Evaluating the Impact of.the Universal Test and Treat Strategy on the Survival of.Patients in the Northwest Region of.Cameroon

Introduction: The main outcome of.ef.iciently implemented universal test and treat (UTT) program is improved survival. UTT implementation has been ongoing in Cameroon since 2016 but evaluation data are scarce. This study aims to assess the survival of.antiretroviral therapy (ART) patients initiated under UTT in Northwest region of.Cameroon. Methods: This retrospective cohort study included HIV-positive patients initiated in 2016 at 27 purposef.lly selected sites and f.llowed until 2021. Data was anonymously abstracted f.om ART registers and patients’ charts. Kaplan-Meier survival estimates and Cox model were used to compare the survival of.patients initiated under UTT with those initiated otherwise, using stata version 14.0. Results: In total, 2490 HIV-positive patients (median age 42.7 years, 94.7% adults, and 69.0% f.male) participated in the study. Of.1389 patients with viral load (VL) test results, 55% were initiated on ART late. The VL suppression rate of.patients initiated late and those initiated early were similar. During f.llow-up, 1020 (40.9%) participants censored. The survival curves of.patients initiated early on ART and those initiated late were similar during the f.rst 2.5 years of.f.llow-up but signif.cantly (p Conclusions: This study conf.rms the expected impact of.UTT. Programs only need to close existing implementation gaps along the critical pathways (diagnosis and treatment) of.UTT, f.cusing more on males.

Validating a tool to assess eye health knowledge, attitude and practice in Cambodia and Vietnam

AIM: To develop an eye health knowledge, attitude and practice(EH-KAP) f.eld-based assessment tool f.r use in implementing ef.ective eye health care ***: An instrument development and validation study. A Vietnam EH-KAP dataset were used to identif. and eliminate redundant questions to develop a standardized tool. f.ce validity was assessed by the KAP survey team. Internal validity(congruency/criterion) was assessed by comparing descriptive analysis of.two datasets(n=531;n=38) collected f.om the same sampling f.ame at dif.erent time points. Weighted scores were calculated f.r each construct. Kappa values f.r test-retest and inter-observer agreement were calculated to check the reliability of.responses. The modif.ed version was assessed by analysing the raw and ungrouped data. Responses were weighted and agreement was tested by comparing construct ***: Totally 38 respondents were included in this validation process(mean age 58.5 y). Mean scores f.r knowledge were 9.15(old questionnaire n=531) and 5.05(modif.ed version). f.r attitude, the scores were 2.23 and 2.42, and f.r practice the scores were 3.33 and 2.21. Testretest agreement was between 62% to 93%(Kappa 0.24 to 0.86) f.r the ungrouped raw data, and 55% to 72%(Kappa 0.42 to 0.65) f.r KAP domain. Inter-observer Kappa value f.r ungrouped data was 0.37 and 0.45 f.r the weighted ***: This standardized tool applied at critical time points can assess trends in KAP within the same population and f.r comparison across groups. If.used alongside a Rapid Assessment of.Avoidable Blindness(RAAB), this tool provides a comprehensive perspective on eye-health of.a population.

Dear Editor, During recent decades, a novel mechanism of.secretion has been identif.ed in a wide range of.mammalian cells. It involves the release of.bioactive membrane nanovesicles （30-100 nm）, termed exosomes, upon the f.sion of.multivesicular bodies with the plasma membrane （Thery et al., 2009）. Exosomes are implicated in diverse f.nctions, such as scavenging of.archaic proteins, intercellular messengers delivering cell-specif.c signals, and vehicles f.r transmissible pathogens. Exosomes have also been described in other organisms such as bacte- ria, Drosophila, and f.ngi.

目的：评价准分子激光治疗近视性散光有字全性和有效性，井与准分子激光治疗近视进行比较。设计：对用准分子激光治疗近视性散光或近视的放性的研究背景：患者从18个私人眼科诊所中收集。患者：54只眼接受散光治疗，66只眼接受近视治疗。1例患者失去随访，另1例遭遇无效切削。方法：使用一台VISX20／20准分子激光机做光学散光屈光性角膜切除术(PARK)及光学屈光性角膜切除术(PRK)。主要结果测量：术前术后均评价屈光度数、矫正视力及裸眼视力。结果：术后6月，接受光学散光屈光性角膜切除术的20例患者中17例(85％)的平面屈光力在1屈光度范围内，且20例中19例(95％)的裸眼视力达到6／12(20／40)或更好。对接受光学屈光性角膜切除术的患者，这两个数字分别是32例中的28例(88％)和32例中的28例(88％)。结论：准分子激光手术为治疗近视性散光提供了一项有效的选择。
