Early Syphilis Presenting as Thickened Penile Plaques With an Embedded recurrent Ulcer (Chancre redux): A Case report

Introduction: The manifestations of syphilis are varied, and serology can establish the diagnosis early, especially in rare cases. We report a case of chancre redux (a rare recurrence of primary syphilis), which was embedded within a secondary syphilitic penile skin plaque. There were missed opportunities at earlier diagnosis as serology for syphilis was not ordered. Case presentation: A 56-year-old man presented with thickened penile plaques for five months. He reported a small penile ulcer approximately one month prior. There was no history of other skin lesions or rash. However, clinically there was an asymptomatic, indurated ulcer embedded within a plaque which was swab-positive for syphilis by PCr. A punch biopsy of a plaque was spirochaete-positive using an immunoperoxidase stain. The patient's lesions resolved three weeks after treatment with intramuscular benzathine penicillin. Discussion: Uncommonly reported even in the preantibiotic era, chancre redux is now rare. The ulcer usually recurs at or near the site of the original chancre, and has similar morphological features. The skin lesions of secondary syphilis can exhibit remarkable morphological variety. The most common skin manifestation, a generalized macular rash, was not present at any time in this case. rather, there were only a couple of nonspecific penile plaques. If biopsy is performed, histologic findings are variable, though typically the inflammatory infiltrate includes plasma cells. A special immunoperoxidase stain can highlight spirochaetes in biopsy sections. Conclusion: This case highlights the importance of considering syphilis in the differential diagnosis of persistent, atypical penile lesions and underscores the need for appropriate serological testing in such instances. Mathematical modelling of ball and plate system with experimental and correlation function-based model validation

The ball and plate system is an inherently nonlinear under actuated benchmark system used for validating the performance of various control schemes. A mathematical model depicting the dynamics close to that of the system is very much required for such a test bed. In this correspondence, the complete nonlinear model, a simplified nonlinear model, and a linearized model of the ball and plate system are developed. The system com.rises a ball and plate mechanism and a rotary servo unit. The ball and plate mechanism is modelled using the Euler–Lagrange method, whereas the rotary servo subsystem is modelled from the first principles. The nonlinear model of the combined system is developed by including the dynamics of the servo motor with gear and rolling resistance between the ball and the plate. The simplified nonlinear model of the system is obtained with suitable assumptions. The model is linearized around the operating point using the j.cobian. The validity of the developed models is investigated through correlation function analysis. The open-loop response of the three models, viz., nonlinear, simplified nonlinear, and linearized models, is analyzed in the MATLAB/Simulink platform. Since the open-loop system is unstable, the experimental validation of the model is performed with a double-loop PSO (particle swarm optimization) PID control scheme.

In this paper,we analyzed the physical characteristics of the ternary alloys rE_(2)rh_(3)Ge(rE=Pr and Sm)using the first principles calculation based on functional density theory integrated in the Wien2k *** calculated formation energies of Pr_(2)rh_(3)Ge and Sm_(2)rh_(3)Ge are−5.3827 and−5.2782 eV/atom.respectively,showing that these alloys may exist *** results of the band structure and density of states indicate that all of the compounds tested are magnetic metals,and their magnetism is mostly related to the contribution of Pr and Sm *** total magnetic moments of Pr_(2)rh_(3)Ge and Sm_(2)rh_(3)Ge are 4.07245 and 11.16161μB/f.u,*** parameters such as heat capacity at constant pressure,entropy,Debye temperature,lattice volume,com.ressibility coefficients,and Grüneisen parameter alloys were calculated using the Gibbs algorithm and studied in depth over a large temperature range of 0-900 *** Debye temperature increases with increasing pressure and decreases with decreasing ***,the Debye temperatures calculated at p=0 GPa and T=300 K are equal to be 342 and 322 K for Pr_(2)rh_(3)Ge and Sm_(2)rh_(3)Ge,*** optical characteristics such as the real and imaginary portions of the dielectric function,the absorption coefficient,the energy-loss function,and the refractive index were investigated and discussed in more depth.

On an isotropic porous solid cylinder:the analytical solution and sensitivity analysis of the pressure

Within this work,we perform a sensitivity analysis to determine the influence of the material input parameters on the pressure in an isotropic porous solid *** provide a step-by-step guide to obtain the analytical solution for a porous isotropic elastic cylinder in terms of the pressure,stresses,and elastic *** obtain the solution by performing a Laplace transform on the governing equations,which are those of Biot's poroelasticity in cylindrical polar *** enforce radial boundary conditions and obtain the solution in the Laplace transformed domain before reverting back to the time *** sensitivity analysis is then carried out,considering only the derived pressure *** analysis finds that the time t,Biot's modulus M,and Poisson's ratio ν have the highest influence on the pressure whereas the initial value of pressure P_(0) plays a very little role.

