AIM: To assess the inflammatory cytokines expression in aqueous humor in diabetic primary open angle glaucoma(POAG) patients. METHODS: A cross-sectional study on 87 eyes, distributed as following: 26 eyes from diabetic patients, 16 eyes with POAG and 21 eyes from diabetic POAG patients; healthy controls(24 eyes) were recruited from patients undergoing conventional cataract surgery. A volume of 100 μL of aqueous humor(AH) was collected during phacoemulsification and 21 inflammatory markers were quantified using a Luminex~? cytometric bead assay: IL-1 Ra, IL-1α, IL-1β, IL-5, IL-6, IL-10, IL-17, GM-CSF, IFNγ, CCL2, CCL3, CCL4, CXCL5, CXCL8, bFGF, VEGF, TNFα. Main changes in cytokine profile were analyzed and compared between groups. Data on demographics, duration of glaucoma, intraocular pressure(IOP), number of anti-glaucoma substances were recorded for correlation analysis and prediction models. RESULTS: Significant differences in cytokine expression between groups were detected for CXCL5(P<0.001), CXCL8(P=0.004), IL-1α(P<0.001), IL-2(P<0.001), CCL4(P=0.003), CCL5(P<0.001) and TNFα(P=0.05). Post-hoc analysis identified IL-2(P=0.009) and CXCL5(P<0.001) as "separation markers" between POAG and diabetic POAG eyes. In POAG patients, the "separation markers" could highly predict the TNFα levels F(1, 16)=14.639, P<0.001, whereas in diabetic patients F(1, 24)=4.844, P=0.006 and diabetic POAG patients F(1, 19)=2.358, P=0.05 the level of prediction was ***: Our results reveal an inflammatory model based on increased TNFα levels in POAG *** co-stimulatory molecules and additional inflammatory pathways need to be further explored in diabetic POAG cases, since the prediction model could only partially explain the increased TNFα level in this category of patients.

Climate change has led human beings to take an interest in the study of meteorological and climatic phenomena. In fact, the main impact of climate change on different sectors of society is caused by extreme events since the occurrence of extreme events leads to more impact related to change in mean climate. Unfortunately, the West African region is vulnerable to extreme rainfall impact because its economy is based on rain-fed agriculture. This study examined the seasonal variability of extreme rainfall in West Africa. Eight (8) climate indices were chosen from among the 27 defined by the Expert Team on Climate Change Detection and Indices (ETCCDI). The nonparametric Mann-Kendall test was used to assess the seasonal trends. The indices of the same types (frequency or intensity) were compared to assess the intra-seasonal variation of extreme precipitation. The results indicate that, regardless of the season, the Gulf of Guinea receives more rainfall than the Sahel. This phenomenon is due to the fact that the coastal part of West Africa is under the influence of evaporation which is observed at the Atlantic Ocean and during the monsoon, while the other part is dominated by the desert. Mann-Kendall’s test revealed upward and downward trends during each season. The increase in extreme rainfall trends in the number of consecutive dry days suggests that droughts, due to global warming, could be observed and could have severe consequences in terms of water availability, energy supply, agricultural yields and ecosystems in West Africa. In addition, it can lead to the loss of biodiversity and health issues. It is therefore essential for policymakers or decisions makers to determine strategies and mitigation measures against climate change and its impacts on populations.

