The consequences of neonatal white matter injury are devastating and represent a major societal problem as currently there is no cure.Prematurity,low weight birth and maternal pre-natal infection are the most frequent causes of acquired myelin deficiency in the human neonate leading to cerebral palsy and cognitive impairment.In the developing brain,oligodendrocyte(OL)maturation occurs perinatally,and immature OLs are particularly vulnerable.Cell replacement therapy is often considered a viable option to replace progenitors that die due to glutamate excitotoxicity.We previously reported directed specification and mobilization of endogenous committed and uncommitted neural progenitors by the combination of transferrin and insulin growth factor 1(TSC1).Here,considering cell replacement and integration as therapeutic goals,we examined if OL progenitors(OLPs)grafted into the brain parenchyma of mice that were subjected to an excitotoxic insult could rescue white matter injury.For that purpose,we used a well-established model of glutamate excitotoxic injury.Four-day-old mice received a single intraparenchymal injection of the glutamate receptor agonist N-methyl-D-aspartate alone or in conjunction with TSC1 in the presence or absence of OLPs grafted into the brain parenchyma.Energetics and expression of stress proteins and OL developmental specific markers were examined.A comparison of the proteomic profile per treatment was also ascertained.We found that OLPs did not survive in the excitotoxic environment when grafted alone.In contrast,when combined with TSC1,survival and integration of grafted OLPs was observed.Further,energy metabolism in OLPs was significantly increased by N-methyl-D-aspartate and modulated by TSC1.The proteomic profile after the various treatments showed elevated ubiquitination and stress/heat shock protein 90 in response to N-methyl-D-aspartate.These changes were reversed in the presence of TSC1 and ubiquitination was decreased.The results obtained in this pre-clinical study indicate that the use

Genotype-based precision nutrition strategies for the prediction and clinical management of type 2 diabetes mellitus

Globally,type 2 diabetes mellitus(T2DM)is one of the most common metabolic disorders.T2DM physiopathology is influenced by complex interrelationships between genetic,metabolic and lifestyle factors(including diet),which differ between populations and geographic regions.In fact,excessive consumptions of high fat/high sugar foods generally increase the risk of developing T2DM,whereas habitual intakes of plant-based healthy diets usually exert a protective effect.Moreover,genomic studies have allowed the characterization of sequence DNA variants across the human genome,some of which may affect gene expression and protein functions relevant for glucose homeostasis.This comprehensive literature review covers the impact of gene-diet interactions on T2DM susceptibility and disease progression,some of which have demonstrated a value as biomarkers of personal responses to certain nutritional interventions.Also,novel genotype-based dietary strategies have been developed for improving T2DM control in comparison to general lifestyle recommendations.Furthermore,progresses in other omics areas(epigenomics,metagenomics,proteomics,and metabolomics)are improving current understanding of genetic insights in T2DM clinical outcomes.Although more investigation is still needed,the analysis of the genetic make-up may help to decipher new paradigms in the pathophysiology of T2DM as well as offer further opportunities to personalize the screening,prevention,diagnosis,management,and prognosis of T2DM through precision nutrition.

Critical assessment of R410A alternatives for mini-split air conditioners in the Egyptian market

In this study,the potential implementation of three different low-GWP refrigerants(R32,R452B,and R454B)as replacements for R410A was investigated.The study was performed using a simulation tool developed by the authors called RACHP-Lab,which is a vapor compression system simulation tool developed based on physics-based simulation for typical mini-split air conditioners.The simulation study was carried out and validated using experimental performance data of 10 different air conditioning units available in the Egyptian market.The units included fixed-speed or variable-speed compressors and operated in cooling or heating modes.Drop-in replace-ment with the new refrigerants was carried out.For R32,the capacity increased between 4.9%and 13%for cooling cases,and 6.3%and 12.4%for heating cases.However,COP did not improve in all cases.For R452B and R454B with direct replacement,the capacity nearly remained the same,with an increase of COP between 1.6%and 8.0%.Soft optimization was also conducted on cooling cases where compressor suction superheat,condenser subcooling,and compressor volumetric speed were optimized to maximize COP while maintaining the original capacity of R410A.R32 showed an improvement of COP over R410A between 4.6%and 15.5%,while for R452B and R454B between 2.2%and 13.2%.

A fracture of the acromion and coracoid processes of scapula is rarely seen in the outpatient clinic,due to the acromion's anatomical position and shape,as well as the strong ligaments and muscles that are attached to it.These fractures are caused by either direct or indirect high-energy trauma injuries to the shoulder joint,leading to severe pain and a grossly restricted range of motion.Several acromial classifications were reported,but this type of longitudinal plane fracture of the acromion process in our case is yet to be described in the current literature.We present a rare combination of the coracoid process and unstable acromion bony projection fractures that have not previously been noted for this type of fracture.The closest to this is Kuhn's type III classification.A 51-year-old male presented to our emergency department complaining of the right shoulder pain and difficulty raising his arm following a 2-wheeler accident.The patient was managed by open reduction and internal fixation with 3 cannulated cancellous screws fixation and progressed well with no postoperative complications.He was asymptomatic postoperatively and regained full range of motion after 4 months.

