The Pattern of Contrast Dye through the Thoracic Paravertebral Catheter Does Not Predict Analgesic Efficacy after Breast Cancer Surgery: A Prospective Cohort Study

Purpose: We examined whether or not the spreading pattern of contrast dye through the catheter into the thoracic paravertebral space (TPVS) was associated with acute post-operative pain relief after breast cancer surgery. Methods: Sixty patients were recruited. After surgery, patients received thoracic paravertebral block (TPVB). The catheter was inserted within the TPVS, and radiographic contrast dye was injected through the catheter. Based on the main spreading patterns of the dye, the patients were divided into a cloud-like (spreading to the posterior compartment of the TPVS) or a longitudinal spread group (spreading to the anterior compartment of the TPVS). The primary endpoint of the study was pain intensity by numeric rating scale (NRS) and additional analgesic requirements within 12 h after surgery. Results: In 22 patients, the spread of contrast dye was not noted in the PVS. Of the 38 eligible patients, 9 patients were assigned to the cloud-like spread group, 28 patients to the longitudinal spread group and 1 patient was excluded. There were no significant differences in the NRS between the two groups both at rest and on moving. No significant differences were found in the frequency of requiring additional analgesics. Conclusions: There is no marked difference in the analgesic efficacy after breast cancer surgery based on the spread of the contrast dye to the posterior or anterior compartment of the TPVS. We should not insert the needle deeply within the TPVS, as the risk of the migration of catheters into the pleural space can be quite high.

The pectoralis major myocutaneous pedicle flap (PMMF) is still being used by many surgeons and plays an important role in head and neck reconstruction. The purpose of this series was to review our 10 years’ experience with the PMMF in head and neck reconstruction. One hundred and two patients who underwent the PMMF technique were reviewed on the clinical records. Postoperative complications were classified into flap loss, hemorrhage, infection, fistula formation, wound dehiscence and donor site complication. Eighty two patients (80.4%) demonstrated no complication. Six patients among 102 patients (5.9%) demonstrated total or partial skin necroses. Three female patients were completely dissatisfied with the cosmetic appearance after the PMMF. One of them required a reconstructive surgery with the latissimusdorsi flap. Without surgical expertise in plastic surgical field, an ear-nose-throat or an oral surgeon can performed the PMMF technique provided the operator is well aware of serious and frequent complications of this “workhorse” procedure.

Four Patients Undergoing Labioplasty and Lip Reconstruction for Lip Deformity

We managed four patients who had neoplastic, posttraumatic, atrophic and postoperative lip deformity, respectively. Case 1: The patient had a hemangioma (17 × 14 mm) confined to the vermilion part of the lower lip. We performed the wedge excision of the tumor and labioplasty. The postoperative appearance and function of the lip were satisfactory. Case 2: The patient had posttraumatic deformity of the upper lip. We performed correction by Cronin’s triangular flap method. The postoperative appearance of the lip was satisfactory. Case 3: The patient had severe atrophy of the upper lip and difficulty closing mouth due to progressive hemifacial atrophy. We performed reconstruction of the upper lip on the atrophic side with a cross-lip vermilion flap from the non-atrophic lower lip. The postoperative appearance and function of the lip were satisfactory. Case 4: The patient had postoperative lower lip deformity and difficulty closing mouth after tumor resection and reconstruction with a myocutaneous pectoralis major flap. We performed debulking of the bulky flap and covered the raw surface with a local rotation flap. The postoperative lip closure was satisfactory. In patients with lip deformity, it is important to perform appropriate correction or reconstruction according to each patient’s clinical features in order to achieve satisfactory cosmetic and functional outcomes.

