Levodopa Pharmacokinetics in Brain after Both Oral and Intravenous Levodopa in One Patient with Advanced Parkinson’s Disease

Objective: One patient received oral levodopa during a study aiming for better understanding of the basal ganglia and of the mechanisms of deep brain stimulation of the subthalamic nucleus (STN DBS) with and without intravenous (IV) levodopa infusion in patients with Parkinson’s disease (PD). The results from oral and IV levodopa treatment are presented. Methods: Five patients with advanced PD were included in the original study. During planned STN DBS surgery microdialysis probes were implanted in the right putamen and in the right and left globus pallidus interna (Gpi). During the study, microdialysis was performed continuously and STN DBS, with and without IV levodopa infusion, was performed according to a specific protocol. After DBS surgery, but before STN DBS was started, one patient received oral levodopa/ benserazide and entacapone tablets out of protocol due to distressing parkinsonism. Results: The levodopa levels increased prompt in the central nervous system after the first PD medication intakes but declined after the last. Immediately the levodopa seemed to be metabolized to dopamine (DA) since the levels of DA correlated well with levodopa concentrations. Left STN DBS seemed to further increase DA levels in left Gpi while right STN DBS seemed to increase DA levels in the right putamen and right Gpi. There was no obvious effect on levodopa levels. Conclusions: The results indicate that PD patients still have capacity to metabolize levodopa to DA despite advanced disease with on-off symptoms and probably pronounced nigral degeneration. STN DBS seems to increase DA levels with a more pronounced effect on ipsilateral structures in striatum.

Is Levodopa Pharmacokinetics in Patients with Parkinson’s Disease Depending on Gastric Emptying?

Levodopa uptake from the gastrointestinal tract in patients with Parkinson’s disease (PD) can be affected by delayed gastric emptying (GE). This might lead to fluctuating levodopa levels resulting in increased motor fluctuations. Continuous dopaminergic stimulation (CDS) improves motor fluctuations and could be a result of smoothening in levodopa uptake. In this study we wanted to study the levodopa pharmacokinetics peripherally in PD patients with motor fluctuations and investigate the relation between levodopa uptake and GE and the effect of CDS. PD patients with wearing off (group 1) and on-off syndrome (group 2) were included. Breath tests were performed to evaluate the half time (T1/2) of GE. Concomitantly 1 tablet of Madopark®was given and the levodopa concentrations in blood and subcutaneous (SC) tissue were analyzed for both groups. Group 2 was then given a 10-d continuous intravenous levodopa treatment and the tests were repeated. Higher levels of levodopa in group 1 compared to group 2 in blood (p = 0.014) were seen. The GE was delayed in both group 1 (p < 0.001) and group 2 (p < 0.05) compared to a reference group with healthy volunteers with T1/2 median values 105 and 78 min vs. 72 min. There was no difference in GE between the two PD groups (p = 0.220) or in group 2 before and after infusion period (p = 0.861). CDS resulted in lower levodopa levels in blood (p < 0.001) and SC tissue (p < 0.01). In conclusion, PD patients in early complication phase have a more favourable levodopa uptake than patients later in disease. We found delayed GE in PD patients with motor fluctuations but no obvious relation between GE and levodopa uptake or GE and PD stage. The effect of CDS indicates no effect of CDS on the mechanisms of GE but on the mechanisms of levodopa uptake.

Crescentic glomerulonephritis in children:a single centre experience

Background:Crescentic glomerulonephritis(CsGN)is characterized by crescents in 50%or more of glomeruli and clinically by a sudden and progressive decline in renal ***:We evaluated the etiology,clinical features,prognostic factors and long-term outcome of *** January 2000 and December 2010,45 children(26 girls,19 boys)with biopsy-proven CsGN(>50%crescents)were investigated ***:The mean age of the patients was 130.86±33.77 *** mean duration of symptoms prior to diagnosis was 26±12 days(4-40 days).Most of the children had hypertension(62.2%),macroscopic hematuria(73.3%),oligoanuria(44.4%),edema(51.1%)and purpuric rash(40%)at *** final clinical status of the patients was complete remission(n=21),partial remission(n=5)or chronic kidney disease(n=19).Adverse outcomes were significantly associated with a long duration between the onset of symptoms and treatment(P=0.038),the presence of oligoanuria(P=0.006),a severe decreased glomerular filtration rate(GFR75%)(P=0.03),and the ratio of fi brous crescents(P=0.015).Conclusion:The outcome of CsGN in children continues to be poor,and it should be treated as a renal emergency.

Strontium Substituted Nanohydroxyapatite Incorporated 3D Printing Scaffold for Bone Tissue Engineering

The customized implants which are composed of polycaprolactone( PCL) and strontium substituted nanohydroxyapatite( SrHA) were fabricated successfully by using fused deposition modeling( FDM),which is a simple 3 D printing technology for fabricating personalized products. The physical and chemical properties of composite scaffolds were characterized by transmission electron microscopy( TEM), Fourier transform infrared spectroscopy( FTIR), X-Ray diffraction( XRD) and inductively coupled plasma-atomic emission spectroscopy( ICPAES). The results suggested that strontium element was successfully doped into nanohydroxyapatite and all scaffolds showed the homogeneous network structure. Furthermore, the in vitro biocompatibility of the scaffolds was evaluated by cell counting kit-8( CCK-8) assay. The data indicated that the prepared scaffolds exhibited excellent biocompatibility to bone marrow mesenchymal stem cells( BMSCs). Besides,strontium element can be released from PCL-SrHA scaffolds in a sustained manner. Therefore,the 3 D printing PCL-SrHA scaffolds hold great potential for bone tissue engineering.

