BACKGROUND Inflammatory bowel diseases(IBD) is a heterogenous, lifelong disease, with an unpredictable and potentially progressive course, that may impose negative psychosocial impact on *** informed patients with chronic illness have improved adherence and outcomes, previous research showed that the majority of IBD patients receive insufficient information regarding their *** large heterogeneity of IBD and the wide range of information topics makes a one-size fits all knowledge resource overwhelming and *** hypothesized that different patient profiles may have different and specific information needs, the identification of which will allow building personalized computer-based information resources in the *** To evaluate the scope of disease-related knowledge among IBD patients and determine whether different patient profiles drive unique information *** We conducted a nationwide survey addressing hospital-based IBD clinics.A Total of 571 patients completed a 28-item questionnaire, rating the amount of information received at time of diagnosis and the importance of information, as perceived by participants, for a newly diagnosed patient, and for the participants themselves, at current *** performed an exploratory factor analysis of the crude responses aiming to create a number of representative knowledge domains(factors), and analyzed the responses of a set of 15 real-life patient profiles generated by the study *** Participants gave low ratings for the amount of information received at disease onset(averaging 0.9/5) and high ratings for importance, both for the newly diagnosed patients(mean 4.2/5) and for the participants themselves at current time(mean 3.5/5).Factor analysis grouped responses into six *** responses of selected profiles, compared with the rest of the participants, yielded significant associations(defined as a difference in rating of >0.5 points with a P < 0.05).Patients w

Xenotransplantation of embryonic pig pancreas for treatment of diabetes mellitus in non-human primates

Transplantation therapy for diabetes in humans is limited by the low availability of human donor whole pancreas or islets. Outcomes are complicated by immunosuppressive drug toxicity. Xenotransplantation is a strategy to overcome supply problems. Implantation of tissue obtained early during embryogenesis is a way to reduce transplant immunogenicity. Pig insulin is biologically active in humans. In that regard the pig is an appropriate xenogeneic organ donor. Insulin-producing cells originating from embryonic pig pancreas obtained very early following pancreatic primordium formation [embryonic day 28 (E28)] engraft long-term in rhesus macaques. Endocrine cells originating from embryonic pig pancreas transplanted in host mesentery migrate to mesenteric lymph nodes, engraft, differentiate and improve glucose tolerance in rhesus macaques without the need for immune suppression. Transplantation of embryonic pig pancreas is a novel approach towards beta cell replacement therapy that could be applicable to humans.

Objective:To perform risk factor analysis for the prediction of hyperbilirubinemia in an African American male neonatal cohort. Study design:A database of 500 previously published term and near-term African American male neonates was further analyzed to determine the role of risk factors for hyperbilirubinemia. Factors studied included birth weight ≥4.0kg,gestational age ≤37 weeks,breast-feeding,glucose-6-phosphate dehydrogenase (G-6-PD) deficiency,and predischarge bilirubin ≥75th percentile. Hyperbilirubinemia was defined as any bilirubin value ≥95th percentile on the hour-of-life-specificbilir-ubin nomogram. Results:Forty-three (8.6%) neonates developed hyperbilirubinemia. At 48 ±12 hours,median transcutaneous bilirubin was 8.3 mg/dL,75th percentile 10.0mg/dL,and 95th percentile 12.6 mg/dL. Of the risk factors,only exclusive breast-feeding,G-6-PD deficiency and predischarge bilirubin ≥75th percentile were significant (Adjusted Odds Ratios 95%Confidence Intervals; CI 3.15 1.39-7.14 ,P = 0.006; 4.96 2.28-10.80 ,P = 0.001; and 7.47 3.50-15.94 ,P < 0.0001,respectively). G-6-PD-deficient neonates who were also premature and breast-feeding had the highest incidence of hyperbilirubinemia (60%). Conclusions:African American male neonates may be at higher risk for hyperbilirubinemia than previously thought. Screening for G-6-PD deficiency and predischarge bilirubin determination may be useful adjuncts in hyperbilirubinemia prediction in these newborns.

