Origin of and therapeutic approach to cardiac syndrome X:Results of the proton pump inhibitor therapy for angina-like lingering pain trial (PITFalL trial)

AIM: To investigate the frequency of gastroen-terological diseases in the etiology and the efficacy of proton pump inhibitors (PPIs) in the treatment of cardiac syndrome X (CSX) as a subform of non-cardiac chest pain (NCCP). METHODS: We investigated 114 patients with CSX using symptom questionnaires. A subgroup of these patients were investigated regarding upper gastrointestinal disorders (GIs) and treated with PPI. Patients not willing to participate in investigation and treatment served as control group. RESULTS: Thirty-six patients denied any residual symptoms and were not further evaluated. After informed consent in 27 of the remaining 78 patients, we determined the prevalence of disorders of the upper GI tract and quantifi ed the effect of treatment with pantoprazole. We found a high prevalence of gastroenterological pathologies (26/27 patients, 97%)with gastritis, gastroesophageal reflux disease (GERD) and acid reflux as the most common associated disorders. If treated according to the study protocol, these patients showed a significant improvement in the symptom score. Patients treated by primary care physicians, not according to the study protocol had a minor response to treatment (n = 19, -43%), while patients not treated at all (n = 26) had no improvement of symptoms (-0%). CONCLUSION: Disorders of the upper GI tract are a frequent origin of CSX in a German population and can be treated with pantoprazole if given for a longer period.

The metabolite lactate (L-lactate) can be generated and released by diverse brain cells,including neurons,astrocytes,and oligodendrocytes (Kann,2023;Rae et al.,2024).Lactate production usually requires the degradation of glucose (D-glucose)-and glycogen in astrocytes-to pyruvate by glycolysis and subsequent conversion of pyruvate to lactate by the enzyme lactate dehydrogenase(Figure 1A;Dienel,2019;Rae et al.,2024).

Anterior cruciate ligament reconstruction:Effect of graft tunnel position on early to mid-term clinical outcomes

BACKGROUND Patient reported outcome measures(PROMs)can be used to assess knee function following anterior cruciate ligament(ACL)***-operatively,femoral and tibial tunnels are created to accommodate the new ACL *** is postulated that there is an optimum position and orientation of these tunnels and that outcomes from this procedure are affected by their *** To evaluate the influence of graft tunnel position on early to mid-term clinical outcomes following ACL *** Six PROMs were collected following ACL reconstruction which included the Knee Injury and Osteoarthritis Outcome Score(KOOS),International Knee Documentation Committee,Lysholm,Tegner,EuroQol-5 Dimension-5 level,and Short Form 12-item Health Survey.A total of 8 radiological parameters were measured from post-operative X-rays relating to graft tunnel *** data was analysed to assess for any correlations between graft tunnel position and postoperative *** A total of 87 patients were included in the study with a mean post-operative follow-up of 2.3 years(range 1 to 7 years).Posterior position of tibial tunnel was associated with improved KOOS quality of life(rho=0.43,P=0.002)and EQ-5D VAS(rho=0.36,P=0.010).Anterior position of EndoButton femoral tunnel was associated with an improved EQ-5D index(rho=-0.38,P=0.028).There were no other significant correlations between any of the other radiological parameters and PROM *** Overall,graft tunnel position had very little correlation with clinical outcomes following ACL reconstruction.A few(posterior)tibial tunnel and(anterior)EndoButton femoral tunnel measurements were associated with better PROMs.

ON KARUSH-KUHN-TUCKER POINTS FOR A SMOOTHING METHOD IN SEMI-INFINITE OPTIMIZATION

We study the smoothing method for the solution of generalized semi-infinite optimization problems from （O. Stein, G. Still： Solving semi-infinite optimization problems with interior point techniques, SIAM J. Control Optim., 42（2003）, pp. 769-788）. It is shown that Karush-Kuhn-Tucker points of the smoothed problems do not necessarily converge to a Karush-Kuhn-Tucker point of the original problem, as could be expected from results in （F. Facchinei, H. Jiang, L. Qi： A smoothing method for mathematical programs with equilibrium constraints, Math. Program., 85（1999）, pp. 107-134）. Instead, they might merely converge to a Fritz John point. We give, however, different additional assumptions which guarantee convergence to Karush-Kuhn：Tucker points.

In sexual reproduction different types of symbiotic relationships between insects and microbes have become established. For example, some bacteria have evolved almost exclusive vertical transmission and even define the compatibility of insect mating partners. Many strictly sexually transmitted diseases have also been described in insects. Apart from such rather specific relationships the role of opportunistic infections in the reproductive process has been widely neglected. Opportunistic microbes transmitted passively during mating might impose an energetic cost, as the immune system will need to be alert and will use resources to fight potential intruders. Through mating wounds and contaminated reproductive organs opportunistic microbes might be transferred to mating partners and even enter the body cavity. Females as the ＂receiving＂ sex are particularly likely to have evolved adaptations to avoid or reduce opportunistic infections. Males of several species show highly complex seminal fluids, which as well as containing components that influence a males＇ fertilization success, also possess antimicrobial substances. The role of antimicrobials in the reproductive process is not well understood. Some evidence hints at the protection of sperm against microbes, indicating a role for natural selection in shaping the evolution of reproductive traits. By highlighting the potential importance of microbes in sexual selection and their role in reproduction in general I will make a case for studies in sexual selection, especially the ones investigating postcopulatory processes, that should incorporate environmental, as well as genotypic variation, in reproductive traits.

This study deals with the optimization of best working conditions in molten melt for the production of hydrogen(H2) *** research has been carried out on how electrochemical process occurs through steam splitting via molten hydroxide.54 combinations of cathode,anode,temperature and voltage have been investigated for the optimization of best working conditions with molten hydroxide for hydrogen gas *** these electrochemical investigations were carried out at 225 to 300℃ temperature and 1.5 to 2.5 V applied voltage *** current efficiency of 90.5,80.0 and 68.6% has been achieved using stainless steel anodic cell with nickel,stainless steel and platinum working cathode *** nickel cathode,an increase in the current directly affected the hydrogen gas flow rate at *** can be hypothesized from the noted results that increase in current is directly proportional to operating temperature and applied *** values were noted when the applied voltages increased from 1.5 to 2.5 V at 300℃,the flow rate of hydrogen gas increased from 1.5 to 11.3 cm^(3) min^(-1),1.0 to 13 cm^(3) min^(-1) in case of electrolysis@stainless steel and@graphite anode *** is observed that the current efficiency of stainless steel anodic cell was higher than the graphite anodic ***,steam splitting with the help of molten salts has shown an encouraging alternate to current methodology for H2 fuel production.

Potential synergistic implications for stromal-targeted radiopharmaceuticals in bone-metastatic prostate cancer

Genetic heterogeneity and chemotherapy-resistant ＇stem cells＇ represent two of the most pressing issues in devising new strategies for the treatment of advanced prostate cancer. Though curative strategies have long been present for men with localized disease, metastatic prostate cancer is currently incurable. Though substantial improvements in outcomes are now possible through the utilization of newly approved therapies, novel combinations are clearly needed. Herein we describe potentially synergistic interactions between bone stromal-targeted radiopharmaceuticals and other therapies for treatment of bone-metastatic prostate cancer. Radiation has long been known to synergize with cytotoxic chemotherapies and recent data also suggest the possibility of synergy when combining radiation and immune-based strategies. Combination therapies will be required to substantially improve survival for men with castrate-resistant metastatic prostate cancer and we hypothesize that bone-targeted radiopharmaceuticals will play an important role in this Drocess.
