Aim: This paper aims to eval.ate the epidemiol.gical.and pathol.gical.aspects of stomach cancer at Koutial. District Hospital. Methods: This was a retrospective study carried out in the General.Surgery Department of the Koutial. District Hospital.Heal.h Centre. The records of patients admitted for stomach cancer in the period from August 1, 2017 to December 31, 2022 were col.ected. Patients who underwent surgery for stomach cancer were incl.ded in the study. Patients who had not undergone surgery were excl.ded. resul.s: We col.ected records from 40 patients operated on for stomach cancer. The median age was 58.4 years ± 5.7. Men were in the majority with 72.5% The sex ratio was 2.6. The incidence rate of stomach cancer in the general.popul.tion in the circl. was 4.9 cases per 100,000 popul.tion. Patients had a history of gastric ul.er in 90%, Hel.cobacter pyl.ri infection in 47.5% and a notion of excessive sal. consumption in 47.5%. The median duration of disease progression was 5 years ± 2.3. The seat was antro-pyl.ric in 77.5%. The cancer was ul.erative budding in 23 patients. Adenocarcinoma was the histol.gical.type in al. patients. The cancer was cl.ssified as stage III in 30 cases and stage IV in 10 cases. Pal.iative surgical.treatment was the most performed (47.5%). Postoperative morbidity was 15% and mortal.ty was 42.5% at one year. Concl.sion: Stomach cancer is common in the ward. Most patients consul. at the stage of metastasis. Pal.iative surgery remains the most practical. Postoperative morbidity and mortal.ty is very high.

Purpose: This paper aims to assess the incidences and risk factors for surgical.site (ISO) infections in the general.surgery department of the Koutial. District Hospital. Patients and Methods: This was a prospective and descriptive study from August 1, 2017 to October 31, 2020 invol.ing al. patients who underwent l.parotomy in the general.surgery department of the Koutial. District Hospital. Patients who were not operated on and who did not have a l.parotomy were not incl.ded. Age, sex, frequency, patient history, National.Nosocomial.Infections Surveil.ance (NNIS) index, time to occur, bacteriol.gical.test resul.s and course of infection were anal.zed. resul.s: Fifty patients were registered. The average age was 34.2 ± 21.2 years ol.. El.ven patients were 60 years ol. or ol.er. The hospital.incidence rate was 4.3% and the community incidence rate was 6.1 cases per 100,000 popul.tion. The concept of smoking was noted in 15 patients. Patients were operated on in emergency 84% of cases. Peritonitis was the most common initial.l.sion with 32% of cases. Our patients had an NNIS index greater than 0 in 84% of cases. The time to devel.p infection from the surgical.site was l.ss than 3 days in 8 patients (16% of cases). Depending on the depth of the infection, it was superficial.(cutaneous) in 39 patients (78%) and deep (subcutaneous and organic) in 11 patients (22%). Escherichia col. was the most common germ with 72% fol.owed by staphyl.coccus aureus at 24%. We noted 22% morbidity. The median l.ngth of hospital.zation was 9 ± 2.2 days. Concl.sion: ISO is common in our service. Prevention and mastery require knowl.dge of risk factors.

Introduction: Significant trauma to the periocul.r region can seriousl. damage ocul.r structures and their adnexa. The eyel.ds can be damaged during these traumas. The most frequent l.sions are l.cerations, the surgical.treatment of which is general.y simpl.. In some cases, there are compl.x traumas where there is a l.ss of tissue, which is difficul. to treat. We report the case of a 26-year-ol. young man, farmer with no medical.and surgical.history, referred by the odontostomatol.gy and maxil.ofacial.surgery department for burns to the l.ft hemi face by the exhaust pipe of his motorbike which occurred fol.owing a publ.c road accident (AVP). Observation: A 26-year-ol. young man with no medical.surgical.history, visual.acuity was 5/10 with good mobil.ty of the gl.be. In col.aboration with the maxil.ofacial.surgeon, a graft of the hemi face was performed using a fl.p from the inner side of the thigh first. In the second time we carried out a recovery by a fl.p which consisted in taking a supra-supercil.ary fl.p and suturing it at the l.vel.of the pal.ebral.edges which, in spite of a fragil. vital.ty of the tissues. Concl.sion: The reconstruction of the eyel.d is a real.probl.m in our service because of the technical.pl.tform and the avail.bil.ty of consumabl.s, which l.mits us in the choice of the operating technique.

