Sigmoid Volvulus, Intestinal Obstruction and Bowel Perforation;an Unusual Presentation of Pedunculated Fibroid Torsion. Case Report

Pedunculated fibroid torsion presenting as a case of acute abdomen from sigmoid volvulus and large bowel perforation is rare. Without prompt diagnosis and intervention, this could lead to serious morbidity and mortality. Ms FM was a 52-year-old perimenopusal woman who was admitted to the Intensive Therapy Unit (ITU) with worsening symptoms of confirmed Covid-19 infection. On the 10th day of her admission, she developed abdominal distension and tenderness. A pelvic ultrasound scan showed a large pedunculated fibroid measuring 23 × 15 × 22 cm. The plan was for conservative management to use pain killers. Following deterioration of her clinical state, an abdominal CT scan was done which confirmed a large uterine fibroid, large bowel distention. CT findings also showed sigmoid volvulus and large bowel perforation. Following a multidisciplinary team assessment, she had an emergency exploratory laparotomy with findings of a large, torted, pedunculated fibroid with adherent sigmoid colon which had become twisted and obstructed. The large bowel segment above the Sigmoid volvulus was grossly distended and there was a gangrenous hepatic flexure with perforation. She had a right hemicolectomy, a de-functioning colostomy and subtotal hysterectomy. Postoperatively, she made very good clinical improvement. Fibroid histology report showed tissue infarction and necrosis which confirmed the torsion. She was discharged home after making good recovery.

Geophysical Investigation of Groundwater Potentials of the Nanka Sand in Obosi and Its Environs, Anambra State Nigeria

Geophysical investigation for Groundwater which entails the use of vertical electrical sounding (VES) methods was carried out in parts of the Idemili area of Anambra State, Nigeria. Geologically the study area falls within the Nanka Sand, belonging to the Ameki Group in the Tertiary Niger Delta Basin. The investigation intends amongst others to assess the groundwater potential of the area, find out the existence of possible aquifer(s) within the area, and estimate the possible depth of a borehole. 15 VES stations were established at the site using Ohmega System Tetrameter and Schlumberger electrode configuration with current electrode spacing (AB/2) of 350 meters. The geologic parameters, such as the number of geo-electric layers, the thickness of the geo-electric units, and their resistivities were interpreted from the VES curves. A qualitative assessment of the curves shows that a model with 5 layers would sufficiently model the field curves. The thickness value ranged from 0.1 to 86 m, the resistivity ranged from 258 to 13,258 Ωm, and the depth ranged between 1.2 and 109 m. Quantitative interpretation of field curves involved inversion with appropriate parameters using Zond Res1D software.

Field Trial of a Thermostable Peste des petits ruminants (PPR) Vaccine in a Semi-Arid Zone of Nigeria

The field trial of a candidate thermostable Peste des petits ruminants (PPR) vaccine was carried out in flocks of sheep and goats under the extensive system of management. The immune response of vaccinated animals was determined using the neutralisation test to detect PPR virus specific antibody. Vaccinated animals seroconverted and a four-fold or more rise in antibody titre were observed between pre-vaccination and post-vaccination antibodies. The vaccine elicited significant antibody response in goats through the different routes of administration (intramuscular, intranasal, intraocular, subcutaneous and orally), but was poorly transmitted between the vaccinees and in-contact animals. The sheep responded poorly to the vaccine administered through most of the routes, except for those vaccinated through intramuscular and subcutaneous routes that seroconverted significantly (≥4 fold rise). The vaccine retained a potent titre of 3.1 log10 TCID50 for more than 8 hours after reconstitution in PBS at room temperature. Based on the response of goats to oral vaccination, it is suggested that the vaccine could be administered on the field through the oral routes and has the potential to be adapted to a feed-based administration for wider application to the scattered livestock populations under the extensive system of management.

Thermal Cycling Effects on the Fatigue Behaviour of Low Carbon Steel

This paper examines the behavior of low carbon steels subjected to a frequency thermal cycling test. A compositional analysis was performed to ascertain the percentage of carbon in the asreceived materials. The specimens were machined to a precise gauge length and diameter and exposed to various cycles of heat at each temperature. A fatigue test was also performed with the use of Avery Dennison and bending stress was obtained using the curve supplied with the machine. The results from the machine were converted to Mega Pascal (MPa) and the values used to plot S-N curves and fatigue resistance for the specimens at various cycles and different temperatures were established.

