Integrating Blockchain Innovation: A Sustainable Adoption Model for Business

Blockchain technology holds significant promise for driving innovations across diverse industries, businesses, and applications. Recognized as a crucial source of competitive advantage in a fast-evolving environment, blockchain is anticipated to contribute substantially to sustainable economic and social development. Despite these high expectations, many blockchain projects currently face high failure rates, leading to negative impacts on various aspects of economic and social sustainability, including corporate governance, risk management, financial management, human resources, culture management, and competitiveness. This paper evaluates adoption models, identifying both risk and success factors. It introduces an integrated adoption model designed to operationalize, measure, and manage blockchain-driven business innovation sustainably. An empirical study involving 20 industry sectors and 125 business leaders was conducted to assess the model’s applicability. The findings indicate that the adoption model has the potential to support the sustainable implementation of blockchain technology for business innovations across various industries and applications. Future research and industry activities should continue validating this model through further case studies.

Accuracy of Sentinel Lymph Node Biopsy after Neoadjuvant Chemotherapy in Breast Cancer Patients;Single Center Experience

Purpose: Most important factor affecting prognosis of breast cancer is axillary nodal involvement. Several studies evaluated the accuracy of Sentinel lymph node biopsy in breast cancer patients post neoadjuvant chemotherapy. In this study, we will examine accuracy and feasibility of using Sentinel lymph node biopsy in predicting axillary lymph node status in breast cancer patients after neoadjuvant chemotherapy. Methods: 45 female patients with resectable, nonmetastatic breast carcinoma cases who received neoadjuvant chemotherapy were enrolled in this study according to the routine Mansoura Oncology Center—guidelines of management of breast cancer. Methylene blue dye used for detection of Sentinel lymph node. Results: Successful Sentinel lymph node detection was 82.2%. Skin involvement (T4 disease) were linked to a low identification (P = 0.005). False negative rate equals 11/27 = (40.7%).With advancement of the stage of the tumor, the incidence of false negative results increases significantly (p = 0.012) with 95% confidence interval;1.2 - 5.4. Conclusion: Sentinel lymph node should be adopted to be the standard method for axillary staging with T1-3 tumors after receiving neoadjuvant chemotherapy, in T4 patients, it is associated with low detection rate & high false negative rate making it doubtful technique for axillary staging.

Radio frequency identification (RFID) has emerged as a pivotal technology in supply chain management (SCM), significantly enhancing its efficiency and effectiveness. When integrated with the internet of things (IoT) to form RFID-IoT, this technology brings transformative advancements to SCM, enabling automated sensing, pervasive computing, and ubiquitous data access across the entire supply chain, from manufacturers and distributors to retailers and consumers. This integration facilitates real-time identification and monitoring of products, enhances various processes, improves logistic tracking, and ensures better product quality management. Despite its promising benefits, the adoption of RFID-IoT in SCM faces several challenges, including technical complexities, data security concerns, and high implementation costs. However, the future potential of RFID-IoT technology remains substantial. It is anticipated that further integration with other emerging technologies, such as block chain and artificial intelligence, will lead to more comprehensive and robust SCM solutions, offering unprecedented levels of transparency, efficiency, and automation in supply chain operations.

Information Technology or AI, a combination of computer science, logic, biology, psychology, and philosophy, is set to dominate our future, revolutionizing social progress, labor productivity, and cost reduction. It is essential for AI database integration and the next generation of computing, supporting Intelligent Information Systems and distributed cooperative labor. AI and IT integration are enhancing efficiency, creativity, and digital capabilities, with applications in customer service, healthcare, personalized medicine, and driverless cars. The paper tends to review the developments and trends in information technology using artificial intelligence (AI) as weapons to re-build the future world. With qualitative analysis of previous research studies, the paper tries to explain the role of AI in present information flow as well as for future. And after studying the selective papers it efficiently explains the advantage of AI in both human and environmental way. AI has the potential to significantly impact our lives, but fundamental questions remain unanswered about its extent, scope, and timeline. Current tools are insufficient to predict future AI achievements or fully comprehend its capabilities. To determine the readiness level for different technologies with AI, criteria are defined based on historical evolution. AI has revolutionized the IT industry, improving efficiency, productivity, and quality. However, concerns arise about the potential impact on the workforce, with concerns about job redundancy and potential unjust outcomes in critical areas like employment, finance, and law enforcement.

