Predicting morbidity and mortality in acute pancreatitis in an Indian population:a comparative study of the BISAP score,Ranson’s score and CT severity index

Objective:Our aim was to prospectively evaluate the accuracy of the bedside index for severity in acute pancreatitis(BISAP)score in predicting mortality,as well as intermediate markers of severity,in a tertiary care centre in east central India,which caters mostly for an economically underprivileged ***:A total of 119 consecutive cases with acute pancreatitis were admitted to our institution between November 2012 and October *** scores were calculated for all cases,within 24 hours of ***’s score and computed tomography severity index(CTSI)were also *** respective abilities of the three scoring systems to predict mortality was evaluated using trend and discrimination *** optimal cut-off score for mortality from the receiver operating characteristics(ROC)curve was used to evaluate the development of persistent organ failure and pancreatic necrosis(PNec).Results:Of the 119 cases,42(35.2%)developed organ failure and were classified as severe acute pancreatitis(SAP),47(39.5%)developed PNec,and 12(10.1%)*** area under the curve(AUC)results for BISAP score in predicting SAP,PNec,and mortality were 0.962,0.934 and 0.846,***’s score showed a slightly lower accuracy for predicting SAP(AUC 0.956)and mortality(AUC 0.841).CTSI was the most accurate in predicting PNec,with an AUC of *** sensitivity and specificity of BISAP score,with a cut-off of≥3 in predicting mortality,were 100%and 69.2%,***:The BISAP score represents a simple way of identifying,within 24 hours of presentation,patients at greater risk of dying and the development of intermediate markers of *** risk stratification method can be utilized to improve clinical care and facilitate enrolment in clinical trials.

目的:使用实验性糖尿病Wistar大鼠模型验证五桠果(Dillenia indica)甲醇提取物的生物活性成分(乙酸乙酯馏分)的降血糖及降血脂作用。方法:通过腹腔内注射链脲佐菌素(60mg/kg)建立大鼠1型糖尿病模型,腹腔内注射烟酰胺(120mg/kg)15min后腹腔内注射链脲佐菌素(60mg/kg)建立大鼠2型糖尿病模型。在不同的实验模型中,用药组大鼠均分别给予200mg/kg和400mg/kg的五桠果甲醇提取物的乙酸乙酯馏分连续21d。使用血糖仪每周1次测定各组大鼠血糖水平。使用自动分析仪器分别测定各组大鼠血清总胆固醇、三酰甘油及高密度脂蛋白胆固醇水平。结果:在两个实验模型中,与模型组比较,五桠果甲醇提取物的乙酸乙酯馏分均能显著降低模型大鼠的血糖水平(P<0.01)以及血清总胆固醇(P<0.05)和三酰甘油(P<0.05)含量,升高高密度脂蛋白胆固醇水平(P<0.05)。结论:五桠果甲醇提取物的乙酸乙酯馏分在400mg/kg剂量时,对于1型及2型糖尿病模型大鼠具有显著的治疗作用。因此这种提取物有望被研究开发成为治疗糖尿病的又一替代药物。

Objective:To investigate the contraceptive effect of intratesticular injection of chlorhexidine gluconate in dogs and compare it with that of zinc gluconate neutralized with ***:Twenty-four sexually mature male mongrel dogs were randomly divided by replicate into four groups(n=6 per group).GroupⅡreceived intra-testicular injection of 2 mL zinc gluconate(10 mg/mL)neutralized with ***Ⅱreceived intratesticular injection of 2 mL chlorhexidine gluconate(5%w/v).GroupⅢreceived intratesticular injection of 2 mL chlorhexidine gluconate(4%w/v).GroupⅣdid not receive any treatment and served as the control *** morphometry was conducted on day 0,7,15 and 30 after *** was collected and evaluated on day 0 and *** were analyzed using repeated measures analysis of ***:There was no difference in the mean values of various parameters between dogs treated with zinc gluconate and those treated with chlorhexidine gluconate at any of the time *** dogs treated with zinc gluconate or chlorhexidine gluconate,there was a significant increase in the testicular morphometric parameters on day 7 followed by a significant reduction thereafter(day 15 and 30).In contrast,there was no change in any of the parameters in the control untreated dogs during the course of the *** to the pre-treatment values,the mean scrotal circumference and the mean paired testicular volume and testicular weight on day 30 were significantly lower in the treated *** samples collected on day 30 from treated dogs were found to be azoospermic,whereas no change in semen quality was observed in the control untreated ***:Intratesticular injection of chlorhexidine gluconate(5%w/v and 4%w/v)is equally as effective as zinc gluconate neutralized with arginine for chemical contraception in dogs.

