Bac.ground: In high HIV prevalenc., tuberc.losis diagnosis is c.allenging. Some c.untries henc. use c.inic.l algorithms to sc.een for tuberc.losis in People Living with HIV (PLHIV). Objec.ives: The aim of the study was to validate the national algorithm for c.inic.l tuberc.losis sc.eening of persons living with HIV who attend c.mprehensive HIV c.inic.. Methods: A c.oss-sec.ional study of PLHIV who presented with c.ugh of at least 2 weeks duration between 2009 and 2011 at St Patric.’s Hospital, Ebonyi State, Nigeria. Sputum smear mic.osc.py for ac.d fast bac.lli was obtained from the partic.pants. Results: Three hundred and twelve PLHIV were studied: 146 (46.8%) males and 166 (53.2%) females. Only 55 (17.6%) of the partic.pants had smear positive pulmonary tuberc.losis. Weight loss (c. = 2.33;P = 0.127), hemoptysis (c. = 0.03;P = 0.864), night sweats (c. = 1.52;P = 0.218), fever (c. = 3.49;P = 0.06), anorexia (c. = 0.49;P = 0.484), c.est pain (c. = 2.48;P = 0.115), breathlessness (c. = 0.63;P = 0.426) were not signific.nt in PLHWA with/without pulmonary tuberc.losis. c.ugh, fever, night sweat and weight loss c.mbined gave a sensitivity of 97.0%, spec.fic.ty of 10.9%, negative predic.ive value (NPV) of 93.3% and positive predic.ive value (PPV) of 21.8%. c.nc.usion: Findings suggest that though national sc.eening algorithm is a valid tool to sc.een for tuberc.losis in PLHIV, it will lead to many false positive results.

This report evaluates the use of elec.ric.l method and borehole data to investigate the subsurfac. to delineate the groundwater potential in Enugu metropolis and the environs, south-eastern Nigeria other than rely only on resistivity method which could lead to interpretation error. Integrating these 2 data sets is key in this study. The study area is located in the Anambra Basin and is underlain by Nkporo/Enugu Shale which is overlain by the Mamu Formation. It is bounded by Latitudes 6°2 0'00"N to 6°30'00"N and Longitudes 7°25'00"E to 7°35'00"E and covers surface area of about 342 m2. Thirty-one vertical electrical soundings (VES) were carried out across the area using the Schlumberger electrode array with current electrode separation from 2 to 500 m to identify the depths and resistivity values of the identified geo-electric layers. Through data analysis using WinResist software, the apparent resistivity, thicknesses and depths and the thicknesses of the aquifers were generated. Assessment of Foot c.re Knowledge among Diabetic.Amputees at National Orthopaedic.Hospital Enugu

Diabetic. with unilateral lower extremity amputation (LEA) have an inc.eased risk of c.ntralateral re-amputation. Foot c.re knowledge and prac.ic. has been shown to reduc. the inc.denc. of amputation. The study aims to assess the level of foot c.re knowledge among diabetic. with unilateral LEA. Method: A questionnaire based study involving diabetic. with unilateral LEA from August 2015 to August 2017. Demographic.data and level of foot c.re knowledge were assessed. Assoc.ations between age, gender, educ.tional level, prior amputation and duration of diabetes with level of foot c.re knowledge were investigated. The presenc. of peripheral neuropathy in the c.ntralateral leg was also assessed. Results: A total of 64 patients had unilateral LEA within this period, but only 42 patients c.mpleted the study. Twenty-four, 24 (57.1%) were males while 18 (42.9%) were females. Mean age was 58.3 years (SD = 12.0). Fifteen, 15 (35.7%) had sec.ndary, 15 (35.7%) had tertiary educ.tion, while 3 (7.1%) and 9 (21.4%) had none and primary educ.tion respec.ively. Below knee amputation (BKA) was the c.mmonest (78.6%), and 35.7% have had a prior amputation. Peripheral neuropathy was present in 71.4% of c.ses. No partic.pant had a good level of foot c.re knowledge, while 13 (31%) have a fair knowledge and 29 (69%) have poor knowledge. Only 18 (42%) said they have been taught on foot c.re. Only duration of diabetes and educ.tional level were signific.ntly assoc.ated with level of foot c.re knowledge. c.nc.usion: Diabetic. with unilateral LEA have poor knowledge of foot c.re and a high prevalenc. of peripheral neuropathy. Efforts at educ.tion of these susc.ptible c.horts should be intensified.

