Diagnostic accuracy of ePOS score in predicting DNR labeling after iCU admission:A prospective observational study(ePOS-DNR)

Background Resuscitation can sometimes be futile and making a do-not-resuscitate(DNR)decision is in the best interest of the *** electronic poor outcome screening(ePOS)score was developed to predict 6-month poor outcomes of critically ill *** explored the diagnostic accuracy of the ePOS score in predicting DNR decisions in the intensive care unit(iCU).Methods This study was conducted at the iCU of a tertiary referral hospital in Saudi Arabia between March and May ***,we calculated ePOS scores for all eligible consecutive admissions after 48 h in the iCU and recorded the DNR *** ability of the score to predict DNR was explored using logistic ***'s ideal cut-off value was calculated using the DeLong method,and different diagnostic accuracy measures were generated with corresponding 95%confidence intervals(Cis).Results We enrolled 857 patients,125 received a DNR order and 732 did *** average ePOS score of DNR and non-DNR patients was 28.2±10.7 and 15.2±9.7,*** score,as a predictor of DNR order,had an area under receiver operator characteristic(AUROC)curve of 81.8%(95%Ci:79.0 to 84.3,Pideal cut-off value>17 was associated with a sensitivity of 87.2(95%Ci:80.0 to 92.5,Pificity of 63.9(95%Ci:60.3 to 67.4,Pitive predictive value of 29.2(95%Ci:24.6 to 33.8,Pive predictive value of 96.7(95%Ci:95.1 to 98.3,Piagnostic odds ratio 12.1(95%Ci:7.0 to 20.8,Pions in this study,the ePOS score performed well as a diagnostic test for patients who will be labeled as DNR during their iCU stay.A cut-off score>17 may help guide clinical decisions to withhold or commence resuscitative measures.

Management of Acute Bowel Obstruction in Adults in Ziguinchor Hospital Establishments: About 163 Cases Followed Up

Acute intestinal obstruction represents a very frequent admission to surgical emergencies. The aim of our study was to make a descriptive analysis of the management of acute intestinal obstructions at the level of hospital establishments in Ziguinchor. Materials and Methods: This was a retrospective, descriptive, multicenter study from the period of January 1, 2017 to December 31, 2021. Patients over 15 years of age admitted for occlusive syndrome in the two surgical departments of the Ziguinchor regional hospitals were included in the study. Epidemiological, diagnostic, therapeutic and evolutionary data were studied. Results: Acute intestinal occlusions accounted for 8.3% of admissions. A total of 163 patients were enrolled, 61.9% were men, sex ratio of 1.6. The mean age was 48.7 years. The average consultation time was 74.8 hours (3.12 days). The total occlusive syndrome was in 48.5% of the cases. Abdominal pain was present in 87.1%, vomiting in 74.2%, cessation of matter and gas in 60.7% and meteorism in 36.2%. Biological lab tests reported hyperleukocytosis in 28.1% of the cases. Abdominal CT scans were performed in 71.2%, confirming the diagnosis in 90.5% and determining the etiology in 93.1%. The zone of the obstruction in the small bowel was 62.7%, and in the colon in 37.3%. Functional occlusions accounted for 22.7% of cases and mechanical for 77.3%, including 120 cases of strangulation occlusion (95.2%) and 6 cases of obstruction. Etiologies were dominated by adhesions and/or fibrous bands (61.2%). Medical treatment resolved the occlusive syndrome in 9.2% of cases, instrumental treatment in 17.1% and surgical treatment in 77.3%. Adhesiolysis/section of fibrous bands (55.7%), detorsion (14.3%) and resection anastomosis (11.9%) were the most common surgical procedures. Overall morbidity was 11.6%, dominated by surgical site infection (7.14%). The mean resumption of intestinal transit was 2.7 days, and the mean hospital stay was 7.9 days. Overall mortality was

This study, carried out in the Léo square degree (west-central Burkina Faso) in the Palaeoproterozoic domain of the Man/Léo ridge, aims to define the shapes and positions of granitic plutons and the organisation of deformation structures using aeromagnetic data. These data have shown that there are small sub-circular granitic bodies to the north of the granitic masses in this region, and a large sub-circular granitic body to the south, around which are other smaller granitic bodies. The lineament map shows that the deformation structures are organised along three main directions and largely form these sub-circular plutonic bodies. We suggest that the granitic plutons are coalescent, pending identification of the internal structures of these granites to further refine the geodynamic model.

Petrographic and Geochemical Characteristics of the Pouni Palaeoproterozoic Formations North of the Léo Square Degree (Burkina Faso, West Africa)

The Pouni area is made up of basalts belonging to the Boromo belt, lamprophyres and granitoids. These geological formations are similar to geological formations of the same type in other regions of the Palaeoproterozoic domain of the Man/Leo shield. This study, which focused on the petrographic and geochemical characteristics of these geological formations, led to the following main conclusions: The lamprophyres are basic plutonic rocks that cut through other geological formations. The basalt belongs to the northern part of the Borormo belt and is thought to be a relic of overthickened oceanic plateaus. There are two groups of granitoid rocks. The granodiorite has a geochemical signature close to that of Archean TTGs and is metaluminous in character. it has a low potassium content. The minor element and rare earth element spectra indicate that it could be derived from partial melting of basic magmatic rocks. Biotite granites are peraluminous and highly potassic. Minor element contents and rare earth spectra indicate that they could be derived from partial melting of felsic materials. Geotectonic diagrams show that the granitoids identified in the Pouni zone were emplaced in an active tectonic context, similar to that of present-day subduction zones.

