Evaluation of Different Ultrasonographic modalities in the Diagnosis of morbidly Adherent Placenta: A Cross-Sectional Study

Objective: To compare the accuracy of different ultrasonographic modalities;two-dimensional ultrasound (2D-US), color Doppler and three-dimensional power Doppler (3D-PD) in the antenatal diagnosis of the morbidly adherent placenta. Setting: Obstetrics and Gynecology Department, Faculty of medicine, Assiut University, Assiut, Egypt. Study Design: A cross-sectional study. methods: All patients fulfill the inclusions criteria: gestational age > 28 weeks, previous one or more cesarean delivery, previous uterine surgery, placenta previa, vitally stable patient and women accepted to participate in the study were included. All patients were evaluated using 2D-US, color Doppler and 3D-PD before delivery. The final diagnosis was established by laparotomy and by histopathology of hysterectomy sample if hysterectomy would be done. Results: One-hundred fifty patients were enrolled in the study. 2D-US has higher sensitivity (86.96%) than 2D color Doppler (84.06%) and 3D-PD (79.71%) in the diagnosis of placenta accreta. On the other hand, 3D-PD has slightly higher specificity (83.95%) than color Doppler (82.72%) and 2D-US (77.78%) in the diagnosis of placenta accreta. The most sensitive parameter in 2D-Us was the loss of retroplacental sonolucent zone (86.96%). As regards color Doppler, the most sensitive parameter was the hypervascularity of the uterine-bladder interface (84.06%). Tortuous vascularity with chaotic branching was the most sensitive parameter in 3D-PD with a sensitivity of 82.61%. Conclusions: The use of 3D power Doppler with both 2D-US and color Doppler as complementary techniques could improve the antenatal diagnosis or exclusion of morbidly adherent placenta.

management of Post-Circumcision Trapped Penis with Glanular Amputation

A phenomenon known as trapped penis has numerous origins, including injudicious circumcision. materials and methods: Surgery for trapped penis with glanular amputation was performed for 5 children. The surgical policy in these cases included complete penile degloving, excising the scarring due to circumcission, fixing the penile skin at the penopubic and penoscrotal angles and reconstructing the penile skin with full thickness skin graft. Results: Improvement was noted in all cases. Conclusion: Surgery for hidden penis using full thickness skin graft achieves very satisfactory aesthetic and functional improvement.

(1) To determine the serum levels of vascular endothelial growth factor (VEGF) and insulin- like growth factor- 1 (IGF1) in women with polycystic ovary syndrome (PCOS). (2) To study Doppler blood flow changes within the ovarian stroma of women with PCOS. (3) To evaluate the relationship between VEGF and IGF- 1 and Doppler indices as well as hormonal profile. Department of Obstetrics and Gynecology, and Department of Biochemistry, Faculty of medicine, Assiut University, Egypt. Cross- sectional study. Fifty infertile women with PCOS diagnosed by ultrasound examination and a history of oligomenorrhea, hirsutism and obesity were studied. Serum levels of vascular endothelial growth factor (VEGF), insulin- like growth factor- 1 (IGF- 1) and hormonal profile were measured. Doppler blood flow velocity waveforms analysis in both right and left intraovarian arteries was done. Twenty healthy and fertile women with regular menstrual cycles served as a comparison group were similarly studied at the third day of the cycle. The serum levels of VEGF,IGF- 1 (4.79 ± 0.91, 253.15 ± 70.07 versus 2.39 ± 0.42, 186.65 ± 42.7) were significantly elevated (P ely) in women with PCOS compared with control. Doppler indices, PI (2.01 ± 0.77, 2.66 ± 1.00 versus 2.98 ± 0.77, 3.75 ± 0.98) and RI (0.77 ± 0.12, 0.82 ± 0.09 versus 0.87 ± 0.09, 0.89 ± 0.09) in both right and left intraovarian vessels were significantly lower in the patients than controls. The VEGF and IGF- 1 levels were negatively correlated with RI and PI in the uterine and intraovarian arteries. VEGF level was positively correlated with IGF- 1 (r = 0.41, P men with PCOS. Higher serum levels of VEGF and IGF- 1 in PCOS women may be related to the increased vascularity that underlies the increased blood flow demonstrated by Doppler blood flow measurements in these women.

