Proliferative Brenner Tumor with Borderline Mucinous Cystadenocarcinoma of the Ovary in a 75-Year-Old Woman

We report here an extremely rare case of proliferative Brenner tumor with borderline mucinous cystadenocarcinoma of the ovary, metastasizing to the pleura in a 75-year-old woman. Massive pleural effusion were present due to metastasis and pseudo-Meigs’ syndrome. The cytological findings from pleural effusion, ascites and tumor imprint were similar, suggesting mucus-producing malignant tumors. After receiving a thoracic drainage for pleural effusion, the patient underwent an exploratory laparotomy to remove the left-ovarian tumor. Histopathological examination revealed proliferative Brenner ovarian tumor with borderline mucinous cystadenocarcinoma, and immunohistochemical examinations for the tumors, even mucus-rich tumor cells were positive for CK 7 and negative for CK 20, suggesting the tumor arising from transitional cells, not from mucus-producing ovarian surface epithelial tumor cells. She underwent four courses of chemotherapy with paclitaxel and carboplatin. The patient showed no signs of recurrence 16 months after the surgery.

We report a recently observed case of primary umbilical endometriosis without previous pelvic surgery. A 41-year-old Japanese woman complained of umbilical nodular tumor. Histopathology revealed endometriosis of an approximate 10 mm resected mass. The stromal cells in endometriosis were immunohistochemically positive for CD10. Two months later the first umbilical surgery, she underwent a left salpingo-oophorectomy and release of adhesion around the left adnexa under a laparoscope. She was treated with dienogest (2 mg/day) for six months after four injections of GnRH analogue for four months. After three years of the follow-up, there were no signs of local relapse and no clinical and ultrasonographic abnormalities due to endometriosis.

Both endoscopic ultrasonography(EUS)-guided choledochoduodenostomy( EUS- CDS) and EUS-guided hepaticogastrostomy(EUS-HGS) are relatively well established as alternatives to percutaneous transhepatic biliary drainage(PTBD). Both EUSCDS and EUS-HGS have high technical and clinical success rates(more than 90%) in high-volume centers. Complications for both procedures remain high at 10%-30%. Procedures performed by endoscopists who have done fewer than 20 cases sometimes result in severe or fatal complications. When learning EUSguided biliary drainage(EUS-BD), we recommend a mentor's supervision during at least the first 20 cases. For inoperable malignant lower biliary obstruction, a skillful endoscopist should perform EUS-BD before EUS-guided rendezvous technique(EUS-RV) and PTBD. We should be select EUS-BD for patients having altered anatomy from malignant tumors before balloon-enteroscope-assisted endoscopic retrograde cholangiopancreatography, EUS-RV, and PTBD. If both EUS-CDS and EUS-HGS are available, we should select EUS-CDS, according to published data. EUSBD will potentially become a first-line biliary drainage procedure in the near future.
