Interaction between Topographic Conditions and Entrainment Rate in Numerical Simulations of Debris Flow

Debris flow simulations are useful for predicting the sediment supplied to watersheds from upstream areas. However, the topographic conditions upstream are more complicated than those downstream and the relationship between the topographic conditions and debris flow initiation is not well understood. This study compared the use of several entrainment rate equations in numerical simulations of debris flows to examine the effect of topographic conditions on the flow. One-dimensional numerical simulations were performed based on the shallow water equations and three entrainment rate equations were tested. These entrainment rate equations were based on the same idea that erosion and the deposition of debris flows occur via the difference between the equilibrium and current conditions of debris flows, while they differed in the expression of the concentration, channel angle, and sediment amount. The comparison was performed using a straight channel with various channel angles and a channel with a periodically undulating surface. The three entrainment rate equations gave different amounts of channel bed degradation and hydrographs for a straight channel with a channel angle greater than 21° when water was supplied from upstream at a steady rate. The difference was caused by the expression of the entrainment rate equations. For channels with little undulation, the numerical simulations gave results almost identical to those for straight channels with the same channel angle. However, for channels with large undulations, the hydrographs differed from those for straight channels with the same channel angle when the channel angle was less than 21°. Rapid erosion occurred and the hydrograph showed a significant peak, especially in cases using the entrainment equation expressed by channel angle. Debris flows include a great diversity of grain sizes with inherent features such as inverse grading,particle size segregation,and liquefaction of fine *** liquefaction of fine sediment affects the fluidity of debris flows,although the behavior and influence of fine sediment in debris flows have not been examined *** study used flume tests to detect the effect of fine sediment on the fluidity of laboratory debris flows consisting of particles with various *** the experiments,the greatest sediment concentration and flow depth were observed in the debris flows mixed with fine sediment indicating increased flow *** experimental friction coefficient was then compared with the theoretical friction coefficient derived by substituting the experimental values into the constitutive equations for debris *** theoretical friction coefficient was obtained from two models with different fine-sediment treatments:assuming that all of the fine sediments were solid particles or that the particles consisted of a fluid phase involving pore water *** the comparison of the friction coefficients,a fully liquefaction state was detected for the fine particle *** the mixing ratio and particle size of the fine sediment were different,some other cases were considered to be in a partially liquefied transition *** results imply that the liquefaction of fine sediment in debris flows was induced not only by the geometric conditions such as particle sizes,but also by the flow conditions.

目的经颈静脉或经股动脉途径被用作当前实践中肝静脉压力梯度(HVPG)测量的常用方法。该研究旨在证实经前臂静脉途径测量HVPG的安全性和有效性。方法针对2020年9月至2020年12月前瞻性地从中国和日本的6所医院招募了经前臂静脉进行HVPG检测肝硬化患者,并收集患者的临床基线资料以及HVPG检测数据。入组患者均选择经右侧肘正中静脉或贵要静脉入路,采用HVPG标准化流程进行测压。研究数据采用SPSS 22.0统计学软件进行分析。定量资料采用中位数(四分位数间距)表示,定性资料采用频数和率表示。两组数据之间的相关性分析采用Pearson相关性分析。结果研究共入组43例患者,其中41例(95.3%)患者成功接受了经前臂静脉途径HVPG检测。无患者出现任何严重并发症。经前臂静脉途径HVPG检测中位操作时间为18.0min(12.3~38.8min)。研究证实HVPG与Child-Pugh评分(r=0.47,P=0.002)、白蛋白-胆红素评分(r=0.37,P=0.001)、Lok指数(r=0.36,P=0.02)、肝脏硬度(r=0.58,P=0.01)、脾硬度(r=0.77,P=0.01)呈正相关,且与白蛋白呈负相关(r=-0.42,P=0.006)。结论多中心回顾性研究结果提示经前臂静脉途径HVPG测量是安全可行的。

Global standardization of laparoscopic liver resection and challenges for the future

