BACKGrOUND Pancreaticoduodenectomy(PD)is a technically complex operation,with a re-latively high risk for *** ability to rescue patients from post-PD complications is as a recognized quality *** protocols were instituted at our low volume facility in the year *** To document the rate of rescue from post-PD complications with tailored protocols in place as a measure of *** A retrospective audit was performed to collect data from patients who experienced maj.r post-PD complications at a low volume pancreatic surgery unit in Trinidad and Tobago between j.nuary 1,2013 and j.ne 30,***-dardized definitions from the International Study Group of Pancreatic Surgery were used to define post-PD complications,and the modified Clavien-Dindo classification was used to classify post-PD *** Over the study period,113 patients at a mean age of 57.5 years(standard deviation[SD]±9.23;range:30-90;median:56)underwent PDs at this *** complications were recorded in 33(29.2%)patients at a mean age of 53.8 years(SD:±7.9).Twenty-nine(87.9%)patients who experienced maj.r morbidity were salvaged after aggre-ssive treatment of their ***(3.5%)died from bleeding pseudoaneurysm(1),septic shock secondary to a bile leak(1),anastomotic leak(1),and myocardial infarction(1).There was a significantly greater salvage rate in patients with American Society of Anesthesiologists scores≤2(93.3%vs 25%;P=0.0024).CONCLUSION This paper adds to the growing body of evidence that volume alone should not be used as a marker of quality for patients requiring *** low volumes at our facility,we demonstrated that 87.9%of patients were rescued from maj.r *** attributed this to several factors including development of rescue protocols,the competence of the pancreatic surgery teams and continuous,and adaptive learning by the entire institution,cul-minating in the development of tailored peri-pancreatectomy protoc

In situ characterization of particle formation in spray flame synthesis using wide-angle light scattering

Wide-angle light scattering(WALS)was used for in situ measurements of droplet and nanoparticle size distributions during the synthesis of titania and iron oxide particles from liquid precursor solutions in the standardized SpraySyn burner for spray flame *** was synthesized from titanium tetraisopropoxide(TTIP)and iron oxide from iron(II)nitrate nonahydrate(INN)using ethanol(EtOH)as *** images were taken at heights up to 120 mm above the burner surface and classified into droplet and particle *** size distributions were derived from a sequential analysis of scattering data containing the oscillating Mie pattern,the lognormal size distribution parameters for spherical and fractal particle fractions from a multivariate approach on averaged particle scattering *** results show that the precursor addition leads to altered evaporation behavior and even droplet disruption probably induced by puffing or micro-explosions compared to pure *** the case of TTIP(a hygroscopic alkoxide),the synthesis of a large fraction of spheres was observed,while the nitrate INN leads to the formation of mostly fractal aggregates.

A decomposition of light’s spin angular momentum density

Light carries intrinsic spin angular momentum(SAM)when the electric or magnetic field vector rotates over time.A familiar vector equation calculates the direction of light’s SAM density using the right-hand rule with reference to the electric and magnetic polarisation *** Maxwell’s equations,this vector equation can be decomposed into a sum of two distinct terms,akin to the well-known Poynting vector decomposition into orbital and spin *** present the first general study of this spin decomposition,showing that the two terms,which we call canonical and Poynting spin,are chiral analogies to the canonical and spin momenta of light in its interaction with *** canonical momentum,canonical spin is directly *** canonical and Poynting spin incorporate spatial variation of the electric and magnetic fields and are influenced by optical *** decomposition allows us to show that a linearly polarised vortex beam,which has no total SAM,can nevertheless exert longitudinal chiral pressure due to equal and opposite canonical and Poynting spins.

Coral-like structures of the Y_(3-x)Pr_(x)Fe_(5-y)Yb_(y)O_(12),(0.00 ≤ x ≤ 0.04, 0.00 ≤ y ≤ 0.02) compound were synthesized using the sol-gel method. Structural investigation certified the YIG cubic crystal structure formation, without any secondary phase. It is shown that, the relatively large ionic radius of the dopant cations results in an expansion of the lattice parameter, variations in the Iona-O-Iondangle, Iona-O,Iond-O and Ionc-O bond distances and decrease in the average crystallite size. Fourier transform infrared(FTIr) and raman measurements are essential to testify the single-phase formation of YIG crystal structure and are observed changes in the stretching and vibrational modes, respectively. The morphological study, energy dispersive spectroscopy(EDS) spectra and textural properties show corallike structures, peaks associated with Pr^(3+) and Yb^(3+) atoms and the effect of dopants on surface area,diameter, and pore volume, respectively. The optical analysis from diffuse reflectance spectra witnessed an increase in the optical gap band, a decrease in Urbach energy and blue shift in the charge transfer,correlated with the expansion of the unit cell due to the dopant's insertion in the YIG structure. A typical ferrimagnetic behavior is exhibited by the Y_(3-x)Pr_(x)Fe_(5-y)Yb_(y)O_(12)compound. The saturation magnetization(M_(s)), cubic anisotropy constant(K_(1)) and coercive field(H_(c)) increase with the Pr^(3+)cations content, as consequence of their magnetic nature and distribution around of Fe^(3+)ions due to the coexistence with the Yb^(3+). Finally, for the first time, antibacterial tests by mean of the direct contact method were performed for YIG co-doped with Pr^(3+)and Yb^(3+)and it is shown that, relatively high dosages of Pr^(3+) cations favored the activity against S. aureus, therefore, a new biological property for YIG doped with rare earths is presented.

Mobile and Internet network coverage plays an important role in digital transformation and the exploitation of new services. The evolution of mobile networks from the first generation (1G) to the 5th generation is still a long process. 2G networks have developed the messaging service, which complements the already operational voice service. 2G technology has rapidly progressed to the third generation (3G), incorporating multimedia data transmission techniques. It then progressed to fourth generation (4G) and LTE (Long Term Evolution), increasing the transmission speed to improve 3G. Currently, developed countries have already moved to 5G. In developing countries, including Burundi, a member of the East African Community (ECA) where more than 80% are connected to 2G technologies, 40% are connected to the 3G network and 25% to the 4G network and are not yet connected to the 5G network and then still a process. The obj.ctive of this article is to analyze the coverage of 2G, 3G and 4G networks in Burundi. This analysis will make it possible to identify possible deficits in order to reduce the digital divide between connected urban areas and remote rural areas. Furthermore, this analysis will draw the attention of decision-makers to the need to deploy networks and coverage to allow the population to access mobile and Internet services and thus enable the digitalization of the population. Finally, this article shows the level of coverage, the digital divide and an overview of the deployment of base stations (BTS) throughout the country to promote the transformation and digital inclusion of services.