Aim: To determine the incidence of post-phototherapy neonatal plasma total bi lirubin (PTB) rebound. Methods: A prospective clinical survey was performed on 2 26 term and near-term neonates treated with phototherapy in the well baby nursery of the Shaare Zedek Medical Center from January 2001 to September 2002. Neona tes were tested for PTB 24 hours (between 12 and 36 hours) after discontinuation of phototherapy, with additional testing as clinically indicated. The main outc ome measure, significant bilirubin rebound, was defined as a post-phototherapy PTB ≥256 μmol/l. Phototherapy was not reinstituted in all cases of rebound, bu t rather according to clinical indications. Results: A total of 30 (13.3%) neon ates developed significant rebound (mean (SD) PTB 287 (27) μmol/l, upper range 351 μmol/l). Twenty two of these (73%) were retreated with phototherapy at mea n PTB 296 (29) μmol/l. Multiple logistic regression analysis showed significant risk for aetiological risk factors including positive direct Coombs test (odds ratio 2.44, 95%CI 1.25 to 4.74) and gestational age 72 hours (4/74, 5.4%) (odds ratio 3.61, 95%CI 1.21 to 10.77). Conclusion: Post-photot herapy neonatal bilirubin rebound to clinically significant levels may occur, es pecially in cases of prematurity, direct Coombs test positivity, and those treat ed ≤72 hours. These risk factors should be taken into account when planning pos t-phototherapy follow up.

Objective:To determine normal concentrations of procalcitonin in preterm infants shortly after birth and to assess its accuracy in detecting bacterial infection. Methods:Blood samples of 100 preterm infants were prospectively drawn during the first 4 days of life for determination of procalcitonin concentration. Infants were classified into four groups according to their sepsis ***:Mean (SD) gestational age and birth weight were 32(2.9) weeks and 1682 (500) g respectively. A total of 283 procalcitonin concentrations from healthy infants were plotted to construct nomograms of physiologically raised procalcitonin concentration after birth,stratified by two groups to 24-30 and 31-36 weeks gestation. The peak 95th centile procalcitonin concentration was plotted at 28 hours of age; values return to normal after 4 days of life. Only 12 infants were infected,and 13 of their 16 procalcitonin concentrations after birth were higher than the 95th centile,whereas samples taken at birth were *** a multivariable analysis,gestational age,premature rupture of membrane,and sepsis status influenced procalcitonin concentration independently,but maternal infection status did ***:The suggested neonatal nomograms of preterm infants are different from those of term infants. Procalcitonin concentrations exceeding the 95th centile may be helpful in detecting congenital infection,but not at birth.

Objective: To test the hypothesis that normalizing the intestinal flora by adm inistration of prophylactic probiotics would provide a natural defense, thereby reducing both the incidence and severity of necrotizing enterocolitis (NEC) in p reterm neonates. Study design: Neonates ≤1500 g birth weight were randomized to either receive a daily feeding supplementation with a probiotic mixture (Bifido bacteria infantis, Streptococcus thermophilus, and Bifidobacteria bifidus; Solga r, Israel) of 109 colony forming units (CFU)/day or to not receive feed suppleme nts. NEC was graded according to Bell’s criteria. Results: For 72 study and 73 control infants, respectively, birth weight (1152 ±262 g vs 1111 ±278 g), gest ational age (30 ±3 weeks vs 29 ±4 weeks), and time to reach full feeds (14.6 ±8.7 days vs 17.5 ±13.6 days) were not different. The incidence of NEC was red uced in the study group (4%vs 16.4%; P = .03). NEC was less severe in the prob iotic-supplemented infants (Bell’s criteria 2.3 ±0.5 vs 1.3 ±0.5; P = .005). Three of 15 babies who developedNEC died, and all NEC-related deaths occurred in control infants. Conclusion: Probiotic supplementation reduced both the incid ence and severity of NEC in our premature neonatal population.