Preoperative Conformal Radiotherapy Concurrently with Paclitaxel/Carboplatin in Gastric Cancer

Background and objectives: Surgery is the primary therapy for localized gastric cancer, but even with the best results only 40% 5-year survival can be achieved with the use of postoperative adjuvant chemoradiotherapy. Preoperative therapy might help increase the R0 resection rate, which is an independent predictor of 5-year OS. Our study hypothesized that the concurrent combination of carboplatin-paclitaxel with radiation therapy would result in a pathological CR rate, which will be in turn associated with OAS and DFS benefits. Patients and methods: prospective phase II study included 32 patients with locally advanced gastric adenocarcinoma including gastroesophageal junction who received a combination of neoadjuvant conformal radiotherapy concurrently with carboplatin-paclitaxel followed by surgery. Results: Pathological CR and R0 resection rates were 18.8% and 75% respectively. With a median follow up of 24 months, 2 years disease-free survival was 28.1% and overall survival was 51.3%. The regimen was tolerated with neither grade 4 toxicities nor deaths. Conclusion: Neoadjuvant radiotherapy concomitant with carboplatin-paclitaxel chemotherapy is a well-tolerated approach for patients with locally advanced gastric adenocarcinoma resulting in significant pathological CR and R0 resection margins as reflected by the good DFS and OS.

Preliminary Results of Solar Flare Induced D-Region Perturbations over UKm Using Stanford AWESOmE Receiver

We present the preliminary results of VLF signal perturbations produced due to solar flare. The data were recorded by the Stanford VLF AWESOmE receiver located at National University of ma-laysia, Selangor. Two new long distance (>1000 km) VLF paths, JJI-UKm (2700 km) and NWC-UKm (3300 km) were analyzed simultaneously. Data from the GOES satellite were used to determine the onset time and type of each of these flares. Results indicated that all five solar flare events with an X-ray peak flux above 10-5 W/m2 (m-class) were recorded, 37.5% for X-ray flux greater than 10-6 W/m2 (C-class), while the weakest X-ray flux recorded was 2.6 × 10-7 W/m2 (B-class) with 0.24% probing potentiality. We found a strong positive correlation (0.84) between solar flare radiation intensity and the values of amplitude and phase perturbations for both paths. The values of amplitude and phase perturbations time-correlated with solar flare, varied from 0.2 to 5 dB and 0.15 to 20 degree respectively. These findings are in complete agreement with previous works and demonstrate that the data obtained by the UKm AWESOmE observation station will provide addi-tional contribution to the study of elF/VLF waves phenomena in the ionosphere/magnetosphere, especially at low latitudes region.

Water management of Falaj Al Khatmain in Sultanate of Oman

Oman is an arid country with an average annual rainfall of 100 mm. Agricultural productions in Oman is almost fully dependent on irrigation. more than one third of irrigation water is supplied by aflaj （singular： falaj）, which provide 680× 10^6 m^3 of water per year and irrigate some 26,500 ha. Aflaj are trenches and tunnels, which are dug in the ground to convey water from one place to another. This study analyzed the existing management of Falaj Al Khatmain and proposed improvements to the system regarding water utilization, enriching community living, distribution of water among the shareholders and the water circulation during day and night. Furthermore, the study estimated the surplus water during the wet period and determined the recharge area of groundwater using the rainfall, falaj flow, wadi flow, water table and lithology of the wells located in or around the study area. The study concluded that under the climatic and hydro-geologic conditions in the study area, the best arrangement for recharging the groundwater is an infiltration pond to store surplus water from thefalaj in the subsurface pond or reservoir without dam construction.

Oral Vinorelbine as Switch maintenance Therapy versus Best Supportive Care in Patients with Advanced Adenocarcinoma Non-Small Cell Lung Cancer EGFR Wild Type