目的：测量中国汉族人与高加索人的健康成年人口腔黏膜血流参数,比较口腔黏膜血流灌注情况在不同人种间的异同,比较口腔黏膜在上颌与下颌、男性与女性之间的差异性,为口腔种植临床中软组织处理提供参考依据。方法：从德国波恩大学在读学生中招募20名高加索人种学生（男性10例,女性10例,平均年龄25.3岁）和20名中国汉族留学生（男性10例,女性10例,平均年龄28岁）,口腔内设计8个测量点M1~M8,其中于上颌中切牙唇侧黏膜、下颌中切牙唇侧黏膜、上颌腭侧黏膜和颊黏膜各2个点。使用O2C激光多普勒血流仪进行血流参数测量,包括血氧饱和度（saturation oxygen,Sp O2）、平均血红蛋白量（relative amount of hemoglobin,r HB）、单位时间内血流量（flow）、血流速度（velocity）。结果：高加索人种和中国汉族两组之间比较,上颌中切牙血氧饱和度平均值分别为75.6%±8.2%和70.4%±7.6%;颊黏膜血红蛋白量平均值分别为79.9±5.8和83.5±6.6,差异有统计学意义,其余各位点血流参数差异无统计学意义。将测量位点各组之间进行比较：上腭黏膜的血氧饱和度为77.1%±10.9%,平均血红蛋白量为67.8±11.1,单位时间血流量194.1±63.7,相比较其他测量部位差异有统计学意义。男性多个测量位点血氧饱和度普遍低于女性,差异均有统计学意义。结论：口腔中不同解剖部位的黏膜血流灌注情况存在差别,性别之间血红蛋白氧结合能力可能存在差别,并且可能对于软组织愈合能力有一定影响;口腔黏膜血流灌注情况在小样本青年人群中的高加索人种与中国汉族人之间差异无统计学意义。

背景:在发达国家,憩室出血是严重的急性下消化道出血最常见的原因。早期结肠镜(≤24小时)的作用仍然存在争议,目前尚无早期肠镜应用于急性憩室出血的数据。本研究旨在通过美国全国住院患者样本库,评估结肠镜诊治的时机对急性憩室出血患者预后的影响。方法:采用第9版国际疾病分类(ICD 9),对全国住院患者样本库中2012-2014年间的病例进行选择并提取资料。纳入出院诊断(包括主要诊断或第二诊断)为憩室病合并出血和憩室炎合并出血的病例,排除年龄不满18周岁以及住院期间未行结肠镜检查的病例。首要结局指标为住院时间和住院总费用。结果:共计88,600例患者纳入研究,其中45,020例(50.8%)在入院24小时内接受了结肠镜检查(早期肠镜组),而另外43,580例(49.2%)则是在入院24小时后行结肠镜检查(晚期肠镜组)。早期肠镜组较晚期肠镜组患者住院时间显著缩短(3.7天vs 5.6天,P<0.001),住院总花费显著降低($9317 vs$11,767,P<0.001)。两组患者病死率的差异无统计学意义(0.7%vs 0.8%)。对潜在致混淆因素进行校正后,两组间住院时间和住院总费用的差异仍然具有统计学意义。结论:对于急性憩室出血,早期结肠镜诊治可显著缩短住院时间,降低住院费用,而且并不会增加病死率。选择适宜的急性憩室出血病例进行早期结肠镜诊治,具有节省医疗资源的潜在意义。进一步研究需明确哪些憩室出血患者能从早期结肠镜诊治中获益。

Assessment of Groundwater Vulnerability in Kaduna Metropolis, Northwest Nigeria

DRASTIC index model was employed in the assessment of the intrinsic groundwater vulnerability to contamination in Kaduna metropolis, Nigeria. The model evaluates the contribution of seven environmental parameters (Depth to water level, Net Recharge, Aquifer media, Soil media, Topography, Impact of vadose zone, and Hydraulic Conductivity) in the protection of groundwater against contamination. The mapping was conducted within the framework of Geographical Information System. The study area has very low, low to slightly moderate vulnerability with highest and lowest DRASTIC values of 131 and 77 respectively. To have better understanding of the spatial vulnerability of groundwater in the area, the DRASTIC map was reclassified into five (very high, high, moderate, low and very low) vulnerability zones. Generally, the distribution of the vulnerability classes indicated the low to moderate vulnerability status of the majority parts of the study area, with high vulnerability at the center. Strict control measures should be put in place when locating land uses with high potential hazards in the high and very high vulnerability areas.