Pattern of Presentation and Associated Morbidities of Women Presenting with Postmenopausal Bleeding

Background: Postmenopausal bleeding (PMB) is caused mainly by benign disorders;however it is sometimes caused by endometrial cancer. Aim: We here attempted to determine what conditions account for PMB in an outpatient clinic of a University hospital in London. Methodology: Study subjects consisted of 179 patients with PMB who were referred to us from July to December 2019. Sociodemographic data including patient’s age, risk factors, diagnosis and management were reviewed. Underlying conditions where determined. Results: Of 179 subjects, the following findings were made: 1) Age 59.63 ± 8.3 (mean and standard deviation). 2) Parity;multiparity, 57.0% (mean ± 1.67). 3) First episode of PMB, 77.1%. 4) The most frequently observed risk factor;obesity 34.6%. 5) The following accounted for PMB (diagnosis in order of incidence rate);genital atrophy 37.4%, submucosal fibroid 28.5%, endometrial polyp 20.7%, endometrial hyperplasia 6.7%, and endometrial cancer 5.6%. All patients were treated appropriately. We did not determine the prognosis of patients with endometrial cancer. Conclusion: Although the incidence of rate among women with PMB has already been reported, its reconfirmation in a single facility is important for making policies in the treatment of PMB.

Prevalence of Some World Health Organization Priority Organisms from an Abattoir at Kwata, Anambra State, Nigeria

The abattoir is one of the important sectors in the food industry, therefore the need for close monitoring of everything concerning it to minimize microbial contamination. The aim of the study is to identify bacteria listed as WHO priority organisms associated with meat from Kwata abattoir in Awka, Anambra state. The study was carried out over a three (3) months period from September to December 2022. Thirteen samples were collected from the floor, table, water, meat, knife and soil in the abattoir. The samples were cultured using streak and spread plate methods on MacConkey and Cetrimide agar. The isolates were identified with the following biochemical tests: catalase, oxidase, citrate and indole tests. Ampiclox levofloxacin, gentamicin, ofloxacin impenem ceftraixone, cefixime, cefuroxime and nitrofurantoin were used for sensitivity test following Kirby Bauer disc diffusion method and biofilm formation was determined using the tube method. The 75 isolates obtained were identified as follows;29.3% E. coli, 26.7% Klebsiella spp., 16% Proteus spp., and 28% Pseudomonas spp. The result of antibiotics sensitivity test interpreted using the European Committee on Antimicrobial Susceptibility Testing (EUCAST) breakpoints showed that (98%) of the isolates were resistant to the antibiotics used. Test for biofilm formation with 52 isolates showed 31 strong, 12 moderate and 9 weak biofilm formers. The result of this study confirms the presence of bacteria contaminants within the WHO priority list in the abattoir, as such there is need for improved handling of animals. Slaughtering, cleaning and distribution of meat should be done using aseptic procedures. .

Control of Postpartum Haemorrhage with Uterine Balloon Tamponade Using Foley Catheter in a Rural Mission Hospital in Ebonyi State, Southeast Nigeria

Background: Postpartum haemorrhage is the leading cause of maternal deaths worldwide, the majority of which occur in low resource settings. Uterine atony is the commonest cause of postpartum haemorrhage. Uterine balloon tamponade (UBT) is an effective method of treating refractory postpartum haemorrhage. Aim: Commercial UBT devices are often not affordable and not readily available in rural settings. The aim of this paper is to report on three cases of postpartum haemorrhage successfully managed with uterine balloon tamponade using Foley catheters. Case Reports: We report on three patients with major obstetric haemorrhage from uterine atony who were successfully managed with uterine balloon tamponade using Foley’s urethral catheter. The first two patients had primary postpartum haemorrhage while the third patient had significant bleeding during the surgical evacuation of the uterus for a molar pregnancy. In each case, uterine bleeding was refractory to pharmacologic uterotonics. They all had uterine tamponade with a Foley catheter with the dramatic resolution of their bleeding. Conclusion: In well-selected patients, uterine balloon tamponade with Foley catheter is cheap, arrests bleeding and prevents clinical deterioration among women with refractory postpartum haemorrhage, especially in low resource settings where commercial balloon tamponade may not be available or affordable.

