Human cD4 ＋ effector T lymphocytes generated upon TcR engagement with self-peptides respond defectively to IL-7 in their transition to memory cells

The peripheral repertoire of cD4＋ T lymphocytes contains autoreactive cells that remain tolerant through several mechanisms. However, nonspecific cD4＋ T cells can be activated in physiological conditions as in the course of an ongoing immune response, and their outcome is not yet fully understood. Here, we investigate the fate of human naive cD4＋ lymphocytes activated by dendritic cells （Dcs） presenting endogenous self-peptides in comparison with lymphocytes involved in alloresponses. We generated memory cells （Tmem） from primary effectors activated with mature autologous Dcs plus interleukin （IL）-2 （Tmauto）, simulating the circumstances of an active immune response, or allogeneic Dcs （Tmallo）. Tmem were generated from effector cells that were rested in the absence of antigenic stimuli, with or without IL-7. Tmem were less activated than effectors （demonstrated by cD25 downregulation） particularly with IL-7, suggesting that this cytokine may favour the transition to quiescence. Tmauto and TmaHo showed an effector memory phenotype, and responded similarly to polyclonal and antigen-specific stimuli. Biochemically, IL-7-treated Tma^o were closely related to conventional memory lymphocytes based on Erk-l/2 activation, whereas Tmauto were more similar to effectors. Autologous effectors exhibited lower responses to IL-7 than allogeneic cells, which were reflected in their reduced proliferation and higher cell death. This was not related to IL-7 receptor expression but rather to signalling deficiencies, according to STAT5 activation These results suggest that ineffective responses to IL-7 could impair the transition to memory cells of naive cD4＋ T lymphocytes recognizing self-peptides in the setting of strong costimulation.

Human thrombin for the treatment of gastric and ectopic varices

AIM:To evaluate the efficacy of human thrombin in the treatment of bleeding gastric and ectopic ***:Retrospective observational study in a Tertiary Referral *** January 1999-October 2005,we identified 37 patients who were endoscopically treated with human thrombin injection therapy for bleeding gastric and ectopic *** details including age,gender and aetiology of liver disease/segmental portal hypertension were *** thrombin was obtained from the Scottish National Blood Transfusion Service and prepared to give a solution of 250 IU/mL which was injected via a standard injection *** patient case notes were reviewed and the total dose of thrombin given along with the number of endoscopy sessions was *** haemostasis rates,rebleeding rates and mortality were catalogued along with the incidence of any immediate complications which could be attributable to the thrombin *** duration of follow up was also *** study was conducted according to the United Kingdom research ethics ***:Thirty-seven patients were included.33 patients(89%) had thrombin(250 U/mL) for gastric varices,2(5.4%) for duodenal varices,1 for rectal varices and 1 for gastric and rectal varices.(1) Gastric varices,an average of 15.2 mL of thrombin was used per ***-bleeding occurred in 4 patients(10.8%),managed in 2 by a transjugular intrahepatic portosystemic shunt(TIPSS)(one unsuccessfully who died) and in other 2 by a distal splenorenal shunt;(2) Duodenal varices(or type 2 isolated gastric varices),an average of 12.5 mL was used per patient over 2-3 endoscopy ***-bleeding occurred in one patient,which was treated by TIPSS;and(3) Rectal varices,an average of 18.3 mL was used per patient over 3 endoscopy *** re-bleeding occurred in this ***:Human thrombin is a safe,easy to use and effective therapeutic option to control haemorrhage from gastric and ectopic varices.

Using lattice configurations for quantum chromodynamics(QcD)generated with three domain-wall fermions at a physical pion mass,we obtain a parameter-free prediction of QcD’s renormalisation-group-invariant process-independent effective charge,α^(k2).Owing to the dynamical breaking of scale invariance,evident in the emergence of a gluon mass-scale,m0=0.43(1)GeV,this coupling saturates at infrared momenta:α^(0)/π=0.97(4).Amongst other things:α^(k2)is almost identical to the process-dependent(PD)effective charge defined via the Bjorken sum rule;and also that PD charge which,employed in the one-loop evolution equations,delivers agreement between pion parton distribution functions computed at the hadronic scale and *** diversity of unifying roles played byα^(k^2)suggests that it is a strong candidate for that object which represents the interaction strength in QcD at any given momentum scale;and its properties support a conclusion that QcD is a mathematically well-defined quantum field theory in four dimensions.

Atlantoaxial rotatory displacement in children

AIM To correlate the Pang and Lee class with the clinical course in a consecutive series of patients presenting with painful *** Forty-seven dynamic rotational computed tomography(cT) scans in 35 patients were classified into one of the five types defined by Pang and Li, including types Ⅰ(atlantoaxial rotatopry fixation), Ⅱ("pathologic stickiness" without crossover of c1 on c2), Ⅲ("pathologic stickiness" with crossover of c1 on c2), Ⅳ(normal or muscular torticollis), and Ⅴ(diagnostic grey zone). The Pang and Li class was then compared with the radiologist's report, which was graded abnormal, diagnosis of rotatory subluxation or fixation, or non-diagnostic. Medical records were reviewed and the clinical course was compared among the five *** We reviewed 47 cT scans in 35 patients, and the majority were performed without sedation. The average age was 7.7 years(4-14 years old) and associated conditions included minor trauma(20%), surgical procedures around the head and neck(29%), and Grisels syndrome(20%). Twenty-six percent of our studies fell within the pathologic spectrum(5% type 1 or rotatory fixation, 21% types 2 and 3 or rotatory subluxation), while 45% were classified as muscular torticollis(45%) and 28% fell within the diagnostic grey zone. Seven radiologists interpreted these studies, and their interpretation was discordant in 45% of cases. clinical resolution occurred in 27 of 29 cases for which follow-up was available. One of two patients with fixed rotatory subluxation required a c1-c2 *** The Pang and Li classification characterizes a spectrum of abnormalities in rotation to facilitate communication, although the indications for dynamic cT scan should be further defined.
