AIM:To characterise the cag pathogenicity island in Helicobacter pylori(H.pylori) isolates by analysing the strains' vacA alleles and metronidazole susceptibilities in light of patient ethnicity and clinical outcome.METHODS:Ninety-five H.pylori clinical isolates obtained from patients with dyspepsia living in Malaysia were analysed in this study.Six genes in the cagPAI region(cagE,cagM,cagT,cag13,cag10 and cag67) andvacA alleles of theH.pylori isolates were identified by polymerase chain reaction.The isolates' metronidazole susceptibility was also determined using the E-test method,and the resistant gene was characterised by sequencing.RESULTS:More than 90% of the tested isolates had at least one gene in the cagPAI region,and cag67 was predominantly detected in the strains isolated from the Chinese patients,compared with the Malay and Indian patients(P < 0.0001).The majority of the isolates(88%) exhibited partial deletion(rearrangement) in the cagPAI region,with nineteen different patterns observed.Strains with intact or deleted cagPAI regions were detected in 3.2% and 8.4% of isolates,respectively.The prevalence of vacA s1m1 was significantly higher in the Malay and Indian isolates,whereas the isolates from the Chinese patients were predominantly genotyped as vacA s1m2(P = 0.018).Additionally,the isolates from the Chinese patients were more sensitive to metronidazole than the isolates from the Malay and Indian patients(P = 0.047).Although we attempted to relate the cagPAI genotypes,vacA alleles and metronidazole susceptibilities to disease outcome,no association was observed.The vacA alleles were distributed evenly among the strains with intact,partially deleted or deleted cagPAI regions.Interestingly,the strains exhibiting an intact cagPAI region were sensitive to metronidazole,whereas the strains with a deleted cagPAI were more resistant.CONCLUSION:Successful colonisation by different H.pylori genotypes is dependent on the host's genetic makeup and may play an important role in the clinical outcome.

Carboxymethyl cellulose(CMC)has emerged in oil and gas industries as a superior substitution to the conventional HPAM and xanthan gum(XG)for high viscosity polymer flooding application.In this study,the combined effect of conventional surfactant,sodium dodecylbenzenesulfonate(SDBS)and CMC for potential surfactant-polymer(SP)flooding in enhanced oil recovery(EOR)has been investigated.Thereafter,SDBS–CMC interaction and the functional groups present in CMC were appropriately identified.The presence of various C–O bonds signifies the existence of carboxymethyl group which greatly influence the rheological properties of CMC solution.The behaviour of SDBS–CMC was characterized by their viscosity,shear rate,solubilization,wettability,and surface tension.Tertiary flooding utilizing SDBS-CMC was performed and compared to commercial SDBS-XG SP flooding.The results indicate several SDBS-CMC combinations are favourable for EOR application.Solution viscosity shows direct relationship with CMC concentrations.Consequently,at any given SDBS concentrations,significant increment was observed at 0.3 wt%and above.However,the trend displayed inconclusive relation to SDBS fractions.Majority of the SDBS-CMC combinations generate Winsor III emulsions particularly at CMC of 0.2 and 0.3 wt%,while Type II were observed in few combinations.Increasing CMC concentrations increased the contact angle,while gradual reductions were observed with SDBS concentrations.The gradual reduction in surface tension was highly influenced by the addition of CMC rather than SDBS.A novel combination of 0.3 wt%SDBS and 0.4 wt%CMC possessed an encouraging criterion in term of viscosity,solubilization,and surface tension reduction for EOR application.Flooding experiment from several SDBS-CMC combinations proved to recover additional oil ranging 16.4–20.2%of oil initially in place(OIIP).The trend in incremental oil recovery is similar to that of when utilising SDBS-XG.

Evolution of Erythromycin Resistance among Streptococcus pneumoniae Isolates in Malaysia from 2005 and 2010

Objectives: This study focuses on the antibiotic susceptibility pattern and distribution of the ermB and mefA virulence genes among the Streptococcus pneumoniae due to an increase in erythromycin resistance in S. pneumoniae worldwide. Methodology: We investigated 255 clinical isolates collected from 2005-2010 to determine the serotype distribution and resistance to erythromycin in comparison to penicillin, clindamycin, clarithromycin, azithromycin, and trimethoprim sulfamethoxazole. Multiplex PCR was carried out to detect erythromycin resistance genes (ermB and mefA). Results: There were 146 (57.3%) isolates resistant to erythromycin. MIC90 for erythromycin is at >256 mg/L and MIC50 is at 16 mg/L. The ermB gene was detected in 25.3% of the erythromycin-resistant isolates and mefA gene was detected in 50.7% of the isolates. The four most common serotypes encountered are 19F, 19A, 23F and 14. The serotype distribution among the erythromycin resistant isolates was 19F (42.0%) followed by serotype 19A (11.3%), serotype 23F (9.2%) and serogroup 14 (7.0%). Conclusion: In conclusion, there is a significant rise in erythromycin resistance among the Malaysian pneumococcal isolates. The emergence of serotype 19A together with increasing prevalence of resistance to macrolide warrants for a more extensive surveillance study.

目的:研究在马来西亚地区幽门螺杆菌菌株对克拉霉素耐药情况与23SrRNA基因突变的关系。方法:通过E test方法测定幽门螺杆菌菌株对克拉霉素的敏感性,通过对克拉霉素耐药菌株和克拉霉素敏感菌株的23S rRNA基因V区的点突变进行分析聚合酶链反应产物的序列分析扩增。应用限制性内切酶(Mbo II,Bsa I)酶切位点检测相对应的克拉霉素耐药菌株的突变已达到对限制性片段长度多态性分析。结果:120例患者的187个菌株中,有4株对克拉霉素耐药,183株对克拉霉素敏感。该耐药菌株的MIC(最低抑菌浓度)范围从1.5到24ug/ml.两个菌株中一个是A2142G突变,另两个是A2143G突变。4个克拉霉素耐药菌株中2个被检测到T2182C突变,14个克拉霉素敏感菌株中13个被检测到T2182C突变。限制性内切酶(Bsa I、MboII)可检测到突变。结论:在马来西亚地区幽门螺杆菌菌株对克拉霉素耐药的发生比较罕见,A2142G和A2143G突变存在低水平耐药。
