Background: Traumatic injuries constitute a leading cause of mortality and morbidity worldwide with a global burden that is on the rise. The aim of this study is to analyse preliminary data of the Limbe trauma registry, to describe the injury patterns and determine the outcome of care of the injured who were treated in the emergency department of the regional hospital Limbe in Cameroon. Methodology: This was a descriptive secondary analysis of prospective data collected on injury at Limbe Regional Hospital in two years, between October 2008 and October 2010. All cases of injury presenting to the emergency department of Limbe Regional Hospital were included in a prospective trauma registry. Data was collected with a pre-tested data entry form and included the age, sex, mechanism of injury, delay before arrival to hospital, body part injured, description of lesions and outcome at the emergency. Descriptive analyses were done using STATA 14. Comparisons between groups were evaluated using Chi-squared test or Kruskal-Wallis and p-values <0.05 were considered statistically significant. Results: A total of 2400 cases of injury were recorded during the study period with more males (68%) than females. The ages ranged from 0 to 90 years with a peak between 20 to 39 years. Road traffic crash was the most common cause of injury representing 60.81% of the population (n = 1434);other causes were burns, falls, domestic injuries, assaults, and labour accidents. The accidents mostly involved motor cycle alone (42.03%). Concerning the outcome of care at the emergency department, 88.73% (n = 1260) were discharged. Conclusion: The burden of road traffic injuries in south west Cameroon is high. Motorcycles are mostly involved alone or with pedestrians and/or cars.

Retroperitoneal liposarcoma is a rare malignant tumour with an incidence of 2.5 per million people annually. It is the most common type of all the retroperitoneal malignant tumours. Early diagnosis of the tumour is difficult because of absence of specific clinical presentation. Symptoms would only occur if the tumour exhibits pressure on the surrounding organs. It usually occurs between the fourth to sixth decades of life affecting both genders equally. Histological subtypes include: well-differentiated, poorly differentiated, myxoid, round cell and pleomorphic. Radiological imaging useful in diagnosis include: ultrasonography, computed tomography and magnetic resonance imaging. Complete surgical resection of the tumour and the adjacent viscera involved or organs remained the mainstay of treatment. Radiotherapy plays a role in reducing the risk of recurrence but its effect on the overall survival of patients is not known. The case presented is a 48-year-old male patient with retroperitoneal liposarcoma, treated by complete surgical resection of the tumour and right hemicolectomy and currently on follow-up for possible recurrence.

Leaves Aqueous Extract of <i>Boerhavia coccinea</i>Induces Antinociceptive Effect but Increases Nitric Oxide Production in Macrophages

Pain treatment is one of the most challenging situations of the modern medicine. To overcome the actual limitations, new strategies should be developed and phytotherapy represents a promising alternative. Boerhavia coccinea is a medicinal plant used for the treatment of pain. The present work was undertaken to evaluate the antinociceptive effects of crude aqueous extract of the leaves of Boerhavia coccinea (AE) on acute pain and examine its mechanism of action. The analgesic effect of AE was evaluated at doses 50, 100, 200 and 400 mg/kg using the formalin-induced nociception in mice. The specific analgesic effect of AE was verified by testing its effect of AE on the sleep induced by diazepam. The anti-inflammatory effects of AE were also tested in vivo (100 and 200 mg/kg) on CFA-induced inflammation and in vitro (1 to 300 μg/ml) on the production of NO by non-activated or activated (LPS, 1 μg/l) macrophages. The effect of AE in non-stimulated macrophages was evaluated in absence and in presence of L-NAME (100 μg/ml) or dexathetasone (30 μg/ml). AE administered orally significantly (p st phase and 90% at the 2nd phase, but did not show any anti-inflammatory effect. AE at 50 mg/kg increased the latency to sleep and reduced sleep duration. AE drastically (p < 0.001) increased the NO production by non-activated or activated macrophages by up to 5654%. L-NAME and dexamethasone potentiated the activation effect of AE on NO production. In conclusion, AE possess potent antinociceptive effect that is not related to any anti-inflammatory activity. Instead, this extract increases the nitric oxide production by an unknown mechanism.

