Medications for diabetic neuropathy

BACKGROUND AND OBJECTIVE Diabetic neuropathy is a common, long-term complication that can decrease quality of life. While various neuropathic agents are useful for treating this pain, choosing one or the other can be challenging. This systematic review investigated the relative effectiveness of various medications used for the treatment of diabetic *** Databases were reviewed for randomized, controlled trials published between January of 2012 and April of 2014. The trials assessed the efficacy of medications for treating diabetic neuropathy, and compared the medication to placebo or to another *** Sixty-five, randomized, controlled trials, including 27 medications with 12, 632 patients analyzed. By drug class, SNRIs, topical capsaicin, TCAs, and anticonvulsants all resulted in larger and statistically significant reductions in pain, as compared with placebo. Head-to-head trials showed that SNRIs and TCAs reduced pain more than did anticonvulsants and topical capsaicin. Studies that evaluated long-term efficacy found that the aldose reductase inhibitors fidarestat, duloxetine and oxcarbazepine are all more effective than placebo. Indirect and direct comparisons among specific medications revealed greater pain control with carbamazepine, venlafaxine, duloxetine and amitriptyline as compared with *** This systematic review and meta-analysis of pharmacologic interventions for painful diabetic neuropathy found that carbamazepine, venlafaxine, duloxetine and amitriptyline are significantly better than placebo for controlling pain.

This paper presents a brief demonstration of Schulz’s first conjecture, which sets the upper and lower limits on the length of the shortest chain of addition. Two methods of the upper limit are demonstrated;the second one is based on the algorithm of one of the most popular methods for obtaining addition chains of a number, known as the binary method.

Outcomes of the Clean Development Mechanism in Argentina

This paper summarizes the results of the implementation of the CDM in Argentina during the first period of commitment of the Kyoto Protocol. From a total of sixty- five projects ideas, forty-four achieved registration under the CDM EB while the rest remained approved at national level (12), under evaluation (2), suspended (5) or rejected (2). Most of the projects focused on the generation of electricity from renewable energy, mainly wind energy and the use of landfill gas and methane for energy. Emissions reductions achieved 33.4% of the expected CERs up to 2012 with strong differences among sectors. Results demonstrated that under the current Argentina’s energy policy framework, the income by the selling of CERs covered less than 6% of the incremental costs for renewable energy projects. A sensitivity analysis to evaluate the impact of CDM in the coverage of incremental costs for renewable energy based on the prices of both the energy in the local market and the CER demonstrated that the best conditions would only cover 15% of those costs. The contribution of CDM to technology transfer in Argentina was minor considering that 45% of the projects qualified as type III where technological learning and capacity building were limited at the level of operation and maintenance of a foreign technology. Domestic and external barriers that prevented a better performance of CDM projects in terms of GHG mitigation, technology transfer and the contribution to a sustainable development were also analyzed.

采用新型无溶剂反应和回流的方法制得锰钾矿型氧化镁(K-OMS-2),同时采用常规方法制得氧化镁,并测试不同催化剂对工业排放气中有机挥发性物质(VOCs)中的模型化合物––乙酸乙酯和乙酸丁酯的催化氧性能.采用N2吸附-脱附、X射线衍射、扫描电镜、程序升温还原和X射线光电子能谱等技术对催化剂进行了表征.所有氧化镁样品均表现出很高的催化乙酸乙酯和乙酸丁酯氧化生成CO2的活性,且制备方法对催化剂性能起着重要作用.新型无溶剂法制得的K-OMS-2纳米棒样品比常规的回流法制得样品表现出更好的催化性能,含锰钾矿型氧化镁的样品比常规方法制得样品表现出更高的活性.性能最好的催化剂也表现出较高的稳定性,在213和202 oC条件下,可分别使90%的乙酸乙酯和乙酸丁酯转化为CO2.催化剂性能的显著差异清楚地表明,对于所选VOCs氧化反应,采用新型无溶剂法制得的K-OMS-2纳米棒样品比常规法制备的氧化镁混合物更好,这可能与样品结构中含有更高的Mn平均氧化态有关.本文表明了催化剂性能与其表面化学性质间存在显著的关联,显示了K-OMS-2内在性质决定了其高的催化性能.

Cardiovascular Risk in Adults and its Association with Health Services Utilization. ENSANUT 2018-2019

Background The use of health services by the adult population is related to cardiovascular risk and its stratification. Cardiovascular risk (CVR) stratification should be a tool for the assessment of the patients and the appropriate control during the process of medical care and utilization of health services for the adults. Objective Evaluate the association between cardiovascular risk (CVR) in adults and the utilization of health services. Material and Methods A secondary analysis was performed of the data from the National Health and Nutrition Survey (ENSANUT) 2018-2019. The CVR classification (risk score) was obtained in 43,070 adults with a previous diagnosis (self-report) and 1,237 adults newly diagnosed. Independent, risk factors and the association between groups of CVR and utilization of preventive, outpatient and hospital services were analyzed. Results More than 85% of adults interviewed have some degree of CVR. Almost half of them have low CVR (48.2%). Older adults with social security predominate in the group with high and very high CVR. Seventy-five percent of adults recently diagnosed have low CVR. In both, there is very little utilization of health services. For adults previously diagnosed CVR, the higher the CVR, the greater the likelihood of utilization of outpatient, preventive and hospital services, in contrast to adults without CVR independent of the marital status, sex, health institution and socioeconomic level. Conclusion The results give evidence of areas of opportunity for improvement in the quality of health services. The evaluation of CVR in primary care and promotion and prevention of CVR should be strengthened.
