Lhermitte-Duclos disease (LDD) or dysplastic gangliocytoma of the cerebellum is a rare benign lesion of uncertain pathogenesis characterized by overgrowth of cerebellar ganglion cells which replace granular cells and Purkinje cells;this results in gross thickening of the cerebellar folia. It is revealed by symptoms of raised intracranial pressure, cerebellar impairment and obstructive hydrocephalus. We reported the case of a 41-year-old male who presented with symptoms of acute raised intracranial pressure. Brain computed tomography (CT) scan revealed hydrocephalus due to compression of the 4th ventricle by a large non-enhancing left cerebellar mass. Magnetic resonance imaging (MRI) showed a space-occupying lesion within the left cerebellar hemisphere with unusual striation. Radical surgery was retained though the margins with normal cerebellar tissue were not distinct. Clinical complications after gross total or partial removal of Lhermitte-Duclos lesions have been rarely reported in the literature;herein we stress the importance of extreme caution in removing these lesions in cerebellar areas that have no distinct margins between the lesion and normal tissue.

Background :Non-Hodgkin lymphomas are the first childhood cancer in sub-Saharan Africa. objective: The purpose of this study was to assess non-Hodgkin lymphomas cases in our setting. Methodology: A retrospective and descriptive study carried out in the pediatric oncology unit of the Gabriel Touré Teaching Hospital Bamako over 10 years from 1st January 2005 to 31th December 2015. Results: We exploited 274 (21.6%) cases of Non-Hodgkin Lymphoma out of 1295 cancer cases registered, the age group 6 - 10 years was the most represented (46.4%);the male sex was predominant with a sex-ratio of 1.8;digestive signs were the most common signs of discovery (44.2%) followed by maxillary swelling (42.7%);the majority of patients (52.9%) consulted between 1 and 3 months after the onset of signs;the malnutrition rate was 39.8%, of which 24.1% were severe cases and 15.7% were moderate rate. Abdominal localization was the most common (43.1%) followed by maxillofacial localization (33.9%). Almost all were Burkitt type cytology (92.7%), the majority (73.4%) were in Murphy stage III. Almost all (96%) had received chemotherapy and the modified LMB 01 protocol was widely used (62.4%). The majority of patients (85%) were chemosensitive at day 7 or after the third cyclophosphamide injection but at the end of induction only 31% were in complete remission. Gastrointestinal toxicity was the most common (37.13%) followed by hematologic toxicity 35.09 %;9.12 % of patients were lost of follow-up and 22.26% died. Tumor progression was the most common cause of death (60.66%) followed by infection (21.31%). Conclusion: In light of these findings, the late diagnosis and the poor management of NHL, as well as the limited ability to primarily treat metabolic complications, explain the high case-fatality rate, hence the important role of early diagnosis and treatment multidisciplinary.

Introduction: Neonatal asphyxia (NA) is one of the most likely causes of neuro-developmental abnormalities in children. In Mali it is responsible for half of the early deaths and the third of neonatal mortality. Updated data would help understand and improve intervention strategies to reduce mortality. objective: It is the study of epidemiological and clinical characteristics, the immediate outcome and the factors associated with newborn (NB) mortality with NA. Material and Methods: This was a prospective cross-sectional study from June 27th to September 3rd 2016 about the NBs admitted for NA in the Hospital Teaching Gabriel Touré of Bamako. The clinical and biological data including the prognosis were collected from the health records of women, the liaison sheets and the medical file. The analysis was done using the software Epi info version 3.5.1. Results: 76 NBs were included which represented 23.45% of hospitalizations. The majority (89.5%, n = 68) were admitted to less than 24 hours of life for NA grade III according to the Sarnat classification (43.4%, n = 33). The average age of mothers was 24.17 ± 5.5 years. Almost half (41.3%, n = 31) were primigravida. The most common obstetrical event was dystocia (64.5%, n = 49). The prognosis was poor in grade III anoxia in our patients (56%) of deaths. Conclusion: The périnatal anoxia (PA) is a major health issue in Mali because of its frequency and severity. Monitoring of pregnancies, delivery assisted by skillful and qualified personnel, mastery of neonatal resuscitation techniques are good means of prevention.

An Application of Machine Learning Methods to Detect Mango Varieties

The mango, a fruit of immense economic and dietary significance in numerous tropical and subtropical regions, plays a pivotal role in our agricultural landscape. Accurate identification is not just a necessity, but a crucial step for effective classification, sorting, and marketing. This study delves into the potential of machine learning for this task, comparing the performance of four models: MobileNetV2, Xception, VGG16, and ResNet50V2. These models were trained on a dataset of annotated mango images, and their performance was evaluated using precision, accuracy, F1 score, and recall, which are standard metrics for image classification. The Xception model, with its exceptional performance, outshone the other models on all performance indicators. It achieved a staggering accuracy of 99.47%, an F1 score of 99.43%, and a recall of 99.43%, showcasing its remarkable ability to accurately identify mango varieties. MobileNetV2 followed closely with performances of 98.95% accuracy, 98.85% F1 score, and 98.86% recall. ResNet50V2 also delivered satisfactory results with 97.39% accuracy, 97.08% F1 score, and 97.17% recall. VGG16, however, was the least effective, with a precision rate of 83.25%, an F1 score of 83.25%, and a recall of 85.47%. These results confirm the superiority of the Xception model in detecting mango varieties. Its advanced architecture allows it to capture more distinguishing features of mango images, leading to greater precision and reliability. Xception’s robustness in identifying true positives is another advantage, minimizing false positives and contributing to more accurate classification. This study highlights the promising potential of machine learning, particularly the Xception model, for accurately identifying mango varieties.

