The aim of this study was to investigate the link between ambulance transports due to heat stroke and air temperature by using daily data of ambulance transports in Okayama prefecture, Japan. Daily observations for ambulance transports due to heat stroke from July to September in 2010 in Okayama prefecture, Japan were obtained from Fire and Disaster Management Agency in Japan. Data of meteorological parameters in Okayama prefecture, Japan were also obtained from Japan Meteorological Agency. Effect of meteorological parameters on ambulance transports due to heat stroke was analyzed. A total of 1133 ambulance transports due to heat stroke were observed in from July to September of 2010 in Okayama prefecture, Japan. Ambulance transports due to heat stroke was significantly correlated with air temperature. In addition, number of subjects with ambulance transports due to heat stroke over 34°C in the highest air temperature was 21.2 ± 9.8 per day. Higher air temperature was closely associated with higher ambulance transports due to heat stroke by using daily data in Okayama, prefecture, Japan.

Objective：To summarize and critically assess the efficacy of Eastern and Western manipulative therapies for the treatment of neck pain in adults. Methods：A search of Pub Med/MEDLINE, the Cochrane Central Register of Controlled Trials, Clinical ***, EMBASE, etc. from their inception date to January 2014 with Chinese, Japanese, and Korean databases. Two reviewers independently selected randomized controlled trials（RCTs） with negative control or blank control, extracted data and assessed methodological quality. Meta-analysis and levels of evidence were performed by Revman5.1 and Grades of Recommendations Assessment, Development and Evaluation（GRADE） approach. Results：Nineteen clinical trials with adequate randomization were included in this review, 11 of them had a low risk of bias. The primary outcome for shortterm pain had no significant differences, however, the secondary outcome, only the Numerical Pain Rating Scale（NPRS） score of intermediate-term [n=916, pooled mean differences（MD） =–0.29, P=0.02], the Neck Disability Index（NDI） score of short-term（n=1,145, pooled MD=–2.10, P〈0.01）, and intermediate-term（n=987, pooled MD=–1.45, P=0.01） were significantly reduced with moderate quality evidence. However, it supported the minimally clinically important difference（MCID） of the Visual Analogue Scale and NPRS pain score to be 13 mm, while NDI was 3.5 points. The meta-analysis only suggested a trend in favor of manipulative therapy rather than clinical significance. Conclusions：The results do not support the existing evidences for the clinical value of Eastern or Western manipulative therapy for neck pain of short-term follow-up according to MCIDs. The limitations of our review related to blinding, allocation concealment and small sample size.

目的分析口腔和胃幽门螺杆菌（Hp）感染的检测结果，探讨口腔Hp感染与胃Hp感染的相关性，及口腔Hp感染对Hp根除治疗的影响。方法采用唾液测定螺旋杆菌抗原技术（HPS）和^13C／^14C尿素呼气试验（UBT）同步检测的方法，对114例有上消化道症状的初诊患者（第1组），129例确诊为胃Hp感染经根除治疗后4周复查的患者（第2组）和33例无消化道症状的健康志愿者（第3组），进行口腔和胃Hp检测。结果第1组、第2组和第3组HPS阳性检出率分别为77．19％、75．97％和81．82％，3组比较差异无统计学意义（x^2=0．47，P值均〉0．05）；UBT阳性检出率第1组（52．63％）比第2组（34．11％）和第3组（21．21％）高，第1组与第2组和第3组比较，差异有统计学意义（x^2=8．48和10．19，P均〈0．05），第2组与第3组之间差异无统计学意义（x^2=2．03，P〉0．05）；在UBT阳性者中，HPS阳性检出率差异无统计学意义（3组分别为81．67％、88．64％和100％，x^2=2．25，P值均〉O．05）。结论唾液中存在高Hp抗原检出现象，口腔可能是Hp在胃以外的“第二定居地”。口服药物治疗对口腔Hp感染几乎无效，口腔Hp的存在可能是胃病发病和复发的一个重要和直接的原因。