Evaluation of Surgical Outcome after Resection of Pancreatic Tumors, Our Experience in Mansoura Oncology Center, a Middle-Volume Center in Egypt

Background: Pancreatic cancer is the fourth most common cause of cancer related deaths in the world. Surgical resection remains the only potentially curative treatment for adenocarcinoma of the pancreas;only 10% - 20% of patients are candidate for standard pancreatic resection. Objective: To evaluate perioperative mortality, morbidity and survival for patients underwent PD in middle-volume center. Patients and Methods: Sixty patients with pancreatic tumors were enrolled in this study for different surgical procedure according to type of tumors. Results: No early postoperative complications were observed in 60% of patients, whereas 40% of patients developed one or more complication, the median survival for this group was 12.3 months with standard deviation 3.8 months.

Diffusion-Weighted MR Imaging of the Benign Hepatic Focal Lesions

Purpose: To determine the role of diffusion weighted MR imaging and apparent diffusion coefficient (ADC) in benign hepatic focal lesions. Method and Materials: This study included 47 patients (29 M, 18 F with mean age of 43 years) with benign hepatic focal lesions. They were cyst (n = 8), hemangioma (n = 23), abscess (n = 5), adenoma (n = 5), focal nodular hyperplasia (n = 4) and nodular regenerative hyperplasia (n = 2). They underwent routine MR imaging and diffusion MR weighted imaging using 1.5 tesla MR unit (Symphony-Siemens). Diffusion MR imaging was done using spin echo type of single shot echo planar imaging (EPI) with b value of 0, 500 & 1000 sec/mm2. The apparent diffusion coefficient (ADC) map was reconstructed and ADC value was measured. The mean ADC values correlated with histo-pathological results as well as follow-up imaging results. Results: Adequate ADC maps were obtained in 47 patients. The mean ADC values were 3.4 ± 0.12 × 10-3 mm2/sec in cystic lesion, 2.23 ± 0.08 × 10-3 mm2/sec in hemangioma, 1.94 ± 0.05 × 10-3 mm2/sec in abscess, 1.72 ± 0.07 × 10-3 in focal nodular hyperplasia, 1.65 ± 0.06 × 10-3 mm2/sec in adenoma, 1.62 ± 0.07 × 10-3 mm2/sec in nodular regenerative hyperplasia. The mean ADC values were significantly different within benign hepatic focal lesions (P < 0.001). The differences between the mean ADC values of FNH, adenoma and NRH were not statistically significant (P < 0.23). Conclusion: Diffusion weighted MR imaging is a new imaging modality for diagnosis and characterization of different benign hepatic focal lesions, particularly in patient with renal dysfunction.

The Effectiveness of Intraoperative Frozen Section Analysis of Safety Margins in Breast Conserving Surgery and the Role of Surgeon in Decreasing the Rate of Positive Margins