Hepatobiliary Surgery and Nutrition recently published a review by X Cai that describes the current status and future of laparoscopic liver resection (LLR) in China. The advent of LLR has led to significant changes in the field of liver surgery, with numerous reports outlining how this procedure offers a better, less invasive approach for the treatment of liver disease. Two international consensus conferences regarding LLR—held in 2008 (Louisville, USA) and 2014 (Morioka, Japan)—highlighted the efficacy and safety of LLR, and discussed the surgical indications, technique, standardizations, and precautions that should be considered when performing LLR (1,2). Delegates deliberated whether open liver resection (OLR) should remain the current standard procedure for liver disease, and the importance of LLR within this arena. There was agreement that minor LLR offers sufficient safety and benefit to be considered standard practice, but major resection requires further investigation. The most recent guidelines from the European meeting on LLR, held in 2017 in Southampton, UK (3), integrate the available evidence and expert knowledge on LLR, taking into consideration relevant stakeholders' opinions and complying with international methodology standards across five domains (indications, patients and complex diseases, procedures, techniques, and implementation). The guidelines emphasize the importance of a team-oriented approach and the need to recruit experts in OLR and laparoscopic surgery within a specialist center.

Hepatitis B virus infection and alcohol consumption

Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer, and the second most common cause of cancer deaths worldwide. The top three causes of HCC are hepatitis B virus(HBV),hepatitis C virus(HCV), and alcoholic liver disease. Owing to recent advances in direct-acting antiviral agents, HCV can now be eradicated in almost all patients. HBV infection and alcoholic liver disease are expected, therefore, to become the leading causes of HCC in the future. However, the association between alcohol consumption and chronic hepatitis B in the progression of liver disease is less well understood than with chronic hepatitis C. The mechanisms underlying the complex interaction between HBV and alcohol are not fully understood, and enhanced viral replication, increased oxidative stress and a weakened immune response could each play an important role in the development of HCC. It remains controversial whether HBV and alcohol synergistically increase the incidence of HCC. Herein, we review the currently available literature regarding the interaction of HBV infection and alcohol consumption on disease progression.

Novel Biological Based Method for Robot Navigation and Localization

The capability and reliability are crucial characteristics of mobile robots while navigating in complex environments. These robots are expected to perform many useful tasks which can improve the quality of life greatly. Robot localization and decisionmaking are the most important cognitive processes during navigation. However, most of these algorithms are not efficient and are challenging tasks while robots navigate through complex environments. In this paper,we propose a biologically inspired method for robot decision-making, based on rat’s brain signals. Rodents accurately and rapidly navigate in complex spaces by localizing themselves in reference to the surrounding environmental landmarks. Firstly, we analyzed the rats’ strategies while navigating in the complex Y-maze, and recorded local field potentials(LFPs), *** recorded LFPs were processed and different features were extracted which were used as the input in the artificial neural network(ANN) to predict the rat’s decision-making in each junction. The ANN performance was tested in a real robot and good performance is achieved. The implementation of our method on a real robot, demonstrates its abilities to imitate the rat’s decision-making and integrate the internal states with external sensors, in order to perform reliable navigation in complex maze.

