The authors consider the problem of estimating the ordered means of two normal distributions with unknown ordered variances. The authors discuss the estimation of two ordered means, individually, in terms of stochastic domination and MSE (mean squared error). The authors show that in estimating the mean with larger variance, the usual estimator under order restriction on means can be improved upon. However, in estimating the mean with smaller variance, the usual estimator can"t be improved upon even under MSE. The authors also discuss simultaneous estimation problem of two ordered means when unknown variances are ordered.

Background: The number of total knee arthroplasty (TKA) surgeries performed each year is increasing worldwide and mechanical alignment (MA) is currently seen as the gold standard procedure. However, taking neutral alignment as the universal goal may be mistaken. In our hospital, we currently conduct kinematically aligned TKA (KA-TKA). Three different types of implants are used: the cruciate-retaining (CR) type, cruciate-sacrificing (CS) type, or bi-cruciate-retained (BCR) type. We aimed to compare the coronal alignment observed following KA-TKA and MA-TKA and in normal knees, as well as that achieved with different types of implants. Methods: The study comprised 206 knees of Japanese patients who underwent KA-TKA using varying implants in our Hospital between May 2019 and April 2020. Measurements of pre- and postoperative coronal alignment were determined from weight-bearing full-leg standing radiographs. The postoperative results were compared to measurements taken from patients who underwent MA-TKA (N = 96) and normal knees (N = 60). Background: Recently, high efficacy of the chemotherapeutic regimen combining clarithromycin (CAM) with lenalidomide (Len) and dexamethasone (Dex) (BiRD) in treating multiple myeloma (MM) patients has been reported. However, the exact mechanism of added CAM has not been fully elucidated. This case report will provide helpful information for understanding the significance and the mechanism of action of CAM as an add-on therapy. Patient: A 78-year-old female patient with IgA-λ type MM was treated with low-dose Len coupled with low-dose Dex (low Rd), and excellent response was achieved for long term, but she later became refractory to this treatment. Then, CAM was added to low Rd (low Rd-CAM, i.e., modified BiRD therapy). This add-on-therapy was found to be effective, but later suspended because of pneumonitis. Then, low-dose Len coupled with CAM (low R-CAM) treatment was applied;but effect of this Dex-free treatment was insufficient. Thus, low Rd-CAM was reapplied and satisfactory reduction of IgA was achieved. This fact suggests that low Rd-CAM is the favorable combination, Dex is requisite and CAM might have enhanced the effect of Dex. In this case, various serum cytokines were examined during the course of illness. Only interleukin-6 showed apparent increase, and tumor necrosis factor-α, transforming growth factor-β, soluble IL-2 receptors and C-reactive protein showed the slight increase during low Rd-CAM treatment. The results seem somewhat conflicting, but it seems that intricate cytokine response due to immune activation might have occurred during low Rd-CAM treatment.

To improve the responsiveness of engine speed control to disturbances, robust controls were investigated by simulation. The intake air control system of a gasoline engine is a typical nonlinear system, and the disturbances and parameter perturbations are generally regarded as being the unstable factors with regard to engine control. In this paper, a Mean-Value Engine Model (MVEM) with disturbances and parameter perturbations is investigated using Sliding Mode Control (SMC), which is a form of variable structure control, with a view to address instability in the idle speed control process. The simulation results confirmed that, compared with a conventional PI (Proportional-Integral) controller, the stability of the idle speed for an engine that is being subjected to disturbances, parameter variations and background noise is greatly improved by the application of SMC.

We demonstrated the insitu observation of a moving atomic force microscope (AFM) cantilever using a laser confocal microscope combined with a differential interference microscope (LCM-DIM). The AFM cantilever scanned or indented the {110} surface of a hen egg-white lysozyme crystal in a supersaturated solution. Using a soft cantilever, we could observe the step growth with high time resolution by LCM-DIM and perform quantitative measurements of the step height by AFM simultaneously. In addition, a hard cantilever was used with LCM-DIM to observe the dynamics of crystal surface scratching and indentation. In the supersaturated solution, the small steps generated from the scratched line aggregated to macro steps, and subsequently flattened the surface.

In the ionosphere, the solar winds generate electrical currents. On the Earth surface, these currents cause magnetic field fluctuations. These fluctuations, penetrating the Earth interior, induce the electrical currents J, and, in the presence of the Earth magnetic field B, generate electromagnetic force, known as Lorentz force F = J × B. To study the relation of earthquakes and the Lorentz force, acting at the near onset times of strong earthquakes, we examine the Kp index, a logarithmic measure of the magnetic field deviation. The time varying Kp index gives us J, which in turn determines F. The variations of the Kp index were stacked by aligning their central times to the times of main earthquake shocks. This stacking method has been a popular and powerful tool in image processing, because it lifts up only the geomagnetic effect like carving a relief. The Lorentz force tilts the subtle force balance in the earth crust towards triggering the release of stress strain energy, initiating an earthquake in a similar way as a mountain climber’s step can trigger the avalanches. The internal dynamics, however, are highly statistical. Conventional statistical methods are used in combination with a newly devised method, which compares the time sequences of hypothetical random earthquakes to real ones. We find that the distinctive patterns of the Kp surges often strongly correlate to the onset of earthquake. This correlation depends on the seismic regions and the magnitudes of earthquakes. The stronger the earthquake is, more closely the Kp surge is associated. The statistical significance of nearly 100% is obtained for the Kp variations, synchronizing with more earthquakes in the Pacific Rim region. In parallel with the data analysis the historical studies are reviewed. The solar activities have been considered to influence the earthquake occurrences and the relation of the two has been studied extensively in the recent years as well as in the past century. According to the reports from Ministry of Health in each country, the average life span is elongated over the past decade. This trend is expected to be keeping constant further in the future and at the same time continued efforts should be made. Next, it is necessary for us to consent about improving the quality of life. In an effort of improving the quality of life, other than the western medicine, we have attempted to introduce many traditional medical practices, including the oriental medicine, from various parts of the world into the medical field as a complementary & alternative medicine (CAM). Judging both positive and negative aspects by the evaluation standard of the Western medicine, we try to obtain numerical aspects in termed of QOL. Some methods of alternative medicine involved in this review are acupuncture, moxibustion, TCM and other traditional medicine. Above all, WHO suggests selecting the suzerain nation of acupuncture moxibustion, which has been developed in the oriental countries as a specialized cure, by identifying merits & demerits and ascertaining which nation can be a the best model for special field. At the moment for evaluating CAM, for example, what kind of methods is/are suitable for evaluating each CAM. We have been trying to propose the peripheral leukocyte, that is one of the best marker for evaluating CAM. Our trial is a hot spring hydrotherapy, acupuncture and moxibution, light exercise and Ondle/floor heating etc. In these trials, we find the common results from leukocyte effects which exhibit a strong correlation for the regulation in number and function, as a new scale for evaluating QOL. The contents are the result from the data showing the much in number of volunteer tend to down regulate, on the other hand, lower number of volunteer is up-regulated at next day by each menu of CAM as a constitution dependent manner. However, even in western medical methodology, double blind and cross over system are the better way to evaluation