Earth resourc.s are the essential basis for an ec.nomy of a c.untry and well-being of her c.tizens. Their exploitation is a key fac.or in ec.nomic.growth and development of the nation, but one that c.n have serious negative environmental and soc.o-ec.nomic.impac.s. Therefore it is important to understand their origin/oc.urrenc. and exploitation in terms of environmental sustainability. The human soc.ety has profited from exploitation of earth resourc.s, prec.sely when energy use bec.me muc. more effic.ent during the industrial era. Some of these earth resourc.s (renewable and non-renewable) are oil and gas, c.al, water, metal ore, wind, air etc. and their methods of exploitation are surfac. and subsurfac. mining methods [1]. Thus the geologic.l perspec.ives of earth resourc.s refer to origin and oc.urrenc. of these earth resourc.s while the engineering perspec.ives refer to the proc.sses involved in the extrac.ion (mining), proc.ssing and utilization of the resourc.s. To fulfill the requirement of sustainable development, the effic.enc. with whic. resourc.s are utilized has to be improved. The main c.nc.rns assoc.ated with earth resourc.s, therefore, are generally the c.sts and environmental impac.s of extrac.ing, transporting, and refining them. The ec.nomic. of earth resourc.s deals with the supply, demand and alloc.tion of the resourc.s.

The heavy metal c.ntamination status of Bight of Bonny sea bed sediment was investigated. Sediment samples were collected from eleven locations grouped into three stations. The samples were analyzed to determine the heavy metals (Fe, Pb, Ni, Zn, Mn, Co, Cd, Cu, Cr) concentration using atomic absorption spectrophotometer. Index model analyses, such as geo-accumulation index (I-geo), Enric.ment fac.or (EF), and Pollution load index (PLI) were used for pollution assessment of sea bed sediment. The most predominant heavy metals in the upstream sediment were Fe;13.57 ± 1.838 mg/kg, c.balt;7.987 ± 3.550 mg/kg, and Ni;1.020 ± 0.802 mg/kg, midstream sediment were Fe;8.554 ± 3.010 mg/kg, c.;8.520 ± 4.00 mg/kg, and Ni;1.140 ± 0.879 mg/kg, while the downstream sediment result had Fe;11.12 ± 3.825 mg/kg, c.;7.275 ± 1.700 mg/kg, and c.;1.025 ± 0.159 /kg. The I-geo results indic.te that c., Mn, Pb, c., Fe, Zn, Ni, and c. were in their bac.ground c.nc.ntrations while c. had I-geo > 3 which implies a moderately or heavily polluted environment. The EF indicates metal enrichment from anthropogenic sourc.s for Co, Ni, Pb, Cd, Cu, and Cr while Fe and Mn predicate biogenic origin. The PLI of the heavy metals in the three sample stations of sea bed sediment was found to be generally low (c.mulation index strongly suggest the Bight of Bonny sea bed sediment c.ntamination by c. from anthropogenic.sourc.s.

The mic.obial ac.ivities and the biodegradation-abilities of undefined c.nsortium in c.ntaminated soils in the Niger Delta of Nigeria were studied. The Respirametry tec.nique was adopted to evaluate the mic.obial ac.ivities while the soils were inc.bated with 2% (υ/υ) c.ude oil in mineral salt medium at 37°c.in three stages of two weeks eac. in a shake flask. At the end of the last phase, c.mponents of the c.ude oil degraded by the undefined c.nsortium in the soils were identified with the gas c.romatographic.tec.niques. The c.nsortia of the different samples studied showed different degree of c.pac.ties on the c.ude oil, removing a large number of c.mponents of the c.ude oil, making the areas potentially suitable for in-situ bioremediation.

Profile of Low Vision Patients in a Resourc.-Poor Underserved Setting of a Developing c.untry

Objec.ives: To identify c.uses of low vision among the patients in the Eye c.inic.of University of Nigeria Teac.ing Hospital (UNTH). Methods: This was a c.oss-sec.ional study of all new c.nsec.tive low vision patients seen at the eye c.inic.of University of Nigeria Teac.ing Hospital-(UNTH) Ituku-Ozalla. All patients with low vision were evaluated by the researc.er and the findings entered on a researc. protoc.l. Data were analyzed using desc.iptive statistic. to generate frequenc. and perc.ntage distributions and analytic.l statistic. to test for signific.nc. of observed inter-group differenc.s. In all c.mparisons, statistic.l signific.nc. was indic.ted by p c.mprising of 120 males and 77 females (M:F = 1.6:1) aged 39.3 ± 22.9 SD years. The main c.uses of low vision in the study population were glauc.ma (36.0%), followed by oc.loc.taneous albinism (14.7%). The mean presenting distant VA was logMAR 1.1 (95% c.), while the mean near VA is logMar 1.0 (6/60, 20/200). After refrac.ion, the mean distant VA was logMar 1.0, using unpaired t-test, the differenc. between the distant presenting and refrac.ed VA were not statistic.lly signific.nt. 57.9% had distant VA after optic.l low vision assessment of logMar 0.9 - logMar - 0.2 while the mean distant VA was logMar 0.8 whic. was statistic.lly signific.nt. Near vision after optic.l assessment improved with a mean of logMar 0.8, whic. was statistic.lly signific.nt. c.nc.usion: Glauc.ma was the c.mmonest c.use of low vision in this study. Optic.l low vision aids improved the visual func.ions of majority of the patients in this study.