Numerical and Dimensional Analysis for Prediction of Line Heating Residual Deformations

Line heating process is a very complex phenomenon as a variety of factors affects the amount of residual deformations. Numerical thermal and mechanical analysis of line heating for prediction of residual deformation is time consuming. In the present work dimensional analysis has been presented to obtain a new relationship between input parameters and resulting residual deformations during line heating process. The temperature distribution and residual deformations for 6 mm, 8 mm, 10 mm and 12 mm thick steel plates were numerically estimated and compared with experimental and published results. Extensive data generated through a validated FE model were used to find co-relationship between the input parameters and the resulting residual deformation by multiple regression analysis. The results obtained from the deformation equations developed in this work compared well with those of the FE analysis with a drop in the computation time in the order of 100 (computational time required for FE analysis is around 7 200 second to 9 000 seconds and where the time required for getting the residual deformation by developed equations is only 60 to 90 seconds).

Effects of Thermal Diffusion and Chemical Reaction on MHD Flow of Dusty Visco-Elastic (Walter’s Liquid Model-B) Fluid

The present note consists, the effects of thermal diffusion and chemical reaction on MHD flow of dusty viscous incom-pressible, electrically conducting fluid between two vertical heated, porous, parallel plates with heat source/sink. The plate temperature is raised linearly with time and concentration level near the plate to Cw. The variable temperature and uniform mass diffusion taking into account the chemical reaction of first order. The series solution method is used to solve the mathematical equations. Effects of various parameters like chemical reaction (K), thermal diffusion (ST) and magnetic field (M) etc. on velocity profile, skin friction, concentration profile and temperature field are displayed graphically and discussed numerically for different physical parameters. The analysis developed here for thermal diffusion, bears good agreement with real life problems.

Role of angiotensin converting enzyme and angiotensinogen gene polymorphisms in angiotensin converting enzyme inhibitor-mediated antiproteinuric action in type 2 diabetic nephropathy patients

AIM To investigate the role of genetic variants of angiotensin converting enzyme(ACE) and angiotensinogen(AGT) genes in the antiproteinuric efficacy of ACE inhibitor therapy in diabetic nephropathy(DN) *** In the present study, 270 type 2 diabetes mellitus patients with nephropathy were enrolled and treated with ACE inhibitor(ramipril) and followed at 6 mo for renal function and albumin excretion by estimating serum creatinine, end stage renal disease, and albumin/creatinine ratio(ACR) in urine. Genotyping of ACE I/D and AGT M235 T polymorphisms were performed by using primer specific polymerase chain reaction(PCR) and PCR-RFLP techniques, respectively. RESULTS Forty-eight percent of DN patients(responders) benefited with respect to proteinuria from ACE inhibitor therapy at 6 mo follow-up. A significant reduction in ACR was observed after 6 mo treatment with ACE inhibitor irrespective of whether DN patients were micro-albuminuric(≥ 30 and < 300 mg/g creatinine) or macro-albuminuric(≥ 300 mg/g creatinine) at the time of enrollment. However, macro-albuminuric patients(55%) showed better response to therapy. A reduction in urinary ACR was found independent of genotypes of ACE I/D and AGT M235 T polymorphisms although macro-albuminuric patients having TT genotype showed statistically insignificant increased response(72%). CONCLUSION ACE inhibitor therapy reduced urinary ACR by ≥ 30% in 50% of DN patients and the response is independent of ACE I/D and AGT M235 T polymorphisms.

