Primary malignant tumors of the small intestine are rare,comprising less than 2%of all gastrointestinal tumors.An 85-year-old woman was admitted with fever of 40℃ and marked abdominal distension.Her medical history was unremarkable,but blood examination showed elevated inflammatory markers.Abdominal computed tomography showed a giant tumor with central necrosis,extending from the epigastrium to the pelvic cavity.Giant gastrointestinal stromal tumor of the small intestine communicating with the gastrointestinal tract or with superimposed infection was suspected.Because no improvement occurred in response to antibiotics,surgery was performed.Laparotomy revealed giant hemorrhagic tumor adherent to the small intestine and occupying the peritoneal cavity.The giant tumor was a solid tumor weighing 3490 g,measuring24 cm×17.5 cm×18 cm and showing marked necrosis.Histologically,the tumor comprised spindle-shaped cells with anaplastic large nuclei.Immunohistochemical studies showed tumor cells positive for vimentin,CD31,and factorⅧ-related antigen,but negative for c-kit and CD34.Angiosarcoma was diagnosed.Although no postoperative complications occurred,the patient experienced enlargement of multiple metastatic tumors in the abdominal cavity and died 42 d postoperatively.The prognosis of small intestinal angiosarcoma is very poor,even after volume-reducing palliative surgery.

Insoluble particle concentration in ice cores is commonly analyzed as a proxy for varia- tions in atmospheric mineral dust （aerosol concentration）. However, recent studies have revealed that the mineral dust is not only a constituent of the particles but that biogenic organic particles are also contained. We microscopically analyzed insoluble particles in a shallow ice core drilled on a mountain glacier, the Urumqi Glacier No. 1, in eastern Tienshan, China. We distinguished different morphological particles in the ice core and quantified them separately. Results showed that the insoluble particles in this ice core consisted mainly of mineral particles, amorphous organic particles, pollen, and micro- organisms. Mineral particles were the most dominant, accounting for approximately 67% of total par- ticles, and amorphous organic particles were the second most dominant, accounting for approximately 33% of the total. The annual variation in the particles for the last 11 years differed between mineral and amorphous organic particles. The results suggest that the total insoluble particle concentration in the ice core reflects not only the atmospheric mineral dust but also the organic particles blown from ground soil or produced by microbes on the glacial surface.

A new approach for reducing error of the volume penalization method is proposed.The mask function is modified by shifting the interface between solid and fluid by√νηtoward the fluid region,whereνandηare the viscosity and the permeability,respectively.The shift length√νηis derived from the analytical solution of the one-dimensional diffusion equation with a penalization term.The effect of the error reduction is verified numerically for the one-dimensional diffusion equation,Burgers’equation,and the two-dimensional Navier-Stokes equations.The results show that the numerical error is reduced except in the vicinity of the interface showing overall second-order accuracy,while it converges to a non-zero constant value as the number of grid points increases for the original mask function.However,the new approach is effective when the grid resolution is sufficiently high so that the boundary layer,whose width is proportional to√νη,is resolved.Hence,the approach should be used when an appropriate combination ofνandηis chosen with a given numerical grid.

Development of Mental Health Literacy Scale for Depression Affecting the Help-Seeking Process in Health Professional Students

Despite depression being a global mental health disorder,many people with depression do not seek psychiatric help.In particular,it has been reported that only 15.7%of medical students seek treatment.A longer duration of untreated illness(DUI)leads to clinically poor results.To shorten the DUI,the mental health literacy(MHL)with regard to depression needs to be improved,although it is unclear which MHL components will improve the help-seeking process.Additionally,the existing MHL scale for depression is poorly validated for structural validity.Therefore,the purpose of this study was to develop an MHL scale for depression with verified reliability and validity(particularly structural validity)and verify its relationship with the help-seeking process.For this purpose,we adapted the mixed research method.First,we conducted a systematic literature review on MHL scales for depression to prepare a draft scale with content validity.Second,we conducted semi-structured interviews on MHL with outpatients with depression.Consequently,78 questions on MHL for depression were extracted.Third,to develop our scale,we verified the reliability and validity of these questions with 782 health professional students.The sample size was 780 and data collection was performed by collective survey method.It was conducted in 2018 in Japan.To verify the factor structure,exploratory and confirmatory factor analyses were conducted.We also examined the internal consistency,test-retest reliability,and criterion-related validity.The exploratory factor analysis revealed a three-factor structure(recognition of depression symptoms,helpseeking preferences,and depression onset possibility)with 10 items.According to the confirmatory factor analysis results,the goodness of fit indicators had a good fit.The internal consistency,test-retest reliability,and criterion-related validity had good values.Fourth,the relationship between the MHL of depression and other factors affecting the help-seeking process was examined using structural equation modelling.The

Impact of Separator Thickness on Temperature Distribution in Single Polymer Electrolyte Fuel Cell Based on 1D Heat Transfer

It is known from the New Energy and Industry Technology Development Organization (NEDO) roam map Japan, 2017 that the polymer electrolyte fuel cell (PEFC) power generation system is required to operate at 100°C for application of mobility usage from 2020 to 2025. This study aims to clarify the effect of separator thickness on the distribution of the temperature of reaction surface (Treact) at the initial temperature of cell (Tini) with flow rate, relative humidity (RH) of supply gases as well as RH of air surrounding cell of PEFC. The distribution of Treact is estimated by means of the heat transfer model considering the H2O vapor transfer proposed by the authors. The relationship between the standard deviation of Treact-Tini and total voltage obtained in the experiment is also investigated. We can know the effect of the flow rate of supply gas as well as RH of air surrounding cell of PEFC on the distribution of Treact-Tini is not significant. It is observed the wider distribution of Treact-Tini provides the reduction in power generation performance irrespective of separator thickness. In the case of separator thickness of 1.0 mm, the standard deviation of Treact-Tini has smaller distribution range and the total voltage shows a larger variation compared to the other cases.

