Healthcare Providers' Perceptions of the Sexual and Reproductive Health Needs (Including Family Planning) of Elderly Women from Selected Sites in Botswana

Much of the sexual and reproductive health services and service delivery including family planning target women of child bearing ages (15 - 49 years) and sometimes men. Hardly are there programmes/interventions that specifically target the needs of the elderly women (50 years and above), yet this group has serious sexual and reproductive health needs as many of them are still sexually active. This cross-sectional study obtained the views of a stratified random sample of 169 healthcare providers (doctors, nurses and pharmacists) from four selected sites, Gaborone, Selibe Phikwe, Barolong and Kweneng East health districts in Botswana on how the healthcare system in the selected sites is meeting the SRH/FP needs of the elderly women. The study found out that while overwhelming majority of the healthcare providers feltthat the healthcare system has no programme that specifically target the SRH/FP needs of this significant others and their SRH/FP needs are not being met ,less than 15% indicated that Pap smear screening as well as screening of cervical cancer were on-going. Although there are SRH/FP services available in the healthcare system, the elderly women are minimally accessing these services. Only condom, combined oral contraceptives, progestogen-only pills, treatment of STIs, screening for HIV/AIDS and screening for cervical cancer are accessed and information is also limited to these services. Reasons given by the healthcare providers for the non-accessibility of these services were cultural diversity (80%), people's sexual behavior and perceptions about sex (79%), lack of knowledge about the desired SRH/FP services (76%), religion (73%) and gender issues (62%). Menopausal Perceptions and Experiences of Older Women from Selected Sites in Botswana

Objective: To assess the menopausal perceptions and experiences of older women from selected sites in Botswana. Design: This study used snowball sampling to recruit 444 older women in four health districts of Botswana. Trained research assistants administered a structured questionnaire to determine respondents’ menopausal symptoms, perceptions and knowledge of menopause and sexual experiences. Multiple logistic regression procedures were used to evaluate the association of socio-demographic characteristics with knowledge of menopause and factor analysis was used to cluster menopausal symptoms. Results: Participants had low levels of knowledge and awareness of menopause. The three most common changes identified were weakening of bones (78%), changes in sex drive (69.6%), and difficulty working (56.2%). The majority of respondents perceived menopause as freedom from menstrual cycles (85%) and cost saving (65%). Employment status was significantly associated with knowledge of menopause. The mean age at menopause was 48.9 years. With an average life expectancy of 54.5 years, there remains about 6 years of postmenopausal life. Recommendation: Public health care systems in and beyond Botswana should mobilize resources and take measures to improve older women’s awareness and knowledge about menopause-related changes through educational training and guidance to maintain active, healthy lives.

航天级钛合金的新型叠层制造技术(ALM)的优势体现在较低的制造成本等方面,并可替代传统加工成型工艺。由等离子弧焊熔敷叠层制造技术制备Ti-6Al-4V合金的显微组织由定向凝固生长的β柱状晶及在其晶内生长的细小的α片层组织构成。在原位拉伸过程中结合应用高速离线电子背散射衍射表征(Offline EBSD)可快速获取试样显微组织和形变特征之间的关系。揭示出不均匀变形的发生取决于柱状晶界间的应变响应。柱状滑移和基面滑移系统被激活进而导致最后出现形变滑移线,即在某些柱状晶中滑移扩展至整个晶粒;而在另一些晶粒中表现为存在应变梯度和应力集中的地方发生形变失配。形变的扩展习性受制于定向凝固生长的柱状晶生长方向及其之间的界面取向关系。在垂直于柱状晶方向的拉伸试验揭示存在剧烈的变形局域化。基于原位拉伸观测及高速的离线电子背散射衍射表征结果,本文作者提出从微观到宏观形变扩展的控制机制。

Prospective evaluation of the cause of acute pancreatitis,with special attention to medicines