Introduction: Nowadays the more accepted surgical option for treating early breast cancer is breast conserving surgery. The main challenge in this type of surgery is to get free safety margins without need of second surgical operation, so many breast surgeons have started to depend on intraoperative frozen sections to ensure free safety margins. Aim of work: To assess our policy that we prefer to depend on intraoperative frozen section analysis to get free safety margin in breast conserving surgery from the 1st surgery, and its oncologic outcome. Patients and Methods: This is a retrospective study conducted in Oncology Center—Mansoura University (OCMU), where the data of 219 patients with breast cancer, who were managed by breast conserving surgery with intraoperative frozen section analysis of the safety margins, was analyzed. Results: The intraoperative frozen section analysis of safety margin was negative from the start in 183 (83.6%) patients, while it was positive in 36 patients (16.4%). Intraoperative decision of margin re-excision was applied for 29 patients (13.2%) in order to reach negative margin, modified radical mastectomy was offered for 4 patients (1.8%), while nipple sparing mastectomy with immediate breast reconstruction using latissimus dorsi flap was offered for 3 patients (1.4%). The postoperative paraffin results were typical with intraoperative frozen section analysis results in 216 patients (98.6%) and different results were obtained in only 3 patients (1.4%) who were managed by modified radical mastectomy in a second operation. Only 4 patients had local recurrence (1.8%) during the period of follow-up duration which was ranged from 1 to 86 months with mean ± SD (22.3 ± 14.1). Conclusion: The intraoperative frozen section analysis of safety margins in breast conserving surgery has very high-rate typical results with the paraffin section analysis and it is very helpful in decreasing the rate of second surgical operation in cases of infiltrated m

Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the *** of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and *** approach to treatment of GLM should be applied according to the different clinical stage of GLM. This evidencebased consensus would be valuable to assist front-line surgeons and medical specialists in the optimal management of GLM.

Value of Repeated Direct Smear Sputum Examination in the Diagnosis of Pulmonary Tuberculosis

Introduction: The examination of three sputum samples per suspect has been severely criticized from a public health viewpoint and several recent trials have documented the relative inefficiency of the third smear and the necessity for confirmation of a positive smear has also been contested. Aim: This study, undertaken in Qena, Egypt, aimed to determine the usefulness of examining the second and third direct smear microscopy (DSM) specimen in the diagnosis of pulmonary TB. Patients and methods: A retrospective study using record review at TB outpatient clinic;Qena Chest Hospital, Egypt, was done from 2010-2013. Direct smear results were collected as one of the following combinations PNN, PPP, PPN, PNP, NNP, NPP, and NPN, NNN, where N is a negative and P a positive smear. The proportion of positive, first, second and third specimen were calculated. Cases were considered positive having at least one positive smear confirmed by another positive one in the absence of sputum culture. Results: Out of 9420 recorded suspects, 719 of them were positive, so smear positivity was 7.6%. The majority of them were diagnosed from the first sample (96.4%). For only 3.6% (26 of 719), the second smear was positive and a third specimen was required (NPP) to make a definitive diagnosis of TB. No recorded isolated positive or negative smears in the third sample (NNP or PPN). Conclusions: These data indicated that, in our locality with limited financial resources, the incremental yield of a second sputum direct smear examination was low, and the third one was negligible indicating that examination of two sputum samples is enough among pulmonary TB patients. A third sample is required only as confirmatory if the second sample was positive. Smear microscopy can be substantially simplified with favourable resource implications.

Influence of Nitrogen Fertilizer and Foliar Application of Salicylic Acid on Wheat

Two cemented plots experiments were carried out during the winter seasons of 2012/2013 and 2013/2014, Soil Salinity Laboratory, Alexandria, Egypt, to study the effect of three levels of salicylic acid (SA) (0, 50, 100 ppm) and three rates of nitrogen fertilizer (75, 100, 125 kg N/feddan) on yield and yield components of wheat (Sakha 93). The experimental design was split plot with four replicates. The results indicated that increasing nitrogen fertilizer rates resulted in significant increase in plant height (cm), number of grain/spike, number of spikes/m2, 1000 grain weight (gm), grain yield (gm/plot), straw yield (gm/plot), and biological yield (gm/plot). Increasing salicylic acid rates resulted in significant increase in all the previous characters, in addition to grain weight/spike (gm). The interactions had no significant effect on the studied characters. The path analysis revealed that 1000 grain weight was the most important character with direct effect of 0.54 followed by number of spikes/m2 with direct effect of 0.33 followed by number of grain/spike with direct effect of 0.16.