Advanced Optimized Anomaly Detection System for IoT Cyberattacks Using Artificial Intelligence

While emerging technologies such as the Internet of Things(IoT)have many benefits,they also pose considerable security challenges that require innovative solutions,including those based on artificial intelligence(AI),given that these techniques are increasingly being used by malicious actors to compromise IoT systems.Although an ample body of research focusing on conventional AI methods exists,there is a paucity of studies related to advanced statistical and optimization approaches aimed at enhancing security measures.To contribute to this nascent research stream,a novel AI-driven security system denoted as“AI2AI”is presented in this work.AI2AI employs AI techniques to enhance the performance and optimize security mechanisms within the IoT framework.We also introduce the Genetic Algorithm Anomaly Detection and Prevention Deep Neural Networks(GAADPSDNN)sys-tem that can be implemented to effectively identify,detect,and prevent cyberattacks targeting IoT devices.Notably,this system demonstrates adaptability to both federated and centralized learning environments,accommodating a wide array of IoT devices.Our evaluation of the GAADPSDNN system using the recently complied WUSTL-IIoT and Edge-IIoT datasets underscores its efficacy.Achieving an impressive overall accuracy of 98.18%on the Edge-IIoT dataset,the GAADPSDNN outperforms the standard deep neural network(DNN)classifier with 94.11%accuracy.Furthermore,with the proposed enhancements,the accuracy of the unoptimized random forest classifier(80.89%)is improved to 93.51%,while the overall accuracy(98.18%)surpasses the results(93.91%,94.67%,94.94%,and 94.96%)achieved when alternative systems based on diverse optimization techniques and the same dataset are employed.The proposed optimization techniques increase the effectiveness of the anomaly detection system by efficiently achieving high accuracy and reducing the computational load on IoT devices through the adaptive selection of active features.

Burden of routine orthopedic implant removal a single center retrospective study

BACKGROUND Open reduction and internal fixation represent prevalent orthopedic procedures,sparking ongoing discourse over whether to retain or remove asymptomatic implants.Achieving consensus on this matter is paramount for orthopedic surgeons.This study aims to quantify the impact of routine implant removal on patients and healthcare facilities.A retrospective analysis of implant removal cases from 2016 to 2022 at King Fahad Hospital of the University(KFHU)was conducted and subjected to statistical scrutiny.Among these cases,44%necessitated hospitalization exceeding one day,while 56%required only a single day.Adults exhibited a 55%need for extended hospital stays,contrasting with 22.8%among the pediatric cohort.The complication rate was 6%,with all patients experiencing at least one complication.Notably,34.1%required sick leave and 4.8%exceeded 14 d.General anesthesia was predominant(88%).Routine implant removal introduces unwarranted complications,particularly in adults,potentially prolonging hospitalization.This procedure strains hospital resources,tying up the operating room that could otherwise accommodate critical surgeries.Clearly defined institutional guidelines are imperative to regulate this practice.AIM To measure the burden of routine implant removal on the patients and hospital.METHODS This is a retrospective analysis study of 167 routine implant removal cases treated at KFHU,a tertiary hospital in Saudi Arabia.Data were collected in the orthopedic department at KFHU from February 2016 to August 2022,which includes routine asymptomatic implant removal cases across all age categories.Nonroutine indications such as infection,pain,implant failure,malunion,nonunion,restricted range of motion,and prominent hardware were excluded.Patients who had external fixators removed or joints replaced were also excluded.RESULTS Between February 2016 and August 2022,360 implants were retrieved;however,only 167 of those who met the inclusion criteria were included in this study.The remaining implants were rejected due

The Impact of Network Topologies and Radio Duty Cycle Mechanisms on the RPL Routing Protocol Power Consumption

The Internet of Things(IoT)has witnessed a significant surge in adoption,particularly through the utilization of Wireless Sensor Networks(WSNs),which comprise small internet-connected devices.These deployments span various environments and offer a multitude of benefits.However,the widespread use of battery-powered devices introduces challenges due to their limited hardware resources and communication capabilities.In response to this,the Internet Engineering Task Force(IETF)has developed the IPv6 Routing Protocol for Low-power and Lossy Networks(RPL)to address the unique requirements of such networks.Recognizing the critical role of RPL in maintaining high performance,this paper proposes a novel approach to optimizing power consumption.Specifically,it introduces a developed sensor motes topology integrated with a Radio Duty Cycling(RDC)mechanism aimed at minimizing power usage.Through rigorous analysis,the paper evaluates the power efficiency of this approach through several simulations conducted across different network topologies,including random,linear,tree,and elliptical topologies.Additionally,three distinct RDC mechanisms—CXMAC,ContikiMAC,and NullRDC—are investigated to assess their impact on power consumption.The findings of the study,based on a comprehensive and deep analysis of the simulated results,highlight the efficiency of ContikiMAC in power conservation.This research contributes valuable insights into enhancing the energy efficiency of RPL-based IoT networks,ultimately facilitating their widespread deployment and usability in diverse environments.

