Dermatomyositis associated with hepatocellular carcinoma in an elderly female patient with hepatitis C virus-related liver cirrhosis

A 79-year-old female patient with hepatitis C virusrelated liver cirrhosis was diagnosed as having hepatocellular carcinoma （HCC） with a diameter of 2.0 cm. She refused therapy for HCC. Nine months after the diagnosis, she developed dermatomyositis when the HCC enlarged to a diameter of 6.0 cm. She underwent therapy for dermatomyositis, and then transcatheter arterial chemoembolization for HCC. Although the manifestations of dermatomyositis improved and entire tumor necrosis was achieved, she died of pneumonia 2 mo after the treatment of HCC. HCC and/or chronic hepatitis C virus infection might be involved in the pathogenesis of dermatomyositis.

Abstract Objectives To explore the potential use of ultrasound measurement (SoundScan 2000 Compact, Myriad Ultrasound Systems Ltd., Rehovot, Israel), and to assess both quantitative and qualitative properties of bone. Methods The speed of sound waves (SOS; m/s) propagating along the cortical bone was determined at the tibial shaft. The performance, reliability, validity, and clinical application of this system were evaluated in Japanese subjects. Results In phantom experiments, validity of this system was 0.04(%), coefficient variation (CV: %) in same-day tests was 0.12, in five-separate-day tests was 0.13, and in independent operators was 0.11, respectively. In experiments with Japanese subjects, CV in same-day tests was 0.38, in five-separate-day tests was 0.99, and in independent operators was 0.90, respectively. In addition, cortical bone status of 1176 Japanese subjects (372 males, 804 females) was clinically evaluated by SoundScan 2000 Compact. SOS was negatively correlated with body fat percentage (%), body fat (kg) and waist hip ratio in females and subjects with higher weight bearing index (leg strength/body weight; WBI) have higher SOS in females under the age of forty. Conclusions These findings indicate that the SoundScan 2000 Compact is a highly reliable and valid method for determining cortical bone status and body composition and WBI are closely related to cortical bone status in Japanese females.

day work and shift work) and cigarette smoking in Japanese workers. We used data of 3,238 men (39.3 ± 10.5 years) and 5,111 women (37.1 ± 10.9 years), aged 20 - 59 years, by cross-sectional clinical investigation study. Work style i.e. day work and shift work, cigarette smoking, status of stress and stress coping were obtained by questionnaires by well-trained medical staff. A total of 227 men (7.0%) and 339 women (6.6%) were shift workers, and 1346 men (41.6%) and 649 women (12.7%) were current smokers. Work style was significantly linked to cigarette smoking, stress and stress coping after adjusting for age in women. In addition, the level of stress coping in subjects with cigarette smoking was significantly lower than that in subjects without cigarette smoking even after adjusting for age in women. However, these associations were not noted in men. Work style was critically associated with cigarette smoking in Japanese female workers.

AIM： To examine the association of inducible nitric oxide synthase （iNOS） C150T polymorphism with gastric cancer, as well as with gastric atrophy and H pylori ***： A single nucleotide polymorphism of iNOS CtSOT was examined for 454 Japanese health checkup examinees （126 males and 328 females） aged 35 to 85 years without a history of cancer and 202 gastric cancer patients （134 males and 68 females） aged 33 to 94 years with pathologically confirmed diagnosis of gastric adenocarcinoma.RESULTS： The iNOS C150T polymorphism was not associated with gastric atrophy or with H pylori seropositivity. The odds ratio （OR） of the C/T ＋T/T for gastric cancer was increased without statistical significance （OR=1.19, 95% confidence interval （CI）： 0.68-2.08）. In the differentiated subgroup （n = 113）, however, the OR of the C/T genotvpe for gastric cancer was significant （OR = 2.02, 95% CI： 1.04-3.92） relative to the C/C genotype. In addition, considering the location of gastric cancer （n = 105）, there were significant differences between the controls and non-cardia group with the ORof 2.13 （95% CI： 1.08-4.18） for C/T and 1.94 （95% CI： 1.00-3.78） for C/T ＋ T/***： The iNOS C150T polymorphism is associated with the risk of H pylori-related gastric cancer in a Japanese population. This polymorphism may play an important role in increasing the risk of gastric cancer in Asian countires with the highest rates of gastric cancer.

Changes in Climate Parameters at Wakkanai Area, Which is Located at Most Northern Part in Japan

We evaluated the climate change in Wakkanai area, which is located at most northern part and typically rural area in Japan. Climate parameters i.e. atmospheric pressure, air temperature, humidity, velocity, daylight hours and snow fall were obtained from Japan Meteorological Agency. Positive changes in parameters in air temperature, especially mean of the lowest air temperature (r = 0.508, p < 0.0001) were noted. In addition, parameters of air temperature in January were significantly and positively correlated with years. However, atmospheric pressure (land) and mean velocity were negatively correlated with years. Global warming was also proved at Wakkanai area, especially in winter, in Japan.

To investigate the link between higher air temperature and ambulance transports due to heat stroke, especially in elderly subjects, using daily data in Sai-tama prefecture in August, 2010, Japan. We obtained daily data of air temperature in Saitama prefecture in August, 2010, Japan from Japan Meteorological Agency. Number of ambulance transports due to heat stroke was also used by Saitama prefecture official website. The effect of high air temperature on am-bulance transports due to heat stroke was evaluated. A total of 1688 ambulance transports due to heat stroke was observed in Saitama prefecture in August, 2010, Japan. Seven hundred and twenty one subjects (42.7%) were over the age of 65. Mean air temperature, the highest air temperature and day light hours were significantly associated with ambulance transports due to heat stroke in total subjects (mean air temperature: r = 0.749, p < 0.0001) and the over the age of 7. In addition, by separate analysis as classified by age groups, the correlation coefficient between ambulance transports due to heat stroke and mean air temperature was the highest in subjects with over 65. However, the lowest air temperature was not cor-related with ambulance transports due to heat stroke. Higher air temperature was closely associated with higher ambulance transports due to heat stroke using daily data in Saitama prefecture in August, 2010, Ja-pan, especially in elderly subjects.