Moringa oleifera Lam.(Shigru)(Moringaceae family) is a traditional medicine used for control of diabetes, obesity, asthma, and cardiac,liver, gastrointestinal, infective, and brain disorders, such as depression and Alzheimer's disease. In Ayurvedic literature, Shigru is among few drugs having Balya(nourishing) as well as Medohara(antiobesity) property. This review focuses on valid connections between the properties documented in ancient literature and current pharmacological knowledge of Moringa, including pharmacological actions, phytochemistry,botanical description, and how Moringa can tackle malnutrition in India, especially Chhattisgarh. All information about M. oleifera was obtained from electronic scientific databases such as PubMed, Web of Science, Scopus, ScienceDirect, Elsevier, Google Scholar, Traditional Knowledge Digital Library, and Indian Traditional Books(Ancient Ayurveda literatures, The Wealth of India, and The Ayurvedic Formulary of India), postgraduate/doctoral thesis, and googling the keyword M. oleifera. M. oleifera have anti-oxidant, antimicrobial, anti-diabetic,anti-obesity, anti-inflammatory, cardioprotective, hepatoprotective, neuroprotective, gastroprotective, wound-healing properties and it can potentially tackle malnutrition. This review describes the key information related to botanical description of M. oleifera, phytochemistry,pharmacological actions, clinical studies, toxicological studies, better utilization as food therapeutics, and ethnobotanical and evidence-based comparative review of M. oleifera. M. oleifera can effectively tackle malnutrition in India, especially Chhattisgarh. The authors emphasize the need for future in-depth ethnopharmacological lead-based research and clinical studies to expand M. oleifera pharmacological activities,clinical efficacy, and safety.

The goal of this project is to use the Semantic Web Technologies and Data Mining for disease diagnosis to assist health care professionals regarding the possible medication and drug to prescribe (Drug recommendation) according to the features of the patient. Numerous Decision Support Systems (DSS) and Expert Systems allow medical collaboration, like in the differential diagnosis specific or general. But, a medical recommendation system using both Semantic Web technologies and Data mining has not yet been developed which initiated this work. However, it should be mentioned that there are several system references about medicine or active ingredient interactions, but their final goal is not the Drug recommendation which uses above technologies. With this project we try to provide an assistant to the doctor for better recommendations. The patient will also able to use this system for explanation of drugs, food interaction and side effects of corresponding drugs.

Linking a Simulated Annealing Based Optimization Model with PHT3D Simulation Model for Chemically Reactive Transport Processes to Optimally Characterize Unknown Contaminant Sources in a Former Mine Site in Australia

Historical mining activities often lead to continuing wide spread contaminants in both groundwater and surface water in previously operational mine site areas. The contamination may continue for many years after closing down the mining activities. The essential first step for sustainable management of groundwater and development of remediation strategies is the unknown contaminant source characterization. In a mining site, there are multiple species of contaminants involving complex geochemical processes. It is difficult to identify the potential sources and pathways incorporating the chemically reactive multiple species of contaminants making the source characterization process more challenging. To address this issue, a reactive transport simulation model PHT3D is linked to a Simulated Annealing based the optimum decision model. The numerical simulation model PHT3D is utilized for numerically simulating the reactive transport process involving multiple species in the former mine site area. The simulation results from the calibrated PHT3D model are illustrated, with and without incorporating the chemical reactions. These comparisons show the utility of using a reactive, geochemical transport process’ simulation model. Performance evaluation of the linked simulation optimization methodology is evaluated for a contamination scenario in a former mine site in Queensland, Australia. These performance evaluation results illustrate the applicability of linked simulation optimization model to identify the source characteristics while using PHT3D as a numerical reactive chemical species’ transport simulation model for the hydro-geochemically complex aquifer study area.

Electron Monopole Duality in Quantum Hall Effect

Starting from the duality between electric and magnetic field, we have made an attempt to discuss the quantum hall effect from the consideration of magnetic monopole in view of electron monopole duality. Starting from the dual dy-namics of electric and magnetic charges, we have reformulated a consistent theory of quantum hall effect in presence of monopole. Speculating the existence of magnetic monopoles in magnetic materials (metals), we have accordingly modi-fied the parameters;like drift velocity, current density, Hamiltonian and eigen values and eigen function for harmonic oscillator;resposible to examine the quantum Hall effect in metals.
