuring the secure delivery of sensitive data has become an essential *** overcome these difficulties,different steganography and data encryption methods have been proposed by researchers to secure *** of the proposed steganography techniques achieve higher embedding capacities without compromising visual imperceptibility using LSB *** this work,we have an approach that utilizes a combinationofMost SignificantBit(MSB)matching andLeast Significant Bit(LSB)*** proposed algorithm divides confidential messages into pairs of bits and connects them with the MSBs of individual pixels using pair matching,enabling the storage of 6 bits in one pixel by modifying a maximum of three *** proposed technique is evaluated using embedding capacity and Peak Signal-to-Noise Ratio(PSNR)score,we compared our work with the Zakariya scheme the results showed a significant increase in data concealment *** achieved results of ourwork showthat our algorithmdemonstrates an improvement in hiding capacity from11%to 22%for different data samples while maintaining a minimumPeak Signal-to-Noise Ratio(PSNR)of 37 *** findings highlight the effectiveness and trustworthiness of the proposed algorithm in securing the communication process and maintaining visual integrity.

Glioblastoma(GB)is the most common and aggressive form of primary brain tumors in adults with a universally poor prognosis despite multimodal management including surgery,chemotherapy and radiation *** the novel therapeutic strategies,immunotherapy deserves particular attention with its potential to evoke biologic response and harness the host immune *** success achieved for other tumors has elicited great enthusiasm and prompted research on immunotherapy for *** the central nervous system has traditionally been thought of as an immune-privileged site,our understanding is being refined with emerging *** studies have been conducted and more are under way to establish the role of immunotherapy in management of *** of GB has yet resulted in mixed success with conflicting research findings,emphasizing the need for extensive study before its integration into routine clinical *** there is a lot of room for improvement,immunotherapy for GB may be feasible and serve as a viable management strategy broadening and strengthening the therapeutic armamentarium to combat this deadly ***,we present a concise review of immunotherapy for GB.

Image Classification Based on Vision Transformer

This research introduces an innovative approach to image classification, by making use of Vision Transformer (ViT) architecture. In fact, Vision Transformers (ViT) have emerged as a promising option for convolutional neural networks (CNN) for image analysis tasks, offering scalability and improved performance. Vision transformer ViT models are able to capture global dependencies and link among elements of images. This leads to the enhancement of feature representation. When the ViT model is trained on different models, it demonstrates strong classification capabilities across different image categories. The ViT’s ability to process image patches directly, without relying on spatial hierarchies, streamlines the classification process and improves computational efficiency. In this research, we present a Python implementation using TensorFlow to employ the (ViT) model for image classification. Four categories of animals such as (cow, dog, horse and sheep) images will be used for classification. The (ViT) model is used to extract meaningful features from images, and a classification head is added to predict the class labels. The model is trained on the CIFAR-10 dataset and evaluated for accuracy and performance. The findings from this study will not only demonstrate the effectiveness of the Vision Transformer model in image classification tasks but also its potential as a powerful tool for solving complex visual recognition problems. This research fills existing gaps in knowledge by introducing a novel approach that challenges traditional convolutional neural networks (CNNs) in the field of computer vision. While CNNs have been the dominant architecture for image classification tasks, they have limitations in capturing long-range dependencies in image data and require hand-designed hierarchical feature extraction.

How has the COVID-19 pandemic changed treatment preferences of patients with proximal ureteral stones?

Background:The impact of the coronavirus disease 2019(COVID-19)pandemic on patient decision making remains *** study aimed to investigate the effect of the pandemic on treatment preferences of patients with proximal ureteral stones.Materials and methods:Retrospective data regarding treatment preferences of patients diagnosed with symptomatic proximal ure-teral stones between July 2018 and November 2021 at a single center were *** from 493 patients were analyzed according to 2 groups,including patients diagnosed during the COVID-19 pandemic and those diagnosed during an equivalent period of time be-fore the pandemic.Results:Preference for conservative treatment increased during the COVID-19 pandemic(p=0.009).In patients who had previously undergone shock wave lithotripsy(SWL),the preference for SWL decreased and the preference for conservative treatment increased during the COVID-19 pandemic(p=0.042).Multiple logistic regression analysis revealed a significant correlation between a preference for conservative treatment during the pandemic and no prior spontaneous stone passage(p=0.003;odds ratio[OR],2.48;95％con-fidence interval[CI],1.45-4.23),no hydronephrosis(p=0.035;OR,3.57;95％Cl,1.34-9.49),and a visual analog scale score of 4 or less(p=0.018;OR,1.97;95％Cl,1.15-3.38).Conclusions:A significant increase in the preference for conservative treatment was observed among patients diagnosed during the pandemic,and patients with a history of SWL demonstrated a preference shift from SWL to conservative treatment.

Breathing adapted radiation therapy for leukemia relapse in the breast: A case report

BACKGROUND Infiltration of the breast by leukemic cells is uncommon but may manifest as an oncological emergency requiring prompt *** relapse of T-cell acute lymphoblastic leukemia(T-ALL)within the breast is exceedingly rare and there is paucity of data in the literature regarding this *** consensus exists on management of isolated extramedullary breast relapses of ***,we report a case of isolated extramedullary breast relapse of T-ALL treated with breathing adapted radiation therapy(BART)using the active breathing control(ABC)*** SUMMARY The patient was a 33-year-old female with diagnosis of *** received intensive systemic chemotherapy that resulted in complete remission of her disease,and then underwent allogeneic hematopoietic stem cell *** a 15 mo period without symptoms and signs of progression,the patient presented with palpable masses in both *** complained from severe pain and swelling of the *** workup showed bilateral breast lesions,and diagnosis of breast infiltration by leukemic cells was confirmed after immunohistopathological *** patient suffering from severe pain,discomfort,and swelling of both breasts due to leukemic infiltration was referred to the Radiation Oncology Department for symptomatic *** breast irradiation was delivered to both breasts of the patient with BART using the ABC *** patient had complete resolution of her symptoms after treatment with *** BART with the ABC system resulted in complete resolution of the patient’s symptoms due to leukemic infiltration of both breasts with *** contemporary treatment technique should be preferred for radiotherapeutic management of patients with leukemic infiltration of the breasts to achieve effective symptomatic palliation.