A Giant Mesenteric Cyst: A Rare Entity in Childhood Surgical Spectrum

Aim: Mesenteric cysts are rare abdominal masses which cause vomiting and distention and may be complicated with volvulus, bleeding, torsion and rupture, gastrointestinal perforation and even urinary obstruction. We present a patient with a mesenteric cyst causing gastrointestinal obstruction which treated surgically. Case: A 3 years old male patient was admitted to our hospital with symptoms of vomiting, constipation and abdominal distention. Plain abdominal anteroposterior X-ray study showed air-fluid levels and abdominal ultrasonography revealed a cystic mass of 15 × 8 cms. Abdominal computerized tomographic examination with and without contrast material revealed a giant cystic mass with septations together with secondary findings of intestinal obstruction. Exploratory laparotomy via midline incision was made. A cystic mass containing a clear fluid was observed. The mass was obstructing the ileal passage. The cyst together with 10 cms. of ileum were removed and an ileo-ileostomy was made. Pathological examination was consistent with mesenteric cyst. He was discharged to full recovery with a symptom-free postoperative tenth day. The child is doing well at 1 year after surgery. Discussion: Mesenteric cysts, although they are rare, can be easily diagnosed modern diagnostic imaging methods and treated surgically.

Teaching Ultrasound Imaging for Central Line Placement—A Resident’s Perspective

The availability of ultrasound (US) devices has refined techniques for vascular access. The resident trainee’s ability to learn US techniques depends upon device availability, skills, and motivation of instructors. We hypothesized that hands-on teaching program of US techniques for vascular access would have a positive impact on the trainee’s perceptions. After IRB approval, PGY3 anesthesiology residents with limited prior experience in US-guided vascular access underwent close supervision by attending staff as they performed US-guided Internal Jugular Vein (IJV) central line placements. A total of 66 land mark (LM) and 75 US-guided IJV cannulation performed by 19 residents were supervised. Residents completing the study were more proactive with 68.4% agreeing to use US based techniques for an anticipated difficulty in accessing central lines. The graduating residents involved in the study all strongly agreed that US imaging for vascular access was within the scope of practice of all anesthesiologists, and that training guidelines be established and incorporated in their practice. Fifty eight percent of residents stated that their confidence and skill levels had significantly improved. Teaching US-guided techniques resulted in an increase employment of such techniques by graduating residents with no impediment attributed to knowledge deficits or inexperience. Residents were more proactive with using US for anticipated difficulty in IJV placement. A short neck, poor landmark, anticoagulation, morbid obesity favored US use by residents. Systematic teaching of US techniques is justified as it significantly improves the resident’s perceptions and confidence.

Aggregation-induced emission luminogens(AIEgens)exhibit efficient cytotoxic reactive oxygen species(ROS)generation capability and unique light-up features in the aggregated state,which have been well explored in image-guided photodynamic therapy(PDT).However,the limited penetration depth of light in tissue severely hinders AIEgens as a candidate for primary or adjunctive therapy for clinical ***,microwaves(MWs)show a distinct advantage for deeper penetration depth in tissues than ***,for the first time,we report AIEgen-mediated microwave dynamic therapy(MWDT)for cancer *** found that two AIEgens(TPEPy-I and TPEPy-PF6)served as a new type of microwave(MW)sensitizers to produce ROS,including singlet oxygen(1O2),resulting in efficient destructions of cancer *** results of 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide(MTT)and live/dead assays reveal that the two AIEgens when activated by MW irradiation can effectively kill cancer cells with average IC-50 values of 2.73 and 3.22μM,***,the ability of the two AIEgens to be activated by MW not only overcomes the limitations of conventional PDT,but also helps to improve existing MW ablation therapy by reducing the MW dose required to achieve the same therapeutic outcome,thus reducing the occurrence of side-effects of MW radiation.

Reconstruction of the urethral defects with autologous fascial tube graft in a rabbit model

Aim： To investigate the feasibility of the autologous fascia graft in urethra defect reconstruction. Methods： In 24 adult male rabbits, a standardized defect （17 mm） was created within the midportion of each urethra. Two-cm long fascial tube grafts were interposed between the cut ends of the urethra. Twenty-four rabbits were divided into 12 groups. At 0, 3, 10, 15, 21, 30, 45, 60, 90, 120, 150, and 180 days postoperatively, one group was killed. In the first four groups, rabbits were killed and specimens were obtained for histological examination. After 21 postoperative days, in the subsequent eight groups, retrograde urethrograms were carried out to evaluate urethral patency and caliber, then rabbits were killed and specimens were obtained. Results： In the histological study, advancement of the urethral transitional epithelium along scaffold provided by the fascial graft was determined. At the 30th day, the new urethra was completely covered with the transitional epithelium. Fistula formation was observed in two of 24 rabbits. In urethrograms, narrowing was determined in three of 16 rabbits. Conclusion： For segmental urethral reconstruction, fascial graft is a good urethral substitute because of its rapid epithelization capacity, low contraction degree and thinness. We therefore propose the use of fascial grafts for reconstruction of male-urethra defects in humans.

Malignant Gastrointestinal Neuroectodermal Tumor (GNET) is a recently described rare malignant neoplasm, resembling “clear cell sarcoma of tendons and aponeuroses” morphologically, immunohistochemically and molecular pathologically. We will describe a case of soft tissue sarcoma located at right cervical region of a 9-year-old male patient, mimicking malignant GNET, which can be considered as extragastrointestinal counterpart of this tumor. These tumors have poor prognosis. This is unique, possibly the first case of a soft tissue sarcoma resembling GNET.