AIM: To investigate the cause of acute pancreatitis(AP) by conducting a thorough investigation of drugs and their possible etiological ***: We investigated the cause of AP in a large retrospective cohort of 613 adult patients admitted with AP at the Akershus University Hospital, Norway, from 2000 until 2009, who were evaluated with standard ward investigations. This group was compared with a prospectively evaluated group(n = 57) admitted from January 2010 until September 2010 who investigated more extensively using medical history and radiological ***: The groups were comparable with regards to gender, age, comorbidity and severity. The most common etiology was bile stones and alcohol, occurring in 60% in both groups. The prospective group was examined more thoroughly with regards to the use of alcohol and medicines. An increased number of radiological investigations during hospital stay and at follow-up were also performed. A more extensive use of radiological evaluation did not increase the detection frequency of bile stones. In the prospective group, more than half of the patients had two or more possible causes of pancreatitis, being mostly a combination of bile stones and drugs. No possible cause was found in only 3.5% of these patients, compared with 29.7% in the retrospective ***: A detailed medical history and extensive radiological evaluation may determine a possible etiology in almost all cases of AP. Many patients have several possible risk factors, and uncertainty remains in establishing the definitive etiology.

Development of Maize-Tigernut Fortified Weaning Food Using Starter Cultures

The effects of starter fermentation on the nutritional qualities of maize-tigernut fortified weaning foods were investigated. The dry-milled, malted maize grains fortified with dry-milled roasted tigernut flours (70:30) were subjected to 48 hrs of spontaneous and starter fermentation (singly and as combined starters). Nutritional, sensory characteristics and feeding trials of the weaning foods were evaluated. Four fermented weaning blends were formulated: FMT (spontaneously fermented maize-tigernut), SFMT1 (Lactobacillus plantarum F2C fermented maize-tigernut), SFMT2 (Lactobacillus plantarum U2A fermented maize-tigernut) and SFMT3 (combined starter-fermented maize-tigernut). SFMT2 had the highest crude protein, fat, fibre, ash and least carbohydrate content among the blends. The highest energy content (456.84 Kcal/100 g) was observed in blend SMFT2 which was also higher than that of both negative (Nutrend) and positive (Conventional animal feed) controls. The least antinutrient and vitamin contents were recorded in SFMT2. Blend SFMT1 had the highest Vitamin B1 (0.67 mg/100 g), Vitamin A (472.60 ug/100 g), phosphorus (75.45 mg/100 g) and zinc (1.05 mg/100 g) contents while the highest calcium (17.17 mg/100 g) and iron (22.82 mg/100 g) were recorded in SFMT2. Sample SFMT2 was rated the highest in all of the sensory characteristics except colour and the highest overall acceptability (6.00) which was not different significantly from all other starter produce blends. Biological evaluation showed blend SFMT2 fed animals having the highest weight by 28 days (73.14 g), mean weight gain (5.46 g), mean feed intake (18.71 g) and mean protein efficiency ratio PER (3.65). However, all the PER values including that of controls (2.30) were higher than the value of 2.10 recommended by the Protein Advisory Group (PAG) for complementary foods. Seasonal Shifts in the Bacterioplankton Assemblages of High Altitude Middle Atlas Lakes

Compositional changes of the bacterioplankton assemblages in four high altitude middle Atlas lakes were monitored over a complete seasonal cycle using combinations of culture-dependent and molecular approaches. Viable bacterial numbers varied between seasons, with the lowest numbers recorded in the winter and the highest in the summer in all four lakes. Also, bacterial occurrences were found to be strongly correlated with water temperature in all the four sites, i.e., Lake Aoua (0.88), Ifreh (0.59), Hechlef (0.77) and Affourgagh (0.79) during the study period. Standard microbiological characterization of bacteria isolates from the lakes revealed majority (68%) to be Gram positive. Seasonal variations in the microbial assemblages among the lakes were also validated by restriction fragment length polymorphism (RFLP). Shifts in microbial assemblages, especially of the fecal indicator bacteria appeared to also be influenced by differences in the morphometric and watershed characteristics among the lakes. This study further reveals the need to employ combinations of methodological approaches, including taxonomic, physiological and molecular methods to adequately delineate and fully understand the ecology of microbial assemblages in extreme environments.