South of Godé, in the central-western region of Burkina Faso, granitoids of Paleoproterozoic age are similar to those of the Man/Leo shield. This study focused on the petrographic and geochemical characteristics of these granitoids, with the following results: 1) The tonalite that outcrops in the south-west of the study area belongs to the TTG group or first generation granitoids. They are most often ribboned at outcrop and have a geochemical signature close to that of Archean TTGs. Tonalite has a metaluminous character and the REE spectrum indicates that it may be derived from partial melting of basic magmatic rocks. 2) Biotite granites have no outcrop structure. They are weakly metaluminous to peraluminous and potassic to highly potassic. Their rare earth spectra indicate that they may be derived from the partial melting of TTG granitoids. 3) Geotectonic diagrams show that the granitoids studied to the south of Godé were emplaced in an active tectonic context similar to that of present-day subduction zones.

The baobab, Adansonia digitata L., plays an important role in the economy of local populations. Nowadays, baobab seed oil is highly prized for its many cosmetic and therapeutic applications, and for its composition of unsaturated fatty acids, sterols, and tocopherols. However, it undergoes numerous reactions during production, processing, transport, and storage, leading to undesirable products that make it unstable. The aim of this study was to provide local processors with innovative solutions for the treatment of unrefined vegetable oils. To this end, an experimental device for filtering crude oil on activated carbon made from fruit capsules was designed. The results obtained after the treatment show a significant decrease at (p in acid value (1.62 to 0.58 mg KOH/g), peroxide value (4.40a to 0.50c mEqO2/Kg), chlorophyll concentration (1.81 to 0.50 mg/Kg) and primary and secondary oxidation products. According to these results, activated carbon’s adsorptive power eliminates oxidation products and certain pro-oxidants such as chlorophyll, resulting in a cleaner, more stable and better storable oil.

Contribution of Bone Scintigraphy in the Metastatic Extension Assessment of Prostate Cancer: A Study of 288 Cases in the Nuclear Medicine Department of idrissa Pouye General Hospital, Dakar

introduction: Prostate cancer is the most frequently diagnosed male malignancy and the fifth leading cause of cancer death in men worldwide. Since the advent of screening methods such as Prostate Specific Antigen (PSA) assay, digital rectal examination (DRE) and prostate biopsy, its incidence has increased significantly. The aim of our study was to analyse aspects of bone scintigraphy (BS) as part of the metastatic extension assessment of prostate cancer in Senegal. Patients and Methods: This was a retrospective descriptive and analytical study, running from January 1er 2022 to August 31 2023. Patients with histologically confirmed prostate cancer were included. Whole-body scans (WBS) were performed using a dual-head SPECT gamma camera (Mediso Nucline TM Spirit DH-V type), 3 hours after intravenous injection of 8 MBq/kg (555 to 740 MBq) of 99mTc-HMDP. Results: A total of 288 patients with a mean age of 68.37 ± 7.79 years were included. The median total PSA level was 97.6 ng/ml, with 144 patients having a level greater than or equal to 20 ng/ml. All patients had adenocarcinoma, and the Gleason score was available in 202 (70.13%) patients, 75.75% of whom had a score greater than or equal to 7. BS was contributory in 70.48% of cases, with 30.90% positive and 39.58% negative. The result was inconclusive in 85 patients (29.51%). The mean PSA for patients with a positive scan was 190.2 ng/ml and 40.6 ng/ml for those with a negative scan. Multiple metastatic lesions predominated (87.35% of cases). Metastatic lesions occurred preferentially in the axial skeleton, with a proportion of 68% versus 32% in the appendicular skeleton. Classification of bone metastases according to the SOLOWAY score revealed grade i (62.07%), grade ii (35.63%) and grade iV (2.30%). introduction: Breast cancer is the most common cancer in women worldwide, accounting for an estimated 22% of all female cancers. it is the leading cause of cancer mortality in women, almost all of which is due to metastases, with 73% of metastases occurring in the bone. in oncology, metastable technetium 99-labelled methylene bisphosphonate bone scintigraphy (BS) remains the standard examination for detecting and assessing the extent of bone metastases. The aim of this study was to assess the role of BS in the evaluation and follow-up of breast cancer in Senegal. Methodology: This was a retrospective study of breast cancer patients who underwent bone scintigraphy with 99mTc-HMDP in the nuclear medicine department of idrissa Pouye General Hospital (iPGHO), from July 2009 to June 2022. Results: We enrolled 165 patients, mean age 46.79 years (27 - 87 years). BS was performed in 94.37% of cases for post-therapeutic monitoring and in 5.63% for pre-therapeutic assessment. Results were contributory in 131 patients (92.25%), of whom 72 cases (50.70%) were normal and 59 cases (41.55%) positive or presenting bone metastases;and non-contributory or doubtful in 11 cases (7.75%). Secondary bone locations were multiple in 57 cases (96.61%) and single or solitary in 2 cases (3.39%). The scintigraphic appearance of bone metastases was hyper-fixative in 58 cases (98.31%) and mixed in 1 case (1.69%). Bone lesions were quantified using the Soloway’s grading classification. Conclusion: BS with 99mTc-labelled bisphosphonates remains the examination of choice for skeletal exploration, in the detection and extension of bone metastases in breast cancer. Performance has been enhanced by the development of SPECT coupled with CT (SPECT-CT).
