Background Management of pacemaker(PM)infections among advanced aged patients possesses particular clinical challenges due to higher rates of concurrent cardiovascular disease and medical *** leadless cardiac pacemakers(LCPs)may provide new opportunities for better management options in this population,however,there is limited data especially in Asian populations to guide the decision *** We reviewed 11 octogenarians(median age:86[minimum 82–maximum 90]years;male:73%;median body mass index(BMI):20.1 kg/m^(2))who received Micra Transcatheter Pacing System(Medtronic Inc,Minneapolis,MN)implantations following transvenous lead extractions(TLEs)for PM *** All patients had more than two medical comorbidities(average 3.7 comorbidities).The indications for LCP implantations were atrioventricular block in four patients,atrial fibrillation bradycardia in five,and sinus node dysfunction in *** patients(73%)were bridged with temporary pacing using active fixation leads(median interval of 14.0 days),while one with severe dementia underwent a concomitant LCP implantation and TLE during the same *** TLEs and LCP implantations were successfully accomplished in all without any *** median time from the TLE procedure to discharge was 22 days(minimum 7–maximum 136).All patients remained free of infections during a mean follow-up period of 17.2±6.5 *** LCP implantations were safe and effective after removing the entire infectious PM system in all *** novel LCP technology may offer an alternative option for considering a re-implantation strategy after transvenous PM infections in elderly patients,particularly those with severe frailty and PM dependency.

目的心脏结节病（CS）可以通过***／CT、心血管磁共振（CMR）和123I-BMIPP／201T1C1双核素心肌SPECT显像来诊断。该研究旨在说明这3种方法之间的关系。方法和结果16例确诊为CS的患者[男性11例，年龄（55±13）岁]在类固醇治疗前进行了18F-FDGPET／CT、CMR和双核素门控SPECT显像。以上显像时左心室心肌均被分为17段，PET／CT显像时计算各段的最大标准摄取值（SUVmax）；双核素SPECT显像时计算放射性缺损评分（0-4；0为放射性分布正常，4为放射性缺损），包括123I—BMIPP显像的缺损评分（BMDS）、201T1C1显像的缺损评分（TLDS）和两者间的不匹配评分（MS）；CMR显像时计算延时钆增强（LGE）评分（0为无延时增强；1为部分增强；2为均匀增强）。17个节段总计BMDS、TLDS和MS分别为18．6±12．6、12．9±10．9和5．7±3．1。各段的BMDS和TLDS评分随着LGE评分增加而显著增加（P〈0．001）。与LGE评分为0的节段相比，LGE评分为1的节段其MS评分显著增加（P〈0．001），但是LGE评分为1和2的节段其MS评分无明显差异。就LGE评分为1的节段而言，其SUVmax与LGE评分为0的节段相比显著增加（P〈O．025），但是与LGE评分为2的节段相比无明显差异。结论CS患者具有较高SUVmax的区域主要位于有延时增强效应的节段，提示有活动性心肌炎性反应，这些患者123I-BMIPP和和201T1C1显像有明显的不匹配。

Correlation between dose-volume parameters and rectal bleeding after 12 fractions of carbon ion radiotherapy for prostate cancer

BACKGROUND Carbon ion radiotherapy(CIRT)is currently used to treat prostate *** bleeding is a major cause of toxicity even with ***,to date,a correlation between the dose and volume parameters of the 12 fractions of CIRT for prostate cancer and rectal bleeding has not been ***,the clinical risk factors for rectal bleeding were absent after 12 fractions of *** To identify the risk factors for rectal bleeding in 12 fractions of CIRT for prostate *** Among 259 patients who received 51.6 Gy[relative biological effectiveness(RBE)],in 12 fractions of CIRT,15 had grade 1(5.8%)and nine had grade 2 rectal bleeding(3.5%).The dose-volume parameters included the volume(cc)of the rectum irradiated with at least x Gy(RBE)(Vx)and the minimum dose in the most irradiated x cc normal rectal volume(Dx).RESULTS The mean values of D6cc,D2cc,V10 Gy(RBE),V20 Gy(RBE),V30 Gy(RBE),and V40 Gy(RBE)were significantly higher in the patients with rectal bleeding than in those *** cutoff values were D6cc=34.34 Gy(RBE),D2cc=46.46 Gy(RBE),V10 Gy(RBE)=9.85 cc,V20 Gy(RBE)=7.00 cc,V30 Gy(RBE)=6.91 cc,and V40 Gy(RBE)=4.26 *** D2cc,V10 Gy(RBE),and V20 Gy(RBE)cutoff values were significant predictors of grade 2 rectal *** The above dose-volume parameters may serve as guidelines for preventing rectal bleeding after 12 fractions of CIRT for prostate cancer.

Purpose: The genus Pseudomonas is a ubiquitous microorganism frequently detected from immunocompromised patients. The inherent resistance to numerous antimicrobial agents contributes to the opportunistic character of this pathogen exhaustive monitoring of this pathogen is considered of critical importance to public health organizations. The reliable identification method able to distinguish genetic close Pseudomonas species is needed, because these organisms are difficult to differentiate by phenotypic or biochemical methods. The purpose of the present study was to design species-specific primers in order to identify and detect four Pseudomonas species which are frequently detected from the human oral cavities, and to investigate the distribution of these organisms in the living environment using a multiplex PCR. Methods: Polymerase chain reaction (PCR) primers were designed based on partial sequences of the rpoD gene of four Pseudomonas species. Swab samples were collected from fifty washstands, and the distribution of Pseudomonas species was investigated using a conventional PCR at genus level and a multiplex PCR at species level. Results: Multiplex PCR method developed in this study was able to distinguish four Pseudomonas species clearly. The genus Pseudomonas was detected from all samples (100%), whereas P. putida, P, aeruginosa, P. stutzeri and P. fluorescens were detected at 44%, 8%, 4% and 2% in fifty swab samples, respectively. Conclusion: Our developed one-step multiplex PCR method is accurate, specific, cost-effective, time-saving, and works without requiring DNA extraction. It was indicated that washstands were the uninhabitable environment for P. putida, P, aeruginosa, P. stutzeri and P. fluorescens.