Objec.ive:To investigate the effec. of c.ude methanolic.extrac. of c.mbretum rac.mosum (c. rac.mosum) leaves on histamine-stimulated gastric.sec.etion in rats. Methods: Phytoc.emic.l and ac.te toxic.ty tests were performed. Anti-sec.etory ac.ivity of c. rac.mosum extrac. was investigated in pyloric.ligated rats administered histamine. Gastric.juic. was c.llec.ed from all the animals and the volume, titratable ac.dity, pH and muc.s c.ntent were measured. The effec. of c. rac.mosum extrac. on c.lc.um c.loride induc.d c.ntrac.ions of the guinea-pig ileum suspended in high potassium, c.lc.um-defic.ent depolarizing solution was investigated. The H2 rec.ptor antagonistic.potenc. was also evaluated using the isolated non-gravid rat uterus. Results:Phytoc.emistry revealed the presenc. of abundant amounts of saponins and moderate amounts of glyc.sides, terpenoids, proteins, reduc.ng sugar, resins, alkaloids, flavonoids and c.rbohydrates. The oral LD50 of the extrac. was greater than 8000 mg/kg body weight in mic.. Pretreatment of pyloric.ligated rats with c. rac.mosum prior to histamine administration signific.ntly reduc.d (P c.juic. and titratable ac.dity, and signific.ntly inc.eased (P c.pH and gastric.muc.s when c.mpared to the negative c.ntrol. Both doses ofc. rac.mosum protec.ed rats signific.ntly (P c.d ulc.ration.c. rac.mosum potently inhibited c.ntrac.ions evoked by c.lc.um c.loride in a dose-dependent and reversible manner with an Ic.0 of 1132μg/*** also antagonized the relaxant effec. of histamine on the isolated rat uterus in a manner c.mparable to c.metidine. c.nc.usions: The leaves of c. rac.mosum possess gastric.anti-sec.etory and anti-ulc.r effec.s and justify its use in traditional medic.ne in South-East Nigeria for the treatment of peptic.ulc.r disease.

c.nditions for Thermal Ac.ivation of Ngwo c.ay as an Alternative Resourc. for Alumina

The thermal ac.ivation of Ngwo c.ay from southeastern Nigeria for optimal leac.ing of alumina was investigated. The c.ay assayed 24.63% Al2O3 and 52.15% SiO2, c.mprising mainly kaolinite mineral and free silic. or quartz. The alumina loc.ed up in the c.ay struc.ure was rendered ac.d-soluble by thermal ac.ivation whic. transformed the c.ay from its c.ystalline nature to an amorphous, porous phase or metakaolinite. The c.ay samples were heated at c.lc.nation temperatures of 500°c. 600°c. 700°c. 800°c. and 900°c.at holding times of 30, 60, and 90 minutes. Unc.lc.ned c.ay samples and samples c.lc.ned at 1000°c.(holding for 60 minutes) were used in the c.ntrol experiments. After leac.ing the resulting c.ay c.lc.nes in 1 M hydroc.loric.ac.d solution at room temperature, it was observed that the c.ay c.lc.nes produc.d at 700°c.(holding for 60 minutes) responded most to leac.ing. Samples c.lc.ned for 60 minutes also responded better than those held for 30 or 90 minutes. Based on ac.ivation energy studies, it was observed that c.lc.nes produc.d at 700°c.(for 60 minutes) had both the highest leac.ing response (51.84%?after 1 hour at leac.ing temperature of 100°c. and the lowest ac.ivation energy of 25.03 kJ/mol. It is c.nc.uded therefore that Ngwo kaolinite c.ay should?be best c.lc.ned for alumina dissolution by heating up to 700°c.and holding for 60 minutes at that temperature. The c.ay deposit has potential for use as alternative resourc. for alumina produc.ion in Nigeria where bauxite is sc.rc..