BACKGROUND： Injection of glutamate （Glu） in normal animals can cause neuronal c-Fos expression; however, whether Glu can induce spinal neuronal c-Fos expression in pain models is unclear. OBJECTIVE： To examine the effects of intraplantar and intrathecal injection of Glu on c-Fos expression in the L5 spinal cord dorsal horn Ⅰ/Ⅱ and Ⅲ/Ⅳ layers after spinal nerve ligation, and to study the effects of the N-methyI-D-aspartic acid （NMDA） receptor antagonist, D-2-amino-5-phosphonopentanoate （D-AP5）, and a selective group I mGluR antagonist, 7-hydroyiminocyclo propan[a]chromen-lacarboxylic acid ethyl ester （cpccoEt）. DESIGN, TIME AND SETTING： A randomized, controlled animal study was performed at the Department of Pharmacology, Oral Anatomy, and Neurobiology, Osaka University Graduate School of Dentistry, from December 2005 to December 2006. MATERIALS： Glu （5 μmol）, D-AP5 （50 nmot） and cpccoEt （250 nmol） were provided by Wako Pure Chemical Industries, Osaka, Japan, and diluted in saline （50 μL）. The pH of all solutions was adjusted to 7.4. METHODS： Twelve rats were randomly divided into sham operation （n = 6） and spinal nerve ligation （SNL; n = 6） groups for behavioral assessments of neuropathic pain after ligation surgery of the left L5-6 nerve segment. Another 60 rats were randomly divided into sham operation, SNL, saline-intraplantar, saline-intrathecal, Glu-intraplantar, Glu-intrathecal, D-AP5-intrathecal, Glu-D-AP5-intrathecal, cpccoEt-intrathecal, and Glu-cpccoEt-intrathecal groups, with 6 rats in each group. All groups except sham operation group received a similar SNL. On day 14, rats received a 50-μL injection of saline, Glu, D-AP5, and/or cpccoEt into the left intraplantar or intrathecal L5-4 segments. Prognosis of hepatocellular carcinoma accompanied by microscopic portal vein invasion

AIM:To investigate the prognostic factors in patients with hepatocellular carcinoma(HCC) accompanied by microscopic portal vein invasion(PVI).METHODS:Of the 267 patients with HCC undergoing hepatic resection at Aso Iizuka Hospital,71 had *** excluding 16 patients with HCC that invaded the main trunk and the first and second branches of the portal vein,55 patients with microscopic PVI were ***:The patients with HCC accompanied by microscopic invasion were divided into two groups:solitary PVI(PVI-S:n = 44),and multiple PVIs(PVI-M:n = 11).The number of portal vein branches invaded by tumor thrombi was 5.4 ± 3.8(2-16) in patients with *** cumulative survival,PVI-M was found to be a significantly poor prognostic factor(P = 0.0019);while PVI-M and non-anatomical resection were significantly poor prognostic factors in disease-free survival(P = 0.0213,and 0.0115,respectively).In patients with PVI-M,multiple intrahepatic recurrence was more common than in the patients with PVI-S(P = 0.0049).In patients with PVI-S,non-anatomical resection was a significantly poor prognostic factor in disease-free survival(P = 0.0370).Operative procedure was not a significant prognostic factor in patients with ***:The presence of PVI-M was a poor prognostic factor in patients with HCC,accompanied by microscopic *** resection is recommended in these patients with *** with HCC and PVI-M may also be good candidates for adjuvant chemotherapy.

Prospective randomized controlled trial investigating the type of sutures used during hepatectomy

AIM: To determine whether absorbable sutures or non-absorbable sutures are better in preventing surgical site infection (SSI), in this paper we discuss the results of a randomized clinical trial which examined the type of sutures used during hepatectomy. METHODS: All hepatic resections performed from January 2007 to November 2008 at the Department of Surgery at Iizuka Hospital in Japan were included in this study. There were 125 patients randomly assigned to an absorbable sutures (Vicryl) group or non-absorbable sutures (Silk) group. RESULTS: SSI was observed in 13.6% (17/125) patients participating in this study, 11.3% in the Vicryl group and 15.8% in the Silk group. Incisional SSI including superficial and deep SSI, was observed in 8% of the Vicryl group and 9.5% of the Silk group. Organ/ space SSI was observed in 3.2% of the Vicryl group and 6.0% of the Silk group. There were no significant differences, but among the patients with SSI, the period for recovery was significantly shorter for the Vicryl group compared to the Silk ***: The incidence of SSI in patients receiving absorbable sutures and silk sutures is not significantly different in this randomized controlled study; however, the period for recovery in patients with SSI was significantly shorter for absorbable sutures.