Single-atom Pt on carbon nanotubes for selective electrocatalysis

Utilizing supported single atoms as catalysts presents an opportunity to reduce the usage of critical raw materials such as platinum,which are essential for electrochemical reactions such as hydrogen oxidation reaction(HOr).Herein,we describe the synthesis of a Pt single electrocatalyst inside single-walled carbon nanotubes(SWCNTs)via a redox *** via electron microscopy,X-ray photoelectron microscopy,and X-ray absorption spectroscopy show the single-atom nature of the *** electrochemical behavior of the sample to hydrogen and oxygen was investigated using the advanced floating electrode technique,which minimizes mass transport limitations and gives a thorough insight into the activity of the *** single-atom samples showed higher HOr activity than state-of-the-art 30%Pt/C while almost no oxygen reduction reaction activity in the proton exchange membrane fuel cell operating *** selective activity toward HOr arose as the main fingerprint of the catalyst confinement in the SWCNTs.

A hydrodynamic modelling of wave-activated bodies for wavelet marine regions

recently, most wave energy research has targeted sea areas with high power density under extreme wave conditions for deployment. This trend often leads to higher energy costs for wave power plants. However, many marine areas with low power density, characterized by wavelet conditions and less destructive forces than extreme wave conditions, remain underexplored. Therefore, this study aimed to propose a wave-activated body model for sea regions with wavelet conditions. The wave-activated body design process encompassed site selection, parameter determination, geometry design, comparison and performance evaluation using the ANSYS (r) AQWA model. The results indicated that the proposed device achieved the desired heave motion, with an amplitude range of 1.2 to -2.5 m, validating its potential for deployment in marine regions with wavelet conditions. Notably, while the proposed design is optimized for wavelet conditions, it was found to have potential limitations in extreme wave environments. This observation emphasizes the challenge of formulating a generalized design suitable for both conditions. Consequently, it is pivotal for wave-activated body designs to be customized based on the specific ocean conditions they target, underscoring the need for specialized designs of wave energy converters that consider the unique wave characteristics of their deployment sites. A wave-activated body is designed for energy generation in ocean regions with wavelet conditions. Hydrodynamic modelling is carried out for two locations with different wave characteristics to determine the optimum conditions for deployment. Graphical Abstract

Dermatomyositis, an idiopathic inflammatory myopathy, is characterized by distinctive skin manifestations, proximal muscle weakness, and multiple organ involvement and can be accompanied by malignancies. To provide a reference for dermatologists and clinicians in other relevant fields of clinical practice, experts from the Dermatology Branch of the China International Exchange and Promotion Association for Medical and Health Care and the National Clinical research Center for Dermatologic and Immunologic Diseases developed this consensus on the diagnosis and treatment of adult dermatomyositis using Chinese and international literature and expert advice. © 2024 Wolters Kluwer Health. Stand dynamics of old-growth hemlock forests in central Bhutan are shaped by natural disturbances

Understanding how past disturbances have influenced the development of forests is critical for deciphering their current structure and composition and forecasting future *** this study,dendrochronological methods were applied to uncover the disturbance history of old-growth hemlock-dominated forests in central *** of tree-ring samples from two old-growth hemlock stands,located in two different topographic settings,identified the importance of gap-phase dynamics in facilitating recruitment and growth releases and producing complex,multi-aged structure s over *** site showed evidence of a near stand-replacing disturbance in the late 1700s,while the other showed no evide nce of high-severity disturbance at any time over the last 400 *** both sites low-to medium-severity disturbances,some of which appear to be associated with cyclones originating in the Bay of Bengal,dominated the disturbance *** hemlock stands exhibited a significant positive association between cyclone occurrence and growth release events and between recruitment pulses and growth release *** 1800 to 1970 there was an increase in recruitment of angiosperm tree species at most sites and a corresponding decline in conifer *** the past 50 years there has been little new recruitment;this may be due to light limitation in the understory from shade-tolerant angiosperms and bamboo in the lower strata of these *** variations in disturbance dynamics and recruitment were observed across the study sites,suggesting that other factors,such as topography and climate,may be influencing long-term stand development *** study highlights the complex interplay between historical disturbance regimes and tree recruitment in shaping the age and size structures of old-growth hemlock forests in central *** also provides new insights into the dynamics of these forests that can be used to support effective forest conservation and management i