The Fetal Outcome and Fetal Wastage Pattern among Different Types of Obstetric Fistula at the National Obstetric Fistula Centre, Abakaliki, Nigeria

BACKGROUND: Obstetric Fistula is a major public health problem in developing world. It is associated with a high fetal wastage rate. The objective of this study was to evaluate different types of obstetric fistula and their fetal wastage rate. METHOD: A retrospective population study was done at the National Obstetric Fistula Centre, Abakaliki between 1st January-31st December, 2016. The calculated minimum sample size was 3, however, the total number of 203 patients were studied. The case notes of all the women who had obstetric fistula repairs over the period were analyzed. RESULTS: The mean age from this study was 38 ± 12.1 years. The fetal wastage rate from this study was 82.76% while the live birth was 17.24%. Seventy percent of the stillbirth were delivered via SVD, while 11% of stillbirth were delivered through EmCS. Twelve different types of fistula were identified in this study using anatomical classifications. Large extensive fistula, urethral loss and multiple fistula had the highest fetal wastage of 100% respectively. This was followed by mid-vaginal fistula (95.7%), Juxtaurethral fistula 94.4%, Juxtacervical fistula 88.5%, Intracervical fistula 85.71%, Ureteric fistula 85.71%, Vesicouterine 84.21%, Vault fistula 62.5%. Rectovaginal fistula had the least fetal wastage of 15.79% and the highest live birth of 84.2%. CONCLUSION: The findings showed a high fetal wastage rate amongst women with obstetric fistula. There was high fetal wastage across different types of obstetric fistula. Rectovaginal fistula had the best outcome in terms of live births.

Assessing the Impact of Gas Flaring and Carbon Dioxide Emissions on Precipitation Patterns in the Niger Delta Region of Nigeria Using Geospatial Analysis

This research utilizes geospatial methodologies to investigate the influence of gas flaring and carbon dioxide emissions on precipitation patterns within the Niger Delta region of *** study relies on average mean precipitation data sourced from CHRS at the University of Arizona and carbon dioxide emissions data from NASA’s AIRS in Giovanni,spanning from July 2002 to November *** carry out the analysis,ArcGIS 5.0 and SPSS 25,employing Inverse Distance Weighting(IDW),were employed to assess CO_(2) emissions and rainfall for both November and July during the period from 2002 to *** the course of this study,it was observed that CO_(2) emission exhibited an upward trend,increasing from 327.5226 parts per million(ppm)in July 2002 to 390.0077 ppm in November ***,the rainfall demonstrated an increase,rising from 56.66 millimeters to 390.78 millimeters for both July and November from 2002 to *** findings emerged from the correlation analysis ***,from July 2000 to 2011,there was a weak positive correlation(0.3858)observed between CO_(2) emissions and minimum rainfall,while a strong negative correlation(–0.7998)was identified for maximum rainfall *** November,both minimum and maximum CO_(2) emissions displayed strong negative correlations with rainfall,with coefficients of–0.8255 and–0.7415,*** findings hold significant implications for comprehending the environmental dynamics within the Niger *** and stakeholders can leverage this knowledge to formulate targeted strategies aimed at mitigating CO_(2) emissions and addressing potential climate change-induced alterations in rainfall patterns.

Background and objectives:Coronavirus disease 2019(COVID-19)is a pandemic that has become a major source of morbidity and mortality worldwide,affecting the physical and mental health of individuals influencing *** the threat,it poses to maternal health in sub-Saharan Africa and Nigeria,there is little or no data on the impact it has on fertility,conception,gestation and *** compare the birth rate between pre-COVID and COVID times using selected months of the *** and methods:This was a secondary analysis of cross-sectional analytical study data from the birth registries of three tertiary hospitals,comparing two years[2019(Pre-COVID)]versus[2020(COVID era)]using three months of the year(October to December).The data relied upon was obtained from birth registries in three busy maternity clinics all within tertiary hospitals in South-East Nigeria and we aimed at discussing the potential impacts of COVID-19 on fertility in *** secondary outcome measures were;mode of delivery,booking status of the participants,maternal age and ***:There was a significant decrease in tertiary-hospital based birth rate by 92 births(P=0.0009;95%CI:-16.0519 to-4.1481)among mothers in all the three hospitals in 2020 during the COVID period(post lockdown months)of October to *** was a significant difference in the mode of delivery for mothers(P=0.0096)with a 95%confidence interval of 1.0664 to 1.5916,as more gave birth through vaginal delivery during the 2020 COVID-19 period than ***:Tertiary-hospital based birth rates were reduced during the *** multi-centre study extrapolated on possible factors that may have played a role in this decline in their birth rate,which includes but is not limited to;decreased access to hospital care due to the total lockdowns/curfews and worsening inflation and economic recession in the country.
