BACKGROUND The introduction of direct-acting antiviral drugs into clinical practice has revolutionized the treatment of chronic hepatitis C,making it highly effective and safe for ***,few researchers have analyzed the factors causing therapy failure in some *** To analyze factors influencing the failure of direct antiviral drugs in the large,multicenter EpiTer-2 cohort in a real-world *** The study cohort consisted of patients with chronic hepatitis C treated at 22 Polish centers from *** collected from the online EpiTer-2 database included the following:hepatitis C virus(HCv)genotype,stage of fibrosis,hematology and liver function parameters,Child-Turcotte-Pugh and Model for End-stage Liver Disease scores,prior antiviral therapy,concomitant diseases,and drugs used in relation to hepatitis B virus(HBv)and/or human immunodeficiency virus(HIv)*** events observed during the treatment and follow-up period were *** standard and machine learning methods were used for statistical *** During analysis,12614 patients with chronic hepatitis C were registered,of which 11938(mean age:52 years)had available sustained virologic response(SvR)data[11629(97%)achieved SvR and 309(3%)did not].Most patients(78.1%)were infected with HCv genotype *** cirrhosis was diagnosed in 2974 patients,while advanced fibrosis(F3)was diagnosed in 1717 *** included patients with features of hepatic failure at baseline[ascites in 142(1.2%)and encephalopathy in 68(0.6%)patients].The most important host factors negatively influencing treatment efficacy were liver cirrhosis,clinical and laboratory features of liver failure,history of hepatocellular carcinoma,and higher body mass *** viral factors,genotype 3 and viral load also exerted an influence on treatment *** statistical analysis revealed that treatment ineffectiveness seemed to be influenced by the male sex,which was not confirmed

AIM To investigate the main current etiologies of cirrhosis in *** We performed a cross-sectional retrospective multicenter study that included eight hospitals in different areas of Mexico. These hospitals provide health care to people of diverse social classes. The inclusion criteria were a histological, clinical, biochemical, endoscopic, or imaging diagnosis of liver cirrhosis. Data were obtained during a 5-year period(January 2012-December 2017). RESULTS A total of 1210 patients were included. The mean age was 62.5 years(SD = 12.1), and the percentages of men and women were similar(52.0% vs 48.0%). The most frequent causes of liver cirrhosis were hepatitis C virus(HCv)(36.2%), alcoholic liver disease(ALD)(31.2%), and nonalcoholic steatohepatitis(23.2%), and the least frequent were hepatitis B virus(1.1%), autoimmune disorders(7.3%), and other conditions(1.0%).CONCLUSION HCv and ALD are the most frequent causes of cirrhosis in Mexico. However, we note that non-alcoholic fatty liver disease(NAFLD) as an etiology of cirrhosis increased by 100% compared with the rate noted previously. We conclude that NAFLD will soon become one of the most frequent etiologies of liver cirrhosis in Mexico.

[背景]年幼时受到低或中等剂量放射性碘131(131I)暴露导致的甲状腺功能紊乱是一个公共卫生问题。然而,鲜见有关这些常见疾病的131I相关风险的定量数据。[目的]评估甲状腺功能紊乱的发病率与童年(≤18岁)时受到来自切尔诺贝利事故放射性微尘131I暴露的关联。[方法]在一项白俄罗斯队列研究中,对甲状腺功能减退、甲状腺功能亢进、自身免疫性甲状腺炎(AIT)、血清促甲状腺激素(TSH)浓度、甲状腺过氧化物酶自身抗体(ATPO)与基于测定的131I剂量估计之间的关联进行横断面分析。这项队列研究筛查10827名参加者的各种甲状腺疾病。[结果]暴露时的平均年龄(±SD)为(8.2±5.0)岁。甲状腺131I剂量的平均(中位数)估值为0.54(0.23)Gy(范围值为0.001～26.6Gy)。131I剂量与甲状腺功能减退(主要为亚临床型及抗体阴性)和血清TSH浓度呈显著正相关。每1Gy对于甲状腺功能减退症的附加比值比为0.34(95%CI:0.15～0.62),并随着暴露和检测时的年龄、存在甲状腺肿大、居住于城市/农村而显著变化。未发现与抗体阳性甲状腺功能减退、甲状腺功能亢进、AIT或ATPO升高之间有正关联的证据。[结论]在此白俄罗斯队列中,131I剂量与甲状腺功能减退之间的关联与以前报道的一项乌克兰队列的研究结果相一致。上述研究提供了强有力的证据表明,儿童期环境131I暴露的影响结果是甲状腺功能减退,而不是其他甲状腺疾病。