Wireless technology is transforming the future of transportation through the development of the Internet of Vehicles(IoV).However,intricate security challenges are intertwinedwith technological progress:Vehicular ad hoc Networks(VANETs),a core component of IoV,face security issues,particularly the Black Hole Attack(BHA).This malicious attack disrupts the seamless flow of data and threatens the network’s overall reliability;also,BHA strategically disrupts communication pathways by dropping data packets from legitimate nodes altogether.Recognizing the importance of this challenge,we have introduced a new solution called ad hoc On-Demand Distance Vector-Reputation-based mechanism Local Outlier Factor(AODV-RL).The significance of AODVRL lies in its unique approach:it verifies and confirms the trustworthiness of network components,providing robust protection against BHA.An additional safety layer is established by implementing the Local Outlier Factor(LOF),which detects and addresses abnormal network behaviors.Rigorous testing of our solution has revealed its remarkable ability to enhance communication in VANETs.Specifically,Our experimental results achieve message delivery ratios of up to 94.25%andminimal packet loss ratios of just 0.297%.Based on our experimental results,the proposedmechanismsignificantly improves VANET communication reliability and security.These results promise a more secure and dependable future for IoV,capable of transforming transportation safety and efficiency.

Ballistic design and testing of a composite armour reinforced by CNTs suitable for armoured vehicles

This paper is investigating the use of composite armour reinforced by nanomaterials, for the protection of light armoured(LAV) and medium armoured military vehicles(MAV), and the interaction between the composite materials and high-performance ballistic projectiles. Four armour materials, consisted of front hybrid fibre reinforced polymer cover layer, ceramic strike-face, fibre reinforced polymer intermediate layer and the metal matrix composite reinforced backplate, were manufactured and assembled by adhesive technology. The proposed laminated protection system is suitable for armoured ground vehicles;however, it could be used as armour on ground, air and naval platforms. The design of the protection system, including material selection and thickness, was elaborated depending on the performance requirements of Level 4 + STANAG 4569 military standard(projectile 14.5 mm × 114 mm API B32) and especially on a design philosophy which is analysed with the specifications. The backplate of this new composite is a hybrid material of Metal Matrix Composite(MMC) reinforced with carbon nanotubes(CNTs), manufactured with the use of powder metallurgy technique. The composite backplate material was morphologically, mechanically and chemically analysed. Results show that all plates are presenting high mechanical properties and ballistic characteristics, compared to commonly used armour plates. Real military ballistic tests according to AEP-STANAG 4569 were carried out for the total composite armour systems. After the ballistic tests, AA2024-CNT3 showed the best protection results, compared with the other plates(AA2024-CNT1 and AA2024-CNT2), with the projectile being unable to fully penetrate the composite plate.

Evaluation of hand infections in the emergency department using point-of-care ultrasound

BACKGROUND:We aimed to evaluate the utility of point-of-care ultrasound(POCUS)in the assessment of hand infections that present to the emergency department(ED)and its impact on medical decision making and patient management.METHODS:We conducted a retrospective review of patients who presented to two urban academic EDs with clinical presentations concerning for skin and soft tissue infections(SSTI)of the hand between December 2015 and December 2021.Two trained POCUS fellowship physicians reviewed an ED POCUS database for POCUS examinations of the hand.We then reviewed patients’electronic health records(EHR)for demographic characteristics,history,physical examination findings,ED course,additional imaging studies,consultations,impact of POCUS on patient care and final disposition.RESULTS:We included a total of 50 cases(28 male,22 female)in the final analysis.The most common presenting symptoms and exam findings were pain(100%),swelling(90%),and erythema(74%).The most common sonographic findings were edema(76%),soft tissue swelling(78%),and fluid surrounding the tendon(57%).POCUS was used in medical decision making 68%of the time(n=34),with the use of POCUS leading to changes in management 38%of the time(n=19).POCUS use led to early antibiotic use(11/19),early consultation(10/19),and led to the performance of a required procedure(8/19).The POCUS diagnosis was consistent with the discharge diagnosis of flexor tenosynovitis 8/12 times,abscess 12/16 times,and cellulitis 14/20 times.CONCLUSION:POCUS is beneficial for evaluating of hand infections that present to the ED and can be used as an important part of medical decision making to expedite patient care.