Thirty-six daily duplicate diet samples were collected from 12 healthy female Japanese vegans and sodium, potassium, calcium, magnesium, phosphorus, iron, zinc, copper, manganese, iodine, selenium, chromium and molybdenum in the diets were measured to estimate mineral and trace element intake by Japanese vegans. Significantly higher intake of potassium, magnesium, phosphorus, iron, copper, manganese and molybdenum was observed in vegans than in general Japanese women, but no difference was observed in sodium, iodine, selenium and chromium intake. Vegan calcium intake tended to be low compared to that of general women but the difference was not significant. Since high potassium, magnesium and iron intakes cannot be achieved by general Japanese diets and high intake of potassium and magnesium may prevent hyperextension and cardiovascular disease in vegans, there are few problems with Japanese vegan diets regarding mineral and trace element intake, except for calcium intake, which is low as it is in the general Japanese people.

Ardisia mamillata Hance (Myrsinaceae) is a widely occurring shrub in southern China. Its roots have been traditionally used to treat respiratory tract infections and menstrual disorders. Other plants of this genus have also been used for this purpose, and are well documented in traditional medicine in Southeast Asia. Recently, we have isolated six new triterpenoid saponins, ardisimamillosides A-F from A. mamillata. In this paper, we report the isolation and structural elucidation of two additional new triterpenoid saponins (1 and 2) from this species.

A Study on Ecotourism Considered from Viewpoint of Landscape

Changes in Climate Parameters at Wakkanai Area, Which is Located at Most Northern Part in Japan

＜正＞Recently, ecotourism has been acknowledged as one form of sightseeing. Ecotourism is an idea of sightseeing that aims at uniting the protection of regional resources such as nature, the history, and the culture, the sightseeing industry, and revitalization of local economy. Landscape architects have been playing a constant role to the sightseeing development and the regional plan in the area of conventional sightseeing. Then, what sort of relationship are ecotourism and ’Landscape’ building now? What does ecotourism have to do with ’Landscape’? Do actual ecotours as marketed tour packages make a point of ’Landscape’ ? In this study, we consider ecotourism from the aspect of ’Landscape’. Specifically, we examine the definition of ecotourism, the history of ecotourism and sightseeing, and actual ecotours. Firstly, definitions of ecotourism were examined by searching for terms related to ’Landscape’, and were examined how the terms were described. Secondly, the chronology was made in connection with ecotourism and ’Landscape’, and was analyzad. Thirdly, terms related ’Landscape’ were searched in the description of actual ecotours registered in ECOTOUR SOURAN, and the results were compared with that of the general sightseeing tours. As a result, we could not find remarkable sign that ’Landscape’ has special recognition in the definition, the history, and ecotour programs of ecotourism. In the definitions of ecotourism, only NACS-J use the term of ’nature （keikan）’ The landscape may be indirectly expressed as in ’nature’, history’, ’culture’, and an inclusive word ’environment’. But, we may say that the concept of ’Landscape’ is not clearly considered in ecotourism. In the history of ecotourism, landscape seems to have stayed as one of sightseeing objects while both objects and forms of the sightseeing are diversified. In the tours registered as ecotours, the number of tours for which the term related to the landscape was used limited to 22 out of 757. The occurrences

We evaluated the climate change in Wakkanai area, which is located at most northern part and typically rural area in Japan. Climate parameters i.e. atmospheric pressure, air temperature, humidity, velocity, daylight hours and snow fall were obtained from Japan Meteorological Agency. Positive changes in parameters in air temperature, especially mean of the lowest air temperature (r = 0.508, p < 0.0001) were noted. In addition, parameters of air temperature in January were significantly and positively correlated with years. However, atmospheric pressure (land) and mean velocity were negatively correlated with years. Global warming was also proved at Wakkanai area, especially in winter, in Japan.