Regular assumption of finite element meshes is a basic condition of most analysis of finite element approximations both for conventional conforming elements and nonconforming elements. The aim of this paper is to present a novel approach of dealing with the approximation of a four-degree nonconforming finite element for the second order elliptic problems on the anisotropic meshes. The optimal error estimates of energy norm and L^2-norm without the regular assumption or quasi-uniform assumption are obtained based on some new special features of this element discovered herein. Numerical results are given to demonstrate validity of our theoretical analysis.

Biomimetic Drag Reduction Study on Herringbone Riblets of Bird Feather

Birds have gradually formed various excellent structures such as streamlined shape and hollow shaft of feather to improve their flying performance by millions of years of natural selection. As typical property of bird feather, herringbone riblets align along the shaft of each feather, which is caused by perfect link of barbs, especially for the primary and secondary feathers of wings. Such herringbone riblets of feather are assumed to have great impact on drag reduction. In this paper, microstructures of secondary feathers of adult pigeons are investigated by SEM, and their structural parameters are statistically obtained. Based on quantitative analysis of feather structure, novel biomimetic herringbone riblets with narrow smooth edge are proposed to reduce surface drag. In comparison with traditional microgroove riblets and other drag reduction structures, the drag reduction rate of the proposed biomimetic herringbone riblets is experimentally clarified up to 16%, much higher than others. Moreover, the drag reduction mechanism of herringbone riblets are also confirmed and exploited by CFD.

BACKGROUND Alpha-1 antitrypsin deficiency is a rare genetic disease and a leading cause of inherited alterations in plasma protein metabolism(APPM).AIM To understand the prevalence,burden and progression of liver disease in patients with APPM including alpha-1 antitrypsin *** We conducted a retrospective analysis of anonymized patient-level claims data from a German health insurance provider(AOK PLUS).The APPM cohort comprised patients with APPM(identified using the German Modification of the International Classification of Diseases-10th Revision[ICD-10-GM]code E88.0 between 01/01/2010-30/09/2020)and incident liver disease(ICD-10-GM codes K74,K70.2-3 and K71.7 between 01/01/2012-30/09/2020).The control cohort comprised patients without APPM but with incident liver *** were incidence/prevalence of liver disease in patients with APPM,demographics/baseline characteristics,diagnostic procedures,progression-free survival(PFS),disease progression and *** Overall,2680 and 26299 patients were included in the APPM(fibrosis,96;cirrhosis,2584)and control(fibrosis,1444;cirrhosis,24855)cohorts,*** 100000 individuals,annual incidence and prevalence of APPM and liver disease was 10-15 and 36-51,*** the APPM cohort,median survival was 4.7 years[95%confidence interval(CI):3.5-7.0]and 2.5 years(95%CI:2.3-2.8)in patients with fibrosis and cirrhosis,respectively.A higher proportion of patients in the APPM cohort experienced disease progression(92.0%)compared with the control cohort(67.2%).Median PFS was shorter in the APPM cohort(0.9 years,95%CI:0.7-1.1)compared with the control cohort(3.7 years,95%CI:3.6-3.8;P<0.001).Patients with cirrhosis in the control cohort had longer event-free survival for ascites,hepatic encephalopathy,hepatic failure and esophageal/gastric varices than patients with cirrhosis in the APPM cohort(P<0.001).Patients with fibrosis in the control cohort had longer event-free survival for ascites,cir

MOTION ERROR ESTIMATION OF 5-AXIS MACHINING CENTER USING DBB METHOD

In order to estimate the motion errors of 5-axis machine center, the double ball bar （DBB） method is adopted to realize the diagnosis procedure. The motion error sources of rotary axes in 5-axis machining center comprise of the alignment error of rotary axes and the angular error due to various factors, e.g. the inclination of rotary axes. From sensitive viewpoints, each motion error is possible to have a particular sensitive direction in which deviation of DBB error trace arises from only some specific error sources. The model of the DBB error trace is established according to the spatial geometry theory. Accordingly, the sensitive direction of each motion error source is made clear through numerical simulation, which is used as the reference patterns for rotational error estimation. The estimation method is proposed to easily estimate the motion error sources of rotary axes in quantitative manner. To verify the proposed DBB method for rotational error estimation, the experimental tests are carried out on a 5-axis machining center M-400 （MORISEIKI）. The effect of the mismatch of the DBB is also studied to guarantee the estimation accuracy. From the experimental data, it is noted that the proposed estimation method for 5-axis machining center is feasible and effective.

Several applications of Origami have been tested in industrial area. However, Origami based design is found only in lamp shade and wrapping in design area, and has not been practically applied especially in clothing. In this paper, the conical truss model is applied in the clothing skirt, and the simulation software from 2D to 3D is developed by parametric Origami module （POM）. Finally, the pattern is copied on felt clothing, and the scaled model of skirt on the basis of conical reverse truss model design is made.
