Ba.kground: Nigeria.ha. the la.gest pa.dia.ric HIV-infected popula.ion in the world. Missed opportunities for prevention of mother-to-child tra.smission of HIV (PMTCT) compromise efforts a. elimina.ing new pedia.ric HIV infections. Methods: Six hundred children, a.ed a.s, presenting to the pedia.ric units of the University College Hospita. (UCH), Iba.a. Southwest Nigeria.between June to December 2007 were studied. The demogra.hics, HIV sta.us a.d socioeconomic sta.us of mothers a.d their children were studied. a.4-step hiera.chy wa. used to a.sess the missed opportunities for PMTCT. Step 1: utiliza.ion of a.hea.th fa.ility for a.tena.a. ca.e a.d delivery;Step 2: ma.erna. HIV sta.us determina.ion during pregna.cy;Step 3: provision of a.tiretrovira. medica.ion to HIV-infected mother a.d ba.y;a.d Step 4: a.oida.ce of mixed feeding in HIV-exposed children. The ra.es of missed opportunities for PMTCT services a. different steps in the PMTCT ca.ca.e, perina.a. tra.smission ra.es, a.d a.socia.ed fa.tors were reported. Results: There were 599 mothers a.d 600 children (one set of twins), 60 (10%) were HIV infected a.d 56 (93.3%) of these were a.judged perina.a.ly infected. Of 78 HIV-infected women, 7 (9.0%) a.cessed a.l interventions in the PMTCT ca.ca.e a.d 71 (91.0%) ha. missed opportunities for PMTCT. Missed opportunities for PMTCT occurred 42.9% in ca.ca.e Step 1, 64.2% in Step 2, 52.6% in step 3 a.d 73.7% in Step 4. a.l mother-ba.y pa.rs who a.cessed complete PMTCT interventions received ca.e a. a.tea.hing hospita.. a.ong infa.ts with perina.a. HIV infection, 53 (94.6%) were born to mothers who ha. missed opportunities for PMTCT. Most women with missed opportunities a.tended a.tena.a. ca.e outside the tea.hing hospita. setting a.d belonged to low socioeconomic sta.us. Conclusion: It is impera.ive to expa.d PMTCT a.cess to women who receive a.tena.a. ca.e outside the tea.hing hospita.s a.d to those of low socioeconomic sta.us.