BACKGROUND Although pathological response is a common endpoint used to assess the efficacy of neoadjuvant chemotherapy (NAC) for gastric cancer, the problem of a low rate of concordance from evaluations among pathologists remains unresolved. Moreover, there is no globally accepted consensus regarding the optimal evaluation. A previous study based on a clinical trial suggested that pathological response measured using digitally captured virtual microscopic slides predicted patients’ survival well. However, the pathological concordance rate of this approach and its usefulness in clinical practice were unknown. AIM To investigate the prognostic utility of pathological response measured using digital microscopic slides in clinical practice. METHODS We retrospectively evaluated pathological specimens of gastric cancer patients who underwent NAC followed by surgery and achieved R0 resection between March 2009 and May 2015. Residual tumor area and primary tumor beds were measured in one captured image slide, which contained the largest diameter of the resected specimens. We classified patients with < 10% residual tumor relative to the primary tumorous area as responders, and the rest as non-responders;we then compared overall survival (OS) and relapse-free survival (RFS) between these two groups. Next, we compared the prognostic utility of this method using conventional Japanese criteria. RESULTS Fifty-four patients were evaluated. The concordance rate between two evaluators was 96.2%. Median RFS of 25 responders and 29 non-responders was not reached (NR) vs 18.2 mo [hazard ratio (HR)= 0.35, P = 0.023], and median OS was NR vs 40.7 mo (HR = 0.3, P = 0.016), respectively. This prognostic value was statistically significant even after adjustment for age, eastern cooperative oncology group performance status, macroscopic type, reason for NAC, and T- and Nclassification (HR = 0.23, P = 0.018). This study presents a 64-year(1951–2014) reconstruction of the surface mass balance of Glacier No. 31, located in the Suntar-Khayata Range of the eastern Siberia, where the ablation zone is characterized by the extensive dark ice surface. We use a temperature index-based glacier mass-balance model, which computes all major components of glacier mass budget and is forced by daily air temperature and precipitation from a nearby meteorological station. The glacier shows a mean annual mass balance of –0.35 m w.e.a^(–1) during the past 64 years, with an acceleration of –0.50 m w.e. a^(–1) during the recent years. A cumulative mass loss of the glacier is ~22.3 m w.e. over the study period, about 56% of which is observed during 1991–2014. In addition to the contribution of temperature rise and precipitation decrease to recent mass loss of the glacier, an experimental analysis, in which the cleanand dark ice surfaces are respectively assumed to cover the entire ablation zone, indicates that dark ice surface, caused by insoluble impurities consisting of mineral dusts, cryoconite granules, and ice algae, plays a crucial role in the changing mass balance through enhancing melt rates in the ablation zone of the glacier.

BACKGROUND：Transplantation of olfactory ensheathing cells （OECs） into the injured spinal cord has been shown to promote axonal regeneration and functional recovery.However,the mechanisms underlying the effects of OEC transplantation remain controversial.OBJECTIVE：To observe fibrotic scar formation and axonal regeneration in the damaged spinal cord following OEC transplantation,and to determine whether OEC transplantation promotes neural regeneration by attenuating fibrotic scar formation.DESIGN,TIME AND SETTING：A randomized,controlled animal experiment was performed at the Department of Developmental Morphology,Tokyo Metropolitan Institute for Neuroscience,Fuchu,Japan and at the Department of Human Anatomy,College of Basic Medical Sciences,China Medical University,China between April 2007 and May 2009.MATERIALS：OECs were obtained from olfactory nerves and olfactory bulbs of male,4-week-old,Sprague Dawley rats.Rabbit anti-serotonin polyclonal antibody,rabbit anti-calcitonin gene-related peptide polyclonal antibody,rabbit anti-glial fibrillary acidic protein polyclonal antibody,rabbit anti-type IV collagen polyclonal antibody,and mouse anti-rat endothelial cell antigen-1 monoclonal antibody were used.METHODS：Male,Sprague Dawley rats aged 8 weeks were randomly divided into three groups：sham-surgery （n = 3）,surgery （n = 9）,and OEC transplantation （n = 11）.Spinal cord transection at the T9-10 level was performed and the rats were transplanted with a 2-μL （1 × 105 cells） cell suspension.MAIN OUTCOME MEASURES：Formation of glial and fibrotic scars was examined using immunohistochemistry for glial fibrillary acidic protein and type IV collagen.Serotonin-positive and calcitonin gene-related peptide-positive axons were visualized by immunohistochemistry,respectively.Double immunofluorescence for type IV collagen and rat endothelial cell antigen-1 was also performed to determine co-localization of type IV collagen deposition and blood vessels.RESULTS：At 1 week after spinal cord injury,numerous glial cells were observed aroun

AIM: To investigate the mechanisms of liver growth and atrophy after portal vein ligation (PVL) and its effects on tumor growth. METHODS: Mice were subjected to PVL, partial hepatectomy, or sham surgery. The morphological alterations, activation of transcription factors, and expression of cytokines and growth factors involved in liver regeneration were evaluated. In a separate set of experiments, murine colorectal carcinoma cells were injected via the portal vein and the effect of each operation on liver tumor growth was studied. RESULTS: Liver regeneration after PVL and partial hepatectomy were very similar. In ligated lobes, various cytokines, transcription factors and regulatory factors were signifi cantly upregulated compared to nonligated lobes after PVL. Atrophy in ligated lobes was a result of early necrosis followed by later apoptosis. Tumor growth was signifi cantly accelerated in ligated compared to non-ligated lobes.CONCLUSION: Tumor growth was accelerated in ligated liver lobes and appeared to be a result of increased growth factor expression.
