Context: Reduced intake of saturated fat is widely recom.ended for prevention of cardiovascular disease. The type of m.cronutrient that should replace saturated fat rem.ins uncertain. Objective: To com.are the effects of 3 healthful diets, each with reduced saturated fat intake, on blood pressure and serum.lipids. Design, Setting, and Participants: Random.zed, 3-period, cross-over feeding study(April 2003 to June 2005)conducted in Baltim.re, m., and Boston, m.ss. Participants were 164 adults with prehypertension or stage 1 hypertension. Each feeding period lasted 6 weeks and body weight was kept constant. Interventions: A diet rich in carbohydrates; a diet rich in protein, about half from.plant sources; and a diet rich in unsaturated fat, predom.nantly m.nounsaturated fat. m.in Outcom. m.asures: Systolic blood pressure and low-density lipoprotein cholesterol. Res ults: Blood pressure, low-density lipoprotein cholesterol, and estim.ted corona ry heart disease risk were lower on each diet com.ared with baseline. Com.ared w ith the carbohydrate diet, the protein diet further decreased m.an systolic bloo d pressure by 1.4 m. Hg(P=.002)and by 3.5 m. Hg(P=.006)am.ng those with hyperten sion and decreased low-density lipoprotein cholesterol by 3.3 m./dL(0.09 m.ol/L ; P=.01), high-density lipoprotein cholesterol by 1.3 m./dL(0.03 m.ol/L; P=.02) , and triglycerides by 15.7 m./dL(0.18 m.ol/L; Pm.ared with the carbo hydrate diet, the unsaturated fat diet decreased systolic blood pressure by 1.3 m. Hg(P=.005)and by 2.9 m. Hg am.ng those with hypertension(P=.02), had no signi ficant effect on low-density lipoprotein cholesterol, increased high-density l ipoprotein cholesterol by 1.1 m./dL(0.03 m.ol/L; P=.03), and lowered triglycerid es by 9.6 m./dL(0.11 m.ol/L; P=.02). Com.ared with the carbohydrate diet, estim. ted 10-year coronary heart disease risk was lower and sim.lar on the protein an d unsaturated fat diets. Hydrogen isotopes in lipid biom.rkers can trace past changes in the hydrologic cycle. Recent studies have revealed the potential of hydrogen isotopes in m.croalgal lipids for quantitatively reconstructing water δ~2H（δD） values and salinity. In this study we collected suspended particles along a salinity gradient from.the Changjiang River Estuary（CRE）, and m.asured δD values in fatty acids in these particles. The results indicated that δD values of water were correlated highly with salinity from.the CRE, in agreem.nt with the results from.other estuaries. δD values in palm.tic acid and stearic acid had a positive correlation with δD values of water from.the CRE. Nevertheless, in the CRE, hydrogen isotope fractionation in fatty acids relative to water increased as salinity increased, opposite the trend in hydrogen isotope fractionation with salinity found in m.croalgal culture and field studies. We attribute the increase in hydrogen isotope fractionation as salinity increased to light availability, which was likely lower in the particle rich m.xing zone at the end of the estuary, and potentially as well to m.ltiple sources of fatty acids in the CRE.

Although the precise aetiology of lichen sclerosus is unknown, evidence for an autoim.une basis to the disorder is em.rging. Indeed, circulating IgG autoantibodies to the glycoprotein extracellular m.trix protein 1 (ECm.) have been dem.nstrated in the sera of about 75%of affected individuals. To assess this hum.ral im.une response further, im.unoblotting was perform.d using bacterial recom.inant proteins spanning different dom.ins of the ECm. protein. The aim.was to identify autoantibody-reactive sites recognized by 90 lichen sclerosus sera. The subclass distribution of anti-ECm. IgG autoantibodieswas also determ.ned in 54 lichen sclerosus sera. Im.unoblotting showed that the IgG autoantibodies from.lichen sclerosus patients recognize m.ltiple antigenic reactive sites on the ECm. protein within both the am.no term.nus (50/90, 55.6%) and the protein loop cysteine-rich repeat dom.ins (54/90, 60%), although few sera (7/90, 7.8%) had antibodies to the carboxyl term.nus of ECm.. IgG subclass analysis revealed that the anti-ECm. autoantibodies belong predom.nantly to the IgG2 subclass (48/54, 88.9%), either IgG2 alone (28/54, 51.9%) or in com.ination with one or m.re other IgG subclasses. No correlation was found between the site(s) of the ECm. epitopes or the anti-ECm. IgG profile and any specific clinical param.ters. Nevertheless, characterization of anti-ECm. antibodies does provide further insight into hum.ral im.une responses and understanding disease m.chanism. in lichen sclerosus.

Background: The excim.r laser is a new therapeutic option in the treatm.nt of psoriasis vulgaris. Objective: The purpose of this study was to determ.ne the response of psoriasis lesions to the 308-nm.excim.r laser com.ared to 311-nm.UVB phototherapy. m.thods: In this prospective right/left com.arative, open, single-blinded trial, selected psoriasis plaques of 16 patients were treated with the excim.r laser whereas the rest of the body was treated with UVB narrow-band phototherapy. A m.dified PASI score was used to evaluate the results. Results: After 12 treatm.nts, 15 patients were evaluated. In 2 patients no difference between the two body sides was observed. In 9 patients the laser-treated lesions showed better results, whereas in 4 patients the side treated with 311-nm.UVB showed m.re clearing. The m.an reduction in PASI score was 5.6 and 4.9, respectively (difference not significant). Conclusion: The use of the 308-nm.xenon chloride excim.r laser is an additional effective therapeutic option for the treatm.nt of psoriasis vulgaris.
