Outcome of Very premature Newborns in a Referral Hospital in a Resource-Limited Setting

Background: In Cameroon, prematurity is considered among the first cause of neonatal mortality and the main cause of sequelae in children under 5 years old. Although some local teams have studied the causes of neonatal deaths, the survival of the very preterm babies in our context remains poorly known. patients and Methods: We conducted a hospital-based cross-sectional study with both a retrospective and prospective data collection, covering a period of 5 years and 8 months, including 120 participants who presented with very preterm birth. Socio-demographic, anamnestic, and outcome features were studied. The assessment of the child’s psychomotor development was evaluated trough gross motor skills, fine motor skills, language and social contact. We calculated the development quotient (DQ) by dividing the developmental age (DA) by the actual age (ra) of the patient. Qualitative variables were expressed as numbers and percentages and quantitative variables as means ± standard deviations. Results: At the clinic, 23.8% of the children developed cerebral palsy, 3.1% had cerebral palsy with mental delay, and 6.6% had praxis disorders. Hearing impairment was observed in 6.3% of the survivors, visual impairment in 9%, and swallowing disorders in 7.6%. The mean developmental quotient was (89.98 ± 19.7) with a median of 93. A delay in developmental milestones was observed in 10.8%, speech disorder in 57%, severe malnutrition in 7.7%. The mortality rate was 48.5%. Conclusion: Very preterm birth is associated with a higher risk of neonatal death. Cognitive and motor disorders should not be underestimated. The harmonised management of very premature babies is highly recommended in our context for early diagnosis of sequelae.

Background: perinatal asphyxia is a common cause of mortality and of morbidity including motor and neurodevelopmental abnormalities. The aim of this study was to evaluate the post-hospital outcome of neonates treated for perinatal asphyxia at the Douala Gynaeco-Obstetric and pediatric Hospital (DGOpH) in Cameroon. patients and Methods: We conducted a hospital-based cross-sectional study with both a retrospective and prospective data collection, conducted over a period of 3 months and involving neonates above 34 weeks of gestational age who were managed for perinatal asphyxia at DGOpH from August 2015 to February 2020. Socio-demographic, perinatal, motor, nutritional and neuro-developmental out-of-hospital data were recorded. The assessment of the child’s psychomotor development was evaluated through gross motor skills, fine motor skills, language and social contact. We calculated the development quotient (DQ) by dividing the developmental age (DA) by the actual age (ra) of the patient. The data were entered and analyzed using excel and Stata version 15 software. Results: Among the 58 newborns included in our study, males were the most represented (59%). The mean age was 36.5 ± 14.16 months (Extremes: 12 months and 66 months). The majority of patients were born at term (79%), had a birth weight between 2500 and 4000 grams (69%), were resuscitated (95%), and had an Apgar score p>thp> minute of life (67%). SARNAT stages II and III counted for 48%. Neurodevelopmental abnormalities were found in 25.5% of patients with gross motor delay (mainly tetraparesis) representing 23.5%, fine motor delay 27.5%, impairment in social contact 31% language speech delay. The majority of the children had a normal development quotient (78.4%). Conclusion: The short-term and long-term outcome of newborns who experienced perinatal asphyxia in our setting is marked by numerous impairments in developmental milestones leading to disability.