Background: Infectious diseases associated with Human Immunodeficiency Virus (HIV), Hepatitis B Virus (HBV), and Hepatitis C Virus (HCV) continue to pose significant public health challenges that necessitate continuous surveillance and interventions. This study investigates the prevalence of HIV, HBV and HCV infections within the academic community of Rivers State University, Port Harcourt, Nigeria. Given the potential for transmission in university settings and the importance of early detection and intervention, this research explores the extent of these infections and evaluates the outcomes of a medical outreach program aimed at addressing them. Methodology: A cross-sectional study design was employed to recruit 513 participants using a convenience sampling approach between the 20th and 22nd of April 2022. Participants included students and staff members who voluntarily consented to the medical outreach and study. Serological assays were utilized to identify HIV, HBV and HCV infections. The medical outreach program’s design, implementation, and outcomes were evaluated, focusing on testing uptake, counselling utilization, and linkage to care. Statistical analysis employed GraphPad Prism version 9 software. Result: The study revealed low prevalence rates of HIV (0.6%), HBV (1.2%), and HCV (0.6%) infections within the university population. Age-based analysis indicated a slightly higher prevalence in the 15 - 24 age group. Gender-wise, HBV showed a marginal difference, with 1.3% among females and 0.9% among males. The medical outreach program achieved a testing uptake of 80.8%, with 75% utilizing counselling services, and 75% of those identified with infections successfully linked to care. Conclusion: This study provides valuable insights into the prevalence of infections and the effectiveness of a medical outreach program within a university community. The present study was carried out with the aim of evaluating the chemical composition and bioactivity of Uvariodendron molundense against Sickle cell disease and associated pathogenic bacteria agents. The antisickling and antibacterial activities were assessed using Emmel and micro-dilution methods respectively. The results revealed that the leaves and stem bark of U. molundense contains various secondary metabolites such as total phenols, flavonoids, anthocyanins, tannins, quinones, saponins, alkaloids, steroids, terpenoids and leuco-anthocyanins. The n-hexane (non-polar solvent) extract displayed poor yield (0.66) than the extracts obtained in the polar solvents which have a high yield (Methanol: 1.68, Ethanol: 1.46 and Ethyl acetate: 1.40). These results indicate that the abundant secondary metabolites/compounds in this plant species are those which pass easily through the polar solvents (methanol, ethanol and ethyl acetate). This is the case of phytomarkers like flavonoids and tannins (which are most concentrated in methanol) and anthocyanins which are concentrated in ethyl acetate. The extraction yield of organic/triterpenoic acids (betulinic acid rich extract) was 1.70%. All tested extracts displayed antisickling activity. At 100 μg/mL, the rates of normalization were 89% for organic extract (ED50 = 0.391 μg/ml) and 82% for anthocyanins extract (ED50 = 0.659 μg/mL). The antibacterial activity of tested extracts was very good toward Staphyloccocus areus (CMI ≤ 31.25 μg/mL) while, for Escherichia coli, only the organic extract exhibit interesting activity (CMI = 31.25 μg/mL). This study validates for the first time the in vitro antisickling activity of U. molundense. The bioactivity profiles of organic acids extract from the studied plant material indicate that they constitute promising fraction to be further investigated phytochemically for the discovery of new lead compounds for pharmaceutical application.

A Retrospective Analysis of the Patterns and Outcomes of Tile B and Tile C Pelvic Fractures in Cameroon

Background: Pelvic fractures constitute about 2% - 8% of all fractures. This incidence may rise up to 25% in poly-traumatised patients. These fractures have a high mortality rate due to the fact that they occur usually as a result of high energy trauma, and most of the injured are poly-traumatised with concomitant haemodynamic instability. The aim of this study is to describe the patterns of injury and complications of unstable pelvic fractures treated in a level III hospital in Cameroon. Methodology: This was a hospital based retrospective analysis of files of patients admitted and treated for unstable pelvic fractures at the Regional Hospital Limbe within a period of 10 years (from 1st of January 2009 to 31st of December 2018). Results: A total of 139 cases of pelvic fractures were identified, amongst which 77 were unstable;68 were finally analyzed. The ages ranged from 18 to 80 years with a mean of 39 ± 5 years. The age group from 20 - 40 years was most represented (58.88%, n = 40). There were 45 males and 23 females giving a sex-ratio of 2:1. Road traffic injuries accounted for most of the cases (73.53%, n = 50). Forty-five (66.2%) were classified Tile B and 23 (33.82%) were Tile C, and 15 cases were open fractures (22.06%). Associated lesions were recorded in 30 cases, fractures of the lower extremity being the most common (33.33%, n = 10). Most of the cases were definitively treated surgically (80.89%, n = 55). The most common complications were surgical site infections (23.64%, n = 13). Conclusion: Unstable pelvic fractures are relatively common. Young males in the age group 20 - 40 years are the most affected, and the most common cause of injury was road traffic accident. Surgical site infections, pressure ulcers, severe anaemia and thrombo-embolism are common complications.

We reported a case of a 30-year-old man who reportedly sustained electrical burns and fell from a high voltage electric pole about 50 meter high onto a metal that caused impalement injury. In addition, he sustained full-thickness burns of the right upper limb (7%), the right hemithorax, the perineum (sparing the penis), the anterior abdominal wall and the lateral aspect of both thighs. There was 43% burned surface area in total. Radiographic examination revealed a slender curved object extending from his perineum into the pelvis. The management of this case was a challenging one which was described in this article.