Background: This study was performed to evaluate the efficacy and safety of switch maintenance therapy with oral vinorelbine in advanced non-small cell lung cancer (NSCLC) with adenocarcinoma limited to epidermal growth factor receptor (EGFR) wild type. materials and methods: In this single randomized trial, patients with advanced stage (IIIB and IV) NSCLC with adenocarcinoma EGFR wild-type status, treated with 6 cycles of platinum based chemotherapy. Patients did not show progression after first-line chemotherapy were randomly assigned to receive switch maintenance with vinorelbine (80 mg/m2, day 1, 8) (group I) or the best supportive care until disease progression (group II). Results: The median progression free survival (PFS) was 9.7 months for group I versus 5.7 months for group II with statistically significant difference between both groups [HR = 1.15;95% CI 1.19 to 1.49;P value = 0.002], while the median overall survival (OS) was 13.2 months for group I versus 11.9 months for group II with no statistically significant differences between both groups [HR = 1.24;95% CI 1.05 to 1.46;P value = 0. 3]. The patients who received oral vinorelbine had tolerable toxicity profile. Conclusion: Switch maintenance therapy with oral vinorelbine, though improve PFS, did not improve OS in patients with NSCLC with adenocarcinoma EGFR wild type.

Alkaloids Production from Callus of Hyoscyamus niger L, in Vitro

Callus cultures of Hyoscyamus niger L. were initiated from leaf segments cultured on murashige and Skoog （mS） medium supplemented with 0.5 mg/L Benzyl Adenine （BA） and 0, 1, 2 and 3 mg/L Naphthalene acetic acid （ NAA ）. Half of cultures were incubated under light of 16 hr/day, while the other half was incubated under complete darkness. The incubation temperature was 25 ± 1 ℃in both incubation conditions. The fresh and dry weight of the produced callus was obtained after six weeks of incubation. Callus produced were recultured on medium that gave the highest production of callus. Constant weight （300 mg） of callus was cultured in each of these medium supplemented with abiotic elicitor of 50 g/L sucrose, 200 mg/L NaCI, 50 or 100 mg/L proline and 2 mg/L BA each one added separately and incubated under complete darkness. The fresh and dry weights of callus were measured after six weeks. HPLC was used to determine the tropane alkaloids （Hyoscyamine and Scopolamine）. The results showed that the significant highest average of fresh and dry weight of callus （112 and 89.6 mg） achieved using the medium contained 0.5 mg/L BA and 2 mg/L NAA under dark condition. The amount of fresh and dry weight of callus produced under dark condition was significantly higher than that produced under light condition, with increase in percentage of 51.3 and 37.62% respectively. The addition of abiotic elicitors caused reduction in both fresh and dry weight of callus, therefore the highest fresh weight average was 1,727 mg using 100 mg/L proline. The results indicated that addition of 50 or 100 mg/L proline led to increase in Hyoscyamine concentration of 58.03 and 21.37% respectively compared with the control. While other abiotic elicitors were found to cause reduction in Hyoscyamine concentration. Percentage of Scopolamine concentration were increased to 129.03, 166.94, 205.51 and 149.20% after addition of sucrose （50 g/L）, NaC1 （200 mg/L） and proline （50 or 100 mg/L） respectively compared with

Effect of Initial Stress on Wave Frequencies of elastic Solid with Rotation

In this paper, the effect of initial stress on the radial vibrations of elastic hollow cylinder with rotation is discussed. The one-dimensional equation of elastodynamic is solved in terms of radial displacement. The frequency equation is obtained when the boundaries are free;fixed and mixed boundary condition is examined numerically. The determination is concerned with the eigenvalues of the natural frequency of the radial vibrations in the case of harmonic vibrations. The effect of rotation and initial stress on the natural frequencies was examined. It was shown that the dispersion curves of guided waves were significantly influenced by the rotation and initial stress of the elastic cylinder. Numerical results are given and illustrated graphically for each case considered. The results indicate that the effect of rotation and initial stress are very pronounced.

Design and Development of Innovative Highland Water Filtration System

The righteous book of Al-Quran mentions that the basic need of the living thing in this world is water. Resident of rural areas such as indigenous people will tap the highland aquatic resources as the main foundation for water. The reservoir is built on the top of hilly area to collect water before it discharges through a pipe system that uses the force of gravity to go down to the settlements. However, the filtration system frequently clogs and requires the occupants to climb up the reservoir hill for cleaning process. A lot of energy and time are required to manage the irrigation systems. Thus, an innovation system has been conducted as the filters will be cleaned automatically using solar energy supply. The designs of the filters are specially designed as the filter is equipped with electric motors, washer rods, power supply units, and automatic control box. The advantages of this project are the product will filtered the water as remain as natural highland watering resources while using the solar energy to accomplished green technology application.
