Desmopression is an effective adjunct treatment for reversing excessive hyponatremia overcorrection

We report a case of a 50-year-old malnourished African American male with hiccups, nausea and vomiting who was brought to the Emergency Department after repeated seizures at home. Laboratory evaluations revealed sodium(Na+) 107 mmol/L, unmeasurably low potassium, chloride < 60 mmol/L, bicarbonate of 38 mmol/L and serum osmolality 217 m Osm/kg. Seizures were controlled with 3% saline Ⅳ. Once nausea was controlled with iv antiemetics, he developed large volume free water diuresis with 6 L of dilute urine in 8 h(urine osmolality 40-60 m Osm/kg) and serum sodium rapidly rose to 126 mmol/L in 12 h. Both intravenous desmopressin and 5% dextrose in water was given to achieve a concentrated urine and to temporarily reverse theacute rise of sodium, respectively. Serum Na+ was gradually re-corrected in 2-3 mmol/L daily increments from 118 mmol/L until 130 mmol/L. Hypokalemia was slowly corrected with resultant auto-correction of metabolic alkalosis. The patient discharged home with no neurologic sequaele on the 11 th hospital day. In euvolemic hyponatremic patient, controlling nausea may contribute to unpredictable free water diuresis. The addition of an antidiuretic hormone analog, such as desmopressin can limit urine output and prevent an unpredictable rise of the serum sodium.

In this paper, we consider T-S fuzzy systems with redundant actuators and design a novel adaptive fault-tolerant control scheme to compensate their actuator failures. The advantage of the proposed method is that it can deal with both the parameter uncertainties and failure uncertainties （i.e. the occurrence time, size and mode of the failure are unknown）, and does not require a fault detection unit. A unified control structure is designed to accommodate different failure situations. When failures occur, the parameter adaptation law can tune the controller parameters automatically to compensate the effects of failure and achieve asymptotical state tracking. Simulation results have demonstrated the effectiveness of the proposed scheme.

次贷危机爆发之后、特别是雷曼破产后,各国政府推出了许多政策措施,目的在于重建市场信心,最明显的途径是向系统重要银行提供正式或非正式的担保。但当银行负债不得不成为政府的或有负债时,就会彻底改变市场对政府自身信用的评价。在希腊等体质较弱的欧洲外围国家,会把政府和银行一道带入恶性循环。不过,欧元的结构比通常理解的更为坚固,脱离欧元是一种代价昂贵的违约方式,相信这是最不可能发生的;相比之下,政府债务重组对经济的负面影响较小,因而更可能被采用。

腐植酸被认为在动物喂饲试验中对健康和产率具有整体积极作用,同时颇有争议的是,其也被作为肿瘤的病原学因素之一。我们尝试评价斯洛伐克地区特定来源腐植酸在使用推荐预防剂量时的体外作用。研究其体外的抗氧化性质、对肝线粒体中酶系和非酶系抗氧化防御系统的作用以及对人工培养肿瘤细胞株的作用。我们观察到不论腐植酸是溶解于二甲基亚砜(DMSO)或是直接加入呼吸介质中线粒体悬浮液,在腐植酸给药后超氧化物歧化酶(SOD)活性显著下降。所测定的其他抗氧化酶如谷胱甘肽过氧化物酶(GPx)和谷胱甘肽还原酶(GR),其活性及还原型谷胱甘肽(GSH)的含量与对照组相比并未表现出显著变化。腐植酸对超氧阴离子自由基(O2-·)的抑制率低于其对羟自由基(·OH)的抑制率。6种不同肿瘤细胞株的存活率显示出只有急性T淋巴性白血病细胞株对测试腐植酸敏感。尽管在水溶液中溶解度相对较低,但特定来源腐植酸参与了氧化还原调控。通过夺取自由基,腐植酸重置了抗氧化防御机制。来源于自然资源的腐植酸体外研究结果显示出其作为有前景的免疫增强剂的潜力。

＜正＞ The hydrothermal diamond-anvil cell （HDAC） was designed for the study of geologic processes in the crust that occur in the presence of water or other fluids （Bassett et al., 1993）. The advantages of the HDAC include: （1） its wide applicable P-T range （temperatures from -190 to 1200℃and pressures up to 10 GPa）（2） its low cost, （3） its safe operation, （4） its ease of loading samples and capability of varying bulk sample density without reloading, （5） the allowance of in-situ sample characterization through optical observation and spectroscopic analysis, including Raman, FTIR, and synchrotron X-ray, and （6） the ease of keeping permanent records on video tapes.