The aim of this study was to carry out a dynamic simulation of the energy and environmental performance of a built space system, with a view to assessing its energy and environmental class. The use of a simulation and modeling tool, supported by various methodological references, formed the basis of our approach. Adopting a systemic perspective, we described the structural and functional aspects of the systems making up built spaces, as well as the associated energy flows. our approach was also based on a typology, taking into account typical days, structural and functional configurations at different scales and angles of observation. The analysis tool we developed in Java was applied to the built space system of the Patte d’oie university campus in ouagadougou. Annual electricity consumption was measured at 124387.34 kWh, closely aligned with the average annual electricity bill (125224.31 kWh), with a maximum relative deviation of 1%, followed by a carbon emission balance of 58337.66 kg eq Co2 per year. This validation confirmed the effectiveness of our tool. In addition, following the analysis of electricity consumption using our tool, the university campus was classified in energy class B and environmental class C. These results will be based on the emission factors of the energy mix of the West African Economic and Monetary Union (WAEMU) territory, with particular emphasis on Burkina Faso.

In this work, we present numerical modelling of coupled heat and mass transfer within porous materials. our study focuses on cinder block bricks generally used in building construction. The material is assumed to be placed in air. Moisture content and temperature have been chosen as the main transfer drivers and the equations governing these transfer drivers are based on the Luikov model. These equations are solved by an implicit finite difference scheme. A Fortran code associated with the Thomas algorithm was used to solve the equations. The results show that heat and mass transfer depend on the temperature of the air in contact with the material. As this air temperature rises, the temperature within the material increases, and more rapidly at the material surface. Also, thermal conductivity plays a very important role in the thermal conduction of building materials and influences heat and mass transfer in these materials. Materials with higher thermal conductivity diffuse more heat.

Pneumonia ranks as a leading cause of mortality, particularly in children aged five and under. Detecting this disease typically requires radiologists to examine chest X-rays and report their findings to physicians, a task susceptible to human error. The application of Deep Transfer Learning (DTL) for the identification of pneumonia through chest X-rays is hindered by a shortage of available images, which has led to less than optimal DTL performance and issues with overfitting. overfitting is characterized by a model’s learning that is too closely fitted to the training data, reducing its effectiveness on unseen data. The problem of overfitting is especially prevalent in medical image processing due to the high costs and extensive time required for image annotation, as well as the challenge of collecting substantial datasets that also respect patient privacy concerning infectious diseases such as pneumonia. To mitigate these challenges, this paper introduces the use of conditional generative adversarial networks (CGAN) to enrich the pneumonia dataset with 2690 synthesized X-ray images of the minority class, aiming to even out the dataset distribution for improved diagnostic performance. Subsequently, we applied four modified lightweight deep transfer learning models such as Xception, MobileNetV2, MobileNet, and EfficientNetB0. These models have been fine-tuned and evaluated, demonstrating remarkable detection accuracies of 99.26%, 98.23%, 97.06%, and 94.55%, respectively, across fifty epochs. The experimental results validate that the models we have proposed achieve high detection accuracy rates, with the best model reaching up to 99.26% effectiveness, outperforming other models in the diagnosis of pneumonia from X-ray images.

Impact of Vascular Variations on Living Donor Kidney Transplantation

Background: Kidney transplantation is the best treatment for end-stage chronic kidney disease. However, its realization is confronted by several difficulties among which are anatomical variations. objective: The objective of our work was to describe the impact of renal pedicle variations on the operative procedure as well as the complications. Method: We conducted a retrospective study on living kidney donors and their recipients in the period from 2012 to 2017. Several variables were studied, in particular socio-demographic, operative and progression. Results: We identified 49 living donors whose mean age was 37.59 years with a male-to-female ratio of 3.9 and 45 ± 10.75 years in the recipients. The prevalence of renal vascular abnormalities was higher with a proportion of 46.94% in recipients. The left kidney was most frequently removed (75.51%) and then kept mainly in HTK (95.92%). These vascular anomalies were associated with a longer operating time but this difference was not statistically significant (p = 0.5804). They had no effect on hot and cold ischemia times (p = 0.9838, p = 0.8389). Complications were observed in 11 patients, all recipients, i.e. 11.22%, and were not related to the presence of vascular abnormalities (p = 0.086). We observed that 4.08% of deaths were all recipients. Conclusion: It seems that kidney transplantation with multiple renal arteries and/or veins does not significantly lengthen the operating time and does not promote the onset of complications.