目的分析2015—2021年甘肃省永昌县结核病的流行特征,并利用灰色预测模型GM(1,1)预测其短期发病趋势,为结核病的重点人群预防和控制措施的制订提供一定的理论支撑。方法从中国疾病预防控制信息系统子系统传染病监测收集2015—2021年甘肃省永昌县居民中结核病发病数据,采用描述流行病学方法分析结核病发病现状与流行特征,采用R 4.1.0软件建立结核病年发病人数的模型并预测其短期发病趋势。结果 2015—2021年永昌县结核病报告发病数为604例,结核病发病总体呈缓慢上升趋势(趋势χ^(2)=7.690,pp>0.05),六坝镇报告发病最多、占18.38%;不同性别结核病构成差异无统计学意义(χ^(2)=6.579,p>0.05),男女性别比2.07∶1;以61~75岁发病为主、占52.81%,不同年龄组结核病构成差异有统计学意义(χ^(2)=45.999,pp>0.05)。灰色预测模型GM(1,1)预测显示,2015—2021年结核病拟合发病数与实际发病数基本吻合,C=0.346,p=0.992,模型预测精度高,预测2022年和2023年结核病发病数为24例和17例。结论 2015—2021年永昌县结核病发病总体呈缓慢上升趋势,六坝镇为重点排查乡(镇);疾控部门应注意“三早”预防与基础健康教育普及,加强对高发月份重点监控。灰色预测模型GM(1,1)预测该县结核病发病效果较好,且显示2022—2023年结核病发病数将持续下降。

AIM:To evaluate the prognostic value of pretreatment F D G p o s i t r o n e m i s s i o n t o m o g ra.p h y c o m p u t e d tomography(pET-CT) in patients with hepatocarcinoma treated by liver transplantation(LT).METHODS:The authors retrospectively analyzed the data of 27 patients(mean age 58 ± 9 years) who underwent FDG pET-CT before LT for *** follow-up was 26 ± 18 *** FDG pET/CT was performed according to a standard clinical protocol:4 MBq FDG/kg body weight,uptake 60 min,low-dose non-enhanced *** authors measured the SUVmax and SUVmean of the tumor and the normal *** tumor/liver activity ratios(RSUVmax and RSUVmean) were tested as prognostic factors and compared to the following conventional prognostic factors:MILAN,CLIp,OKUDA,TNM stage,alphafoetoprotein level,portal thrombosis,size of the largest nodule,tumor differentiation,microvascular invasion,underlying cirrhosis and liver ***:Overall and recurrence free survivals were80.7%and 67.4%at 3 years,and 70.6%and 67.4%at 5 years,*** to a multivariate Cox model,only FDG pET/CT RSUVmax predicted recurrence free *** though the MILAN criteria alone were not predictive,it is worth noting that none of the patients outside the MILAN criteria and with RSUVmaxpET/CT with an RSUVmax cutoff value of 1.15 is a strong prognostic factor for recurrence and death in patients with HCC treated by LT in this retrospective *** prospectivestudies should test whether this metabolic index should be systematically included in the preoperative assessment.

Introduction: Rheumatoid arthritis (ra) is the most common chronic inflammatory rheumatism. The increasing use of bone and joint ultrasound as a tool to aid in the detection of unrefined synovitis in developed countries, has led us to study the performance of ultrasound in the diagnosis of ra in subjects consulting in rheumatology at Cotonou. Study Method: Descriptive cross-sectional study of patients with ra, selected on the basis of ACR/ EULAR 2010 criteria. A complete clinical examination with imaging including X-rays and ultrasounds of the hands and feet were performed in all patients who have given their consent. The data collected was analyzed using EpI data 3.1 and SpSS 24.0 software. The significance rate was defined with a p-value ppa (k) test was used to assess agreement. Results: All 53 patients included had lesions on ultrasound. Synovitis was found in 98.1% and bone erosions in 88.7%. There was no agreement in the wrists between clinical examination and ultrasound (k = -0.116), MCp (k = -0.097) and MTp (k = -0.031). Agreement was very low at the ppI level (k = 0.03). It was low at the different sites, between ultrasound and radiographic detection of lesions (k between 0.022 - 0.22). Age, positive immunologic ra factor and biological inflammatory syndrome were associated with ultrasound erosions (p = 0.0001) and only age was associated with active synovitis (p = 0.022). Conclusion: Bone and articular ultrasound is a good complementary alternative to clinical examination and radiography diagnostic in early diagnosis of ra in our developing countries, in the absence of MRI which remains a luxury.