To investigate the link between higher air temperature and ambulance transports due to heat stroke, especially in elderly subjects, using daily data in Sai-tama prefecture in August, 2010, Japan. We obtained daily data of air temperature in Saitama prefecture in August, 2010, Japan from Japan Meteorological Agency. Number of ambulance transports due to heat stroke was also used by Saitama prefecture official website. The effect of high air temperature on am-bulance transports due to heat stroke was evaluated. A total of 1688 ambulance transports due to heat stroke was observed in Saitama prefecture in August, 2010, Japan. Seven hundred and twenty one subjects (42.7%) were over the age of 65. Mean air temperature, the highest air temperature and day light hours were significantly associated with ambulance transports due to heat stroke in total subjects (mean air temperature: r = 0.749, p < 0.0001) and the over the age of 7. In addition, by separate analysis as classified by age groups, the correlation coefficient between ambulance transports due to heat stroke and mean air temperature was the highest in subjects with over 65. However, the lowest air temperature was not cor-related with ambulance transports due to heat stroke. Higher air temperature was closely associated with higher ambulance transports due to heat stroke using daily data in Saitama prefecture in August, 2010, Ja-pan, especially in elderly subjects.

Backgroud:Blood flow re striction(BFR) with low-intensity re sistance training has been shown to result in hypertrophy of skeletal *** this study,we tested the hypothesis that BFR during the rest periods between acute,high-intensity resistance exercise sessions(70% of 1 repetition maximum,7 sets with 10 repetitions) enhances the effects of the resistance ***:A total of 7 healthy young men performed squats,and between sets BFR was carried out on one leg while the other leg served as a *** BFR was applied during rest periods,even severe occlusion pressure(approximately 230 mmHg),which almost completely blocked blood flow,was well-tolerated by the *** muscle-specific microRNAs were measured from the biopsy samples,which were taken 2 h after the acute ***:Doppler data showed that the pattern of blood flow recovery changed significantly between the first and last ***-206 levels significantly decreased in the BFR leg compared to the *** mRNA levels of RAC-β serine/threonine-protein kinase v22,nuclear re spiratory factor 1,vascular endothelial growth factor,lupus Ku autoantigen protein p70 genes(p <0.05),and paired box 7(p <0.01) increased in the BFR *** protein levels of paired box 7,nuclear respiratory factor 1,and peroxisome proliferator-activated receptor y coactivator 1α did not differ between the BFR leg and the control ***:BFR,during the rest periods of high-load resistance training,could lead to mRNA elevation of those proteins that regulate angiogenesis,mitochondrial biogenesis,and muscle hypertrophy and ***,BFR also can cause DNA damage,judging from the increase in mRNA levels of lupus Ku autoantigen protein p70.

Developing a Scale for Measuring Uncertainty in Patients with Unruptured Intracranial Aneurysms Undergoing Endovascular Coiling

A measure called the Uncertainty in Unruptured Intracranial Aneurysm Patients Undergoing Endovascular Coiling Scale (UUIACS) was developed and its validity and reliability were examined. The 49 questions that comprised the original draft of the UUIACS were created based on interview data. Based on data from 172 participants, exploratory and confirmatory factor analyses were conducted. As a result of exploratory factor analysis, the UUIACS e retained 17 items and extracted four factors (“Lack of decision-making cues”, “Lack of information and complexity of information interpretation”, “The ambiguous nature of the disease”, and “The unpredictable living with UIA”). All of the UUIACS items showed adequate internal consistency. Between the UUIAC scale and the Universal Uncertainty in Illness Scale (UUIS), the Health Locus of Control (HLC) scale, and the SF-36v2® (Japanese version), positive correlations were found between the UUIACS and UUIS, and the HLC scale at a 1% significance level indicating concurrent validity. According to confirmatory factor analysis, the UUIACS had an acceptable goodness of fit. Given these findings, the UUIACS was judged to have satisfied the criteria for use in a clinical setting, although further investigation was required.