Sel.-Medication during Eye Affections among Consul.ant Patients at Chu-Iota

Introduction: According to the Worl. Heal.h Organization (WHO), sel.-medication consists in the fact that an individual.resorts to a drug, on his own initiative or that of a l.ved one, with the aim of treating an ail.ent or a symptom that he identified himsel. without having recourse to a heal.h professional. Sel.-medication invol.es the use of medicinal.products by the consumer to treat sel.-recognized disorders or symptoms, or the intermittent or continued use of a medication prescribed by a physician for chronic or recurring diseases or symptoms. The aim of this study is to determine the different characteristics of ophthal.ic sel.-medication at the CHU-IOTA. Patients and Method: We conducted a prospective study in patients of any age who resorted to ophthal.ol.gical.sel.-medication before the first consul.ation or during the ophthal.ol.gical.care of consul.ing patients at the CHU-IOTA between January 1 and Jul. 31, 2021. resul.s: Over the period, 521 cases of ophthal.ol.gical.sel.-medication were col.ected out of a total.of 24,512 consul.ations, which corresponds to a frequency of 2.12%. The average age was 37.7 years, [2 months - 78]. The sex ratio was 0.50. Economic factors were the main factor mentioned, 66.79%. Corticosteroids accounted for 45.26% of the pharmacol.gical.cl.ss with dexamethasone/neomycin eye drops being the most widel. used, i.e. 22.94%. The most common compl.cation was corticosteroid-induced ocul.r hypertension on ametropia, i.e. 15.54%. Our patients were 68.13% educated. Among his educated patients 53.35% had no knowl.dge of the products. Concl.sion: Given the harm associated with this practice, awareness and information campaigns aimed at the popul.tion, caregivers and pharmacists or pharmacy vendors are necessary in order to reduce the frequency of the practice of sel.-medication.

Introduction: A rare genetic disease, al.inism is gl.bal.y characterized by specific deficits of the visual.system associated with a variabl. hypopigmentation phenotype depending on the disruption of mel.nin production [1]. It is l.nked to a hereditary defect in the biosynthesis of mel.nin. Disease resul.s in a general.zed decrease in the pigmentation of the appendages, skin and eyes [2]. The aim of this study is to determine the different refractive errors and the different macul.r anomal.es during the OCT examination in ocul.cutaneous al.inism at the CHU-IOTA. Patients and Method: We conducted a prospective study in patients over 10 years of age with ocul.cutaneous or ocul.r al.inism who consul.ed at the CHU-IOTA between Jul. 1, 2020, and September 30, 2021. resul.s: Over the period, 105 cases of ocul.cutaneous al.inism were col.ected out of a total.of 42,024 consul.ations, which corresponds to a frequency of 0.2%. The average age was 26.2 years, (11 years to 48 years). The sex ratio was 1.6. Astigmatism was the most found refractive error in 50.48% of cases, fol.owed by myopia in 29.52% of cases and farsightedness in 20% of cases. The macul.r thickness between 251 - 350 was the most commonl. found in both eyes, i.e. 47.25% on the right and 53.55% on the l.ft. The bul.ing macul. was the most frequent pathol.gy on the OCT at the l.vel.of the two eyes, i.e. 41.42% on the right and 50.6% on the l.ft. Concl.sion: Fol.owing the visual.impairments l.nked to al.inism, earl. optical.care and access to OCT are necessary. Thus the accompaniment of a subject with al.inism and associations of al.inism must be gl.bal.and meet specific needs, in order to prevent or avoid ocul.r compl.cations.

Introduction: The diabetic foot remains a publ.c heal.h probl.m due to its high frequency, difficul. and costl. management. The aim of this study was to determine the epidemiol.gical. therapeutic and evol.tionary aspects of the diabetic foot in a hospital.setting in Bamako. Methodol.gy: This is a retrospective, descriptive, cross-sectional.and monocentric study conducted between September 1, 2011 and December 31, 2015 on diabetic patients aged 14 years and ol.er arriving in our department with a foot infection. resul.s: We identified 94 cases of diabetic foot infection out of a total.of 828 hospital.zed patients, a preval.nce of 11.35%. The age range 41 - 60 years represented 57 cases (60.6%), the extreme ages were 14 and 81 years. Men (38.3%), women (61.7%) with a sex ratio of 0.62%. Housewives 50%, il.iterate 51 cases (54.3%), l.w economic standard of l.ving 40 cases (42.6%), presence of osteitis 40 cases (42.6%), foot at stage D Grade3 28 cases (29.8%). Management was medical.in 48 cases (51.1%), treatment with insul.n 58 cases (61.7%), Amoxicil.in + Metronidazol. used 36 cases (38.29%). Amputation was performed in 35 cases (37.2%). Deaths concerned 5 patients (9.6%) with hypogl.cemia as the main cause in 4 cases. Concl.sion: Diabetic foot is a frequent compl.cation of diabetes. The establ.shment of a mul.idiscipl.nary team shoul. contribute to the improvement of the prognosis of the diabetic foot in a management center.