Foreset profiles of deltaic topography change corresponding to hydraulic conditions,and are influenced by water discharge and change rates of water *** investigate the correlation between the transition of deposition rates on the foreset and the change rate of water discharge of temporary waxing and subsequent waning flows,we perform experiments on silty deltas that developed under several different hydrographic *** findings are as follows:(1) Under flows with constant discharge,the deposition rate on the foreset was low at high water discharge and high at low water discharge because the separated flow became vigorous at high water discharge and prevented sediment supply onto the *** the water discharge is not constant,the deposition rate shifted concurrently with water discharge only at extremely low change rate.(2i) In a waxing flow at low(but not extremely low) increase rate,the deposition rate gradually decreased with some time lag.(2ii) In the case of high increase discharge,intense erosion on the topset caused by rapid waxing made a large amount of silt move onto the foreset and the deposition rate temporary increased.(3) With ample time after waning stage,the deposition rate recovered to that at constant low water discharge in all ***,the transitional processes depended on not only waning rate but also waxing rate at before waning stage.(3i) In the case of high decrease rate of water discharge,the deposition rate abruptly decreased regardless of increase rate.(3ii) Even in the case of low decrease rate of water discharge,if the increase rate before waning stage was high,the deposition rate abruptly decreased.(3iii) Only in the case where both the increase rate and the decrease rate were low,deposition rate gradually increased.

Strain-relaxed SiGe is an attractive material for use as a substrate of strained Si, in which carrier mobility is higher than that of bulk Si. The concept of this study is the use of porous Si as a sponge like substrate so that a SiGe lattice can relax without introducing dislocations. We produced porous Si specimens by electrochemical anodization and annealed them under a H2 atmosphere. Then, SiGe thin films were grown by gas-source molecular beam epitaxy. We observed the microstructure of the specimens using transmission electron microscopy. The result showed that we succeeded in producing a single-crys- tal continuous Si0.73Ge0.27 film with a 10% relaxation ratio and a low dislocation density on porous Si.

A novel method of thin film formation of organic materials with nano-structure has been successfully developed by using vacuum technique is *** diarylethene(Ci8Hi8N2S2) was selected as a model compound for the evaluation of this method. Polymer film of PMMA, PBzMA, PHPM and PC were prepared for present experiment. By comparing these kinds of polymer, we found that the tendency of dye dispersion into the polymer is as follows:PC>PBzMA>PMMA>PHPMA, where no dispersion is observed for PHPMA under the condition of 115℃for 24 hours,Thin film of polymer alloy composed of PMMA and polystylene(PS)was loaded into a glass ample with diarylethene,and treated for three days at 100℃.Dispersed state of the dye was evaluated by transmission electron microscope, and concluded that the dye is distributed only in PS domains selectively. Photochromic properties of the PS domain will be evaluated by using a scanning nearfield optical microscope.

Physiological changes in the photosynthesis, respiration and cell division of Chlamydomonas reinhardtii, a freshwater green alga, in response to adenine nucleotides were investigated. In advance of this investigation, two adenine nucleotides, di(2'-O-methyl)-cyclic bis(3'-5')diadenylic acid (1) and its N-benzoyl-protected analog 2 were synthesized from the commercially available adenosine phosphoramidite. The respective analogs significantly promoted the cell division (cell number) of C. reinhardtii strains 137c mt+ and BR mt+. Moreover, they significantly enhanced the O2 evolution (photosynthesis) and O2 uptake (respiration) of both strains. c-di-AMP analogs seem to play an effective role as a physiological activator in planta.

Swallowing Function before and after Subtotal Glossectomy and Reconstruction with a Rectus Abdominis Musculocutaneous Flap: A Case Report

Purpose: Swallowing function was in a patient with tongue cancer that was evaluated with video fluorography before and after subtotal glossectomy and reconstruction with a rectus abdominis muscuocutaneous flap. Materials and Methods: A 41-year-old man underwent subtotal glossectomy and retained the unilateral posterior mylohyoid and unilateral stylohyoid muscles. The structure of the flap was evaluated postoperatively. To assess swallowing function, video fluorography was performed before surgery, 1 month after surgery and 1 year after surgery. Testing involved 1) ability to hold 10 mL of liquid in the oral cavity, 2) epiglottis turnover, 3) the presence of aspiration, 4) hyoid bone movement, and 5) maximum width of the esophageal entrance. Results: The flap was protuberant postoperatively. The patient was able to hold the test diet in the oral cavity before and after surgery. Epiglottis turnover was good before surgery but insufficient after surgery. Aspiration during swallowing was not observed before or after surgery. At rest, the hyoid bone sagged postoperatively, in comparison with preoperatively. Hyoid bone movement and width of the esophageal entrance decreased after surgery;however, they demonstrated gradual recovery. Conclusions: For good postoperative swallowing function after subtotal glossectomy, it is necessary to perform reconstruction with protuberant flap and to retain the suprahyoid muscles as much as possible.