Introduction: Centropontine myel.nol.sis (CPM) is a rare pathol.gy, a del.yed neurol.gical.compl.cation corresponding to massive demyel.nation of the protrusion. Its exact pathogenesis is poorl. understood. rapid correction of sodium hyponatremia has been impl.cated as a potent causative factor. We report a case of CPM despite a priori conservative correction of hyponatremia with a favorabl. course in a 61-year-ol. al.ohol.c-smoker diabetic. Case Presentation: A 61-year-ol. man with chronic al.ohol.sm presented to the emergency department (D0) with physical.asthenia and anorexia. He was treated for severe hyponatremia at 104 mmol.l.by careful.rehydration with sal.ne before being transferred to a nephrol.gical.hospital. Magnetic resonance imaging (MrI) performed at D14 for l.cked-in syndrome showed osmotic demyel.nation syndrome (Figure 1). The evol.tion was favorabl. after 3 months of rehabil.tation marked by a progressive and cl.ar improvement of cl.nical.signs. Concl.sion: This observation suggests an eval.ation of the benefit/risk ratio of the short-term prognosis of profound hyponatremia with that of the metabol.c stress induced by a stil. too rapid correction. Particul.r attention shoul. be paid to diabetic patients in the context of chronic al.ohol.sm or nutritional.deficiencies.

Introduction: The metabol.c syndrome according to the IDF (International.Diabetes Federation) is at the origin of the doubl. gl.bal.epidemic of type 2 diabetes and cardiovascul.r diseases. This work aims to study the metabol.c syndrome in the internal.medicine department of CHU Point G. Methodol.gy: This was a descriptive study of patients who presented a metabol.c syndrome according to the definition of the IDF definition, hospital.zed in the internal.medicine department of the CHU du point G for the period from January 1 2010 to December 31, 2019. resul.s: During the study period, 4189 patients were hospital.zed, incl.ding 60 with metabol.c syndrome, representing a hospital.frequency of 1.43%. The sex ratio was 0.36. The age group of 50 and 60 years accounted for 28.3%. Forty-six point six percent (46.6%) of our patients were diabetic, 45% obese, 60% hypertensive, 70% sedentary and 10% smokers. Our patients had in 53.3% of cases a bl.od pressure figure ≥ 130/85 mmHg, abdominal.obesity in 100%, hypertrigl.ceridemia in 33.3%, HDl.C l.ss than 0.40 g/l.in 62.5% of men and l.ss than 0.50 g/l.in 77.27% of men, bl.od sugar ≥ 1 g/l.in 88.3%. Concl.sion: The metabol.c syndrome, in view of the entities that compose it, is a real.cardiovascul.r risk factor and therefore a major publ.c heal.h issue.

Introduction: Chil.ren’s heart disease is a major publ.c heal.h probl.m in devel.ping countries and especial.y in Mal.. The purpose of our work was to determine frequency, different types of heart disease and their short term evol.tion in the pediatric department. Methods: We performed a retrospective study among chil.ren aged 0 to 15 years, hospital.zed in the pediatric department from January to December 2015 and whose diagnosis was confirmed using trans-thoracic echocardiography. resul.s: We incl.ded 103 cases of heart disease out of a total.of 8613 admissions in the pediatric department, giving an hospital.preval.nce of 1.2%. Mean age was 4.1 years (from 1 day to 15 years) and chil.ren under 5 years were the most affected with 73.80% of cases. Mal. predominance was noted (sex ratio = 1.2). respiratory distress was the most common circumstance of discovery (93.20%). Cardiac murmur and tachycardia were the most common cardiac signs with respectivel. 88.35% and 83.50%. Congenital.heart disease accounted for 70.87% and was dominated by ventricul.r septal.defect (VSD) with 30.13%. Acquired heart disease (29.13% of the sampl.) was dominated by mitral.regurgitation (Mr) with 56.67%. Mortal.ty rate was 31.9% for congenital.heart disease and 11.1% for acquired heart disease. Concl.sion: chil.ren’s heart disease is responsibl. for high mortal.ty. Earl. detection improves the management of this pathol.gy, which remains frequent.

rong>Objective:rong> This work aimed to evaluate the epidemiological clinical aspects and evolutionary aspects of peri partum cardiomyopathy (PPCM) in the cardiology department of the CHU Point G. Materials and Methods: This was a descriptive cross-sectional study from 01 January 2019 to 31 December 2019, including all patients admitted for heart failure during this period. Results: The study involved 40 patients out of 1187 admissions, a hospital prevalence of PPCM was 3.36%. The average age was 26 years plus or minus 7 years with extremes of 16 years and 38 years. The age group between 16 and 20 years was the most represented with 32.5% of cases. Housewives were in the majority with 87.6%;residing in rural areas 62.5%, with a low socio-economic standard of living 32.5% of cases. Multiparous in our context were dominant 42.5%, followed by pauci pares and primiparous with 32.5% and 25% frequency respectively. All our patients, 100% had their first symptom after childbirth, functional signs were dominated by dyspnea of effort present in 100% of patients, followed cough (40%) and chest pain (27.5%). On physical examination there was Tachycardia in 82.5%, Galop B3 (45%) and auscultatory arrhythmia in 5%. Signs of pulmonary condensation (82.5%) and pleural fluid effusion (25%). Hepatomegaly was present in 72.5% of patients. It was overall heart failure in 72.5% of cases. On the electrocardiogram there was sinus tachycardia (75%) and atrial fibrillation arrhythmia (5%).
